Objectives: This study investigated the nanoleakage of root canal obturations using calcium silicate-based sealer according to different drying methods. Materials and Methods: Fifty-two extracted mandibular premolars with a single root canal and straight root were selected for this study. After canal preparation with a nickel-titanium rotary file system, the specimens were randomly divided into 4 groups according to canal drying methods (1: complete drying, 2: blot drying/distilled water, 3: blot drying/NaOCl, 4: aspiration only). The root canals were obturated using a single-cone filling technique with a calcium silicate-based sealer. Nanoleakage was evaluated using a nanoflow device after 24 hours, 1 week, and 1 month. Data were collected twice per second at the nanoscale and measured in nanoliters per second. Data were statistically analyzed using the Kruskal-Wallis and Mann-Whitney U-tests (p < 0.05). Results: The mean flow rate measured after 24 hours showed the highest value among the time periods in all groups. However, the difference in the flow rate between 1 week and 1 month was not significant. The mean flow rate of the complete drying group was the highest at all time points. After 1 month, the mean flow rate in the blot drying group and the aspiration group was not significantly different. Conclusions: Within the limitations of this study, the canal drying method had a significant effect on leakage and sealing ability in root canal obturations using a calcium silicate-based sealer. Thus, a proper drying procedure is critical in endodontic treatment.
Background: A transforaminal epidural steroid injection has been used for the treatment of radicular arm or leg pain, which has the advantage of ventral epidural spreading compared to either an interlaminar or caudal approach. However, several factors are known to affect the epidural spread pattern of contrast dye; therefore, the injected medication can not be delivered to the target site. The objective of our study was to observe any differences in the contrast flow patterns according to several factors. Methods: A total of 34 epidurographies in 29 patients, who underwent fluoroscopically guided transforaminal epidural steroid injections, were evaluated. After confirming the location of the needle tip within the anterior superior aspect of the neural foramen in the lateral view, and at the 6 o'clock position to the pedicle in the anteroposterior view, 2 ml of contrast dye was injected. The contrast flow patterns of ventral, ventral and dorsal, and dorsal epidural filling were analyzed, according to age, gender, magnetic resonance imaging finding and history of previous back surgery. Results: Ventral contrast flow occurred in 30 out of the 34 epidurographies (88%). Both ventral and dorsal contrast flow patterns were observed 13 out of 16 (81%) patients in the older age group. Also, both ventral and dorsal contrast flow patterns were observed in 16 out of 18 (88%) patients with spinal stenosis. Conclusions: Transforaminal epidural steroid injections, performed uner fluoroscopy, provide excellent nerve root filling and ventral epidural spreading. Patients with spinal stenosis or an old age have both ventral and dorsal epidural spreading patterns.
The purpose of this study is to evaluate the two warm gutta-percha filling techniques by measuring the weight changes of resin blocks before and after canal filling in ribbon shaped canal. Simulated ribbon shaped root canals in 30 transparent resin blocks were instrumented to #40 using .06 taper Profile. 15 resin blocks were obturated with gutta-percha using cold lateral condensation. Warm lateral condensation using the Endotec II was then accomplished on the same 15 blocks. Another 15 resin blocks were obturated using the System B. All canals were obturated without sealer. The resin blocks were weighed after canal preparation and after each subsequent obturation, and then weight changes of the resin blocks were calculated The results were as follows. 1. Warm lateral condensation using Endotec II and continuous wave of condensation using System B produced a denser obturation of gutta-percha compared with conventional cold lateral condensation (p<0.01). 2. There was no significant difference between warm lateral condensation and continuous wave condensation. In conclusion, the warm gutta-percha condensation techniques like warm lateral condensation and continuous wave condensation can be expected to bring favorable canal obturation results in ribbon shaped canals.
In this paper, we propose a signal propagation modeling technique for generating a positioning fingerprint DB based on Long Term Evolution (LTE) signals. When a DB is created based on the location-based signal information collected in an urban area, gaps in the DB due to uncollected areas occur. The spatial interpolation method for filling the gaps has limitations. In addition, the existing gap filling technique through signal propagation modeling does not reflect the signal attenuation characteristics according to directions occurring in urban areas by considering only the signal attenuation characteristics according to distance. To solve this problem, this paper proposes a Deep Neural Network (DNN)-based signal propagation functionalization technique that considers distance and direction together. To verify the performance of this technique, an experiment was conducted in Seocho-gu, Seoul. Based on the acquired signals, signal propagation characteristics were modeled for each method, and Root Mean Squared Errors (RMSE) was calculated using the verification data to perform comparative analysis. As a result, it was shown that the proposed technique is improved by about 4.284 dBm compared to the existing signal propagation model. Through this, it can be confirmed that the DNN-based signal propagation model proposed in this paper is excellent in performance, and it is expected that the positioning performance will be improved based on the fingerprint DB generated through it.
The purpose of this study was to investigated the effect of root planing and decalcified freeze dried allografts on the resorption of transplanted roots and the healing of preveously diseased recipient extraction sockets. The experimental chronic periodontitis was induced by elastic ligatures on the 2nd and 3rd mandibular premolars of 4 adult dogs, and after 8 weeks, crowns were removed and the teeth extracted. The extracted roots were split in half along the long-axis, and the extednt of plaque exposure was morked on the root surfaces with burs. The roots were either root-planed(Test group), or left uninstrumented(Control group), and transplanted in the extraction sockets with decalcified freeze-dried allografts filling the void. The flaps were sutured to cover the sockets completely. The animals were sacrificed after 12 weeks of healing, and the specimens were examined histologically. The results were as follows : 1. No signs of inflammation or disease activity were observed in either groups. 2. Replacement root resorption was observed in both groups. 3. More connective tissue attachments and less ankylosis were observed in the test groups compared to the control. 4. The unresorbed remains of DFDB particles were observed in both groups. 5. DFDB particles in the apical portion of the alveolar sockets were encased in newly-formed bone, while those in the coronal areas were seen encapsulated with connective tissue. 6. No significant difference was found between root-planed and uninstrumented roots relative to the healing and the bone fromation in the recipient extraction sockdets. From the present study, there seemed to be no significant benefits in root planing the transplanted roots or grafting the sockets with DFDB in order to curve the replacement resorption, although the root-planed roots showed more connective tissue attachments. There was also no significant benefits in root transplantation and DFDB for and enhanced healing and bone formation in alveolar extraction sockets.
Background/Purpose: The purpose of this study was to evaluate the effect of Nd:YAG irradiation on adherence of retrograde filling materials (mineral trioxide aggregate [MTA] and Super-EBA) by micro-computed tomography (CT) measurement and to observe the dentinal surface after irradiation by scanning electron microscopy (SEM). Materials and methods: Forty retrofilling models using extracted human teeth were divided into four groups according to the material and method used: ProRoot MTA (MTA group), Super-EBA (EBA group), MTA with Nd:YAG laser irradiation (LMTA group), and Super-EBA with Nd:YAG laser irradiation (LEBA group). All specimens were stored in 100% humidity for 24 hours until micro-CT was performed. The gap volume of the tooth/material interface was measured using the CTAn program. In six samples, the laser-irradiated dentin surface was observed using SEM. Results: The mean percent difference in gap volume was not statistically significant between the Nd:YAG laser-irradiated groups and non-irradiated in both materials(P > 0.05). The gap volume in the MTA group was significantly lower than that in the EBA group (P < 0.05). Examination of the non-irradiated specimens by SEM showed patent dentinal tubules. In contrast, alterations in the texture of the dentin surface and obliteration of the dentinal tubules were evident in the Nd:YAG laser-irradiated specimens. Conclusion: In this study, changes in the dentinal surface after Nd:YAG irradiation did not affect adherence between the apical filling material and the dentin wall.
The present report presents a case of dens invaginatus (DI) in a patient with 4 maxillary incisors. A 24-year-old female complained of swelling of the maxillary left anterior region and discoloration of the maxillary left anterior tooth. The maxillary left lateral incisor (tooth #22) showed pulp necrosis and a chronic apical abscess, and a periapical X-ray demonstrated DI on bilateral maxillary central and lateral incisors. All teeth responded to a vitality test, except tooth #22. The anatomic form of tooth #22 was similar to that of tooth #12, and both teeth had lingual pits. In addition, panoramic and periapical X-rays demonstrated root canal calcification, such as pulp stones, in the maxillary canines, first and second premolars, and the mandibular incisors, canines, and first premolars bilaterally. The patient underwent root canal treatment of tooth #22 and non-vital tooth bleaching. After a temporary filling material was removed, the invaginated mass was removed using ultrasonic tips under an operating microscope. The working length was established, and the root canal was enlarged up to #50 apical size and obturated with gutta-percha and AH 26 sealer using the continuous wave of condensation technique. Finally, non-vital bleaching was performed, and the access cavity was filled with composite resin.
This study investigates, analyzes, and summarizes Dansplantation techniques and methods through practical methodology centering on fieldwork in order to present effective planting methods for large trees that have important significance. The conclusions are as follows : 1. The transplantation process of a large tree generally consists of the stages of digging up a tree, manufacturing a carrier frame, loading the tee on a vehicle, transporting, transplanting the tree, installing a strut and maintaining and managing the new transplant. In addition, planting a tree on a mounted place includes the primary procedures of trimming out the root, and preparing for transplanting the tree on a mounted place, as well as the secondary work of trimming out the root, transplanting a tree on a mounted place, maintenance and management. 2. In order to decide on a transplantation method for a large-sized tree, a structure calculation has to be performed first. That is, one must calculate the weight of the tree and the allowable stress of the strut (H-beam, etc.) fhst and then decide on the upper method through computer modeling based upon this structural calculation. 3. As a result of the analysis of a transplanted tree using the life soil method, it was confirmed that large quantities of feeder roots had developed around the root within a short time after the transplantation. The life soil method has proven to be very effective for transplantation of large-sized trees. 4. As for the production method of an H-beam strut frame, it was found that the manufacturing process and disassembly process were simple and proper; therefore, the H-beam frame is an appropriate structure to be used in the transplantation of large trees. 5. The concavo-convex method, which consists of filling the life soil in the concavo-convex area around the root, was found to be a method that promotes the growth of feeder roots within a short period of time and saves the supply of water at the same time.
Objectives: This study evaluated the solubility, dimensional alteration, pH, electrical conductivity, and radiopacity of root perforation sealer materials. Materials and Methods: For the pH test, the samples were immersed in distilled water for different periods of time. Then, the samples were retained in plastic recipients, and the electrical conductivity of the solution was measured. The solubility, dimensional alteration, and radiopacity properties were evaluated according to Specification No. 57 of the American National Standards Institute/American Dental Association (ANSI/ADA). Statistical analyses were carried out using analysis of variance (ANOVA) and Tukey's test at a significance level of 5%. When the sample distribution was not normal, a nonparametric ANOVA was performed with a Kruskal-Wallis test (${\alpha}$ = 0.05). Results: The results showed that white structural Portland cement (PC) had the highest solubility, while mineral trioxide aggregate (MTA)-based cements, ProRoot MTA (Dentsply-Tulsa Dental) and MTA BIO ($\hat{A}$ngelus Ind. Prod.), had the lowest values. MTA BIO showed the lowest dimensional alteration values and white PC presented the highest values. No differences among the tested materials were observed in the the pH and electrical conductivity analyses. Only the MTA-based cements met the ANSI/ADA recommendations regarding radiopacity, overcoming the three steps of the aluminum step wedge. Conclusions: On the basis of these results, we concluded that the values of solubility and dimensional alteration of the materials were in accordance with the ANSI/ADA specifications. PCs did not fulfill the ANSI/ADA requirements regarding radiopacity. No differences were observed among the materials with respect to the pH and electrical conductivity analyses.
In endodontic treatment, calcium hydroxide has been used as intracanal medicament. Although calcium hydroxide should be removed thoroughly before permanent root canal filling, no effective method for its removal has been reported. Because of irregularity of root canal walls, root curvatures and anatomic variations, it is insufficient to remove calcium hydroxide from the canal wall only by mechanical instrumentation Considering the chemical effects of irrigants on calcium hydroxide, $Ca^{++}$ dissolving effect from two calcium hydroxide products is investigated, using dis- tilled water, NaOCl, citric acid and EDTA. Vitapex$^{(R)}$ 0.1g and calcium hydroxide 0.03g were dissolved in distilled water, 5% NaOCl, 50% citric acid and 17% EDTA respectively, at 1, 3, 5, 10min. time interval. The solution was filtered using filter paper(pore size $5{\mu}m$) and $Ca^{++}$ concentration was determined by ion chromatography. The result were as follows : 1. Distilled water, NaOCl, citric acid and EDTA abstracted more $Ca^{++}$ from calcium hydroxide than Vitapex$^{(R)}$ except NaOCl 1, 5, 10 time interval. 2. EDTA and citric acid abstracted more $Ca^{++}$ from Vitapex$^{(R)}$ and calcium hydroxide than distilled water or NaOCl. The overall result support the view that water-based calcium hydroxide product is easily removed than oil-based calcium hydroxide product and EDTA, citric acid are more effective in $Ca^{++}$ elution than NaOCl or distilled water.
