Micro-computed tomographic evaluation of canal retreatments performed by undergraduate students using different techniques

  • Objectives: This study evaluated the amount of remaining root canal filling materials after retreatment procedures performed by undergraduate students using manual, rotary, and reciprocating techniques through micro-computed tomographic analysis. The incidence of instrument fracture and the instrumentation time were also evaluated. Materials and Methods: Thirty maxillary single rooted teeth were prepared with Reciproc R25 files and filled with gutta-percha and AH Plus sealer by the continuous wave of condensation technique. Then, the specimens were assigned to 3 groups (n = 10), according to the retreatment technique used: manual, rotary, and reciprocating groups, which used K-file, Mtwo retreatment file, and Reciproc file, respectively. Retreatments were performed by undergraduate students. The sample was scanned after root canal filling and retreatment procedures, and the images of the canals were examined to quantify the amount of remaining filling material. The incidence of instrument fracture and the instrumentation time were recorded. Results: Remaining filling material was observed in all specimens regardless of the technique used. The mean volume of remaining material was significantly lower in the Reciproc group than in the manual K-file and Mtwo retreatment groups (p < 0.05). The time required to achieve a satisfactory removal of canal filling material and refinement was significantly lower in the Mtwo retreatment and Reciproc groups (p < 0.05) when compared to the manual K-file group. No instrument fracture was observed in any of the groups. Conclusions: Reciproc was the most effective instrument in the removal of canal fillings after retreatments performed by undergraduate students.

포졸란 시멘트를 기반으로 하는 근관전색제의 치근단부 미세누출 평가 (Microleakage Assessment of a Pozzolan Cement-based Mineral Trioxide Aggregate Root Canal Sealer)

  • 본 연구는 포졸란 시멘트를 기반으로 하는 근관전색제인 Endoseal MTA (Maruchi, Wonju, Korea)를 근관전색제와 근관충전제로 사용하였을 때 나타나는 치근단부 미세누출을 기존의 근관전색제인 AH $Plus^{(R)}$ (Dentsply DeTrey, Konstanz, Germany)와 비교하여 평가하고자 하였다. 만곡되지 않은 영구 전치를 3개의 실험군(A군, E1군, E2군)으로 나누고 A군은 AH $Plus^{(R)}$와 gutta-percha (GP), E1군 은 Endoseal MTA와 GP, E2군은 Endoseal MTA 단독으로 충전하였다. 충전이 완료된 치아는 완전한 경화를 위해 24시간동안 멸균 증류수에 보관하였고 염색액 누출을 위해 치근단 부위를 0.2% 로다민 B 염색 용액에 24시간 동안 노출시켰다. 입체 현미경을 이용하여 미세누출 정도를 평가하였으며 3개의 실험군 사이에 유의한 차이가 관찰되지 않았다. 결론적으로 Endoseal MTA는 GP의 사용에 관계 없이 AH $Plus^{(R)}$와 유사한 밀폐력을 보였으며 이러한 결과는 Endoseal MTA의 근관전색제 및 근관 충전제로써 유용한 사용에 대한 가능성을 제시한다.

MTA와 4-META/MMA & TBB레진 혼합 재료의 치근단 미세누출에 관한 연구 (APICAL MICROLEAKAGE OF MTA WITH 4-META/MMA & TBB RESIN AS A ROOT-END FILLING MATERIAL)

  • 이 연구에서는 Mineral Trioxide Aggregate (MTA)와 4-methacryloxyethyl trimellitate anhydride (4-META) / methyl methacrylate (MMA) & tri-n-butylborane (TBB) 레진을 혼합하여 치근단 역충전 재료로 사용하였을 때의 미세누출을 MTA, 포틀랜드 시멘트와 비교하였다. 발치된 사람 치아의 근관형성 및 충전 후 치근단 절제와 역충전 와동을 형성하였다. MTA, Portland cement, MTA와 4-META/MMA & TBB레진을 혼합한 각각의 재료로 치근단을 역충전한 후 methylene blue dye에 72시간 동안 침적하여 염료가 침투한 길이를 10배 현미경 하에서 측정하였다. MTA와 4-META/MMA & TBB 레진 혼합 충전 군에서는 포틀랜드 시멘트나 MTA 충전군보다 통계적으로 유의하게 적은 미세 누출을 보였으며, 포틀랜드 시멘트와 MTA 충전군의 미세 누출 정도는 통계적으로 차이가 없었다. 이 실험의 결과로 보아 MTA와 4-META/MMA & TBB레진 혼합 재료는 치근단 역충전시 포틀랜드 시멘트나 MTA에 비해 낮은 초기 미세누출을 보여, 치근단 봉쇄 효과가 우수한 것으로 사료된다.

System BTM를 이용한 열가압 충전시의 치주인대내에 미치는 영향 (THE EFFECTS OF THERMOCONDENSATION TECHNIQUE USING SYSTEM BTM ON THE PERIODONTIUM)

  • Thermocondensation root canal filling technique have been used to fill accessary canals or to obtain homogeneous root caral fillings. But these thermocondensation technique inevitably produce heat in the canal which can be transmitted through the dentin and cementum and consequently damage periodontal ligamental cells and osteoblasts. In this study, System $B^{TM}$(Analytic technology, WA.D.S.A.) was used to evaluate the reaction of periodontal ligament tissue to "Continous Wave condensation technique" introduced by Buchanan, and the transmitted root surface temperature was measured according to measured root thicknesses. 12 Mandibular incisors of two adult dogs were used for the experiment. 6 controls were filled by lateral condensation technique with sealer.3 specimens were apically filled by Continuous Wave technique at $200^{\circ}C$ for 5 seconds and remaining 3 specimens were additionally backfilled using System $B^{TM}$(Analytic technology, WA.D.S.A.) was used to evaluate the reaction of periodontal ligament tissue to "Continous Wave condensation technique" introduced by Buchanan, and the transmitted root surface temperature was measured according to measured root thicknesses. 12 Mandibular incisors of two adult dogs were used for the experiment. 6 controls were filled by lateral condensation technique with sealer.3 specimens were apically filled by Continuous Wave technique at $200^{\circ}C$ for 5 seconds and remaining 3 specimens were additionally backfilled using System $B^{TM}$ at $100^{\circ}C$ for 20 seconds. Six weeks later, the dogs were sacrificed and the teeth stained with Hematoxylin and Eosin for histologic examination. 6 extracted human teeth were used to measure the transmitted temperature. After cutting off the crown, the canals were prepared and divided into 3 groups with root thickness of 1.5mm, 1.0mm, 0.5mm, 2 teeth in each group. Inside each root canal, System $B^{TM}$ was heated as with the temperature for the apically condensed and the back filled group, and the transmitted heat was measured on the external surface of the root. The temperature of System $B^{TM}$ heat spreader at $200^{\circ}C$ and $100^{\circ}C$ was also measured at root temperature. It can be concluded as follows: 1. In the thin area (200-$250{\mu}m$) of the root, root resorption could be seen even with heating at $200^{\circ}C$ for 5 seconds. 2. When the spreader was heated at $200^{\circ}C$ for 5 seconds and additionally at $100^{\circ}C$ for 20 seconds for backfill, all teeth showed root resorption regardless of their root thickness. 3. The transmitted external root surface temperature was higher as the root thickness decreased and as the heating time increased. In the thermocompaction technique using System $B^{TM}$, the spreader should be heated for the minimal time and used only in the apical area. The heated spreader shouldn't inserted to the binding point of the canal and backfilling should be done with other means of minimally heated gutta percha technique.

CBCT-based assessment of root canal treatment using micro-CT reference images

  • Purpose: This study compared the root canal anatomy between cone-beam computed tomography (CBCT) and micro-computed tomography (micro-CT) images before and after biomechanical preparation and root canal filling. Materials and Methods: Isthmus-containing mesial roots of mandibular molars(n=14) were scanned by micro-CT and 3 CBCT devices: 3D Accuitomo 170 (ACC), NewTom 5G (N5G) and NewTom VGi evo (NEVO). Two calibrated observers evaluated the images for 2-dimensional quantitative parameters, the presence of debris or root perforation, and filling quality in the root canal and isthmus. The kappa coefficient, analysis of variance, and the Tukey test were used for statistical analyses(α=5%). Results: Substantial intra-observer agreement (κ=0.63) was found between micro-CT and ACC, N5G, and NEVO. Debris detection was difficult using ACC (42.9%), N5G (40.0%), and NEVO (40%), with no agreement between micro-CT and ACC, N5G, and NEVO (0.05<κ<0.12). After biomechanical preparation, 2.4%-4.8% of CBCT images showed root perforation that was absent on micro-CT. The 2D parameters showed satisfactory reproducibility between micro-CT and ACC, N5G, and NEVO (intraclass correlation coefficient: 0.60-0.73). Partially filled isthmuses were observed in 2.9% of the ACC images, 8.8% of the N5G and NEVO images, and 26.5% of the micro-CT images, with no agreement between micro-CT and ACC, and poor agreement between micro-CT and N5G and NEVO. Excellent agreement was found for area, perimeter, and the major and minor diameters, while the roundness measures were satisfactory. Conclusion: CBCT images aided in isthmus detection and classification, but did not allow their classification after biomechanical preparation and root canal filling.

Microorganism penetration in dentinal tubules of instrumented and retreated root canal walls. In vitro SEM study

  • Objectives: This in vitro study aimed to investigate the ability of Candida albicans (C. albicans) and Enterococcus faecalis (E. faecalis) to penetrate dentinal tubules of instrumented and retreated root canal surface of split human teeth. Materials and Methods: Sixty intact extracted human single-rooted teeth were divided into 4 groups, negative control, positive control without canal instrumentation, instrumented, and retreated. Root canals in the instrumented group were enlarged with endodontic instruments, while root canals in the retreated group were enlarged, filled, and then removed the canal filling materials. The teeth were split longitudinally after canal preparation in 3 groups except the negative control group. The teeth were inoculated with both microorganisms separately and in combination. Teeth specimens were examined by scanning electron microscopy (SEM), and the depth of penetration into the dentinal tubules was assessed using the SMILE view software (JEOL Ltd). Results: Penetration of C. albicans and E. faecalis into the dentinal tubules was observed in all 3 groups, although penetration was partially restricted by dentin debris of tubules in the instrumented group and remnants of canal filling materials in the retreated group. In all 3 groups, E. faecalis penetrated deeper into the dentinal tubules by way of cell division than C. albicans which built colonies and penetrated by means of hyphae. Conclusions: Microorganisms can easily penetrate dentinal tubules of root canals with different appearance based on the microorganism size and status of dentinal tubules.

한국인 여성 상악중절치의 근관면적비에 관한 연구 (A Study on Root Canal Index of the Maxillary Central Incisorsin Korean Female)

  • The author had selected the roots and root-canal as measurable parts and sought the area ratio by measuring the respective areas of the root. Further, heplotted out a root caual index and studied the correlation with age. The teeth used as reserch material were permanent maxillary central incisors of Korean female. Some 296 teeth of known age were selected on condition that there is no caries or filling material and that they were not malformed in showing normal signs in roentgenograms. The $3" {\times} 4"$ printing paper so as to measure easily. On the ocassion of measureing the area of measured parts with a planimeter (Koizumi, type kp-27, Japan), the cervical lines were joined up into a straight line on a photograph (Figure 1) Root canal index = Area of the root / Area of the root canal The results of the root canal index in Korean female age groups were as follows : 1. The root canal index of maxilary central incisor in women was 4.74 im 20 years of age, 5.44 in 30, 5.90 in 40, 6.32 in 50, 6.63 in 60 in the order. 2. Root canal index and age were in positive correlation ; there was a tendency that the root canal index increase as age advances. 3. The regression equation was as follows : Y = 5.36x + 7.71 (r = 0.54, n= 296 ) (Y = estimated age, x = root canal index)

근관 소독제의 효과에 관한 임상적 연구 (A CLINICAL STUDY ON THE EFFECT OF INTRACANAL MEDICATION IN ENDODONTIC TREATMENT)

  • The purpose of this study was to clarify the effect of intracana1 medication on the clinical symptoms and the number of visits before canal filling in endodontic treatment The experimental teeth was divided into three groups. The teeth of group 1 was irrigated with saline solution and the teeth of group 2 was irrigated with sodium hypochlorite during biomechanical preparation. The intracanal medicament was not applied in the canals of group 1 and group 2. The teeth of group 3 was irrigated with sodium hypochlorite and hydrogen peroxide during canal enlargement and applied formocresol as intracanal medicament after drying the root canals with paper points. The incidence of interappintment pain and percussion pain after 1 st visit and the number of appointments before canal filling was examined, and the results was statistically compared. The following results were obtained. 1. The incidence of interappointment pain and percussion pain and the number of visits before canal filling was not effected by the types of irrigant and the use of the intracanal medicament 2. There was no significant difference in the incidence of interappintment pain arid percussion pain and the number of appointement before canal filling between vital teeth and non vital teeth, and also between single rooted teeth and multirooted teeth. 3. The use of formocresol as intracanal medicament was not able to aid the elimination of clinical symptoms and to decrease the number of appointment before canal filling.

Fluid transport model을 이용한 치근단 역충전 재료의 혈액오염시 미세누출평가 (EVALUATION OF MICROLEAKAGE WITH RETROGRADE FILLING MATERIALS IN BLOOD CONTAMINATION USING FLUID TRANSPORT MODEL)

  • Leakage studies have been performed frequently, since a fluid-tight seal provided by various dental fill-ing materials has been considered clinically important. The leakage of the various root-end filling materials has been widely investigated mostly dye penetration method. These dye studies cannot offer any information about the quality of the seal of a test material over a long period of time The purpose of this study was to evaluate the microleakage of root end cavities in blood contamination filed amalgam, intermediate restorative material(IRM), light cured glass ionomer cement(GI) and mineral trioxide aggregate(MTA) by means of a modified fluid transport model. Fifty standard human root sections, each 5mm high and with a central pulp lumen of 3mm in diameter, were and filled with our commonly used or potential root end fill ing materials after they were contaminated with blood. At 24h. 72h, 1, 2, 4, 8, and 12 weeks after filling, leakage along these filling materials was determined under a low pressure of 10KPa(0.1atm) using a fluid transport model. The results were as follows : 1 MTA group showed a tendency of decreasing percent of gross leakage (20m1/day) in process of time, whereas the other materials showed a tendency of increasing in the process time. 2. At the all time interval, GI group leaked significantly less than amalgam group and IRM group (p<0.05). 3. At the 4 weeks, the percentage of gross leakage in MTA group decreased to 0% thereafter, the low per-centage of gross leakage was maintained in MTA group until the end of the experiment, whereas the percentage in IRM group increased to 100% 4. At the 12 weeks, percentage of gross leakage was significantly low in MTA group(0%), comparison with GI group(40%), amalgam group(90%) and IRM group(100%), but there was no significant difference between latter two materials.

근관충전방법에 따른 폐쇄효과의 비교 (COMPARISON OF THE SEALING ABILITY OF VARIOUS ROOT CANAL FILLING TECHNIQUES)

  • The purpose of this study was to evaluate the sealing ability of various canal filling methods. Palatal roots of ninety extracted human maxillary molar teeth were resected at cementoenamel junction. Eighty of them were randomly assigned to four experimental groups, ten were served as positive and negative controls. All canals were prepared to # 40 using Profile. Experimental groups were obturated by lateral condensation technique, Thermafil technique, Continuous Wave of Condensation Technique, and down-pack & back-fill technique of Obtura-II, each with root canal sealer. Control groups were not obturated. Teeth were immersed in resorcinol-formaldehyde resin for 5 days at $4^{\circ}C$, and the resin was allowed to polymerize completely for 4 days at room temperature. Teeth were then ground horizontally at 1.5mm(level 1), 2.5mm(level 2), 3.5 mm(level 3) from the anatomical apex and examined with a stereomicroscope at ${\times}40$ magnification. The gap between the canal wall and the filling material, which was filled with the resin, was measured at each of the three levels. Each ratio of leakage was obtained by calculating the ratio of the area of the resin to the total area of the canal and was analyzed statistically (Rank-sum test). The results were as follows : 1. At the level 1, there was the greatest leakage in the Thermafil group and Obtura-II group, and the difference between the Obtura-II group and Continuous Wave of Condensation Technique group was statistically significant(p<0.05). 2. At the level 2, there was the least leakage in the Continuous Wave of Condensation Technique group, but there was no statistically significant difference between each group (p>0.05). 3. At the level 3, there was no statistically significant difference between each group(p>0.05).
