인체 호흡 모사를 위한 기계적 장치 연구 (The Study of Mechanical Simulation for Human Respiratory System)

  • A patient with respiratory disorders such as a sleep apnea is increasing as the obese patient increase on the modern society. Positive Airway Pressure (PAP) devices are used in curing patient with respiratory disorders and turn out to be efficacious for patients of 75%. However, these devices are required for evaluating their performance to improve their performance by the mechanical breathing simulator. Recently, the mechanical breathing simulator was studied by the real time feedback control. However, the mechanical breathing simulator by an open loop control was specially required in order to analyze the effect of flow rate and pressure after operating the breathing auxiliary devices. Therefore the aims of this study were to make the mechanical breathing simulator by a piston motion and a valve function from the characteristic test of valve and motor, and to duplicate the flow rate and pressure profiles of some breathing patterns: normal and three disorder patterns. The mechanical simulator is composed cylinder, valve, ball screw and the motor. Also, the characteristic test of the motor and the valve were accomplished in order to define the relationship between the characteristics of simulator and the breathing profiles. Then, the flow rate and pressure profile of human breathing patterns were duplicated by the control of motor and valve. The result showed that the simulator reasonably duplicated the characteristics of human patterns: normal, obstructive sleep apnea (OSA), mild hypopnea with snore and mouth expiration patterns. However, we need to improve this simulator in detail and to validate this method for other patterns.

Characteristics of Active Tuberculosis Patients Requiring Intensive Care Monitoring and Factors Affecting Mortality

  • Background: One to three percent of cases of acute tuberculosis (TB) require monitoring in the intensive care unit (ICU). The purpose of this study is to establish and determine the mortality rate and discuss the causes of high mortality in these cases, and to evaluate the clinical and laboratory findings of TB patients admitted to the pulmonary ICU. Methods: The data of patients admitted to the ICU of Yedikule Chest Diseases and Chest Surgery Education and Research Hospital due to active TB were retrospectively evaluated. Demographic characteristics, medical history, and clinical and laboratory findings were evaluated. Results: Thirty-five TB patients (27 males) with a median age of 47 years were included, of whom 20 died within 30 days (57%). The Acute Physiology and Chronic Health Evaluation II (APACHE II) and Sequential Organ Failure Assessment (SOFA) scores were significantly higher, and albumin and $PaO_2/FIO_2$ levels were significantly lower, and shock, multiple organ failure, the need for invasive mechanical ventilation and drug resistance were more common in the patients who died. The mortality risk was 7.58 times higher in the patients requiring invasive mechanical ventilation. The SOFA score alone was a significant risk factor affecting survival. Conclusion: The survival rate is low in cases of tuberculosis treated in an ICU. The predictors of mortality include the requirement of invasive mechanical ventilation and multiple organ failure. Another factor specific to TB patients is the presence of drug resistance, which should be taken seriously in countries where there is a high incidence of the disease. Finding new variables that can be established with new prospective studies may help to decrease the high mortality rate.

Evaluation of Respiratory Protection Program in Petrochemical Industries: Application of Analytic Hierarchy Process

  • Background: Respiratory protection equipment (RPE) is the last resort to control exposure to workplace air pollutants. A comprehensive respiratory protection program (RPP) ensures that RPE is selected, used, and cared properly. Therefore, RPP must be well integrated into the occupational health and safety requirements. In this study, we evaluated the implementation of RPP in Iranian petrochemical industries to identify the required solutions to improve the current status of respiratory protection. Methods: This cross-sectional study was conducted among 24 petrochemical industries in Iran. The survey instrument was a checklist extracted from the Occupational Safety and Health Administration respiratory protection standard. An index, Respiratory Protection Program Index (RPPI), was developed and weighted by analytic hierarchy process to determine the compliance rate (CR) of provided respiratory protection measures with the RPP standard. Data analysis was performed using Excel 2010. Results: The most important element of RPP, according to experts, was respiratory hazard evaluation. The average value of RPPI in the petrochemical plants was $49{\pm}15%$. The highest and lowest of CR among RPP elements were RPE selection and medical evaluation, respectively. Conclusion: None of studied petrochemical industries implemented RPP completely. This can lead to employees' overexposure to hazardous workplace air contaminants. Increasing awareness of employees and employers through training is suggested by this study to improve such conditions.

  • The effects of continuous administration of propofol on the blood, liver and kidney of dogs were evaluated. Propofol(5mg/kg) was repeatedly administered to 8 mongrel dogs intravenously for consecutive seven days. Time to recovery, body temperature, heart rate, and respiratory rate were measured and recorded during experimental period. Hematology (WBC, RBC, PCV) and serum chemistry (AST, ALT, BUN, Creatinine) were also monitored for eight days. Time to standing significantly decreased on day 4 and 6 compared with that on day 1. Body temperature 5 minutes after injection on day 5 and 6, and respiratory rate before injection on day 2, 3, 5 and 6 were significantly increased, respectively. No significant changes were observed in total WBC counts, total RBC counts and PCV values. The AST and AIT values were decreased on day 2 compared with those on day 1, but were within clinically normal range. There were no significant changes in BUN and Creatinine levels. This study suggests that consecutive day propofol anesthesia in dogs can be used for the purpose of the treatment and restraint.

당진지역 돼지생식기호흡기증후군(PRRS) 항체가 조사 (Seroprevalence of porcine reproductive and respiratory syndrom(PRRS) in Dangjin)

  • The purpose of this study was sero-epidemiological survey of porcine reproductive and respiratory syndrom(PRRS) in Dangjin area. 411 samples from 26 pig farms were analyzed by enzyme linked immunosorbant assay (ELISA). The data indicate that 66% of the pigs and 92% of the farms showed sero-positives to the PRRS viruc. Sows showed 58% of sero-positive rate and fattening pigs showed 85% of seropositive rate. The rate of sero-positive in boars was 63%. No significant regional differences were detected in sero-epidemiological survey.

파킨슨 환자의 클리어 스피치 전후 음향학적 공기역학적 특성 (An aerodynamic and acoustic characteristics of Clear Speech in patients with Parkinson's disease)

  • An increase in speech intelligibility has been found in Clear Speech compared to conversational speech. Clear Speech is defined by decreased articulation rates and increased frequency and length of pauses. The objective of the present study was to investigate improvement in immediate speech intelligibility in 10 patients with Parkinson's disease (age range: 46 to 75 years) using Clear Speech. This experiment has been performed using the Phonatory Aerodynamic System 6600 after the participants read the first sentence of a Sanchaek passage and the "List for Adults 1" in the Sentence Recognition Test (SRT) using casual speech and Clear Speech. Acoustic and aerodynamic parameters that affect speech intelligibility were measured, including mean F0, F0 range, intensity, speaking rate, mean airflow rate, and respiratory rate. In the Sanchaek passage, use of Clear Speech resulted in significant differences in mean F0, F0 range, speaking rate, and respiratory rate, compared with the use of casual speech. In the SRT list, significant differences were seen in mean F0, F0 range, and speaking rate. Based on these findings, it is claimed that speech intelligibility can be affected by adjusting breathing and tone in Clear Speech. Future studies should identify the benefits of Clear Speech through auditory-perceptual studies and evaluate programs that use Clear Speech to increase intelligibility.

손씻기 교육이 간호사의 손씻기 행위와 중환자의 호흡기로의 MRSA 분리율 감소에 미치는 영향 (Effects on Nurses' Hand Washing Behavior and Reduction of Respiratory Isolation Rate of MRSA of the Hand Washing Education)

  • Purpose: This is a clinical experimental study aimed to provide basic data to ensure quality care. The purpose of this study is to increase nurses hand washing behavior and decrease respiratory isolation rate of MRSA. Method: This study was conducted in Kangnam St. Marys hospital. The subjects included 5 nurses working at the NS ICU and 165 specimen for nasal swabs and 46 specimen for sputum cultures from 106 patients admitted to the NS ICU. The data were collected from June to September 2000. Results: 1.The percentage of the nurses hand washing was 15.6percent but was improved to 46.5 percent after the educational intervention. In particular, hand washing behavior was increased in situations such as after suctioning, before parenteral nutrition, and after providing hygiene care. 2. Results of nasal swab showed that MRSA isolation rate was reduced from 42.8 percent before the education to 18.6percent after the education. The sputum culture results also showed that the MRSA rate was reduced from 40.7percent before the education to 34.6percent after the education. Conclusion: The findings showed the nurses' strict hand washing behavior and use of disposable gloves and paper towers as well as use of hand sterilization spray by visitors or families can decrease the MRSA isolation rate in the NS ICU patients. Since the NS ICU patients may have respiratory complications due to long-term hospitalization, the nurses regular hand washing is important enough to be emphasized. Future research should be focused on the impact of nurses' hand washing behavior on the incidence of pneumonia, an iatrogenic infection.

Positive impact of integrated amrita meditation technique on heart rate, respiratory rate and IgA on young healthy adults

  • The objective of the current study was to find out the effect of Integrated Amrita Meditation Technique (IAM) on blood pressure (BP), heart rate (HR), respiratory rate (RR) and IgA. One hundred and fifty subjects were randomized into three groups IAM, Progressive Muscle Relaxation (PMR) and Control. Baseline values were collected before giving the training for all the subjects and the IAM and PMR groups were given training in the respective techniques. BP, HR, RR and IgA were recorded manually at 0 h, 48 h, 2 months and 8 months after the first visit. HR was found to be reduced in the IAM group 48 h onwards and the fall sustained till 8 months (p < 0.05). IAM group showed significant drop when compared to the PMR group and control group in all the subsequent visits (p < 0.05). RR decreased significantly in the IAM group in the third and fourth visits (p < 0.05). RR of IAM showed significant decrease when compared to PMR and control from the third visit onwards. IgA showed significant increase in comparison with PMR and control in the third and fourth visits. BP did not show any difference in any of the visits. There was subject dropout from randomization to completion of the study, in all the three groups. The significant decrease in HR and RR and increase in IgA in the IAM group when compared to the PMR and control group shows the efficacy of the technique in reducing the physiological stress indicators for up to 8 months.

돼지 생식기호흡증후군 바이러스의 항체분포 및 역학조사 (Seroprevalence and epidemiological analysis of porcine reproductive and respiratory syndrome virus in Korea)

  • A nation wide sero-epidemiological survey of porcine reproductive and respiratory syndrome(PRRS) was carried out to analyze the current status of the PRRS virus infections in the field using the indirect immunofluorescent antibody assay(IFA) with the field isolate PL96-1. Since the first report of the antibody detection to PRRSV in 1993, the prevalence of seropositive pigs has increased dramatically and the data indicate that over 21% of the pigs and around 60% of the farms showed seropositives to the PRRS virus. A slightly higher positive rate was recognized in breeders than fattenings and it might be due to the higher age at the time of testings. No significant regional differences were detected in the sero-epidemiological survey. Higher sero-positive rate in growers indicates that PRRSV infection in the field was common after weaning(around 40 days). However, the number of seropositive pigs were declined in fattening pigs. Sows showed around 26% of sero-positive rate that there is a higher chance of continuous virus circulation in the infected farms. Low rate of sero-positivity in boars(9.8%) implies that there is high demand in proper control measures to prevent virus spreading through breeding procedures such as natural or artificial insemination. Therefore it was concluded that PRRSV infection in domestic swine herds is endemic and the positive rate and economic loses will be increased by spontaneous infections in naive farms.

개에서 주사 및 도입마취제로서 Propofol의 마취효과 및 심폐기능에 미치는 영향 (Anesthetic and Cardiopulmonary Effects of Propofol as Infusion and Induction Anesthesia in Dogs)

  • To investigate the cardiopulmonary and anesthetic effects of propofol in dogs, experimental dogs were randomly divided into 4 groups (propofol infusion anesthesia, P/INF, propofol intermittent anesthesia, P/INTER, propofol induction anesthesia, P/ISO, thiopental Na induction anesthesia, T/ISO) and monitored analgesic and anesthetic effects, recovery time, body temperature, heart rate, mean arterial pressure, respiratory rate, systolic and diastolic pressure. In all groups, apnea was not observed. In the P/INF group, the respiratory rate(RR) was slightly decreased, but in the P/INTER group, RR was increased and shallowing. In the groups of P/ISO and T/ISO, the respiratory rate was decreased. Heart rate(HR) was increased after induction anesthesia in all groups, but gradually decreased. Mean arterial pressure(MAP) was decreased after injection anesthesia in the groups of P/INF and P/INTER. In the groups of P/ISO and T/ISO, however, MAP was slightly increased. Systolic and diastolic arterial pressure were gradually decreased after induction anesthesia, but not significantly. In the groups of P/INF and P/ISO, recovery time was shorter than the groups of P/INTER and T/ISO. In all groups, body temperature of animals was decreased gradually according to time but no significant changes were observed. Propofol injection doesn't make the complete loss of responses of animals, especially, in the P/INTER group. In the P/INF group, deep pain was present until the end of anesthetic period. During recovery period, any other side effects except incoordination were not monitored. The present study suggested that infusion anesthesia was superior to intermittent anesthesia as injection anesthetic agent, and propofol was better than thiopental Na as induction anesthetic agent.
