Encryption-based Image Steganography Technique for Secure Medical Image Transmission During the COVID-19 Pandemic

  • COVID-19 poses a major risk to global health, highlighting the importance of faster and proper diagnosis. To handle the rise in the number of patients and eliminate redundant tests, healthcare information exchange and medical data are transmitted between healthcare centres. Medical data sharing helps speed up patient treatment; consequently, exchanging healthcare data is the requirement of the present era. Since healthcare professionals share data through the internet, security remains a critical challenge, which needs to be addressed. During the COVID-19 pandemic, computed tomography (CT) and X-ray images play a vital part in the diagnosis process, constituting information that needs to be shared among hospitals. Encryption and image steganography techniques can be employed to achieve secure data transmission of COVID-19 images. This study presents a new encryption with the image steganography model for secure data transmission (EIS-SDT) for COVID-19 diagnosis. The EIS-SDT model uses a multilevel discrete wavelet transform for image decomposition and Manta Ray Foraging Optimization algorithm for optimal pixel selection. The EIS-SDT method uses a double logistic chaotic map (DLCM) is employed for secret image encryption. The application of the DLCM-based encryption procedure provides an additional level of security to the image steganography technique. An extensive simulation results analysis ensures the effective performance of the EIS-SDT model and the results are investigated under several evaluation parameters. The outcome indicates that the EIS-SDT model has outperformed the existing methods considerably.

  • In the 4th Industrial Revolution, the healthcare industry is undergoing a paradigm shift from post-care and management systems based on diagnosis and treatment to disease prevention and management based on personal precision medicine. To optimize medical services for individual patients, an open ecosystem for the healthcare industry that allows the exchange and utilization of personal health records (PHRs) is required. However, under the current system of hospital-centered data management, it is difficult to implement the linking and sharing of PHRs in practice. To address this problem, in this study, we present the design and implementation of a patient-centered PHR platform using blockchain technology. This platform achieved transparency and reliability in information management by eliminating the risk of leakage and tampering/altering personal information, which could occur when using a PHR. In addition, the patient-consent system was applied to a PHR; thus, the patient acted as the user with ownership. The proposed blockchain-based PHR platform enables the integration of personal medical information with scattered distribution across multiple hospitals, and allows patients to freely use their health records in their daily lives and emergencies. The proposed platform is expected to serve as a stepping stone for patient-centered healthcare data management and utilization.

Clinical Effect of Injinho-tang on Hyperbilirubinemia in Hepatobiliary Disorders: A Systematic Review (간담도 질환에서의 고빌리루빈혈증에 대한 인진호탕의 임상 효과 : 체계적 고찰)

  • Purpose: This systematic review was conducted to evaluate the clinical effects of Injinho-tang on hyperbilirubinemia in hepatobiliary disorders. Methods: We searched for randomized controlled clinical trials that had administered Injinho-tang as an intervention in the following medical databases: Public/Publisher MEDLINE (PubMed), Excerpta Medica dataBASE (EMBASE), Cochrane library, Research Information Sharing Service (RISS), ScienceON, Oriental Medicine Advanced Searching Integrated System (OASIS), and China National Knowledge Infrastructure (CNKI). Among the retrieved studies, only trials that met the inclusion criteria were selected, and serum total bilirubin values were extracted and analyzed from the finally selected trials. Results: The serum total bilirubin values of 1,302 patients with various hepatobiliary diseases were synthesized through a meta-analysis, which confirmed a decrease in serum total bilirubin of 21.03 𝜇mol/L (95% CI -29.58~-12.49, p<0.01) in the group administered with Injinho-tang compared with the control group. Conclusions: Injinho-tang is effective in alleviating hyperbilirubinemia in hepatobiliary diseases when administered with conventional treatment. However, the potential risk of bias, high heterogeneity among the included trials, and differences in herbal composition are limitations of the results of this meta-analysis.

Identification of Critical Success Factors (CSFs) for Public-Private Partnerships Across Infrastructure Sectors

  • Public-private partnerships (PPP) projects are becoming popular in both developed and developing countries due to their ability to access new financing sources and transfer certain project risks to the private sector. PPP has been an active research area where the concept of Critical Success Factors (CSF) is often discussed by researchers. This study aims to identify the CSFs for various PPP infrastructure projects that have been explored in previous CSF studies. This article reviewed the literature about CSF in PPP projects from the years 2002 to 2021, compared the findings of studies regarding the identified CSFs, and consolidated the CSFs that can be applied to various PPP infrastructure projects. The results showed that dominant research focused on general infrastructure, where CSFs can be applied to all infrastructure sectors rather than any specific sector. The most identified CSFs from the study are favorable and efficient legal frameworks, appropriate risk allocation and sharing, a robust and reliable private consortium, a competitive and transparent procurement process, and political support and stability. The findings from the study can provide an overview of CSFs that are relevant to specific PPP infrastructure sectors like building infrastructure, transportation, water, etc. as well as for general infrastructure. In addition, the results can also be used for further empirical analysis.

Communication with Family Members about Positive BRCA1/2 Genetic Test Results in Korean Hereditary Breast Cancer Families (한국인 유전성 유방암 가계에서 BRCA1/2 유전자 돌연변이 사실에 대한 가족과의 의사소통 실태)

  • Purpose: Sharing genetic information with family members is important for cancer awareness and prevention. The purpose of this study is to examine disclosure patterns of positive BRCA genetic test results to patients' relatives. Materials and Methods: A total of 106 probands who had positive BRCA genetic test results from the Korean Hereditary Breast Cancer Study participated in our study. Subjects were asked whether they had disclosed their genetic test results to first-, second-, and third-degree relatives. Univariate and multivariate analyses were used to identify factors associated with positive result sharing with close and distant relatives. Results: In total, 99 respondents (93.4%) informed at least one at-risk relative of the test result, and they all reported that they had disclosed their genetic test result to a first-degree relative. Communication of test results to other relatives occurred significantly less often, with only 31 of 99 subjects (31.3%) sharing their results with second- or third-degree relatives. In the results of univariate analyses, disclosure of genetic test results to more distant relatives was associated with marital status and months since post-test counseling. The reasons for communication were to provide information about the BRCArelated cancer risk and to recommend the genetic test. Conclusion: Most individuals with the BRCA mutation share their test results with first-degree family members; however, these results reach more distant relatives significantly less often. Therefore, it is necessary to encourage patients' communication with extended family members through systematic genetic counseling.

A Study on the Plans for Effective Use of Public Data: From the Perspectives of Benefit, Opportunity, Cost, and Risk (인터넷기반 공공데이터 활용방안 연구: 혜택, 기회, 비용, 그리고 위험요소 관점에서)

  • With the request for the advent of new engine toward economic growth, the issue regarding public-owned data disclosure has been increasing. The Korean governments are forced to open public-owned data and to utilize them in solving the various social problems and in promoting the welfare for the people. In contrast, due to the distrust of the effectiveness for the policy, many public owned organizations hesitate to open the public-owned data. However, in spite of communication gap between the government and public organizations, Ministry of Government Administration and National Information Society Agency recently planned to accelerate the information disclosure. The study aims to analyze the perception of the public organization for public data utilization and to provide proper recommendations. This research identified mutual weights that the organization recognize in opening and sharing the public data, based on benefit, opportunity, cost, and risk. ANP decision making tool and BOCR model were applied to the analyses. The results show that there are significant differences in perceiving risk and opportunity elements between the government and public organizations. Finally, the study proposed the ideal alternatives based on four elements. The study will hopefully provide the guideline to the public organizations, and assist the related authorities with the information disclosure policy in coming up with the relevant regulations.

High Seroprevalence of Helicobacter pylori Infection in Korean Adolescent Athletes (한국의 청소년 운동선수들에서 Helicobacter pylori IgG의 높은 혈청 양성률)

  • Purpose: Poor living conditions such as overcrowding and sharing a bed are reported to be significant risk factors for H. pylori infection throughout childhood. We investigated the prevalence of Helicobacter pylori infection in Korean athletes who are obliged to live together in training camps over six months a year. Methods: Blood sampling and surveying through questionnaire were performed on 440 regular high school students (228 boys and 212 girls) and 220 athletes (148 boys and 72 girls) of a physical education high school. We measured serum IgG Ab to H. pylori in order to compare the prevalence of H. pylori infection between normal adolescents and athletes. In addition, we conducted a nutritional analysis and questionnaire survey for socioeconomic status in order to compare other risk factors that might influence H. pylori infection between groups. Results: The prevalence (43.2%) of H. pylori infection in athletes was higher than that (22.7%) in the controls. No significant differences in the nutritional factors or socioeconomic status, such as Hollingshead index, type of house, and crowding index, were found between the groups. Multivariate analysis showed that crowding and the difference in school type were the significant predictors of H. pylori seropositivity after controlling for dietary and socioeconomic factors. Conclusion: Living together in training camps might be a significant risk factor for H. pylori infection in athletes.

  • The police, as an advance guard for performing governmental functions, should conduct their duties to protect lives, physical bodies, and properties in the nation against various kinds of crimes, prevent from a public risk by detecting a factor that can menace public safeguard and public order and suppress it when a risk factor occurs. Practically it is impossible to protect lives, physical bodies, and properties in the nationprovided that social stability is not taken as security. The roles of the criminal investigation police that take a countermeasure to prevent a risk in prior by detecting hindrance factor of social stability and process any occurred crime in conformity with pertinent laws could control a successful performance in each of the police activities themselves. The study assumes that the organizational capabilities of the criminal investigation police exercises a significant influence upon organizational concentration. In spite of the fact that it is natural that the organizational capabilities of the criminal investigation police exercises a significant influence upon organizational concentration, until now it is very deficient in studies of this field. It is a viewpoint that an organizational concentration would be improved when a capability of the organizational staff will be improved, they would have a targeting capability for sharing organizational goals and vision and a systematic capability would be supported by the dimension of the criminal investigation police. The study was aimed to deduce a developmental method for the criminal investigation police by conducting an analysis on the impact of the organizational capabilities of the criminal investigation police upon organizational concentration that it suggest method that the criminal investigation police will make a contribution for maintaining a social order.

Aviation Safety Regulation and ICAO's Response to Emerging Issues (항공안전규제와 새로운 이슈에 대한 ICAO의 대응)

  • Aviation safety is the stage in which the risk of harm to persons or of property damage is reduced to, and maintained at or below, an acceptable level through a continuing process of hazard identification and risk management. Many accidents and incidents have been taking place since 2014, while there had been relatively safer skies before 2014. International civil aviation community has been exerting great efforts to deal with these emerging issues, thus enhancing and ensuring safety throughout the world over the years. The Preamble of the Chicago Convention emphasizes safety and order of international air transport, and so many Articles in the Convention are related to the safety. Furthermore, most of the Annexes to the Convention are International Standards and Recommended Practices pertaining to the safety. In particular, Annex 19, which was promulgated in Nov. 2013, dealing with safety management system. ICAO, as law-making body, has Air Navigation Commission, Council, Assembly to deliberate and make decisions regarding safety issues. It is also implementing USOAP and USAP to supervise safety functions of member States. After MH 370 disappeared in 2014, ICAO is developing Global Tracking System whereby there should be no loophole in tracking the location of aircraft anywhere in world with the information provided by many stakeholders concerned. MH 17 accident drove ICAO to install web-based repository where information relating to the operation in conflict zones is provided and shared. In addition, ICAO has been initiating various solutions to emerging issues such as ebola outbreak and operation under extreme meteorological conditions. Considering the necessity of protection and sharing of safety data and information to enhance safety level, ICAO is now suggesting enhanced provisions to do so, and getting feedback from member States. It has been observed that ICAO has been approaching issues towards problem-solving from four different dimensions. First regarding time, it analyses past experiences and best practices, and make solutions in short, mid and long terms. Second, from space perspective, ICAO covers States, region and the world as a whole. Third, regarding stakeholders it consults with and hear from as many entities as it could, including airlines, airports, community, consumers, manufacturers, air traffic control centers, air navigation service providers, industry and insurers. Last not but least, in terms of regulatory changes, it identifies best practices, guidance materials and provisions which could become standards and recommended practices.

  • Innovation is the development of new customers value through solutions that meet new needs, inarticulate needs, or old customer and market needs in value adding new ways. This is accomplished through more effective products, processes, services, technologies, or ideas that are readily available to markets, governments, and society. Innovation differs from invention in that innovation refers to the use of a better and, as a result, novel idea or method, whereas invention refers more directly to the creation of the idea or method itself. Therefore, the utilization of patents is the most important of the three aspects (creation, protection and utilization) in the patent system for getting to actual innovation. In order to increase the patent use, the rates of the applied patents to a real industrial process and the bargain and license of patent rights should be promoted. For promoting technological innovation substantially, there is necessary to escape from dichotomy of weakening or enhancing patent protection in the discussion on the changes of patent system for "Adequate patent guarantees". Furthermore, the legal and institutional improvements to enable the use of patent should be discussed together. Recently, open innovation is a new paradigm that assumes that firms can and should use external ideas as well as internal ideas, and internal and external paths to market, as the firms look to advance their technology" or "Innovating with partners by sharing risk and sharing reward." The boundaries between a firm and its environment have become more permeable; innovations can easily transfer inward and outward. The central idea behind open innovation is that in a world of widely distributed knowledge, companies cannot afford to rely entirely on their own research, but should instead buy or license processes or inventions (i.e. patents) from other companies. In addition, internal inventions not being used in a firm's business should be taken outside the company. In the era of open innovation, more flexible patent use strategy is essential. It is important that improvements for the legal system in order that patented technology can be applied for products and processes in a real industrial between outside and inside of the firms.
