Retreatability of two endodontic sealers, EndoSequence BC Sealer and AH Plus: a micro-computed tomographic comparison

  • Objectives: Recently, bioceramic sealers like EndoSequence BC Sealer (BC Sealer) have been introduced and are being used in endodontic practice. However, this sealer has limited research related to its retreatability. Hence, the aim of this study was to evaluate the retreatability of two sealers, BC Sealer as compared with AH Plus using micro-computed tomographic (micro-CT) analysis. Materials and Methods: Fifty-six extracted human maxillary incisors were instrumented and randomly divided into 4 groups of 14 teeth: 1A, gutta-percha, AH Plus retreated with chloroform; 1B, gutta-percha, AH Plus retreated without chloroform; 2A, gutta-percha, EndoSequence BC Sealer retreated with chloroform; 2B, gutta-percha, EndoSequence BC Sealer retreated without chloroform. Micro-CT scans were taken before and after obturation and retreatment and analyzed for the volume of residual material. The specimens were longitudinally sectioned and digitized images were taken with the dental operating microscope. Data was analyzed using an ANOVA and a post-hoc Tukey test. Fisher exact tests were performed to analyze the ability to regain patency. Results: There was significantly less residual root canal filling material in the AH Plus groups retreated with chloroform as compared to the others. The BC Sealer samples retreated with chloroform had better results than those retreated without chloroform. Furthermore, patency could be re-established in only 14% of teeth in the BC Sealer without chloroform group. Conclusion: The results of this study demonstrate that the BC Sealer group had significantly more residual filling material than the AH Plus group regardless of whether or not both sealers were retreated with chloroform.

Buckling resistance, torque, and force generation during retreatment with D-RaCe, HyFlex Remover, and Mtwo retreatment files

  • Objectives: This study compared the buckling resistance of 3 nickel-titanium (NiTi) retreatment file systems and the torque/force generated during retreatment. Materials and Methods: The buckling resistance was compared among the D-RaCe (DR2), HyFlex Remover, and Mtwo R25/05 retreatment systems. J-shaped canals within resin blocks were prepared with ProTaper NEXT X3 and obturated by the single-cone technique with AH Plus. After 4 weeks, 4 mm of gutta-percha in the coronal aspect was removed with Gates-Glidden drills. Retreatment was then performed using DR1 (size 30, 10% taper) followed by DR2 (size 25, 4% taper), HyFlex Remover (size 30, 7% taper), or Mtrwo R25/05 (size 25, 5% taper) (15 specimens in each group). Further apical preparation was performed with WaveOne Gold Primary. The clockwise torque and upward force generated during retreatment were recorded. After retreatment, resin blocks were examined using stereomicroscopy, and the percentage of residual filling material in the canal area was calculated. Data were analyzed using 1-way analysis of variance with the Tukey test. Results: The HyFlex Remover files exhibited the greatest buckling resistance (p < 0.05), followed by the Mtwo R25/05. The HyFlex Remover and Mtwo R25/05 files generated the highest maximum clockwise torque and upward force, respectively (p < 0.05). The DR1 and DR2 files generated the least upward force and torque (p < 0.05). The percentage of residual filling material after retreatment was not significantly different between file systems (p > 0.05). Conclusions: NiTi retreatment instruments with higher buckling resistance generated greater clockwise torque and upward force.

열연화주입형 gutta percha와 resilon의 유변학적 특성 (Rheological characterization of thermoplasticized injectable gutta percha and resilon)

  • 연구목적: 본 연구의 목적은 열연화주입형 gutta percha와 resilon의 온도 변화에 따른 점탄성 변화를 관찰하고 조작성을 비교하기 위함이다. 연구 재료 및 방법: Obtura-II 시스템을 이용하여 세 종류의 gutta percha와 resilon을 $140^{\circ}C$$200^{\circ}C$로 가열한 후 사출 온도를 측정하였다. 점도계를 이용하여 온도 변화에 따른 재료들의 점탄성 특성(전단탄성계수, G'; 전단점성계수, G"; 손실 탄젠트, tan${\delta}$; 복소점도, ${\eta}^*$)을 관찰하였다. 점도계와 차동주사열측정기(DSC)로 재료들의 상전이 온도를 측정하였고 가압법으로 $60^{\circ}C$$40^{\circ}C$에서 재료들의 점조도를 비교하였다. 결과: 세 종의 gutta perchas는 온도에 따라 서로 다른 점탄성 특성을 나타냈다. $40-50^{\circ}C$에서 연화된 재료의 고체화 상변이가 일어났고, 점도계와 DSC로 측정된 상변이 시작 온도는 유사하였다. 상변이 시작 온도와 가압 시 점조도는 재료들마다 차이를 보였다(p < 0.05). Resilon은 gutta percha와 비슷한 유변학적 특성을 보였다. 결론: 열연화된 근관충전재는 냉각과정 동안 유변학적 특성의 변화를 나타냈고 재료들마다 서로 다른 점탄성 특성은 근관 내주입 시와 충전 시 서로 다른 조작성을 보임을 알 수 있다.

비외과적 근관치료의 임상적 성공에 영향을 미치는 예측 인자들의 평가 (Prognostic factors influencing clinical outcome of nonsurgical endodontic treatment)

  • 연구목적: 이 연구는 근관치료 전문의에 의해 시행된 비외과적 근관치료의 임상적 성공률을 전향적으로 평가하고, 치료 성공율과 관련된 환자요인과 치아요인의 영향력을 평가하는 것을 목적으로 하였다. 연구 재료 및 방법: 비외과적 근관치료가 이루어진 441개 치아 중 175개의 치아를 1-2년 후 임상적 검진과 방사선촬영을 하였다. 결과: 치근단 방사선 병소의 유무로 평가된 비외과적 근관치료의 성공율은 81.1% 였다. 치아요인 중 재근관치료, 괴사된 치수, 치수병변에서 유래된 치은의 부종 또는 sinus tract, 그리고 치료 전 치근단 병변의 존재는 이변수분석에서 치료성공에 부정적 영향을 미치는 것으로 나타났다 (p < 0.05). 환자요인, 치아요인과 근관충전의 길이를 포함한 단계적 로지스틱 회귀분석에서는 치수병변에서 유래된 치은의 문제 (odds ratio [OR]: 4.4; p = 0.005), 치료 전 치근단 병변의 존재 (OR: 3.6; p = 0.011), 그리고 치근단에서부터 1-2 mm 짧은 근관충전 (OR: 9.6; p = 0.012) 이 치료 실패와 관련된 주요한 요인으로 나타났다. 결론: 치료 전 치근단 병변의 존재뿐만 아니라 치수병변에서 유래된 치은의 부종 또는 sinus tract의 존재는 비외과적 근관치료의 실패에 영향을 줄 수 있는 것으로 나타났다.

Influence of the root canal filling technique on the success rate of primary endodontic treatments: a systematic review

  • Objectives: This study aimed to investigate the influence of different obturation techniques compared to cold lateral compaction on the success rate of primary non-surgical endodontic treatments. Materials and Methods: Systematic searches were performed for studies published up to May 17th, 2022 in MEDLINE/PubMed, Cochrane Library, Web of Science, Scopus, EMBASE, and Grey Literature Reports. Randomized clinical trials and nonrandomized (nonrandomized clinical trials, prospective or retrospective) studies that evaluated the success rate of primary non-surgical endodontic treatments obturated with the cold lateral compaction (control) and other obturation techniques were included. The revised Cochrane risk of bias tools for randomized trials (RoB 2) and nonrandomized studies of interventions (ROBINS-I) were used to evaluate the risk of bias. The Grading of Recommendations Assessment, Development, and Evaluation (GRADE) tool was used to evaluate the certainty of evidence. Results: Eleven studies (4 randomized clinical trials (RCTs), 4 prospective, and 3 retrospectives) were included. Two RCTs were classified as having some concerns risk of bias and 2 as a low risk of bias. Two nonrandomized studies were classified as having a critical risk of bias and 5 as having a moderate risk of bias. The GRADE analysis demonstrated a very low to moderate certainty of evidence. Conclusions: This systematic review generally evidenced no differences in the success rate of primary non-surgical endodontic treatments when the cold lateral compaction technique and other obturation techniques are performed. Further well-designed studies are still necessary.

치근단 역충전용 포틀랜드 시멘트 신복합재료의 물리적 성질 고찰 (Physical properties of novel composite using Portland cement for retro-filling material)

  • 연구목적: 이 연구의 목적은 새로운 근단 역충전 재료를 개발하기 위해 MTA, MTA와 AH-plus 혼합물 (AMTA), 그리고 실험적 개발재료인 Portland cement-Epoxy resin composite (EPPC)의 미세 누출 및 물리적인 성질을 비교하는 것이다. 연구 재료 및 방법: 발거 치아 49개를 근관 성형하고 gutta-percha와 sealer로 충전하였다. 각 치아의 치근을 근단부 3 mm에서 절단하고 3 mm 깊이의 역충전 와동을 형성하였다. 15개씩 무작위로 분류한 치근을 세 군으로 나누어 각각 MTA, AMTA, EPPC로 충전하였고 네 개의 치근은 대조군으로 사용하였다. 각 군의 재료로 역충전이 완료된 모든 실험군을 젖은거즈로 덮고 24시간 동안 경화시켰다. 경화 후 72시간 동안 1% methylene blue 염색액에 담근 후, 치근단을 수직 절단하여 사진을 촬영하고 미세 누출을 평가하였다. Vicat apparatus를 사용하여 실험군 별로 경화시간을 측정하였으며, aluminum step wedge를 사용한 디지털 방사선 사진을 촬영하여 각 군의 방사선 불투과도를 평가하였다. 미세누출과 경화시간에 대해 각 군간의 차이를 일원배치분산분석 및 Scheffe 사후 검증으로 유의수준 95%에서 평가하였다. 결과: AMTA와 EPPC는 MTA군에 비해 적은 미세누출량을 보였다 (p < 0.05). AMTA는 가장 높은 방사선 불투과도를 보였으며 개발 재료인 EPPC군은 5 mm aluminum 두께의 방사선 불투과도를 보였다. MTA가 가장 긴 경화시간을 나타낸반면 EPPC군은 다른 군에 비해 초기경화와 최종경화 모두 가장 짧은 경화시간을 나타냈다 (p < 0.05). 결론: 이 연구 조건하에서, 신개발 재료인 EPPC는 치근단 역충전 재료로서 기존의 MTA보다 양호한 밀폐능력과 방사선 불투과성 및 짧은 경화시간 등의 역충전 재료로서 적절한 물리적 성질을 가진 것으로 보인다.

여러 치과 와동 기저재용 재료들의 미세누출 및 압축강도 비교 (Comparison of Microleakage and Compressive Strength of Different Base Materials)

  • 이 연구는 깊은 와동에서 기저재로 사용되는 5개의 기저재용 재료를 대상으로 미세누출 및 압축강도 평가를 시행하였다. 미세누출 평가를 위해 발거된 영구 소구치 50개를 준비하여 베이스 재료에 따라 10개씩 군을 나누었다. 치아의 순면에 가로 5.0 mm, 세로 3.0 mm, 높이 3.0 mm 크기의 와동을 형성하였다. 형성된 와동에 1.0 mm 두께로 각 베이스 재료를 충전하였다. 이후 와동의 상방부를 composite resin으로 최종수복 시행하였으며, 시편을 2% 메틸렌블루 용액에 침적시킨 후 치아를 절삭하였고 실체현미경(× 30)을 이용해 미세누출 정도를 평가하였다. 압축강도 평가를 위해 각 재료 별로 5개씩의 원통형 시편을 제작하였다. 이후 만능시험기를 이용해 압축강도를 평가하였다. 미세누출 평가에서 Riva light cureTM가 가장 큰 미세누출을 보였으며, Well-Root PT와 Biodentine이 가장 적은 미세누출을 보였다. 압축강도는 모든 군이 베이스 재료로서 받아들일 만한 강도를 보였다. Fuji II LC가 가장 높은 압축강도를 보였으며 Well-Root PT가 가장 낮은 강도를 보였다.

The effect of individualization of fiberglass posts using bulk-fill resin-based composites on cementation: an in vitro study

  • Objectives: This study evaluated the bond strength of various fiberglass post cementation techniques using different resin-based composites. Materials and Methods: The roots from a total of 100 bovine incisors were randomly assigned to 5 treatment groups: G1, post + Scotchbond Multi-Purpose (SBMP) + RelyX ARC luting agent; G2, relined post (Filtek Z250) + SBMP + RelyX ARC; G3, individualized post (Filtek Z250) + SBMP; G4, individualized post (Filtek Bulk-Fill) + SBMP; G5, individualized post (Filtek Bulk-Fill Flow) + SBMP. The samples were subjected to the push-out (n = 10) and pull-out (n = 10) bond strength tests. Data from the push-out bond strength test were analyzed using 2-way analysis of variance (ANOVA) with the Bonferroni post hoc test, and data from the pull-out bond strength test were analyzed using 1-way ANOVA. Results: The data for push-out bond strength presented higher values for G2 and G5, mainly in the cervical and middle thirds, and the data from the apical third showed a lower mean push-out bond strength in all groups. No significant difference was noted for pull-out bond strength among all groups. The most frequent failure modes observed were adhesive failure between dentine and resin and mixed failure. Conclusions: Fiberglass post cementation using restorative and flowable bulk-fill composites with the individualization technique may be a promising alternative to existing methods of post cementation.

Compomer와 Ketac Silver로 성견 상악 이개부 병소 충전시 조직반응에 미치는 영향 (Effects on the tissue reaction using compomer & Ketac Silver in the maxillary furcation in the beagle dogs)

  • Procedures for treatment of molar furcation invasion defects range from open flap debridement, apically repositioned flap surgery, hemisection, tunneling or extraction, to regenerative therapies using bone grafting or guided tissue regenerative therapy, or a combination of both. Several clinical evaluations using regenerative techniques have reported the potential for osseous repair of treated furcation invasions. Regenerative treatment of maxillary molars are more difficult due to the multiple root anatomy and multiple furcation entrances therefore, purpose of this study was to evaluated histologically compomer and Ketac Silver as a barrier in the treatment of a bi-furcated maxillary premolar. Five adult beagle dogs were used in this experiment. With intrasulcular and crestal incision, mucoperiostcal flap was elevated. Following decortication with 1/2 high speed round bur, furcation defect was made on maxillary premolar. 2 month later one premolar was filled with compomer and the other premolar was filled with Ketac Silver. After 4, 8 weeks, the animals were sacrificed by vascular perfusion. Tissue block was excised including the tooth and prepared for light microscope with H-E staining. Results were as follows. 1. Compomer & Ketac Silver restoration were encapsulated fine connective tissue. 2. In 4 weeks, compomer & Ketac Silver restoration slightly infiltrated inflammatory cells but not disturb the new bone or new cementum formation. 3. In 8 weeks, compomer & Ketac Silver restoration were less infiltrated iflammatory cell and encapsulated fine connective tissue. 4. Therefore, compomer & Ketac Silver filling to the grade III maxillary furcations with multiple root anatomy and multiple furcation entrances is possible clinical method and this technique is useful method for maxillary furcation involvement but it is thought that periodic maintenance should be needed

Intentional Replantation에 의한 상악 제 1 소구치 치료의 증례보고 (A CASE REPORT ON INTENTIONAL REPLANTATION OF MAXILLARY FIRST BICUSPID)

  • Intentional replantation of the posterior teeth has been performed as a last resort except extraction when it is impossible to perform the conventional endodontic or surgical endodontic treatment. Many studies have done with the prognosis, and root resorption, of which the responsibility might have been injury of periodontal ligament and remaining periodontal lesion, proved to be major cause of failure. Intentional replantation, however, can exclude anatomic difficulty, risk of nerve injury, and maxillary sinus trauma that surgical access can raise, especially in posterior teeth, and it is reported that success rate is not less than surgical endodontics, independent of replantation of traumatically avulsed teeth. The success of intentional replantation can be evaluated with, clinically, reduction of pocket depth, pain and mobility(physical mobility), and, radiographically, hard tissue formation, reduction of periapical radiolucency. In this case report, for the maxillary left first bicuspid with dens evaginatus that had periapical lesion, painfulness, mobility and showed no reduction of mobility after even extirpation and a number of irrigation, extraction followed by extraoral root filling with calcium hydroxide and replantation was performed. Based on many studies, it is concluded that hard tissue formation capacity of calcium hydroxide, preceded extirpation before extraction, and careful extraction without injuring periodontal ligament affected successful results of this case.
