Study about the complications associated with implant surgery and prosthetic treatment (임프란트 수술 및 보철물에 따른 합병증 연구)

  • Purpose : The aim of this study was to compare surgical complications between simple implant placement and implant placement combined with complicated surgical procedures. We also evaluated prosthetic complications according to the specific types of prosthesis. Material and Method : A retrospective analysis of dental chart of patients who was performed implant therapy during the period from June 2003 to December 2005 was carried out. This study was performed on 408 patients (208 male, 200 female). In addition, 1671 implants were performed. Based on their medical record and radiographs, the authors evaluated surgical and prosthetic complications, surgical procedures accompanied at the time of implant, risk factors of implant failure etc. Result : Surgical complications were developed on 358 implants(21.4% on total placed implants) and wound dehiscence was most prevalent complication. On maxillary posterior area, surgical complications developed more frequently on implants with major surgery and showed a significant difference. And complication rate of implants accompanied with GBR was higher than that of simple implants placement and also showed significant difference. The implant supported prosthesis showed no statistical difference in the occurrence of complications according to the types of prosthesis, and food retention was the most common post-prosthetic complication. Also we speculated that length and width of implant showed significant correlation to the failure of implant primary osseointegration. Conclusion : Based on the result, clinician should provide more careful maintenance for patients with implant placement accompanied by complicated surgical procedure. And periodic maintenance for the patient is requested for long-term survival of implant therapy.

Implant and root supported overdentures - a literature review and some data on bone loss in edentulous jaws

  • PURPOSE. To present a literature review on implant overdentures after a brief survey of bone loss after extraction of all teeth. MATERIALS AND METHODS. Papers on alveolar bone loss and implant overdentures have been studied for a narrative review. RESULTS. Bone loss of the alveolar process after tooth extraction occurs with great individual variation, impossible to predict at the time of extraction. The simplest way to prevent bone loss is to avoid extraction of all teeth. To keep a few teeth and use them or their roots for a tooth or root-supported overdenture substantially reduces bone loss. Jaws with implant-supported prostheses show less bone loss than jaws with conventional dentures. Mandibular 2-implant overdentures provide patients with better outcomes than do conventional dentures, regarding satisfaction, chewing ability and oral-health-related quality of life. There is no strong evidence for the superiority of one overdenture retention-system over the others regarding patient satisfaction, survival, peri-implant bone loss and relevant clinical factors. Mandibular single midline implant overdentures have shown promising results but long-term results are not yet available. For a maxillary overdenture 4 to 6 implants splinted with a bar provide high survival both for implants and overdenture. CONCLUSION. In edentulous mandibles, 2-implant overdentures provide excellent long-term success and survival, including patient satisfaction and improved oral functions. To further reduce the costs a single midline implant overdenture can be a promising option. In the maxilla, overdentures supported on 4 to 6 implants splinted with a bar have demonstrated good functional results.

  • From the study of the symptom, pathology in prescription of Ha-Tong from The Bangyakhapeun. I have reserched 163 prescription. It can be concluded as follows. 1. Prescription about Fecal disease which was the most as 11.66% of the whole, following order Internal disease(6.75%), Uterus Disease(5.52%) Sick-by-Cold Disease(5.52%), Eye Disease(4.91%), Blood Disease(4.91%), Unbalanced humoral status Disease(4.91%), Gynecologic Disease(4.91%). 2. The Fecal Disease divide diarrhea and dysentery; The Internal Disease divides with Sik-sang(食傷) Chu-sang(酒傷), Sik-juck-yu-sang-han(食積類傷寒), Carbonic acid, Vomiting acid; The Uterus Disease divides with Urinnary Disadvantage, Urinary retention, Incontinence; The Sick-by-Cold Disease divides with yang-myung-byung(陽明病), sang-han-goi-jng(傷寒壞證), sang-han-bun-gal(傷寒煩渴), sang-han-sum-ou, sang-han-hyul-jng(傷寒血證), sang-han-ja-ri(傷寒自利), sang-han-bun-jo-jng(傷寒煩燥證). 3. The Diarrhea and Dysentery many used o-ryung-san, hwng-gum-jak-yak-tang(黃芩芍藥湯) hyang-ryun-hwan(香連丸) etc, and The Internal Disease many used pyung-we-san(平胃散) as a basic prescripton. 4. The organ problem use the Sil-yuol(實熱) of the liver, stomach, lung, uterus, small intestine; six natural factors problem used the Sil-jng(實證) of the wind, fire, heat, cold, dampness; And used Unbalanced humoral status, lntrnal hurt, qi and blood, seven extream feeling.

Synthesis of Multi-component Olivine by a Novel Mixed Transition Metal Oxalate Coprecipitation Method and Electrochemical Characterization

  • The multi-component olivine cathode material, $LiMn_{1/3}Fe_{1/3}Co_{1/3}PO_4$, was prepared via a novel coprecipitation method of the mixed transition metal oxalate, $Mn_{1/3}Fe_{1/3}Co_{1/3}(C_2O_4){\cdot}2H_2O$. The stoichiometric ratio and distribution of transition metals in the oxalate, therefore, in the olivine product, was affected sensitively by the environments in the coprecipitation process, while they are the important factors in determining the electrochemical property of electrode materials with multiple transition metals. The effect of the pH, atmosphere, temperature, and aging time was investigated thoroughly with respect to the atomic ratio of transition metals, phase purity, and morphology of the mixed transition metal oxalate. The electrochemical activity of each transition metal in the olivine synthesized through this method clearly was enhanced as indicated in the cyclic voltammetry (CV) and galvanostatic charge/discharge measurement. Three distinctive contributions from Mn, Fe, and Co redox couples were detected reversibly in multiple charge and discharge processes. The first discharge capacity at the C/5 rate was $140.5\;mAh\;g^{-1}$ with good cycle retention. The rate capability test showed that the high capacity still is retained even at the 4C and 6C rates with 102 and $81\;mAh\;g^{-1}$, respectively.

  • A new analytical procedure for the measurement of monomeric isocyanates and total isocyanate group in workplaces has been investigated. The method described herd involves derivatization of the isocyanate sample upon collection with the reagent 9-anthracenylmethyl 1-piperazinecarboxylate (PAC). Laboratory investigations have demonstrated that excess PAC reagent can be satisfactorily removed from PAC-derivatized monomeric isocyanates-a requirement for the success f the analytical procedure. After removal of excess PAC reagent, the PAC derivatives of butyl isocyanate, phenyl isocyanate, HDI, MDI, and TDI were reacted with sodium thiomethoxide to convert them all to 9-anthracenylmethyl methyl sulfide (AMMS). Total isocyanate group was determined by HPLC analysis and quantification of the single AMMS peak. This circumvents many of the disadvantages associated with current HPLC methods. There were no longer problems associated with quantifying late-eluting peaks and analysis times were very short. A single detector was used for quantification because a standard of the analyte existed and the retention time could be determined. Because all species were converted to a single analyte, the problem of variability of response factors among different species was averted. Finally, there were no complex chromatograms to interpret. Monomers of other individual species were measured by analysis of the sample before the individual species were converted to AMMS. The favorable performance of PAC warrants its further study as a reagent for the determination of total isocyanate group in air.

Relationship among Inflow Volume, Water Quality and Algal Growth in the Daecheong Lake (대청호 유입유량 변동과 수질 및 조류증식의 관계)

  • Changes in water quality and algal growth according to the differences in the inflow volume were investigated in the Daecheong Lake from 1998 to 2001. Until year 2000, inflow volume considerably depended on the rainfall throughout the basin. However, the correlation was low since 2001 when water storage in the upstream Yongdam Lake was started. According to inflow volume-TP relationship analyses, significant correlation was found at up- and middle-stream sites, excluding down-stream site of the Daechong Lake. For chlorophyll-a, correlation was found with flow volume at all sites except for Choo-So. In a dry year, although nutrients loads were relatively lower than those in rainy years, there were higher concentrations of chlorophyll-a and massive bloom of Microcystis. Limiting factors for algal growth seems to be not the volume of nutrients load but retention time and physical disturbance of the water body influenced by inflow volume. Thus, in the Daecheong lake, it would be more important to focus on the management of eutrophication in dry years than in rainy ones.

  • PTT(polytrimethylene terephthalate) is a thermoplastic that can be melt-spun into fibers and has extensive applications in carpets, textiles and apparel, engineering thermoplastics, nonwovens, and films or sheets. This polymer combines the good properties of nylon and polyester. Compared with other synthetic fibers such as nylon and acrylic, the PTT fibers feel softer, dye easier with vibrant colors, stretch and recover better. Moreover, the PTT fibers for carpets resist most stainings, clean better, and dry faster. The PTT was first patented in 1941, but it was not until the 1990's, when Shell Chemicals developed the practical method of producing PDO, the raw material for PTT. Many studies have been done including the retention of carpet texture using an image analysis technique, or compressional resilience of the carpet for long term use. In this study, PTT and nylon BCF carpets were compared in terms of the compressional properties including the resilience, using one of the KES system for repetitive measurements. The compression resilience(RC) values of the PTT BCF carpets far exceed those of nylon 6 BCF carpets. The RC values of the PTT BCF carpet(cut) specimens are $42{\sim}45%$ for 5 successive compression deformations, while those of the nylon BCF carpet specimens(cut) are $26{\sim}28%$. There is also a similar trend in the RC values for the other type of carpet which is the loop type. This resilience is one of the important factors of carpet usage evaluation.

Tooth selectivity on venus clam (Gomphina melanaegis) dredge (민들조개 (Gomphina melanaegis) 형망의 갈퀴에 의한 어획선택성)

  • The tooth selectivity of the dredge for catching venus clam (Gomphina melanaegis) was described in which the teeth penetrated the bottom and lifted the shell into the bag. Some factors affecting the selection action of the teeth of the dredge were analyzed related to shell length and shell height. The retention probability of venus clam not sifting through the gaps between the teeth was calculated for various shell lengths and was fitted to two parameter logistic selection curve. The formula obtained is as follows: $P= \frac{1}{1+exp[8.24](\frac{d}{L_1}-0.649)}$, where d is distance between teeth and $L_1$ is a shell length. For biological minimum size(25mm) of venus clam to be catchability 50% the distance between teeth was estimated 16.2mm from the logistic curve. Therefore it is desirable to extend that current spacing between teeth from 12mm to 16mm for the venus clam dredge. That space increasing enables fuel oil of vessel to drag a dredge to be reduced and also man power to sift through sifter smaller venus clams on boards to be reduced.

The produced mealworm meal through organic wastes as a sustainable protein source for weanling pigs

  • This study was designed to evaluate the influence of dietary supplementation of Tenebrio molitor larvae (TM) as an alternative source of protein to fish meal on growth response, retention of nutrient, hematological factors, immune system, and intestinal integrity in weaned pigs. A total of 180 weanling pigs (initial bodyweight of 6.27 ± 0.15 kg; 21 d) were distributed based on the bodyweight between 3 treatments with 6 replicates in each treatment including 10 piglets per replicate. The diets corresponding to treatments consisted of a fish meal diet (CON), a fish meal-dried mealworm diet (TM1, 50% replacement of fish meal with TM meal), and a dried mealworm diet (TM2, 100% replacement of fish meal with TM meal). This study was performed in two phases as phase 1 (d 0 to 14) and phase 2 (d 15 to 28). The pigs in the TM2 treatment showed a greater gain to feed ratio compared with the TM1 pigs in phase 1. Throughout the experimental period, the average daily gain (ADG) of the TM1 pigs was significantly greater than that of the TM2 treatment. The IgG concentration was increased in the TM1 and TM2 treatments compared with the CON pigs in phase 1. In conclusion, the supplementation of TM meal (50% replacement of fish meal) did not show any adverse effects in the performance of weanling pigs, however, 100% replacement of fish meal with TM meal is not recommended.

PSYCHOLOGICAL ASPECT OF PATIENT SATISFACTION AND ACCEPTANCE OF COMPLETE DENTURES (총의치 환자 만족도에 대한 정신의학적 측면)

  • In spite of the progress in techniques and materials of the prosthodontic rehabilitation of patients with complete edentulous arches, prosthodontists still face patients' complaints about dissatisfaction and discomfort from their dentures. In the past several decades, prosthodontists tried to find the factors that influence the patient's satisfaction with dentures. However the results are contraversial especially with the psychological factor. In this study using 'patient denture satisfaction questionnaire' and Hopkins Symptom Checklist, we tried to find the correlation between the patient's denture satisfaction and the patient's psychological aspects. 23 complete edentulous patients who have visited the Department of Prosthodontics, Yonsei University Dental Hospital from September 1998 to June 1999 for complete denture treatment were asked to complete the questionnaires 4 to 6 weeks after the upper and lower complete delivery. After the measurement of validity of the questionnaires, correlations between the patient's general satisfaction with their new upper and lower complete dentures and the other satisfaction questions including the satisfactions with esthetics, retention, mastication, speech, comfort and other people's opinion and between the patient's general satisfaction with their new upper and lower complete dentures and the 5 symptoms of Hopkins Symptoms Checklist including somatic, obsessive-compulsive, interpersonal sensitivity, depression and anxiety were analyzed. Among the several satisfaction questionnaire items, comfort with the lower denture showed highest relation to the patient's general satisfaction with dentures. However, only the anxiety scale of Hopkins Symptoms Checklist among the other symptom scales was related to the patient's general satisfaction with dentures. The two questionaires used in this study turned out to be valid means of analyzing patient's denture satisfaction and psychological status before and after the complete denture treatment.
