Purpose: The purposes of this study were to evaluate the degree of social support, self-esteem, and motivation for rehabilitation in stroke patients and examine correlations between activities of daily living and study variables. Furthermore, this study explored factors that influence on activities of daily living. Methods: The study population was 192 stroke patients admitted in two general hospitals and two rehabilitation hospitals in M city. We collected the data using the structured questionnaires from July 11 to September 30, 2013. Results: The higher degree of social support, self-esteem, motivation for rehabilitation, the higher performance level for activities of daily living. Influencing factors of activities of daily living in the participants were motivation for rehabilitation(${\beta}$=.32, p<.001) and social support(${\beta}$=.31, p<.001). The model explained 45.9% of the variance. Conclusion: This study shows that intrinsic motivation of change in stroke patients themselves, a strong will to return to society and social supports are important factors on activities of daily living. Therefore, it is needed to develop a systemic nursing intervention improving activities of daily living in stroke patients through positive social support from patients' family members and health care providers.

Objective: We evaluate temporal correlations between postoperative symptomatic and electrophysiological improvements, and assessed the recovery time required for patients with carpal tunnel syndrome(CTS) before returning to routine activities. Methods: 30 CTS patients were treated via the endoscopic monoportal approach, from March 2001 to September 2003. We assessed the symptoms (hyperesthesia in the finger tips, or abnormal sensations and painful numbness or night pain) and electrophysiological changes in the preoperative state, 1 month and 6 months after surgery. We marked the times at which patients became able to return to activities of daily living and work, after undergoing endoscopic carpal tunnel release. Results: At the end of the follow-up period, high levels of achievement and good outcomes were observed, with respect to both the symptoms and electrophysiological studies. We discovered significant differences between the preoperative and postoperative periods, especially in terms of motor nerve onset latency from $4.50{\pm}1.43$ to $3.97{\pm}0.69$ and sensory nerve conduction velocity, the wrist-to-finger from $19.81{\pm}10.03$ to $28.18{\pm}11.01$ and wrist-to-palm from $23.34{\pm}13.40$ to $31.79{\pm}13.38$(P<0.05 for each comparison). The average time interval required before return to activities of daily living was 26.4 days, and time interval required before return to work was 48.08 days. Conclusion: Electrophysiological improvements are largely consistent with symptomatic relief, but there is some disparity between electrophysiological and symptomatic improvement.
Background: Promoting patients' safe return home at discharge and reducing length of stay in hospital is key for Restorative Rehabilitation Institution (RMI). Objects: This study was designed to identify the factors influencing the return to home and length of stay among various factors. Methods: A total of 120 stroke patients (76 males and 44 females) who were hospitalized in an adult inpatient unit of a RMI for more than 2 months were retrospectively analyzed for this study (multivariate logistic regression analyses, p < 0.001). As predictor variables for assessing the return to home and length of stay, demographic data (sex, age, duration between onset and admission, length of stay, caregiver after discharge, occupation after discharge, reason for discharge, and household type after discharge) were collected. Additionally, following measurements were selectively collected from patient's medical records: scores of Mini-Mental State Examination Korean version (K-MMSE), modified Barthel Index Korean version (K-MBI), Berg Balance Scale and Functional Ambulation Category were obtained at admission and discharge. Results: The K-MMSE at admission and K-MBI at discharge were found to be the predictors of return to home. Additionally, K-MBI at admission influenced the length of stay. Conclusion: This study suggests cognitive functioning at admission and the level of activities of daily living at discharge predicted the return to home and length of stay.
목적 : '재활의료기관 지정운영 시범사업'을 진행하고 있는 기관 내 작업치료사 및 환자들에 대한 설문을 통하여, 시범사업 안에서의 작업치료가 환자들의 가정 및 사회로의 복귀를 위한 적절한 지원체계를 확립하고 있는지 알아보고자 하였다. 방법 : 시범사업을 운영 중인 전국 13개 기관의 작업치료사 293명, 환자군의 응답을 허가한 8개 기관의 환자 296명의 응답지를 대상으로 설문분석 하였다. 치료사용 설문에는 일상생활활동 관련 평가도구 및 적용하는 중재에 관한 항목들로 구성되었으며, 환자용 설문에는 퇴원 후 직업복귀 여부 및 이를 위한 희망 중재 활동을 묻는 항목으로 구성되었다. 결과 : '재활운영 지정운영 시범사업'이 환자들의 빠른 사회복귀를 위한 목적으로 시행되고 있음에도 이를 위한 가장 중요한 수가이며 치료적 목표인 일상생활활동이 효율적이고 효과적으로 적용되지 못하고 있음을 파악할 수 있었다. 또한 기존 의료기관에서 활용하고 있는 건강보험 수가를 본 시범사업에서도 그대로 적용하고 있기 때문에, 실제적인 지역사회 복귀를 위한 수단적 일상생활활동 및 직업재활을 위한 평가 및 중재의 기반이 전혀 마련되어 있지 않다는 것을 알 수 있었다. 결론 : 시범사업에 대한 면밀한 검토를 통하여, 대상자들의 조기 사회복귀를 통한 삶의 질 향상과 불필요한 사회적 지출 감소를 위한 실제적인 개선이 이루어지도록 해야 할 것이다.
Purpose: This study investigated the effects of virtual reality-based task training (VRBTT) using a smart glove on upper extremity function and activity of daily living in stroke patients. Methods: Twenty-nine patients with chronic stroke disease were randomly allocated to two groups: the VRBTT group (n=14) and the control group (n=15). All patients received 30 minutes of standard occupational therapy, 5 times a week, for 8 weeks. The VRBTT group performed an additional 30 minutes of virtual reality-based rehabilitation training, 5 times a week, for 8 weeks. Results: Both groups showed significant improvements in upper extremity function, yielding an increase in FMA and K-WMFT (p<0.05). There was a more significant increase in the VRBTT group before and after interventions (p<0.05). There was no significant difference in MAS for the control group (p>0.05); however, there was a significant increase for the VRBTT group (p<0.05). In the activities of daily living, there was a significant difference in the values for K-MBI (p<0.05). In addition, both groups showed a significant increase for K-MBI and K-RNLI (p<0.05). Conclusion: This study showed that VRBTT using smart gloves can have a more positive effect on upper extremity function and activities of daily living in stroke patients than conventional intervention methods. A variety of virtual reality-based contents and glove-shaped wearable devices will help stroke patients in rehabilitation clinics recover and return to society.
PURPOSE: This study examined the differences between male and female workers by investigating the various factors that affected the workers' abilities to return to work according to the International Classification of Functioning standards. METHODS: We analyzed the personal factor, environmental factor, work performance and participation factor related to ICF according to worker's gender. For this purpose data from the third Worker's Compensation Insurance panel survey conducted by the Korea Workers' Compensation & Welfare Service were analyzed. In order to verify the research model, we used frequency analysis, cross analysis to compare the differences between male and female workers according to personal, environmental, work performance, and participation factors and hierarchical regression analysis to identify significant factors affecting job performance. RESULTS: The results, indicate that the level of education, license status, working period, socioeconomic status and employment type of female workers were lower than those of male workers. Factors that have the greatest influence on job performance are grade of disability, status of disability, economic activity status, and instrumental activities of daily living (p<.05). CONCLUSION: It is necessary to provide socially stable employment support and social policy support without discrimination to allow disabled female workers to return to work and maintain their jobs and to study factors influencing job performance further.

As positive opinions stating that such consumer activities not only relieve stress, but also help psychological stability are emerging, consumer trends for the development of fashion products which reflect 'Kidult' emotions are becoming increasingly popular. Not only that, with interior products, the need for the development of differentiated designs which are focused on consumers and which reflect consumer demands is being magnified as they are very important as living designs within the consumption environments of modern people. As such, the goal of this study is to develop textile pattern designs for kidult interior products for adults.
The only treatment method for crushed soft tissue injuries in the proximal leg involving the knee joint is the microsurgical free flap transplantation, especially latissimus dorsi myocutaneous free flap is useful to cover the extensive soft tissue defects, therefore prevents iatrogenic below knee amputation and facilitates early wound healing, early ambulation and shortens hospital stay. Authors have treated the open amputation with crushed soft tissue and bone injuries in the proximal leg, and the repeated abrasion and infectious wound with traumatic neuroma in the below knee with myocutaneous free flap and succeeded to wear below knee amputation prosthesis and return to normal activities of the daily living in the relatively shorter period than usual.

Background: This study aimed to analyze the efficacy of single-dose corticosteroid injection (CSI) administered at 6 weeks postoperative to treat stiffness following arthroscopic rotator cuff repair (ARCR). Methods: In this prospective, multicentric, case-control study, post-ARCR stiffness at 6 weeks was treated with either a single dose of intra-articular CSI (CSI group) or physical therapy with oral analgesics (non-CSI group). Pain intensity according to visual analog scale (VAS), functional outcome using the Constant Murley Shoulder Score, time to return to activities of daily living (ADLs), and retear rate were recorded at 6 weeks, 9 weeks, 12 weeks, 6 months, 12 months, and 18 months postoperatively in both groups. Results: A total of 149 patients (54.5%) in the CSI group and 124 patients (45.5%) in the non-CSI group were included in this study. Pain and function were significantly better in the CSI group at 9-week, 12-week, and 6-month (P<0.001) follow-up, whereas they were not significantly different when the groups were compared at 12- and 18-month follow-up. The mean duration to return to ADLs was significantly shorter (P<0.001) in the CSI group. The incidence of retears was not significantly different (P=0.36) between groups at the end of 18 months of follow-up. Conclusions: Single-dose intra-articular CSI administered at 6 weeks postoperative to treat post-ARCR stiffness significantly improved pain, function, and duration of return to ADLs without increasing the risk of retears compared to patients who did not receive intra-articular CSI. Purpose: The purpose of this study was to evaluate the clinical results of the 4+5th extensor compartmental artery pedicled vascularized bone graft in advanced Lichtman stage III Kienbock's disease. Materials and Methods: Eight patients with advanced Lichtman stage III Kienbock's disease who underwent the 4+5th extensor compartmental artery pedicled vascularized bone graft and followed up more than 1 year were analyzed retrospectively. There were 3 men and 5 women. The mean age was 43.6 years old. Two patients were Lichtman stage IIIA and six patients were IIIB. The clinical outcomes were evaluated with radiocarpal joint pain, range of motion, grip strength, carpal-height ratio, radioscaphoid angle, return to daily living activity and/or work. The mean follow up period was 38.5 months (range from 12 to 86 months). Results: On last follow up, the pain was disappeared in 6 patients, and mild occasional pain was remained in 2 patients. Mean radiocarpal joint flexion and extension were 55 degrees and 60 degrees, 87% and 88% of the normal side, respectively. The carpal-height ratio was maintained or improved in 6 patients and slightly decreased in 2 patients. Radioscaphoid angle were improved or maintained in 7 patients. Mean grip strength was 67 lb, 93% of the normal side. All 8 patients returned to daily living activities and/or their previous works. Conclusion: The 4+5th extensor compartmental artery pedicled vascularized bone graft prevented the progression of disease and provided clinical improvement even in advanced Lichtman stage III Kienbock's disease.
