eUCP 11조의 잠재적 하자의 규명과 합리적 개정방안의 모색 (A Study on the Inherent Defects in the eUCP Article 11 & Implication for the Revision)

  • This study analyzes some important implications for the forthcoming revision of eUCP through the methodology of expected utility maximization theory. The overall results are as follows. First, beneficiary with an initial wealth has a risk-averse utility in traditional letter of credit transaction, and he would be more risk-averse in eUCP transaction. Secondly, the beneficiary who has risk-averse utility will pay for the risk premium to reduce the risk of corruption of an electronic record by means of cost of loss reduction activities. Thirdly, the cost of loss reduction activities is represented by a convex cost function, Fourthly, a risk averse beneficiary pursues loss reducing activities to the point where the expected marginal product of loss reduction is less than its marginal cost. Fifthly, a more risk-averse eUCP beneficiary will always select a higher level of loss reduction as long as the effectiveness of loss reduction is certain. Sixthly, when the effectiveness of loss reduction is uncertain, the more risk-averse eUCP beneficiary does not necessarily choose a higher level of loss reducing activities. Finally, it would be more reasonable that eUCP Article 11 should protect eUCP beneficiary who pursues a higher level of loss reducing activities.

Epidemiological Correlates of Breast Cancer in South India

  • Background: Breast cancer is the most frequent cancer in women globally and represents the second leading cause of cancer death among women (after lung cancer). India is going through epidemiologic transition. It is reported that the incidence of breast cancer is rising rapidly as a result of changes in reproductive risk factors, dietary habits and increasing life expectancy, acting in concert with genetic factors. Materials and Methods: In order to understand the existing epidemiological correlates of breast cancer in South India, a systematic review of evidence available on epidemiologic correlates of breast cancer addressing incidence, prevalence, and associated factors like age, reproductive factors, cultural and religious factors was performed with specific focus on screening procedures in southern India. Results: An increase in breast cancer incidence due to various modifiable risk factors was noted, especially in women over 40 years of age, with late stage of presentation, lack of awareness about screening, costs, fear and stigma associated with the disease serving as major barriers for early presentation. Conclusions: Educational strategies should be aimed at modifying the life style, early planning of pregnancy, promoting breast feeding and physical activity. It is very important to obtain reliable data for planning policies, decision-making and setting up the priorities.

Factor Structure of a Korean-Language Version of the Patient Satisfaction with Procedural Aspects of Physical Therapy Instrument

  • Purpose: The aim of the study was to survey satisfaction with physical therapy. Methods: After the physical therapy consultation, patients filled in a Korean-language version of the 20-tiem version of the MedRisk Instrument developed for measuring Patient Satisfaction with physical therapy. Items are scored on a five-point Likert scale ranging from strongly disagree to strongly agree. The last two items are general satisfaction and future return to the clinic. Age and gender information was also collected anonymously. Exploratory factor analysis based on principal components analysis with varimax rotation was performed on the first 18 items of the MedRisk Instrument using SPSS v.20. Results: Four factors emerged with eigenvalues greater than 1, and these cumulatively explained 55% of the total variance in item scores. The factors were labelled: Internal, External Positives, External Negatives, and Clinic Presentation. Correlations of the factor scores with the two global items ranged from 0.29 to 0.70 (both p<0.001). Gender differences were only found on the last factor, with male Korean patients rating Clinic Presentation significantly higher than females (p=0.001). Conclusion: Using factor analysis, the proposed factor structure was revealed using the positive and negative components of the external aspects of the physical therapy and by identifying a clinic presentation which contributes to patients' satisfaction. The largest proportion of the variance in Patient Satisfaction was related to clinicians' attention and behaviour. The results of the analysis provide guidelines as to the dimensions of professional physical therapy care and the implications for service delivery and patient experience.

Risk Factors for Stage IV Breast Cancer at the Time of Presentation in Turkey

  • Background: Breast cancer (BC) is the one of the most common cancers in women. It is also a leading cause of death. Unfortunately, some patients initially present with distant metastases and are diagnosed with stage IV disease that is nearly always, by then, incurable. This retrospective analysis investigated the risk factors for stage IV BC that may underlie such late presentation. Materials and Methods: In all, 916 patients with BC who visited the medical oncology polyclinic of eight different centres in Turkeybetween December 2011 and January 2013 were analysed. Results: A total of 115 patients (12.6%) presented with stage IV disease. In univariate analysis; to comparing these with patients at other stages, no statistical difference was found for median diagnosis age or age at menarche (p=0.611 and p=0.820), whereas age at menopause and age at first live birth were significant (p=0.018 and p=0.003). No difference was detected in terms of accompanying diseases, use of oral contraceptives and hormone replacement therapy, smoking, alcohol consumption and the rate of family history of BC between the patients (p=0.655, p=0.389, p=0.762, p=0.813, p=0.229, p=0.737). However, screening methods were employed less often, the rate of illiteracy was higher, and the rate of other cancers was higher in patients with stage IV BC (p=0.022, p=0.022, p=0.018). No statistical difference was observed between the patients in terms of tumour histopathology, and status of oestrogen receptor, progesterone receptor, or human epidermal growth factor-2 receptor (p=0.389, p=0.326, p=0.949, p=0.326). Grade 3 tumours were more frequent in patients with stage IV disease (p<0.001). On multivariate analysis, risk factors for stage IV breast cancer at the time of presentation were found to be age at first live birth and educational level (p=0.003 and p=0.047). Conclusions: Efforts should be made to perform mammography scans, in particular, at regular intervals through national training programs for all women, particularly those with family histories of breast and other types of cancer, and to establish early diagnosis of BC long before it proceeds to stage IV. Additionally, women's education had better be upgraded. In order to make women aware of BC, national education-programmes must be organised.

Employee Perceptions of Their Organization's Level of Emergency Preparedness Following a Brief Workplace Emergency Planning Educational Presentation

  • A brief emergency planning educational presentation was taught during work hours to a convenience sample of employees of various workplaces in Northern Missouri, USA. Participants were familiarized with details about how an emergency plan is prepared by management and implemented by management-employee crisis management teams - focusing on both employee and management roles. They then applied the presentation information to assess their own organization's emergency preparedness level. Participants possessed significantly (p < 0.05) higher perceptions of their organization's level of emergency preparedness than non-participants. It is recommended that an assessment of organizational preparedness level supplement emergency planning educational presentations in order to immediately apply the material covered and encourage employees to become more involved in their organization's emergency planning and response. Educational strategies that involve management-employee collaboration in activities tailored to each workplace's operations and risk level for emergencies should be implemented.

  • Pediatric stroke is relatively rare but may lead to significant morbidity and mortality. Along with the advance of brain imaging technology and clinical awareness, diagnosis of pediatric stroke is increasing wordwide. Pediatric stroke differs from adults in variable risk factor/etiologies, diverse and nonspecific clinical presentation depending on ages. This review will be discussed pediatric stroke focusing on their clinical presentations, diagnosis and etiologies/risk factors.

혁신 리더십 척도 개발 및 효과성 검증 (Development of the Innovation Leadership Scale)

  • The present study investigatesthe reliability and validity of the innovation leadership scale. Originally the seven factors with 25 items were developed through literature review. With a sample of 177 employees in a large company, the results of factor analyses showed that the five-factor model with 14 items had a better fit to the data than the seven-factor model. These five factors were innovativeness pursue, problem solving, vision presentation, risk-taking, and showing initiative. All of these factors were significantly related to various criteria such as identification with the group, attachment to the group, organizational commitment, and supervisor satisfaction, confirming criterion-related validity of the scale. Results of multiple regression analyses showedthat risk taking and showing initiative were more important predictors in explaining criteria. Finally, implications and limitations were discussed. The findings suggest that the key factors of innovation leadership were initiative and risk-taking.

  • 현재 국내표준으로 제시된 모델은 공공시스템에 적용하도록 만들어진 일반 모델로서 비용과 보안관리 능력 등 여러 가지 측면에서 볼 때 일반 기업체에는 적용하기가 어렵다. 본 논문에서는 국내위험분석 모델을 바탕으로 보안관리 위험분석 시뮬레이션모델을 제안하여 일반 기업체에서 실제 적용, 운영이 가능하도록 하였다. 본 모델은 위험분석 특성상 임의적, 주관적으로 도출하게 되는 많은 기준들을 가능한 객관적이고 보편 타당한 기준을 제시하려고 하였고, 대응책 분석 및 제시에서도 현실성 있고 실제 운영 가능한 대응책을 도출하도록 하였다.

Natural history and clinical manifestation of Pediatric Brain Arteriovenous Malformations

  • Brain arteriovenous malformations (bAVMs) are aberrant arteriovenous shunts through a vascular nidus with no intervening capillary beds. They are one of the commonest causes of spontaneous intracranial haemorrhage in children and may be associated with significant morbidity and mortality in cases of rupture. Treatment strategies include microsurgical resection, endovascular embolisation, stereotactic radiosurgery, multimodality treatment with a combination thereof, and particularly in high-grade bAVMs, conservative management. Clinicians involved in treating bAVMs need to have familiarity with the natural history pertaining to bAVMs in terms of risk of rupture, risk factors elevating rupture risk as well as understanding the clinical manifestations of bAVMs. This invited review serves to provide a synthesis on natural history and clinical presentation of bAVMs with particular focus in children to inform decision-making pertaining to management.

경부 악성 림프종과 동반된 유두상 갑상선암 (Synchronous Presentation of Papillary Thyroid Cancer and Malignant Lymphoma)

  • The increasing risk of subsequent malignancy after treatment of malignant lymphoma is well known, which is mainly due to longer survival of these patients. Radiotherapy at an early stage of Hodgkin's disease or non-Hodgkin's lymphoma is accepted to be associated with future occurrence of secondary thyroid cancer. Nevertheless, the synchronous presentation of these malignancies is extremely rare. Well differentiated thyroid cancer, a slow-growing tumor that responds to therapy with surgery and radioactive iodine, is associated with prolonged survival. therefore, it is important to make this diagnosis in patients who show evidence of malignant lymphoma. Furthermore, appropriate treatment must be considered for thyroid cancer to improve the prognosis of these patients. We herein reported 4 cases of synchronous thyroid cancer and malignant lymphoma in patients who had not previously recieved radiotherapy or chemotherapy.
