Effects of Acupotomy on Pain and Functional Improvement in Acute Low Back Pain Patients: A Retrospective Study (도침치료가 급성 요추 염좌 환자의 통증 및 기능개선에 미치는 영향: 후향적 연구)

  • Objectives The aim of this study is to investigate the effect of Acupotomy on the pain reduction and functional improvement of patients with Acute low back pain. Methods We studied 60 patients with Acute low back pain with Numeric Rating Scale (NRS) 5 or higher in admission and had admitted to Department of Korean Medicine Rehabilitation of Daejeon University from March 1, 2017 to October 31, 2017. 30 patients had received Acupotomy combined Korean Medicine treatment(such as acupuncture, moxabustion, herbal medicine, etc) and the other 30 had received Korean Medicine treatment only. The analysis was conducted as a retrospective study which analyzes the patient's medical records. Statistical analysis was performed using the IBM SPSS Statistics 24 program. We used NRS to evaluate pain reduction, used Range of Motion (ROM) and Roland Morris Disability Questionnaire (RMDQ) to evaluate function improvement, and used EuroQol-5 Dimension (EQ-5D) and EuroQoL-Visual Analogue Scale (EQ-VAS) to assess quality of life. Finally, a five-point Likert scale was used to assess treatment satisfaction. Results The analysis revealed that patients who were treated with Acupotomy showed statistically significant NRS reduction, improvement of ROM and RMDQ, improvement of EQ-5D and EQ-VAS and satisfaction compared to those who just recevied Korean Medicine treatment. Conclusions In conclusion, we found that the Acupotomy showed a positive effect on pain resolving, functional rehabilitation and quality of life in patients with Acute low back pain.

Effects of different anesthetic techniques on the incidence of phantom limb pain after limb amputation: a population-based retrospective cohort study

  • Background: General anesthesia (GA) has been considered the anesthetic technique which most frequent leads to phantom limb pain (PLP) after a limb amputation. However, these prior reports were limited by small sample sizes. The aims of this study were to evaluate the incidence of PLP according to the various anesthetic techniques used for limb amputation and also to compare the occurrence of PLP according to amputation etiology using the Korean Health Insurance Review and Assessment Service for large-scale demographic information. Methods: The claims of patients who underwent limb amputation were reviewed by analyzing the codes used to classify standardized medical behaviors. The patients were categorized into three groups-GA, neuraxial anesthesia (NA), and peripheral nerve block (PNB)-in accordance with the anesthetic technique. The recorded diagnosis was confirmed using the diagnostic codes for PLP registered within one year after the limb amputation. Results: Finally, 7,613 individuals were analyzed. According to the recorded diagnoses, 362 patients (4.8%) developed PLP after amputation. Among the 2,992 patients exposed to GA, 191 (6.4%) were diagnosed with PLP, whereas 121 (4.3%) of the 2,840 patients anesthetized with NA, and 50 (2.8%) of the 1,781 patients anesthetized under PNB developed PLP. The relative risks were 0.67 (95% confidence interval [CI], 0.53-0.84; P < 0.001) for NA and 0.43 (95% CI, 0.32-0.59; P < 0.001) for PNB. Conclusions: In this retrospective cohort study, using large-scale population-based databases, the incidence rates of PLP after limb amputations were, in the order of frequency, GA, NA, and PNB.

Effect of initial placement level and wall thickness on maintenance of the marginal bone level in implants with a conical implant-abutment interface: a 5-year retrospective study

  • Purpose: Implant wall thickness and the height of the implant-abutment interface are known as factors that affect the distribution of stress on the marginal bone around the implant. The goal of this study was to evaluate the long-term effects of supracrestal implant placement and implant wall thickness on maintenance of the marginal bone level. Methods: In this retrospective study, 101 patients with a single implant were divided into the following 4 groups according to the thickness of the implant wall and the initial implant placement level immediately after surgery: 0.75 mm wall thickness, epicrestal position; 0.95 mm wall thickness, epicrestal position; 0.75 mm wall thickness, supracrestal position; 0.95 mm wall thickness, supracrestal position. The marginal bone level change was assessed 1 day after implant placement, immediately after functional loading, and 1 to 5 years after prosthesis delivery. To compare the marginal bone level change, repeated-measures analysis of variance was used to evaluate the statistical significance of differences within groups and between groups over time. Pearson correlation coefficients were also calculated to analyze the correlation between implant placement level and bone loss. Results: Statistically significant differences in bone loss among the 4 groups (P<0.01) and within each group over time (P<0.01) were observed. There was no significant difference between the groups with a wall thickness of 0.75 mm and 0.95 mm. In a multiple comparison, the groups with a supracrestal placement level showed greater bone loss than the epicrestal placement groups. In addition, a significant correlation between implant placement level and marginal bone loss was observed. Conclusions: The degree of bone resorption was significantly higher for implants with a supracrestal placement compared to those with an epicrestal placement.

  • Purpose: This work is a retrospective clinical case study evaluating the efficacy of Injinoryung-san granule administration to patients who showed an abnormal hepatic function test level. Methods: We analyzed the chart records of seven patients with elevated levels of the hepatic function test treated with Injinoryung-san granules. Results: The patients who were treated with Injinoryung-san showed a significant improvement in their elevated hepatic function test levels. Conclusion: In patients with elevated hepatic function test levels, Injinoryung-san granules can improve the hepatic level without adverse effects or changes in existing drugs.

Effectiveness of Additional Administration of a Banha-sasim-tang Formulation to Patients with Gastrointestinal Symptoms (KCD K-code) Not Improved by Western Medicines: A Retrospective Chart Review (합성의약품으로 호전되지 않은 소화기 증상 환자(KCD K-코드)에 대한 반하사심탕 제제 병용의 효과 : 후향적 차트 분석)

  • Objectives: The aim of this study was to evaluate the effectiveness of additional administration of Banha-sasim-tang formulation to patients with gastrointestinal symptoms that did not improve with Western medicines. Methods: The patients who met the inclusion and exclusion criteria were analyzed retrospectively. A paired t-test was performed on the Nepean Dyspepsia Index-Korean version (NDI-K) scores before and after administering the Banha-sasim-tang formulation. Results: A total of 27 subjects were included. The mean duration of taking the Banha-sasim-tang formulation was 15.6±3.1 days. The most commonly used Western medicine before taking the Banha-sasim-tang formulation was rebamipide. The means of the NDI-K score were 57.4±21.8 before administering Banha-sasim-tang formulation and 34.9±16.5 after administration, and the difference was statistically significant (p<0.0001). No serious side effects were encountered while taking the Banha-sasim-tang formulation. Conclusions: For patients whose gastrointestinal symptoms do not improve with Western medicines, additional administration of Banha-sasim-tang formulation may be an option. However, as this study is a retrospective chart review, prospective, large-scale, double-blind studies are needed in the future.

Effect of Complex Korean Medicine Treatment on Obese Patient: A Retrospective Study (비만 환자의 한의복합치료 후 비만 개선 효과: 후향적 연구)

  • Objectives: The purpose of this study was to observe the effect of improving obesity after 4 week complex Korean medicine treatment in obese patients by retrospectively analyzing. Methods: A retrospective chart review was conducted on 44 patients who received obesity treatment at 23 Korean medicine clinics in Daejeon city. We analyzed weight, body mass index (BMI), body fat percentage, body fat mass, muscle mass, hip and waist circumference, EuroQol-5D (EQ-5D), Korean version of obesity-related quality of life scale (KOQOL), and satisfaction surveys at 4 and 8 weeks after treatment. Results: The weight, BMI change after 4 weeks and 8 weeks was decreased significantly. In addition, there were significant differences in fat mass, muscle mass, waist circumference, and hip circumference. Life quality scale EQ-5D and KOQOL showed decreased significantly. In the evaluation of overall satisfaction, safety, help level, recommendation intention, and revisit intention, more than 89% answered positively that they were satisfied. There were no serious side effects. Conclusions: It was significant effect on weight loss and obesity improvement after complex Korean medicine. And quality of life evaluation and satisfaction survey also show positive results.

Factors influencing repeated suicide attempts among suicide attempters who visited the emergency room due to acute drug intoxication: a retrospective study (급성약물중독으로 응급의료센터를 내원한 자살시도자의 자살 재시도 영향요인: 후향적 연구)

  • Purpose: This retrospective study was conducted to identify factors associated with repeated suicide attempts by analyzing the electronic medical records of suicide attempters who visited emergency medical centers due to drug intoxication. Methods: Data from January 1, 2020 to December 31, 2021 were collected for this study and analyzed using SPSS/WIN 25.0. Results: In this study, the factors affecting repeated suicide attempts among individuals who visited the emergency medical center due to acute drug intoxication were the presence or absence of psychiatric diseases, the season of drug intoxication, and age. In order to identify the factors affecting repeated suicide attempts in those who presented with acute drug intoxication, logistic regression analysis was performed by inputting psychiatric diseases, season of drug intoxication, and age, all of which exhibited significant differences between those who did and did not attempt suicide again. The presence of psychiatric diseases (odds ratio [OR] = 1.99, 95% confidence interval [CI] = 1.04~3.80), intoxication occurring in the fall (vs. spring; OR = 3.35, 95% CI = 1.28~8.75), and age ≥ 65 (vs. those under the age of 18; OR = 12.96, 95% CI = 2.02~83.17). The regression model was found to be statistically significant (F = 30.01, p < .001), and the coefficient of determination of Cox and Snell, which represents the explanatory power of the model, was 12.0%. Conclusion: This study suggests that to prevent repeated suicide attempts, it is necessary to develop a tool to evaluate the risk of suicide, use it to evaluate the risk of suicide, and then manage individuals accordingly.

A Study of Gamitonggyung-tang's Clinical Effects on Primary Dysmenorrhea: A Retrospective Chart Review (가미통경탕(加味通經湯)의 원발성 월경통에 관한 임상적 효과 : 후향적 차트 리뷰)

  • Objectives: The purpose of this study is to evaluate the effects of Gamitonggyung-tang on Primary Dysmenorrhea. Methods: We treated 33 cases of primary dysmenorrhea patients with Gamitonggyung-tang for three menstrual cycles. We estimated the effects by Visual Analog Scale (VAS), Menstrual Distress Questionnaire (MDQ) and checked amount of the analgesics taken during menstrual phase. Results: After treatment, the mean of VAS, MDQ Score and amount of the analgesics taken were decreased. Conclusions: This case shows that Gamitonggyung-tang is effective in treating primary dysmenorrhea patients.

  • The discount survival model is proposed for the application of the Cox model on the analysis of survival data with time-varying effects of covariates. Algorithms for the recursive estimation of the parameter vector and the retrospective estimation of the survival function are suggested. Also the algorithm of forecasting of the survival function of individuals of specific covariates in the next time interval based on the information gathered until the end of a certain time interval is suggested.

  • Objective: Many studies have demonstrated that hydrosalpinx has a detrimental effect on the outcome of IVF. Treating hydrosalpinges prior to the IVF procedure in women with hydrosalpinges is thought to improve the likelihood of successful IVF outcome. Vaginal ultrasound-guided aspiration of hydrosalpinx fluid (HSF) with injection of the sclerosing agent in situ might be simpler than invasive procedures like salpingectomy. Therefore, we carried out a retrospective study on the effects of ultrasound-guided HSF aspiration and injection of the sclerosing agent of ultrasonically diagnosed hydrosalpinx on IVF outcome. Methods: In our retrospective study, 97 tubal factor infertile female patients that underwent IVF treatment between January 2005 and December 2012 at the Reproductive Medicine Center of CHA Hospital were divided into two study groups. Fifty-six patients underwent interventional ultrasound sclerotherapy (group 1), and the remaining 41 patients received laparoscopic salpingectomy (group 2) before IVF. We compared the IVF outcomes of the two groups. Results: The results showed that ultrasound-guided HSF aspiration and sclerotherapy have IVF outcomes comparable to laparoscopic salpingectomy. Conclusion: Interventional ultrasound guided sclerotherapy before IVF is an effective and less invasive prophylactic intervention alternative to salpingectomy with hydrosalpinx.
