PURPOSE. Porcelain fused to metal (PFM) crowns provide the best treatment option for teeth that have a large or defective restoration. More than 20% of teeth with PFM crowns or bridges require non-surgical root canal treatment (NSRCT). This may be due to the effect of restorative procedures and the possible leakage of bacteria and or their by-products, which leads to the demise of the tooth pulp. Thus, this study was planned to compare the ability of the restorative materials to seal perforated PFM specimens. MATERIALS AND METHODS. The study evaluates the ability of amalgam, composite or compomer restorative materials to close perforated PFM specimen's in-vitro. Ninety PFM specimens were constructed using Ni-Cr alloys and feldspathic porcelain, and then they were divided into 3 groups: amalgam (A), composite + Exite adhesive bond (B) and compomer + Syntac adhesive bond (C). All the PFM samples were embedded in an acrylic block to provide complete sealing of the hole from the bottom side. After the aging period, each group was further divided into 3 equal subgroups according to the thermocycling period (one week for 70 cycles, one month for 300 cycles and three months for 900 cycles). Each subgroup was put into containers containing dye (Pelikan INK), one maintained at $5^{\circ}C$ and the other at $55^{\circ}C$, each cycle for 30 sec time. The data obtained was analyzed by SPSS, 2006 using one way ANOVA test and student t-test and significant difference level at (P<.01). RESULTS. The depth of dye penetration was measured at the interfaces of PFM and filling materials using Co-ordinate Vernier Microscope. The lowest levels of the dye penetration for the three groups, as well as subgroups were during the first week. The values of dye leakage had significantly increased by time intervals in subgroups A and C. CONCLUSION. It was seen that amalgam showed higher leakage than composite while compomer showed the lowest level of leakage.

Talon cusp is cusp-like structure that develops additionally in crown of anterior tooth. And it is one of dental anomalies. Talon cusp can cause various diagnostic, functional, aesthetic problems depending on the size and configuration of cusp. there is a tendency for caries to occur in the developmental grooves, and advanced attrition, periodontal problems, irritation the tongue and temporomandibular pain, occlusal interference and displacement of affected tooth may result. Therefore, early diagnosis and appropriate treatment of each case is important so that it minimize local problems, such as caries, periodontal diseases, and malocclusion. These cases which are in permanent and primary anterior teeth are about gradual reduction forming reparative dentine and complete reduction of talon cusp and root canal therapy which is an alternative and effective form of treatment when gradual reduction of talon cusp may not be possible.

Dens invaginatus is a malformation of tooth resulting from an infolding of the enamel epithelium during tooth development. This malformation shows a broad spectrum of morphologic variations. This invagination frequently allows the entry of irritants and microorganism, which usually lead to necrosis of the adjacent pulp tissue and then to periapical or periodontal abscess. Root canal treatment of such tooth is often difficult because of the un usual form and complicated pulpal space. This article reports 2 cases of dens invaginatus in maxillary lateral incisors. The first case was successfully treated with $Ca(OH)_2$. In the second case, involved tooth was extracted and this extracted tooth was observed using the micro-computed tomography.

X-linked dominant hypophosphataemic rickets(XLHR) is a hereditary disease generally transmitted as an X-linked dominant trait. Oral manifestations include poorly mineralized dentin, enlarged pulp chambers and root canals, early tooth loss. Also, spontaneous dental abscesses could occur in the absence of a history of trauma or caries. This report describes the clinical finding and dental treatment of a child, who came to the hospital for treatment caries but was refered to pediatrics because the child showed clinically short of status, bow-leg and radiographically enlarged pulp chamber and canal, there as diagnosed as XLHR. After the diagnosis, 13 years follow-up and treatment were performed.

Garre's osteomyelitis is a unique form of osteomyelitis characterized rediographically by localized thickening of the periosteum and deposition of laminated subperiosteal bone. The most common inciting factor is a mandibular infection in permanent first molar with necrotic pulp. This disease occurs primarily in children and to date in all instances it has occured only in mandible. It usually results in hard swelling over the jaws, producing facial asymmetry with little or no pain. The overlying skin is normal but can occasionally be inflammed mostly when pain is present. Palpation reveals a usually smooth, bone-hard lesion which feel like an inherent part of the mandible. Unlike other forms of osteomyelitis, there is no marked increase in fever, white bloods cell count, sedimentation rate or alkaline phosphatase value. The treatment of Garre's osteomyelitis usually consist of elimination of the sourses of infection, i.e., either extration of an offending infected teeth or root canal therapy. This treatment almost always results in resolution of the Garre's osteomyelitis. Resistant cases have involved secondary surgery, i.e., decortication and sequestrectomy. This report presents three cases of Garre's osteomyelitis resolved by endodontic treatment. Cliniqtl examination revealed swelling on the face with no tenderness. Periapical radiograph showed deep caries lesion extending into pulp chamber and periapical radiolucency. Occlusal radiograph showed an enlargement of bone and stretching the periosteum. A clinical diagnosis of the Garre's osteomyelitis was made. Endodontic treatment was accomplished with conventional method and restored facial symmetry. Long-term check-ups are necessary to evaluate the results of endodontic treatment.

Whereas a somatic pain notifies tissue damage, a neuropathic pain presents disorder of the nerve itself. The causes of neuropathic pains are trauma, infection, chronic irritation by adjacent tissue and so on. The iatrogenic trauma or infection also causes traumatic neuropathy, which may exert a bad influence on doctor-patient relationship. Some of related dental treatments are implantation (directly or indirectly through heating), root canal treatment, teeth extraction, block anesthesia, mandibular surgery. If inappropriate management is performed after nerve trauma, there will be many chances to develop chronic neuropathy for the patient. It is important that the sign of nerve trauma have to be caught by the practitioner as soon as possible and treated properly.
The purpose of this study was to evaluate a rotational stability of endodontic electronic motors by comparing the changes of rotational speed, depending on the number of usages and with/without static load. Twelve new endodontic electronic motors were used in this study. Non contact type digital tachometer was used for measuring the rotational speed of handpiece. True RMS Multimeter was used for measuring the voltages and the electric currents. All measurements were recorded every 10 seconds during 10 minutes and repeated 9 times. Five repetition was done per each electronic motor. To statistical analysis, student t-test, repeated measures and Scheffe's post-hoc tests were performed. In the same motor group, there was no significant difference in all measurements. In all groups, there was no significant difference in the amount of rotational speed changes depending on the number of usages and with/without static load. In the limitation of this study, the results showed that all kinds of endodontic electronic motors in this study had an established rotational stability. Therefore they could be safely used in root canal treatment with a reliable maintenance of rotational speed, regardless of the number of usages and with/without load.

The radicular cyst is the most common odontogenic cyst which is caused by pulpal inflammation, pulp death, and secondary to trauma or dental caries. Usually, the radicular cyst is asymptomatic, but a secondary inflammation can cause pain, swelling and redness. Getting larger, the radicular cyst can cause facial asymmetry and paresthesia by pressure on nerves. It requires conservative endodontic treatment or surgical approach. When the size of cyst is large or invasion of the adjacent tissue is not expected, cyst enucleation is carried out. And most of the case can be completely cured and shows low recurrence. In these radicular cysts cases, by cyst enucleation or apicoectomy after root canal treatment simultaneously, the infected teeth can be preserved successfully.

It is common for clinicians to encounter endodontically tl·treated teeth that contain posts within their roots. If endodontic treatment is failed, these posts must be removed to facilitate successful nonsurgical retreatment. There have been many techniques such as ultrasonic instrument, Ruddle post removal system, Eggler post remover and Masserann kit developed to facilitate removal of posts from the root canal space. But these methods may be disadvantageous because long length of time required for post removal and fracture of post or teeth. In now days new post removal technique using ATD automatic bridge remover was introduced. Advantages of this method are simple and short time consuming compare to others. This article served as a successful case report of post removal using ATD automatic bridge remover.
The purpose of this study was to determine the incidence and the degree of interappointment pain and post obturation pain associated with the pretreated clinical factors or conditions, and to examine the correlation between the success or failure and the pretreated clinical factors or conditions and postoperative pain. The author experienced the conventional root canal therapy in one hundred fifty-one teeth after recording the following clinical factors or conditions - sex, age, pulp vitality status, presence or absence of periapical rarefaction, single or multirooted teeth, pretreatment pain, and investigated the pain during and after treatment for seven days. After six months all the cases were re-evaluated through the clinical sign, symptom, and the recalled radiograph. The following results were obtained; 1. Of the 151 teeth (73.5%) had no interappointment pain, 31 teeth (20.5%) slight pain; and 9 teeth (6.0%) moderate to severe pain. 2. The presence of the pretreatment pain significantly increased the incidence and degree of interappointment, and there were no significant relationship between interappointment pain and other clinical factors or conditions. 3. Of the 151 teeth, 142 teeth (4.0%) experienced no post obturation pain, 3 teeth (2.0%) slight pain, 6 teeth (4.0%) moderate to severe pain. 4. There were no statistical correlation between postobturation pain and any of the clinical factor or conditions. 5. 141 teeth (3.4%) of 151 teeth were evaluated as success in this study, and success rate was decreased significantly in the cases of periapical rarefaction before treatment and postobturation pain. But there were no significant relationship between success or failure and other clinical factors or conditions.
