Efficacy of Intraoperative Facial Electromyographic Monitoring in Patients with Hemifacial Spasm

  • Objective : Hemifacial spasm has characteristic and specific electrophysiological finding, lateral spread response[LSR]. We study the correlation between change of lateral spread response during microvascular decompression[MVD] and clinical outcome after MVD. Methods : Sixty two patients with hemifacial spasm who were treated with microvascular decompression from March 2000 to February 2003 were included in this study. The monitoring of intraoperative facial electromyography[EMG] and brain stem auditory evoked potential were performed. Results : In 28 [44.7%] patients, there was persistence of lateral spread response after vascular decompression in root exit zone of facial nerve. Among these 28 patients, 9 had mild hemifacial spasm at discharge. Three out of 34 patients who had intraoperative disappearance of lateral spread response after MVD had mild hemifacial spasm. But Both groups, disappearance of LSR [Group I], and persistence [Group II] had only 2 patients with mild hemifacial spasm, and 5 patients at 3 months, respectively. Conclusion : Although intraoperative EMG monitoring is very useful in assessing the efficacy of MVD, the clinical outcome of MVD in patient with hemifacial spasm does not always correlate with EMG finding. The prognostic value of intraoperative LSR monitoring in the long-term results is questionable.

Diagnostic Usefulness of CISS Image in Preoperative Evaluation of Trigeminal Neuralgia and Hemifacial Spasm (삼차신경통과 반측안면경련에서 CISS 영상의 진단적 유용성)

  • Objectives : Trigeminal neuralgia and hemifacial spasm are caused by vascular compression of the REZ(root entry or exit zone) of the 5th and the 7th cranial nerve. Preoperative detection of neurovascular compression is essential for accurate diagnosis, appropriate treatment, and the good operative results. Three dimensional Fourier Transformation-Constructive Interference in Steady State(3DFT-CISS) images are known to give good contrast between CSF, nerve, and vessels. We applied a 3DFT-CISS imaging technique for the preoperative evaluation of patients with these diseases and estimated the diagnostic accuracy and usefulness of this study. Methods : A series of 71 patients with trigeminal neuralgia and hemifacial spasm were treated by microvascular decompression. Among them 34 patients with trigeminal neuralgia and 24 patients with hemifacial spasm had preoperative CISS images. We compared the radiologic finding with the operative finding, and analysed the diagnostic usefulness of 3DFT-CISS imaging. Results : The sensitivity of CISS images of detecting the neurovascular compression was 90.3% in trigeminal neuralgia and 100% in hemifacial spasm. There were one false-positive and three false-negative cases in trigeminal neuralgia, and one false-positive case in hemifacial spasm. The accuracy in diagnosing the causative vessel was 73.5% in trigeminal neuralgia and 83.3% in hemifacial spasm. Conclusion : CISS image is very useful diagnostic tool for preoperative evaluation of neurovascular compression in patients with trigeminal neuralgia and hemifacial spasm. No additional neuroradiologic examination other than CISS image and MRA is needed for preoperative evaluation of patients with trigeminal neuralgia and hemifacial spasm.

  • This paper proposes heuristic greedy algorithm that can be find the solution within polynomial time with solution finding rule for the capacitated minimum spanning tree(CMST) problem, known as NP-hard. The CMST problem can be solved by computer-aided meta-heuristic because of the Esau-Williams heuristic polynomial time algorithm has a poor performance. Nevertheless the meta-heuristic methods has a limit performance that can't find optimal solution. This paper suggests visual by handed solution-finding rule for CMST. The proposed algorithm firstly construct MST, and initial feasible solution of CMST from MST, then optimizes the CMST with the subtree gates more adjacent to root node. As a result of total 30 cases of OR-LIB 10 data, Q=3,5,10, the proposed algorithm gets the best performance.

INDIRECT IMMUNOFLUORESCENCE FOR THE IDENTIFICATION OF ACTINOMYCES SPECIES IN PATIENTS WITH PERIAPICAL LESION (치근단 병변에서 Actinomyces 종의 검정을 위한 간접 면역형광법적 연구)

  • Actinomyces are Gram-positive, non-acid-fast, anaerobic or microaerophilic filamentous bacteria. These organisms are frequently detected from infected root canals and periapical lesion. The purpose of this study was to use indirect immunofluorescence to determine the prescence of select Actinomyces species in a survey of teeth associated with periapical lesion, to clarify the relationship between clinical symptoms of periapical lesions and the Actinomyces species and to study on the cross reaction among Actinomyces. Actinomyces israelii serotype I (ATCC 12102), Actinomyces israelii serotype II (ATCC 29322), Actinomyces viscosus serotype II (ATCC 19246), Actinomyces naslundii serotype I (ATCC 12104) were cultured in anaerobic condition. Rabbit antisera were prepared by intravenous injection of formalized whole cells. Indirect immunofluorescence method was used to achieve the purpose. The following results were obtained. 1. There was a relationship between Actinomyces and periapical disease. 2. A. israelii serotype I, II were frequently identified with Indirect Immunofluorescence and most often assosiated with periapical disease. In culture finding, there was no significant difference between each group. 3. Indirect Immunofluoresence is both more sensitive and more rapid than culture for identification of Actinomyces species in patients with periapical lesion. 4. A. israelii serotype I, II was highly isolated in infected root canals with local swelling, A. naslundii serotype I was highly isolated in those with foul odor, and A. israelii serotype I was found in higher frequncy in those with exudate than other bacteria. 5. In the Indirect Immunofluorescence (1 : 320), A positive cross reaction was obtained between A. israelii serotype I and A. israelii serotype II, also, A. viscosus serotype II and A. naslundii serotype I. There was no cross reaction between A. israelii serotype I, II and A. viscosus serotype II, A. naslundii serotype I.

  • In constructing projects, there exist various kinds of work interferences, which cause a delay of the outset and completion of planned schedule because of some attributions, such as variability, uncertainty and complexity. and so schedule delay has been treated as a natural phenomenon. To reduce or prevent the schedule delay, a constant confirmation of schedule delay and a preparation of counter plans for finding out the cause structure of schedule delay should have been done. However, all this time the research has been mostly done on the calculation method or claim cases of schedule delay. Moreover, the analysis method did not consider the trait, which cause the schedule delay, in constructing projects. This paper restricts the range of the cause analysis of schedule delay to the field of site management in the projects and divides the cause structure of schedule delay into the cause objects and cause attributes of schedule delay according to the input elements. The system of classifying causes of schedule delay is examined by interviews with experts and questionnaire. Additionally, this paper analyzes the attributes of cause attributes and cause subjects and presents the analysis method and procedure of schedule delay with the application of VSM.

  • This research analyzed type of corrective action of findings of audit result and grasped item that must execute question investigation about operation actual conditions and corrective action to investigate actually situation in establishment. present item that must consider when adopt regard and organization that must consider when confirm description and follow up of an audit that certification body and jury publish as judging result do corrective action for improvement. Formation achieve continual improvement of IMS, first, when certification audit, findings are drawn faithfully and describing finding should be described as systematic problem. Second, must do so that can be removed grasping systematic root cause that do corrective action. Third, must confirm whether systematic corrective action assumed to prove continual improvement securing that when confirm succession about result of corrective action, root cause is removed and reappearance of resemblant problem is prevented.

  • This paper presented the application of GAs as a means of finding optimal solutions over a parameter space in the controller design for a fuzzy control system. The performance can involve a weighted combination of various performance characteristics such as rise-time, settling-time, settling-time, overshoot. The results obtained here are compared with those for a traditional design obtained using the root-locus method. In contrast to traditional methods, the GA-based method does not require the usual mathematical processess or mathematical model of the system. In this paper, the Ga-based Fuzzy control system combining Fuzzy control theory with the GA, that is known to be very effective in the optimization problem, will be proposed The effectiveness of the proposed control system will be demonstrated by computer simulations using task tracking position system in stable and unstable linear systems. It is shown that the GA-based controller is better than the traditional controller used It stable and unstable linear systems.

  • In this paper, we proposed a 16-element array antenna design to improve signal detection performances. The array antenna characteristics, such as mutual coupling, pattern deviation, and half power beamwidth of the active element, were examined to obtain an optimal spacing between individual elements. The single element of the array antenna consists of an indirect feed using L-shaped feed and shorted radiating patch to achieve a broadband operation. Root mean square(RMS) errors based on the incident angle of the signal were calculated to verify the signal detection performance of the proposed antenna. The results demonstrate that the proposed array antenna with optimal spacing is suitable for detecting interference signals with low RMS error.

  • Purpose: This study aimed to evaluate the anatomic circle around the impacted lower third molar to show, document, and correlate essential findings that should be included in the routine radiographic assessment protocol as clinically meaningful factors in overall case evaluation and treatment planning. Materials and Methods: Cone-beam computed tomographic images of impacted lower third molars were selected according to specific inclusion criteria. Impacted teeth were classified according to their position before assessment. The adjacent second molars were assessed for distal caries, distal bone loss, and root resorption. The fourth finding was the presence of a retromolar canal distal to the impaction. Communication with the dentist responsible for each case was done to determine whether these findings were detected or undetected by them before communication. Results: Statistically significant correlations were found between impaction position, distal bone loss, and detected distal caries associated with the adjacent second molar. The greatest percentage of undetected findings was found in the evaluation of distal bone status, followed by missed detection of the retromolar canal. Conclusion: The radiographic assessment protocol for impacted third molars should consider a step-by-step evaluation for second molars, and clinicians should be aware of the high prevalence of second molar affection in horizontal and mesioangular impactions. They also should search for the retromolar canal due to its associated clinical considerations.

  • The Newton-Raphson iterative algorithm for finding a floating point reciprocal square mot calculates it by performing a fixed number of multiplications. In this paper, a variable latency Newton-Raphson's reciprocal square root algorithm is proposed that performs multiplications a variable number of times until the error becomes smaller than a given value. To find the rediprocal square root of a floating point number F, the algorithm repeats the following operations: '$X_{i+1}=\frac{{X_i}(3-e_r-{FX_i}^2)}{2}$, $i\in{0,1,2,{\ldots}n-1}$' with the initial value is '$X_0=\frac{1}{\sqrt{F}}{\pm}e_0$'. The bits to the right of p fractional bits in intermediate multiplication results are truncated and this truncation error is less than '$e_r=2^{-p}$'. The value of p is 28 for the single precision floating point, and 58 for the double precision floating point. Let '$X_i=\frac{1}{\sqrt{F}}{\pm}e_i$, there is '$X_{i+1}=\frac{1}{\sqrt{F}}-e_{i+1}$, where '$e_{i+1}{<}\frac{3{\sqrt{F}}{{e_i}^2}}{2}{\mp}\frac{{Fe_i}^3}{2}+2e_r$'. If '$|\frac{\sqrt{3-e_r-{FX_i}^2}}{2}-1|<2^{\frac{\sqrt{-p}{2}}}$' is true, '$e_{i+1}<8e_r$' is less than the smallest number which is representable by floating point number. So, $X_{i+1}$ is approximate to '$\frac{1}{\sqrt{F}}$. Since the number of multiplications performed by the proposed algorithm is dependent on the input values, the average number of multiplications Per an operation is derived from many reciprocal square root tables ($X_0=\frac{1}{\sqrt{F}}{\pm}e_0$) with varying sizes. The superiority of this algorithm is proved by comparing this average number with the fixed number of multiplications of the conventional algorithm. Since the proposed algorithm only performs the multiplications until the error gets smaller than a given value, it can be used to improve the performance of a reciprocal square root unit. Also, it can be used to construct optimized approximate reciprocal square root tables. The results of this paper can be applied to many areas that utilize floating point numbers, such as digital signal processing, computer graphics, multimedia, scientific computing, etc.
