간암 호흡동조 방사선치료 환자의 호흡신호분석 (Respiratory signal analysis of liver cancer patients with respiratory-gated radiation therapy)

  • 목 적 : 외부표지자 호흡움직임 측정 장치(RPM; Real-time Position Management, Varian Medical System, USA)를 이용한 간암 호흡동조 방사선치료 시 호흡신호와 방사선 조사 시간 및 실제 조사된 호흡위상을 분석하여 호흡움직임 측정 장치를 이용한 호흡동조 방사선 치료의 정확도를 평가하였다. 대상 및 방법 : 2014년 5월부터 9월까지 Novalis Tx.(Varian Medical System, USA)와 RPM을 이용하여 간암 호흡동조 방사선치료(Duty Cycle 20%, Gating window 40% ~ 60%)를 시행한 환자 총 16명의 치료 시 기록된 호흡움직임을 분석하였다. RPM에 기록된 외부표지자의 호흡움직임을 후행적 분석을 통해 호흡위상으로 재구성하였으며, 재구성된 호흡위상을 이용하여 기록된 Beam-on Time과 Duty Cycle에 대해 RPM을 사용한 호흡동조 방사선치료의 예측 정확도를 분석하고, 호흡움직임의 재현성에 따른 Duty Cycle과 예측 정확도의 상관관계를 분석하였다. 결 과 : 대상 환자 16명의 치료계획 시와 실제 치료 시 호흡주기 차이는 평균 -0.03초(범위 -0.50초 ~ 0.09초)로 분석되었으며 두 호흡간의 통계적 차이는 확인할 수 없었다(p=0.472). 치료 시 평균 호흡주기는 4.02 sec (${\pm}0.71sec$), 치료 중 호흡주기 표준편차의 평균값은 7.43%(범위 2.57% ~ 19.20%)로 분석되었다. 실제 Duty Cycle은 평균 16.05%(범위 13.78% ~ 17.41%)로 나타났고 이 중 후행적 분석을 통해 평균 56.05%(범위 39.23% ~ 75.10%)가 계획된 호흡위상(40% ~ 60%)에서 조사되었음을 확인하였다. 호흡주기의 표준편차와 Duty Cycle과 계획된 호흡위상에서 조사된 비율의 상관관계는 각각 -0.156 (p=0.282)와 -0.385 (p=0.070)으로 분석되었다. 결 론 : 본 연구는 실제 치료 중 기록된 외부표지자의 호흡움직임을 후행적으로 분석하여 치료 중 호흡움직임의 재현성 및 Duty Cycle, 계획된 호흡동조창에서의 실제 치료 비율 등을 확인하였다. 4DCT를 이용한 치료계획과의 오차를 최소화하고 효율적인 치료를 위해 호흡훈련 및 호흡신호 모니터링의 강화가 필요 할 것으로 판단된다.

마황윤폐탕(麻黃潤肺湯)과 Ciprofloxacin의 병용(倂用)이 호기성 Gram(-) 세균주(細菌株)에 대한 시험관내(試驗管內) 항균력(抗菌力)에 미치는 영향 (Synergic Effect of Mahwangyounpae-tang and Ciprofloxacin on 5 Strains of Aerobic Gram-negative Bacteria)

  • In order to evaluate the in vitro synergic effect of Mahwangyounpae-tang which was a traditional poly-herbal formula has been used in the treatment of respiratory diseases in oriental medicine, and quinolone antibiotics, ciprofloxacin (CPFX), the minimal inhibitory concentration (MIC), $MIC_{50}$ and MIC90 of single use of quinolones and concomitant treatment with Mahwangyounpae-tang against 5 strains of aerobic gram negative bacteria, Escherichia coli, Klebsiella peumoniae, Hemophilus influenzae, Citrobacter freundii and Pseudomonas aeruginosae. The obtained results were as follows : In the case of aerobic gram negative bacteria, the MIC, $MIC_{50}$ and $MIC_{90}$ against Klebsiella peumoniae and Pseudomonas aeruginosae was significantly decreased in concomitant treated groups with Mahwangyounpae-tang compared to those of single treated groups of CPFX, respectively. However, no significant changes were demonstrated against Echerichia coli, Hemophilus influenzae and Citrobacter freundii. According to these results, it is considered as the in vitro antibacterial activity of CPFX was dramatically increased by concomitant use of Mahwangyounpae-tang against some strains of aerobic gram negative bacteria and the increase and selectivity of antibacterial activities against strains were chosen by the selectivity of Mahwangyounpae-tang not CPFX activity.

  • Objective & Methods: To evaluate the in vitro synergic effect of Eunkyo-san, a traditional poly-herbal formula used in the treatment of respiratory diseases in oriental medicine with quinolone antibiotics, represented by ciprofloxacin (CPFX), which was used in the minimal concentration (MIC), $MIC_{50}$ and $MIC_{90}$. of single use of quinolones in concomitant treatment with Eunkyo-san against 9 strain$ of gram positive bacteria. Results: In. the case of aerobic gram positive bacteria, the MIC, $MIC_{50}$ and $MIC_{90}$ against Staphylococcus aureus, Staphylococcus aureus smith, Staphylococcus epidermidis, Streptococcus pyogenes, Streptococcus pneumoniae Type I, Type II and Type III were significantly decreased in concomitant treated groups with Eunkyo-san compared to those of single treated groups of CPFX, respectively. However, no significant changes were demonstrated against Bacillus subtilis and Enterococcus faecalis. Conclusion: The in vitro antibacterial activity of CPFX were increased against some strains of gram positive strains, especially, pneumococcus such as Staphylococcus and Streptococcus, by concomitant use of Eunkyo-san.

다한증 환자의 한의학적 변증특성 및 자율신경계 기능과의 상관관계 (Correlation between Oriental Medicine Diagnosis and the Autonomic Nervous System Functions of Hyperhidrosis Patients)

  • Objective : Hyperhidrosis is a condition characterized by excessive sweating. Some studies suggest that hyperhidrosis is associated with autonomic nervous system dysfunction. Hyperhidrosis is often accompanied by hypersensitivity, tension, irritability, heat flashes, fatigue, etc. This study was designed to evaluate the correlation between Oriental Medicine diagnosis and the autonomic nervous system function in patients with hyperhidrosis using Heart Rate Variability (HRV) analysis. Methods : 23 palmar and plantar hyperhidrosis patients and 10 systemic hyperhidrosis patients were recruited and evaluated by Oriental Medicine diagnostic questionnaire and HRV analysis. The Oriental Medicine diagnostic questionnaire used a three-dimensional diagnosis that classified patients into Cold or Heat Syndrome, Yin or Yang Syndrome, and Deficiency or Excess Syndrome. Measured indices of HRV used the frequency domain analysis(i.e. TP, VLF, LF, HF and LF/HF ratio). Also, measure indices of HRV adjusted for aging effects were evaluated. Results : Cold/Heat Syndrome was not associated with hyperhidrosis prevalence nor the HRV analysis in hyperhidrosis patients. The Yang Syndrome group(78.8%) was more strongly correlated than the Yin Syndrome group(21.2%), and character of Yang was correlated with the adjusted TP and adjusted LF. The LF/HF ratio in the Excess Syndrome group was significantly higher than the LF/HF ratio in the Deficiency Syndrome group. Conclusion : Hyperhidrosis was not associated with the Cold/Heat Syndrome, but was found to be closely associated with the Yang Syndrome. The LF/HF ratio was significantly higher in the Excess Syndrome group than in the Deficiency Syndrome group in hyperhidrosis patients.

한방병원에 내원한 급성기 이후 코로나-19 환자 15명에 대한 임상적 특징 및 치료 분석 : 사례군 연구 (Effects of Korean Medicine Treatment in Post-acute COVID-19 Syndrome: A Retrospective Case Series of 15 Patients)

  • Objective: The aim of this study was to identify the clinical features of post-acute COVID-19 syndrome and the efficacy and safety of Korean medicine treatment. Methods: This study was conducted on 15 patients with post-acute COVID-19 syndrome who visited the outpatient Allergy, Immune, and Respiratory System Department at Kyung Hee University Korean Medicine Hospital from January 10, 2021 to April 10, 2022. We retrospectively analyzed the charts of 15 patients and collected clinical characteristics, Korean medicine treatments, outcome variables (Numeral Rating Scale (NRS), modified Medical Research Council scale (mMRC), Leicester Cough Questionnaire (LCQ), Quality of Life Visual Analog Scale (QOL-VAS), The Post-COVID-19 Functional Status (PCFS)), adverse events, etc. Results: Of the 15 patients, seven (46.7%) were men, and the average age of all patients was 49.7±13 years. The most common symptom was cough (n=9, 60%), and it was followed by dyspnea or increased respiratory effort, fatigue, insomnia, anosmia, etc. The herbal medicine was prescribed for all 15 patients, and Saengmaek-san (n=8, 53.5%) was the most prescribed. Additionally, acupuncture and cupping were performed in four patients (26.7%) each, and electroacupuncture was applied to one patient (6.7%). As a result of Korean medicine treatment, NRS, mMRC, LCQ, QOL-VAS, and PCFS showed improvement, and adverse events were mild. Conclusions: This study presented the clinical features of post-acute COVID-19 syndrome and suggested that Korean medicine treatment may be effective in alleviating related symptoms and enhancing quality of life.

노인 건강 증진을 위한 호흡근 트레이닝의 코어 안정화 효과 (The core stabilization effect of respiratory muscle training to promote the health of the elderly)

  • This study reviews studies on the core stabilization of respiratory muscle training for the elderly health. Previous research data and presenting basic literature data suggest that respiratory activation is an important mechanism for core strengthening via exercise interventions for the elderly. The review found that first, the mechanism of improving the respiratory muscles weakened by aging to address the loss of core function due to old age sarcopenia among the elderly results entails promoting the autonomic nervous system by focusing on the respiratory muscle activation pattern, the core muscle sensation mobilized for body centering. Second, nerve roots, intraperitoneal pressure, and deep muscles in the trunk of the body can be promoted while controlling respiratory stimulation with cognitive feedback. Effortful inspiration increases the activation of respiratory assistive muscles and effortless exhalation can improve the core muscle mobilization by involving abdominal muscles. Third, through respiratory muscle training, the elderly can increase their awareness of spinal centering and improve the ability to control the deep core muscles that must be mobilized for core stabilization. In conclusion, respiratory muscle training to increase the utilization of the trunk muscles seems to be a useful core stabilization exercise for the elderly with chronic tension and joint degeneration.

일회성 호흡훈련 피드백이 중년여성의 HRV-자율신경시스템 변화에 미치는 영향 (The Effects of Acute Respiratory Training Feedback upon a Change on HRV-Autonomic Nervous System in Middle-aged Women)

  • 본 연구는 일회성 호흡훈련 피드백이 중년여성의 HRV-자율신경시스템 변화에 미치는 영향을 분석고자 하였다. 연구대상자는 중년여성(40-60세) 총 24명을 호흡군 12명, 통제군 12명으로 무선할당하여 일회성 호흡 훈련을 실시하였다. 호흡군의 피드백 훈련은 총 15분 동안 실시되었으며, 10분 간 전문가 지도에 따른 호흡 자각 훈련 후 5분 간 자율호흡 훈련을 실시하였다. 자료의 분석은 SPSS WIN 20.0으로 반복측정 변량분석(Repeated Measures ANOVA)을 실시하였으며, 이를 통해 얻어진 결론은 다음과 같다. 중년여성들은 일회성 호흡 훈련 후 SDNN, RMSSD, LF, HF가 유의하게 높아졌으며, 호흡군이 통제군보다 SDNN, RMSSD, LF, HF이 유의하게 높아진 것으로 나타났고, Mean HR과 LF/HF는 집단과 시기의 주효과, 집단과 시기의 상호작용효과 모두 유의한 차이는 나타나지 않았다. 이상의 결과로 일회성 호흡훈련 피드백은 중년여성들의 SDNN, RMSSD, 교감활성, 부교감활성에 효과적이며, 이러한 호흡 훈련 프로그램은 자율신경계 균형 능력을 향상시켜 중년여성들의 스트레스 완화 및 자율신경실조증을 회복할 수 있는 운동중재의 효용 가치를 기대할 수 있다고 사료된다.

기상 및 대기질이 건강취약집단에 미치는 영향 (Effects of Meteorology and Air Pollution on Respiratory Disease affecting Vulnerable Populations)

  • It is well known that atmospheric environments, including both meteorology and air quality, significantly affect public health, such as chronic lung disease and cancer, and respiratory infections. In this study, we have analyzed correlations between the number of daily respiratory outpatients and the atmospheric environments data for about ten years for the city of Busan, South Korea. The respiratory problem patients data have been categorized into two health-vulnerable groups by age over 65(DayPA_O65) and under 20(DayPA_U20), each of which shows relatively higher correlations with air quality and meteorology, respectively. However, time series analysis with factor separation results in that DayPA_O65 and DayPA_U20 show a higher relation with variance components and daily irregular factors of atmospheric concentrations, respectively.

Effects of aqueous extracts from Lonicera japonica and Tussilago farfara on RAW 264.7 Macrophages

  • Inhalational drug is an attractive modality for local therapy of pulmonary diseases as well as systemic drug delivery. Flower of Lonicera japonica (FLJ) and flower of Tussilag farfara (FTF) are medicinal herbs for respiratory disease in traditional Korean medicine. As a preliminary study for effective inhalable formulation of FLJ and FTF, this study was to provide the toxicity and anti-inflammatory effect on murine macrophages. The dried FLJ and FTF were extracted with distilled water, filtered and freeze-dried. After treatment with FLJ and FTF extract on RAW 264.7 cells, the cell viabilities were measured by MTT assay. FLJ and FTF did not show cytotoxicity on RAW 264.7 cells. LPS stimulated RAW 264.7 cells were treated with 3 and $30\;{\mu}g/ml$ of FLJ or FTF. FLJ and FTF did not inhibit TNF-a and IL-6 secretion in both concentration of treatment. We suggest that FLJ and FTF may be useful drugs for respiratory disease. Future work will focus on the physical characteristics for inhalable formulation.
