노인의 인지기능상태에 따른 식품과 영양소 섭취실태 (Food and nutrient intake status of Korean elderly by degree of cognitive function)

  • 본 연구는 용인시에 거주하는 노인들을 대상으로 간이인지기능검사 (MMSE-DS)와 영양건강위험도 조사 (NSI DETERMINE checklist) 및 식품섭취실태를 통하여 인지기능 저하와 영양 식생활 실태와의 관련성을 살펴보고자 하였다. 1) 전체 조사대상 노인 (n = 409)의 평균 나이는 73.3세였고, 남자가 32.3%, 여자가 67.7%이었다. 인지기능 저하로 판정된 사람은 105명으로 대상자의 25.7%가 인지기능 저하로 판정되었으며, 성별에 따른 인지기능 정도의 차이는 없었다. 2) 조사대상 노인들의 운동 빈도는 유의차는 없었으나, 인지기능저하군에서 정상군보다 '운동을 거의 하지 않는다'는 비율은 더 높고, '거의 매일 운동을 한다'는 비율은 낮은 편이었다. 1일 평균 운동 시간은 인지기능저하군이 정상군보다 하루에 1~2시간씩 운동을 하는 사람들은 더 적고, 30분~1시간씩 운동을 하는 사람들은 더 많아서 총 운동시간이 적은 것으로 나타났다. 3) NSI DETERMINE 검사를 이용한 영양건강 위험도는 저위험군 270명 (66%), 중위험군 90명 (22%), 고위험군 49명 (12%)이었으며, 인지기능저하군에서 정상군보다 영양건강의 중위험군과 고위험군의 비율이 유의적으로 높았다. 4) 조사대상자의 평균 에너지 섭취량은 1,528.6 kcal로 에너지 필요추정량 (EER)의 84.4%를 섭취하였고, 인지기능저하군에서 정상군보다 다불포화지방산을 유의하게 적게 섭취하였다. 또한, 인지기능저하군은 티아민, 리보플라빈, 철을 평균필요량 미만으로 섭취하는 사람의 비율이 정상군보다 유의적으로 많았다. 5) 식품섭취빈도조사에서 조사한 63개 식품 중 조사대상자의 인지기능에 따라 유의차를 보인 식품은 6가지로 쌀밥과 과자류의 섭취빈도는 인지기능저하군에서 정상군보다 유의하게 높았고, 고등어, 고추, 귤과 수박의 섭취빈도는 인지기능저하군에서 유의하게 낮았다. 본 연구를 통하여 인지기능이 저하된 노인의 경우 정상노인보다 운동 빈도와 운동 시간이 적고, 영양건강위험도는 높으며, 불포화지방산의 섭취가 적고, 티아민, 리보플라빈, 철 등이 정상군보다 더 부족할 가능성이 높음을 확인하였다. 또한, 인지기능저하군에서 쌀밥과 과자류의 섭취는 더 많으나, 고등어, 고추, 귤, 수박의 섭취 빈도는 더 적은 것으로 나타났다. 앞으로 인지기능 손상을 지연시켜 개인의 건강 장수와 사회적 의료비용 절감을 가져오기 위해서는 인지기능 관련 식품들이 어떤 기전으로 인지기능에 영향을 미치는 가에 대한 후속 연구가 필요하다고 하겠다.

한국 노인의 주관적 불안·우울 상태에 따른 식품 및 영양소 섭취 실태 : 2013~ 2015년 국민건강영양조사 자료를 이용하여 (Food and nutrient intake status of Korean elderly by perceived anxiety and depressive condition: data from Korean National Health and Nutrition Examination Survey 2013~ 2015)

  • 본 연구는 국민건강영양조사 제 6기 (2013 ~ 2015년) 자료를 이용하여 우리나라 65세 이상 노인의 주관적 불안 우울 상태에 따른 식품 및 영양소 섭취 실태를 분석한 것으로 그 결과는 다음과 같다. 남자 노인의 불안 우울군이 비불안 우울군에 비하여 저체중군 비율이 높고 과체중군의 비율이 낮았다. 불안 우울군의 교육 수준과 경제 수준이 더 낮았으며 독거가구의 비율은 더 높아서 혼자 사는 노인이 더 많은 것으로 나타났다. 남녀 노인 모두 불안 우울군의 주관적 건강상태가 더 나쁜 것으로 나타나 스스로 건강하지 않다고 생각하는 비율이 높았다. 남자 노인의 불안 우울군이 점심식사와 저녁 식사 시 혼자 식사하는 비율이 높았고 여자 노인의 경우, 불안 우울군의 저녁식사 빈도가 낮은 것으로 나타났다. 남자 노인의 불안 우울군에서 총 식품섭취량, 버섯류, 어패류, 해조류, 유지류, 조미료류의 섭취가 비불안 우울군에 비하여 낮았다. 반면 여자 노인의 경우 총 식품섭취량과 그 외의 모든 식품군 섭취량에서 주관적 불안 우울에 따른 유의적 차이가 나타나지 않았다. 남자 노인의 불안 우울군의 경우 식이섬유, 비타민 C, 리보플라빈, 니아신, 칼륨, 철의 섭취량이 유의적으로 낮게 나타나, 여자 노인에 비하여 남자 노인이 주관적 불안 우울에 따른 영양소 섭취 상태가 불량한 것을 알 수 있었다. 남녀 노인 모두 탄수화물, 단백질, 지방의 열량 기여율은 불안 우울에 따른 유의적 차이는 없었으나, 한국인 영양섭취기준의 에너지 적정비율 (AMDR)인 탄수화물 (55 ~ 65%), 단백질 (7 ~ 20%), 지방 (15 ~ 30%)의 범위와 비교했을 때 두 군 모두 기준보다 탄수화물은 많이 섭취하고, 지방은 부족하게 섭취하고 있는 것으로 나타났다. 특히 남자 노인의 경우는 불안 우울군의 비타민 C와 니아신의 적정섭취비가 유의적으로 낮았다. 전반적인 영양소 섭취 상태를 평가한 평균영양소 적정섭취비도 비불안 우울군에 비해 불안 우울군이 유의적으로 낮았다. 여자 노인의 경우 불안 우울군의 단백질 적정섭취비가 유의적으로 낮았으며 그 외 다른 영양소의 적정섭취비는 유의적 차이를 보이지 않았다. 본 연구 결과를 종합해보면 남자 노인은 여자 노인에 비해 불안 우울에 따른 영양소 섭취상태가 더욱 불량한 것으로 나타나서 남자노인을 위한 심리적 지원 프로그램과 함께 적절한 식사 섭취를 위한 프로그램이 필요하다고 생각된다.

Factors Related to Dietary Fiber Intake Among Sixth Grade Elementary School Children in Daejon City

  • This study was conducted to investigate variables related to dietary fiber intake among sixth grade children in an elementary school in Daejon city. One hundred and forty-seven children completed a questionnaire for determining their socioeconomic background and their food habits. Anthropometric measurements were taken, and a 24-hour diet recall method was used to collect three-day food intakes. The socioeconomic status of the children's families belonged to the upper middle class; 53.0% of their fathers and 25.8% of their mothers had completed college or higher degrees. 27.9% of the mothers had jobs, including part-time jobs. Approximately 30% of the children skipped breakfast, 66.4% of the children preferred animal foods to plant foods, and 52.4% of the children preferred green vegetables to yellow or pale vegetables. Grilled meat dishes, such as Grilled beef rib with seasoning, Bulgogi, grilled pork belly and beef steaks, were the most popular types of food eaten outside home by the children. Daily dietary fiber intake was 14.5 g in boys and 14.5 g in girls, and these intakes are low compared to the standard guidelines. The average intakes of energy and protein of the children were 84.5% and 114.0% of the Korean Recommended Dietary Allowances (RDAs), respectively. Besides energy, riboflavin, iron and calcium intakes were below the RDAs, and especially calcium intake was only 50% of the RDAs. On the other hand, thiamin, niacin, ascorbic acid, protein and phosphorus intakes exceeded the Korean RDAs. Family income or the children's body mass index (BMI) was not directly related to dietary fiber intakes. However, higher dietary fiber intakes tended to be related to higher intakes of green vegetables and fruits. Children with higher dietary fiber intake tended to prefer plant foods to animal foods. Energy and most nutrients, except heme iron and retinol, showed positive relationships with dietary fiber intake. Especially potassium and plant origin protein and calcium were highly correlated with dietary fiber intakes(r>0.6). From these results, it is concluded that dietary fiber intakes of these sixth grade elementary school children were less than the standard reference and it is anticipated to decrease further in the future with increased incomes. Therefore, increased intakes of dietary fiber by elementary school children should be promoted through nutrition education, together with the development of cooking methods and recipes utilizing green vegetables and fruits.

농촌(農村) 가정보건사업지역(家庭保健事業地域)의 가임여성(可妊女性)의 영양(營養) 및 기생충조사(寄生蟲調査) (Nutrition and Parasite Survey of Rural Fertile Women of the Family Health Project Area in Korea)

  • A survey was conducted from January 23 to 31 and February 14 to 18, 1981, in Whaseounggun, Kyunggj-do, to investigate the dietary and nutritionary status, and prevalence of parasitic infection of the fertile women, in an attempt to provide an integrative perspective on the rural family health programmes. The results are summarised as follows; The total number of fertile women, randomly selected, was 197 ; 21 women were in the latter half of pregnancy; 38 women, lactating, the remaning, non-pregnant. Average age of the subjects was 31.4 years; average number of children, 2.7; average number of abortion, 0.9; average family number in home, 5.5. The families had a mean income of \ 2,470,000 per fear. The majority, 83.8% of the families, were engaged in farming. 61.4% of the subjects had completed schooling 6 years or less. The mean energy intake of the lactating women was 73.4% of the recommended allowance, while that of non-pregnant and pregnant women reached 86.9 to 88.6% of the recommendation. In the average diet of the subjects, carbohydrates contribute 74.6 to 81.7% of the total energy intake; protein 11.3 to 11.9%; fat 7.0 to 13.9%. Protein intake of non-pregnant, pregnant, and lactating women met 72.6%, 60.8%, and 65.2% of allowance, respectively; and those proteins were mainly derived from vegetable sources. The subjects ingested less than one-half of the allowances for iron, calcium, vitamin A, and riboflabin ; in contrast, thiamin and ascorbic acid levels were adequate or fairly acceptable. There were significant correlations of meal balance and food diversity scores to the mean intake levels of protein, calcium, iron, vitamin A, thiamin, and riboflavin. Only 10% of the pregnant women and 11.4% of non-pregnant and lactating women reached the acceptable level of hemoglobin. Positive prevalence of parasitic infection was found among about one-third of the subjects, 30.3% of women. Hematologic findings had no significant correlation to parasitic infection.

지역사회 코호트 여성의 요골과 경골에서의 골다공증 유병률과 관련 요인 분석 (Osteoporosis Prevalence of Radius and Tibia and Related Factors Using Multiple Bone Sites Quantitative Ultrasound Measurement of the Korean Health and Genome Study Cohort Women)

  • This study was conducted to investigate osteopenia and osteoporosis prevalence of radius and tibia using Quantitative Ultrasound (QUS) and to identify affecting factors of osteoporosis. A total of 4,340 women aged 40 - 69 years, living in Ansung (rural) and Ansan (mid-sized) area, and free of illnesses affecting bone metabolism participated in the community-based cohort study. Among them 4,059 subjects measured radius bone density and 4,089 measured tibia. The T-score threshold, defined as < -1.0 and $\le$-2.5, was used to identify subjects with osteopenia and osteoporosis by WHO criteria. The crude prevalence of osteoporosis in radius and tibia was $8.4\%$ and $23.3\%$ respectively; after adjustment for age, it changed $6.3\%$ and $18.8\%$. In simple logistic regression analysis, the prevalence of osteoporosis increased by aging, non-marital status, low education, low income. Otherwise, high intakes of Ca/P, thiamin, riboflavin, vitamin B6, and vitamin E were decreased osteoporosis prevalence. Compared to the normal BMI (body mass index) group 08.5 $\le$ BMI < 23), the odds ratio (ORs) of the low BMI group (BMI < 18.5), and high BMI groups (BMl25-30, BMI $\ge$ 30) were significantly increased. The OR of osteoporosis decreased across increasing quartiles of intakes of Ca, P and Ca/P. Therefore, maintaining normal BMI and increasing Ca intake and Ca/P ratio may have a beneficial effect on bone health of Korean women.

서울 일부지역 남자 고등학생의 주중과 주말의 식품 및 영양소 섭취에 관한 연구 (A Study on Food and Nutrient Intakes of Weekday and Weekend Among High School Boys in Seoul)

  • This study was conducted to examine food and nutrient intakes of weekday and weekend with 329 high school boys residing in Seoul using the 3-day food record. Frequency for breakfast skipping was the highest as compared with lunch and dinner skipping. Frequencies for breakfast and lunch skipping were significantly higher in weekend than weekday (p<0.05). The daily average food, vegetable food and animal food intakes were 1,327.6 g, 800.5 g, and 425.8 g, respectively. Total food and vegetable food intakes of weekday were significantly higher than weekend(p<0.05). The dietary variety score(DVS) was 20.5 in daily average, 23.3 in weekday and 15.1 in weekend, which showed significantly higher in weekday than weekend(p<0.05). The daily averages of energy, protein, fat and carbohydrate intake were 2244.9 kcal, 89 g, 72.6 g, and 311.2 g, respectively. Fat intake was significantly lower and carbohydrate, fiber, phosphate, iron, sodium, potassium, vitamin A, niacin, folate, and vitamin C intakes were significantly higher in weekday than weekend (p<0.05). The percentages of energy intake from carbohydrate, protein, and fat were 55.4%, 15.8%, 28.8% in daily average, 56.8%, 15.8%, 27.4% in weekday and 53.6%, 15.8%, 30.7% in weekend, respectively. The percentages of energy intake from carbohydrate in weekday and weekend were below 60%, and that from fat was above 27% in weekday and weekend. Carbohydrate intake was significantly higher and fat intake was significantly lower in weekday than weekend(p<0.05). Energy intakes of daily average, weekday and weekend were above 83% as compared with estimated energy requirement(EER). Intakes of dietary fiber, calcium, potassium, vitamin C, riboflavin and folate were below 75% as compared with adequate intake(AI) or recommended intake(RI). Mean adequacy ratios(MAR), an index of overall dietary quality were 0.78 in daily average, 0.80 in weekday and 0.75 in weekend. MAR of weekend showed significantly lower than weekday(p<0.05). This study revealed that the overall nutrient intake status was worse in weekend than weekday among high school boys.

Difference in food and nutrient intakes in Korean elderly people according to chewing difficulty: using data from the Korea National Health and Nutrition Examination Survey 2013 (6th)

  • BACKGROUND/OBJECTIVES: Chewing difficulty is a factor contributing to a poor nutritional status in the elderly. The aim of this study was to examine disparities in food and nutrition intakes among Korean elderly people with and without chewing difficulty. SUBJECTS/METHODS: This study utilized data from the sixth Korea National Health and Nutrition Examination Survey conducted in 2013. The study subjects included males and females over 65 years of age who were not required to adhere to a special diet due to disease or sickness. They were divided into groups according to their chewing ability. Those who found chewing "very difficult" or "difficult", were combined to form the chewing difficulty group. Similarly, those who found chewing "moderately difficult", "easy", and "very easy" were combined to form the normal chewing group. RESULTS: Of the 999 subjects, 47.7% had chewing difficulties and the prevalence of chewing difficulty was higher in females than in males (P = 0.03) and higher in those 75 years of age and over than in younger individuals (P < 0.001). The chewing difficulty group had a significantly lower intake of fruits and vegetables (P < 0.05) and lower vitamin C and potassium intake than those in the normal group. Comparison of the percentages of Dietary Reference Intakes for Koreans (KDRIs) in the two groups indicated that the intake of most nutrients (energy, vitamin C, thiamin, riboflavin, niacin, calcium, phosphorus, sodium, potassium, and iron) were significantly lower in the chewing difficulty group than in the normal group. In particular, calcium intake was inadequate (51% of KDRIs) in the chewing difficulty group. CONCLUSIONS: The results indicate that chewing difficulty is closely related to food and nutrient intake in the elderly and can result in vitamin and mineral intake deficiencies. It is evident that the care of elderly subjects with chewing difficulty is essential for maintaining a healthy lifestyle.

어촌과 도시지역 여고생의 영양실태, 체조성 및 혈액성상에 관한 연구 (A Study on Nutrients Intakes, Body Composition and Hematological Status of High School Girls in Fishing and Urban Areas)

  • The purpose of this study was to examin height, weight and body composition nutrient intakes and serum lipids for high school girls in urban and fishing area. The subjects consisted of 78(44 fishing and 34 urban) high school girls aged in average 17.3 and 17 years. Their dietary intakes were assesed for 1 day by means of 24 hours dietary recall method. Concentrations of serum lipids, percentage of body fat and volume of total body water were measured. The mean energy intake of the subjects were 1431 ㎉ in fishing and 1659 ㎉ in urban area. The subjects in fishing area(urban) consumed 58(67) g protein, 27(36) g lipid, 459(634) mg calcium, 676(945) mg phosphorous, 11(14) mg iron, 786(574) RE retinol, 2.8(3.6) mg tocopherol, 0.9(1.0) mg thiamin, 1.2(1.1) mg riboflavin, 16.9(20.5) mg niacin and 64.1(92.8) mg ascorbic acid, respectively. Energy, calcium, iron and tocopherol intakes were lower than the Korean RDA in both girls. The mean percentage of fishing girls were 19.8% and significantly lower than 25.6% in urban. These values seem to fall in the desirable range in fishing girls, but high school girls in urban is seemed to overweight. The mean saturated fatty acid(SFA), monounsaturated fatty acid(MUFA), polyunsaturated fatty acid(PUFA) and dietary cholesterol(Chol) intakes were 7.3 g, 8.1 g, 7.2 g and 263 mg for fishing girls were 13.1 g, 12.1 g, 9.1 g and 216 mg for urban girls respectively. The fat, SFA and MUFA intakes in fishing girls were significantly lower than urban girls. The P/M/S ratio was 0.9/1.1/1 for fishing, was 0.7/0.9/1 for urban. The mean serum triglyceride(TG), total cholesterol, HDL cholesterol, LDL-cholesterol concentrations were 77.7(61.8) mg/dl, 124.5(142.3) mg/dl, 59.3(22.4) mg/dl and 49.6(107.8) mg/dl in fishing girls(urban girls), respectively. All of these values seem to fall in the desirable range but HDL-chlesterol concentrations in fishing girls were significantly higher than urban girls and LDL-cholesterol concentrations in fishing girls were significantly lower than urban girls. There were positive correlations between body fat percent and total cholesterol or LDL-cholesterol concentration : negative correlations between between body fat percent and HDL-cholesterol concentration : positive correlation between fat intake and LDL-cholesterol concentration : negative correlations between serum HDL-cholesterol concentration and fat or SFA or MUFA intakes. 한국 당뇨병 노인의 혈액생화학적 특성, 식이 섭취, 당화혈색소 관련 위험 요인 및 위험도: 국민건강영양조사 제4기(2007-2009)와 제7기(2016-2018) 비교 (Blood Biochemical Characteristics, Dietary Intake, and Risk Factors Related to Poor HbA1c Control in Elderly Korean Diabetes Patients: Comparison between the 4th(2007-2009) and the 7th(2016-2018) Korea National Health and Nutrition Examination Surveys)

  • Objectives: The purpose of this study was to investigate the blood biochemical characteristics, comorbidities, dietary intake, and other risk factors leading to poor glycated hemoglobin (HbA1c) control in elderly Korean diabetes patients over 65 years of age. Methods: Data from the 4th (2007-2009) and the 7th (2016-2018) Korea National Health and Nutrition Examination Surveys (KNHANES) were used. Socio-demographic characteristics (age, gender, region, household income, education level, marital status, nutrition education, diabetes duration and diabetes treatment), lifestyle characteristics (drinking, smoking, regular walking, and subjective health perception), anthropometric characteristics (height, weight, and waist circumference), blood biochemical characteristics (HbA1c, high- density lipoprotein cholesterol, triglycerides, total cholesterol, blood urea nitrogen, and blood creatinine), co-morbidities (obesity, hypertension, hypercholesterolemia, hypertriglyceridemia, and anemia), energy and nutrients intake, food group intake, and HbA1c control-related risk factors were compared. Results: Compared to the 4th survey, the 7th survey showed an increase in diabetes prevalence among men, an increase in the prevalence of diabetes in the older patients, and an increase in the duration of diabetes. The energy ratio from carbohydrate consumption in the 7th survey was lower than in the 4th. Compared to the 4th survey, thiamine and riboflavin intake had improved, and the intakes of vitamin A, vitamin C, and niacin had worsened in the 7th. A comparison of food group intakes showed that there was a decrease in the consumption of whole grains, potatoes, and milk and an increase in the intake of beverages and alcoholic beverages. The risk factors for poor control of HbA1c were the duration of diabetes and co-morbid hypertriglyceridemia in the 4th survey, whereas subjective health perception, obesity, and hypercholesterolemia as co-morbidities were found to be risk factors in the 7th in addition to the risk factors highlighted in the 4th survey. Conclusions: For the future management of elderly Korean diabetes patients, greater care is indicated for men over 75 years, and those with low levels of education. It is necessary to increase the intake of milk and vegetables, and reduce the intake of beverages and alcoholic beverages. In addition, it is necessary to reduce the incidence of obesity, hypercholesterolemia, and hypertriglyceridemia for proper control of blood sugar.

한국 청소년의 소득계층에 따른 혈액 생화학적 특성 및 영양소섭취상태 : 제6기(2013 ~ 2015) 국민건강영양조사를 이용하여 (Biochemical Characteristics and Dietary Intake according to Household Income Levels of Korean Adolescents: Using Data from the 6th (2013 ~ 2015) Korea National Health and Nutrition Examination Survey)

  • Objectives: The purpose of this study was to examine the biochemical characteristics, intake of energy, and nutrients by household income levels of Korean adolescents aged 12 to 18 years. Methods: Data from the 6th (2013 ~ 2015) Korea National Health and Nutrition Examination Survey (KNHNES) were used for the study. A total of 1,839 (966 boys, 873 girls) subjects were included, and they were divided into four income groups according to their household income level. We examined general characteristics (gender, region of residence, skipping or not-skipping breakfast, lunch, dinner, frequency of eating-out), anthropometric characteristics (height, weight, weight status), biochemical characteristics (fasting plasma glucose, blood urea nitrogen, creatinine, triglycerides, cholesterol, HDL-cholesterol, hemoglobin, and hematocrit), the quantitative intake of energy and nutrients using the Korean Dietary Reference Intakes (KDRI), and the qualitative intake evaluated by the nutrition adequacy ratio (NAR) and mean nutrition adequacy ratio (MAR) of the four groups. Results: There were significant differences by income group within the region of residence and the rate of skipping breakfast, lunch, and dinner. The low-income group had a higher rate of skipping breakfast, lunch, and dinner. According to the income group, there was a difference in the height of boys, and there was no difference in the weight and obesity of boys and girls. In the biochemical characteristics, only the hematocrit of girls showed differences by income group. The quantitative intake of energy and nutrients compared with KDRI differed by income group. There were differences in energy, carbohydrates, proteins, thiamin, riboflavin, niacin, and phosphorus levels in boys and protein, vitamin A, niacin, and sodium levels in girls. The qualitative intake of energy and nutrients examined using NAR and MAR also differed according to the income group. The NAR showed differences in calcium in boys and vitamin C and calcium in girls. The MAR revealed differences in both boys and girls by income group. Conclusions: Among adolescents in the low-income group, the rate of skipping meals was high, and the quantitative and qualitative intake of energy and some nutrients was low. It is suggested that the nutritional intake can be improved by lowering the rate of skipping breakfast, lunch, dinner. We suggest that even just providing breakfast in schools can be considered highly effective in improving the rate of avoidance of skipping meals and improving nutrient intake. Also, we suggest that it is necessary to improve the food environment, food availability, and food accessibility through national and social support for low-household income adolescents.
