폐엽절제술 후 견배부 통증을 호소한 폐암환자 치험 1례 (A Clinical Case Report of a Lung Cancer Patient with Shoulder Pain after Lobectomy)

  • Objectives : Quality of life(QoL) in patients with lung cancer usually goes down after pneumoectomy, and the pain is the most common cause. Uncontrolled postoperative pain can restrict the movements of shoulders resulting in frozen shoulder. This case report is to report the effect of herb medicine, Ohjuksan on a 49-year-old female lung cancer patient with shoulder pain after lobectomy. Methods : The patient was treated with Ohjuksan for 10 days. The patient's symptoms were assessed by range of motion(ROM) of shoulder, Visual analogue scale(VAS) of shoulder pain and abdominal diagnosis. Results & Conclusions : The patient showed improvement of ROM, VAS and abdominal diagnosis. As the shoulder pain is relieved from VAS6 to VAS3, ROM of abduction, adduction and internal rotation were increased. Also discomfort of abdomen was disapeared to a certain degree. This suggests that herb medicine, Ohjuksan is effective for treating postoperative pain of shoulder for a Lung cancer patient.

다한증 입원 프로그램을 통해 치료한 소아 및 청소년 수족다한증 11례에 대한 증례보고 (A Case Series Report on 11 Patients of Primary Palmar/Plantar Hyperhidrosis in Children and Adolescents Treated with Hospitalization Program of a Hyperhidrosis Clinic)

  • Objectives : The purpose of this study was to report the clinical effect of short- term admission program on primary palmar/plantar hyperhidrosis in children and adolescents. Methods : Between January 2011 and January 2012, a total of 11 patients with clinical diagnosis of primary palmar/plantar hyperhidrosis were treated with hospitalization which consisted of iontophoresis, electro-acupuncture, herbal medicine, topical therapy and physical therapy. The effects were evaluated as quality of life with dermatology life quality index (DLQI), severity of discomfort using visual analogue scale (VAS), and the patients' subjective satisfaction. We analyzed the patients' improvement before and after admission treatment, as well as six month later. Results : After treatment, the 11 patients' quality of life and severity of discomfort improved significantly. They also were satisfied with oriental treatments for hyper hidrosis. Six months later, 6 of 11 patients had visited the hyperhidrosis clinic. Their symptoms had improved and remained stable. Conclusions : The short-term admission program is effective on primary palmar/plantar hyperhidrosis. Further studies with a larger number of cases will be needed in the future.

Ovalbumin으로 유발된 천식 동물모델에서 GGX의 효과 (Effects of GGX on an Ovalbumin-induced Asthma Mice Model)

  • Objective: The purpose of this study is to evaluate the effects of GGX on an ovalbumin (OVA)-induced asthma mice model. Methods: Balb/c mice were challenged with OVA and then treated with three concentrations of GGX (100, 200, and 400 mg/kg). After sacrifice, the bronchoalveolar lavage fluid (BALF) or lungs of the mice were analyzed by fluorescence-activated cell sorting, ELISA, real-time PCR, H&E, Masson's trichrome, PAS and AB-PAS staining, and immunohistofluorescence staining. Results: GGX significantly inhibited the increase of total cells, immune cells (lymphocyte, neutrophils, macrophage, CD4+, CD8+, CD4+CD69+, CD62L-CD44high+, Gr-1+SiglecF-), and the expression of cytokines (IL-4, IL-5, IL-13, IFN-γ) in BALF. It also significantly inhibited the increase of total cells, immune cells (lymphocyte, neutrophils, eosinophil/macrophage, CD3+, CD19+, CD3+CD193+, CD4+, CD8+, CD4+CD69+, CD62L-CD44high+, and Gr-1+SiglecF-), and the expression of IL-13, TARC, and MCP-1 in lung tissue. GGX decreased the severity of histological lung injury and the expressions of STAT3 and GATA3. Conclusion: This study suggests the probability of using GGX for the treatment of asthma by inhibiting inflammatory immune response.

코로나 19 후 폐섬유화(Post COVID-19 pulmonary fibrosis)에 대한 복합 한의치험 1례 (Korean Medicine Treatment for a Patient with Post-COVID-19 Pulmonary Fibrosis: A Case Report)

  • Background: Post-COVID-19 pulmonary fibrosis (PCPF) is a common complication in severe COVID-19 cases, often associated with acute respiratory distress syndrome or mechanical ventilation. Patients with PCPF frequently experience a decline in their quality of life due to persistent COVID-19 sequelae, including cough and chest pain. However, there is currently no established standard treatment, and the efficacy of existing medications remains uncertain. Case Report: A 65-year-old female patient presenting with cough, dyspnea, chest pain, and fatigue due to PCPF received Korean medicine treatment for 25 days. Symptom evaluation utilized the modified Medical Research Council scale, the Leicester Cough Questionnaire, and the Numeral Rating Scale. Quality of life and functional status were assessed using the Post-COVID-19 Functional Status and the EuroQol 5-Dimensional 5-Level. The extent of pulmonary fibrosis was assessed by comparing chest computed tomography (chest CT) scans before and after hospitalization. Following treatment, the patient demonstrated clinically meaningful improvement in clinical symptoms, enhanced quality of life, and decreased fibrotic lesions on CT scans. Conclusion: This case report suggests that Korean medicine treatment may be effective in improving clinical symptoms, such as cough and dyspnea caused by PCPF, while also enhancing post-COVID-19 quality of life and ameliorating pulmonary fibrotic lesions.

Development of Respiration Sensors Using Plastic Optical Fiber for Respiratory Monitoring Inside MRI System

  • In this study, we have fabricated two types of non-invasive fiber-optic respiration sensors that can measure respiratory signals during magnetic resonance (MR) image acquisition. One is a nasal-cavity attached sensor that can measure the temperature variation of air-flow using a thermochromic pigment. The other is an abdomen attached sensor that can measure the abdominal circumference change using a sensing part composed of polymethyl-methacrylate (PMMA) tubes, a mirror and a spring. We have measured modulated light guided to detectors in the MRI control room via optical fibers due to the respiratory movements of the patient in the MR room, and the respiratory signals of the fiber-optic respiration sensors are compared with those of the BIOPAC$^{(R)}$ system. We have verified that respiratory signals can be obtained without deteriorating the MR image. It is anticipated that the proposed fiber-optic respiration sensors would be highly suitable for respiratory monitoring during surgical procedures performed inside an MRI system.

영상유도 체부정위방사선 치료시 호흡동조를 위한 휴대형 호흡연습장치의 개발 및 유용성 평가 (Development and Utility Evaluation of Portable Respiration Training Device for Image-guided Stereotactic Body Radiation Therapy (SBRT))

  • 본 연구에서는 영상유도 로봇 정위방사선치료장비(Stereotactic Radiation Therapy, SRT) 사이버나이프의 Synchrony 호흡추적장치의 사용에 있어 중요한 요소 중에 하나인 호흡의 안정성을 향상 시키고자 휴대형 호흡연습장치(portable respiratory training device)를 개발하였다. 그래프와 막대 형식의 2가지 디스플레이 중 사용자가 원하는 방식을 선택할 수 있도록 인터페이스를 제작하고, 자신의 호흡주기에 대한 리듬감을 향상 시켜 다음 호흡을 예측할 수 있도록 도와주는 청각시스템을 지원하여 편안한 호흡유도를 제공하였다. 5명의 지원자를 대상으로 자체 프로그램을 통해 검출한 개인고유 호흡주기를 적용하여, '자유호흡(free respiration)'에서 획득한 신호데이터와 시청각시스템을 통해 호흡을 유도하는 '모니터호흡(guide respiration)'의 신호데이터를 획득하고, 호흡주기(period)와 호흡깊이(amplitude)의 편차 평균값을 비교하여 유용성을 평가하였다. 호흡주기의 경우 자유호흡에 비하여 $55.74{\pm}0.14%$로 감소하였고, 호흡깊이의 경우에도 자유호흡의 비해 $28.12{\pm}0.10%$ 감소함으로써 호흡의 규칙성과, 안정성이 향상됨을 확인하였다. 이러한 결과를 바탕으로 개발한 휴대형 호흡연습장치를 이용한 간암, 폐암 등의 체부정위방사선치료에 있어, 호흡 불안정에 의해 발생되는 치료시간의 지연을 줄이고 치료정확도 향상에 도움을 줄 수 있을 것으로 평가되며, 차후 안드로이드(Android)기반의 휴대용단말기를 대상으로 한 호흡연습 어플리케이션 개발에 적용한다면 사용 편의성과 더불어 경제적 효율까지 기대할 수 있을 것으로 판단된다.

Innate immune recognition of respiratory syncytial virus infection

  • Respiratory syncytial virus (RSV) is the leading cause of respiratory infection in infants and young children. Severe clinical manifestation of RSV infection is a bronchiolitis, which is common in infants under six months of age. Recently, RSV has been recognized as an important cause of respiratory infection in older populations with cardiovascular morbidity or immunocompromised patients. However, neither a vaccine nor an effective antiviral therapy is currently available. Moreover, the interaction between the host immune system and the RSV pathogen during an infection is not well understood. The innate immune system recognizes RSV through multiple mechanisms. The first innate immune RSV detectors are the pattern recognition receptors (PRRs), including toll-like receptors (TLRs), retinoic acid-inducible gene-I (RIG-I)-like receptors (RLRs), and nucleotide-biding oligomerization domain (NOD)-like receptors (NLRs). The following is a review of studies associated with various PRRs that are responsible for RSV virion recognition and subsequent induction of the antiviral immune response during RSV infection.

Respiratory syncytial virus prevention in children with congenital heart disease: who and how?

  • Respiratory syncytial virus (RSV) is a major cause of respiratory infection in children. Most of the pediatric population have RSV infection before the age of 2, and recurrent infections are common even within one season. Chronic lung disease, prematurity, along with congenital heart disease (CHD) are major risk factors in severe lower respiratory infection. In hemo-dynamically significant CHD patients with RSV infection, hospitalization is usually needed and the possibility of treatment in intensive care unit and the use of mechanical ventilator support are known to increase. Therefore the prevention of RSV infection in CHD patients is mandatory. The current standard for RSV prevention is immunoprophylaxis by palivizumab. Immunoprophylaxis is recommended monthly in hemodynamically significant CHD patients, up to 5 months. Motabizumab, a second generation drug and newly developing RSV vaccines are also expected to play a key role in RSV prevention in the future. The prophylaxis of RSV infection in CHD patients is cost-effective in both the medical aspect of the patients as well as the socio-economic aspect. Therefore an effort to promote prevention should be made by not only the family of the patients but also by the government.

발성기법중 호흡기 질환이 호흡조절과 공명에 미치는 영향에 관한 영상해부학적 고찰 (The Imaging Anatomical Consideration about Effects of Respiratory Disease on the Breath Control and Resonance in Vocal Technique)

  • This study was undertaken to examine the differences between normal vocal technique and abnormal vocal technique because of diseases in respiratory system(emphysema). The breath control for vocalizing with diaphragmatic respiration must be sustained clearly. But if there is an abnormality in lung(emphysema), it is not sustained to hold diaphragmatic respiration when we sing. In order to examine the differences of the width in diaphragmatic respiration among professional singer's case, non professional case and the case of respiratory disturbance, Shimadzu X-ray remote control TV system was used. The results obtained were summerized as follows: When we vocalize, breath control is very important not only to sustain a resonance but also to form focus for being scattered to cranial resonance. We must know the differences between normal and abnormal diaphrogmatic respiration in order to teach vocal technique right. professional singer's vocal technique with Piaphragmatic respiration was better than that compared with non professional case and respiratory disturbance. Non professional case was very similar to the case of respiratory disturvance in diaphragmatic respiration until 10 sec. after full inhalation, but diaphragmatic respiration in the case of 20 sec. after full inhalation was very different.

Development of Reverse Transcriptase Polymerase Chain Reaction Primer Sets and Standard Positive Control Capable of Verifying False Positive for the Detection of Severe acute respiratory syndrome coronavirus 2

  • Severe acute respiratory syndrome coronavirus (SARS-CoV2) is a major coronavirus that infects humans with human Coronavirus (HuCoV)-229E, HCoV-OC43, HCoV-HKU1, HCoV-NL63, Severe acute respiratory syndrome coronavirus (SARS-CoV) and Middle east respiratory syndrome coronavirus (MERS-CoV). SARS-CoV2 is currently a global pandemic pathogen. In this study, we developed conventional RT-PCR based diagnostic system for the detection of SARS-CoV2 which is relatively inexpensive but has high stability and a wide range of users. Three conventional RT-PCR primer sets capable of forming specific band sizes by targeting the ORF1ab [232 nucleotide (nt)], E (200 nt) and N (288 nt) genes of SARS-CoV2 were developed, respectively, and it were confirmed to be about 10~100 times higher detection sensitivity than the previously reported methods. In addition, a standard positive control that can generate specific amplicons by reacting with the developed RT-PCR primers and verify the false-positiv from contamination of the laboratory was produced. Therefore, the diagnostic system that uses the RT-PCR method is expected to be used to detect SARS-CoV2.
