Garre's osteomyelitis of the mandible managed by nonsurgical re-endodontic treatment

  • Chronic osteomyelitis with proliferative periostitis, known as Garre's osteomyelitis, is a type of osteomyelitis characterized by a distinctive gross thickening of the periosteum of bones. Peripheral reactive bone formation can be caused by mild irritation or infection. Garre's osteomyelitis is usually diagnosed in children and young adults, and the mandible is more affected than the maxilla. The following is a case report of a 12-year-old female patient with Garre's osteomyelitis of the mandible due to an infection of a root canal-treated tooth. Without surgical intervention, the patient's symptoms were relieved through nonsurgical root canal re-treatment with long-term calcium hydroxide placement. A cone-beam computed tomography image obtained 6 months after treatment completion displayed complete healing of the periapical lesion and resolution of the peripheral reactive buccal bone. Due to the clinical features of Garre's osteomyelitis, which is characterized by thickening of the periosteum, it can be mistaken for other diseases such as fibrous dysplasia. It is important to correctly diagnose Garre's osteomyelitis based on its distinctive clinical features to avoid unnecessary surgical intervention, and it can lead to minimally invasive treatment options.

A survey of the intravenous sedation status in one provincial dental clinic center for the disabled in Korea

  • Background: The objective of the present study was to examine the status of patients who had received dental treatment under intravenous (IV) sedation at Chungnam Dental Clinic for the Disabled in Korea from its inception to the present time, and to review the analysis results. Methods: Retrospective analysis was performed on 305 cases of patients who had received dental treatments under IV sedation between January 2011 and May 2016. The analysis examined the patient's sex, age, primary reason for IV sedation, duration of anesthesia and dental treatment, type of dental treatment performed, number of clinical departments involved in the dental treatment and level of multidisciplinary cooperation, and annual trends. Results: Most dental treatments using intravenous sedation were performed on medically disabled patients or dentally disabled patients with an extreme gag reflex or dental phobia. The mean duration of IV sedation was 72.5 min, while the mean duration of treatment was 58.0 min. The types of dental treatments included surgical treatment (n = 209), periodontal treatment (n = 28), prosthodontic treatment (n = 28), restorative treatment (n = 23), implant surgery (n = 22), endodontic treatment (n = 9), reduction of temporomandibular joint dislocation (n = 1), and treatment of traumatic injuries (n = 1), with treatments mostly performed on adult patients. Conclusions: With increasing demand for minimally painful treatment, cases using IV sedation are on an upward trend and are expected to continue to increase.

A review of the regenerative endodontic treatment procedure

  • Traditionally, apexification has been used to treat immature permanent teeth that have lost pulp vitality. This technique promotes the formation of an apical barrier to close the open apex so that the filling materials can be confined to the root canal. Because tissue regeneration cannot be achieved with apexification, a new technique called regenerative endodontic treatment was presented recently to treat immature permanent teeth. Regenerative endodontic treatment is a treatment procedure designed to replace damaged pulp tissue with viable tissue which restores the normal function of the pulp-dentin structure. After regenerative endodontic treatment, continued root development and hard tissue deposition on the dentinal wall can occur under ideal circumstances. However, it is difficult to predict the result of regenerative endodontic treatment. Therefore, the purpose of this study was to summarize multiple factors effects on the result of regenerative endodontic treatment in order to achieve more predictable results. In this study, we investigated the features of regenerative endodontic treatment in comparison with those of other pulp treatment procedures and analyzed the factors that have an effect on regenerative endodontic treatment.

아주대학교병원 소아치과에서 시행된 전신마취 하 치과치료에 관한 실태조사 (A Survey of Dental Treatment under General Anesthesia in Division of Pediatric Dentistry, Ajou University Hospital)

  • Background: General anesthesia is a necessary method for successful dental treatment for children, compromised patients and the disabled who have difficulty in cooperation. The aim of this study was to assess dental treatment under general anesthesia at the department of pediatric dentistry, Ajou university hospital on children and the disabled. Methods: 217 general anesthesia from June 2010 to June 2014 were assessed for this study. Patient's distribution, treatment pattern, reasons for general anesthesia, distribution of combined operation, duration of anesthesia, treatment, frequency of general anesthesia and agents for general anesthesia were examined. Results: The proportion of male, the disabled were higher and above 19 years age group was the highest. Combined operation with otolaryngology was highest. Main reasons for general anesthesia were mental retardation for the disabled and uncooperative behavior for the non-disabled. Percentage of restorative treatment was the highest. Average anesthesia duration was 186 minutes and average treatment time was 143 minutes. Most of the airway was maintained by nasotracheal intubation and induction was done by sevoflurane. Conclusions: General anesthesia for dental treatment has been increasing for children, the disabled who have a difficulty of cooperation. The demand for dental treatment under general anesthesia is expected to continuously increase. Therefore, continuous research and studies should be done to establish efficiency and safety of general anesthesia and provide an enhanced environment for treatment.

변형된 치관부 파절편 재부착술식을 이용한 치관치근파절의 치료 (Treatment of crown-root fracture with a modified crown fragment reattachment technique)

  • 치관부 파절편 재부착술식은 접착치의학의 발달과 더불어 치관파절 치료 방법 중 하나로 자리매김하였다. 하지만 치관치근파절된 치아에 있어서 기존의 치관부 파절편 재부착술식으로는 치은연하 영역의 수복 및 생물학적 폭경의 유지가 어려워 부가적인 치관연장술 또는 치아 정출술 등이 요구되게 된다. 하지만 이번 증례보고는 치수노출을 동반한 치관-치근파절을 치료함에 있어서 이러한 부가적인 술식 없이 변형된 치관부 파절편 재부착술식만을 이용하여 처치한 사례이다. 환자는 근관치료 후 post 삽입 및 재부착 술식을 시행 받았으며 이때 치관 파절편의 변형을 통해 생물학적 폭경을 위한 공간 유지 및 적절한 접착을 위한 영역을 확보할 수 있었고 이후 한차례의 재파절이 발생하였지만 재차 재부착 후 2년간의 follow up을 통해 치주부착상실 없이 치관 파절편이 잘 유지되는 것이 관찰되었다.

  • 본 증례는 뇌성마비가 있는 혼합치열기의 청소년에서 치간이개를 개선하는 다양한 방법을 모색해보았다. 그 중 복합레진을 이용한 보존적 치료는 이동에 어려움이 있는 뇌성마비 환아에서 최소한의 내원 횟수, 짧은 술식 시간, 경제적이며, 가역적이고 단순한 치료법이다. 이를 통해 높은 심미적 만족도와 발음 및 교합력 개선을 통해 본 환아의 삶을 질을 높일 수 있을 것으로 기대한다.

  • As in all other parts in the body, oral tissue also undergoes dramatic changes with increasing age. Since these changes occasionally go beyond physiological scope, which may result in pathological changes, it is essential for dentist to understand changes caused by normal aging process. With increasing age, tooth morphology and occlusion also varies, especially loss of hard tissue, which is taking place in lifelong time, occurs as a result of tooth wear. When this loss of hard tissue is presented rapidly or excessively, functional and esthetical problems are raised, resulting in lowering quality of life of patient as well as making dental treatment for oral rehabilitation even more complex. Therefore, based on understanding of change in occlusion with increasing age, strategic approaches for maintenance of oral health in both functional and esthetic aspect are required as appropriate restoration and maintenance for progressive tooth wear enables desirable occlusal relationship. Carefully planned-restorative treatment in accordance with changed occlusal relationship is also required in the same context. Instead of taking changes in oral tissue as only a consequence of ageing, it is vital to educate patient and his or her guardian, assuring maintenance of oral hygiene and regular dental check-up are of utmost importance for improved oral health.

  • A major cause of missing permanent incisors is congenital abscence and extraction because of trauma and pathologic condition. The request for restoration of missing or spaced anterior teeth is common in dental practice. Problems, such as the tilting, drifting, and rotation of teeth adjacent to the space, complicate the restoration of apperance, and a normally simple restorative dental procedure may become difficult. There are two primary treatment alternatives to improving a dentition's irregular and spaced apperance-closing the space by orthodontic means or providing a prosthesis to disguise the space. The treatment choice depends on many variables, but, as a general rule, patients with a normal overbite, overjet, and buccal relationship are better treated by maintaining the sapce and providing a prosthesis, either fixed or removable. This case report presents two cases : Traumatic loss of maxillary right and left central incisors, Extraction of malformed mandibular right central inciosr. The loss of central incisor space was regained by the fixed-removable and fixed orthodontic appliance, and then Maryland bridge was cemented.

The effect of zirconia surface architecturing technique on the zirconia/veneer interfacial bond strength

  • PURPOSE. The purpose of this study was to evaluate the effect of the zirconia surface architecturing technique (ZSAT) on the bond strength between veneering porcelain and zirconia ceramic. MATERIALS AND METHODS. 20 sintered zirconia ceramic specimens were used to determine the optimal surface treatment time, and were randomly divided into 4 groups based on treatment times of 0, 1, 2, and 3 hours. After etching with a special solution, the surface was observed under scanning electron microscope, and then the porcelain was veneered for scratch testing. Sixty 3 mol% yttria-stabilized tetragonal zirconia polycrystal ceramic blocks were used for tensile strength testing; 30 of these blocks were surface treated and the rest were not. Statistical analysis was performed using ANOVA, the Tukey post-hoc test, and independent t-test, and the level of significance was set at ${\alpha}=.05$. RESULTS. The surface treatment of the zirconia using ZSAT increased the surface roughness, and tensile strength test results showed that the ZSAT group significantly increased the bond strength between zirconia and veneering porcelain compared to the untreated group (36 MPa vs. 30 MPa). Optimal etching time was determined to be 2 hours based on the scratch test results. CONCLUSION. ZSAT increases the surface roughness of zirconia, and this might contribute to the increased interfacial bond strength between zirconia and veneering porcelain.

Recognition and management of palatogingival groove for tooth survival: a literature review

  • Palatogingival groove (PGG) is an anomaly in the maxillary anterior teeth, often accompanied by the area of bony destruction adjacent to the teeth with no carious or traumatic history. The hidden trap in the tooth can harbor plaque and bacteria, resulting in periodontal destruction with or without pulpal pathologic change. Related diseases can involve periodontal destruction, combined endodontic-periodontal lesions, or separate endodontic and periodontal lesions. Disease severity and prognosis related to PGG depend on several factors, including location, range, depth, and type of the groove. Several materials have been used and recommended for cases of extensive periodontal destruction from PGG to remove and block the inflammatory source and recover the health of surrounding periodontal tissues. Even in cases of severe periodontal destruction, several studies have reported favorable treatment outcomes with proper management. With new options in diagnosis and treatment, clinicians need a detailed understanding of the characteristics, treatment, and prognosis of PGG to successfully manage the condition.
