  • The study is designed to develop an educational CD-Title for Basic Life Support. The study is conducted from July to December, 2002, Based on the course of program development suggested by Dick and Cray. the study followed the planning, development, education and evaluation of a program. The developed CD-Title consists of 8 parts. 1. Intro include Introduction, Adult CPR, Child CPR, Infant CPR, Adult Foreign-Body Airway Obstruction, Infant Foreign-Body Airway Obstruction, and Exercise in Menu at Right of screen. 2. Introduction consist of death process at cardiac arresst, chain of survival, introduction of CPR, respiratory & ciculatory anatomy and physiology. 3. Adult CPR consist of assessment responsiveness, activating EMS system, position victim, opening a airway, checking for breathing, rescue breathing, checking for sign of circulation, chest compression, 1 person & 2 persion adult CPR video. 4. Child & Infant CPR consist of, causes, assessment responsiveness, position victim, opening the airway, checking for breathing, rescue breathing, checking for sign of circulation, chest compression, activating EMS system, child & infant CPR video. 5. Adult Foreign-Body Airway Obstruction include assessment responsiveness, activating EMS system, position victim, opening a airway, checking for breathing, 2 rescue breathing, reopening the airway, 2 rescue breathing, finger sweep, 5 abdominal thrusts, unresponsiveness adult Foreign-Body Airway Obstruction video. 6. Infant Foreign-Body Airway Obstruction consist of, causes, assessment responsiveness, position victim, opening the airway, checking for breathing, 2 rescue breathing, reopening the airway, 2 rescue breathing, checking foreign-body object in oral cavity of victim, 5 back blow, 5 chest thsusts, activating EMS system, video. 7. 32 exercise consist of 8 Adult CPR, 12 Child & Infant CPR, 5 Adult Foreign-Body Airway Obstruction, 7 Infant Foreign-Body Airway Obstruction. Every part consist of animations to trigger students interests. This CD-Title will be useful education for first responders and lay rescuers.

Nonspecific Mouse Hepatitis Virus Positivity of Genetically Engineered Mice Determined by ELISA

  • Mouse hepatitis virus (MHV) is a major pathogen in laboratory mice that usually leads to fatal diseases, such as hepatitis, multiple sclerosis, encephalitis, and respiratory disease. MHV has a high infection rate, and it needs to be detected as soon as possible to prevent its spread to other facilities. However, MHV detection by enzyme-linked immunosorbent assay (ELISA) often gives false positives; thus, it is very important that the results are confirmed as true positives in the early infection stage or distinguished as false positives with more accurate, reliable methods. Under microbiological screening, MHV ELISA-positive mice were found in four GFP-tagging transgenic mice. To verify the detection of the MHV antigen directly, reverse transcription polymerase chain reaction (RT-PCR) was performed, and the mice were determined to be MHV negative. Additional serum antibody-based screening was conducted with three different ELISA kits, and multiplexed fluorometric immunoassay (MFIA) was performed to confirm their accuracy/sensitivity. In brief, the ELISA kit for A59 nucleocapsid protein (MHV-A59N) revealed MHV ELISA positivity, while other ELISA kits (MHV-S lysate and MHV-JHM lysate) demonstrated MHV negativity. In MFIA, only the test for the recombinant A59 nucleocapsid antigen was MHV positive, which was consistent with the ELISA results. These results suggest that the ELISA kit with the recombinant A59 nucleocapsid antigen might induce non-specific MHV ELISA positivity and that confirmation is therefore essential.

Polyhydroxyalkanoate Chip for the Specific Immobilization of Recombinant Proteins and Its Applications in Immunodiagnostics

  • In this study, a novel strategy was developed for the highly selective immobilization of proteins, using the polyhydroxyalkanoate (PHA) depolymerase substrate binding domain (SBD) as an active binding domain. In order to determine the appropriacy of this method for immunodiagnostic assays, the single-chain antibody (ScFv) against the hepatitis B virus (HBV) preS2 surface protein and the severe acute respiratory syndrome coronavirus (SARS-CoV) envelope protein (SCVe) were fused to the SBD, then directly immobilized on PH A-coated slides via microspotting. The fluorescence-labeled HBV antigen and the antibody against SCVe were then utilized to examine specific interactions on the PHA-coated surfaces. Fluorescence signals were detected only at the spotted positions, thereby indicating a high degree of affinity and selectivity for their corresponding antigens/antibodies. Furthermore, we detected small amounts of ScFv-SBD (2.7 ng/mL) and SCVe-SBD fusion proteins (0.6ng/mL). Therefore, this microarray platform technology, using PHA and SBD, appears generally appropriate for immunodiagnosis, with no special requirements with regard to synthetic or chemical modification of the biomolecules or the solid surface.

Rhodococcus equi Infections in Foals (Rhodococcus equi에 의한 망아지 감염증)

  • This work described 3 infection cases caused by Rhodococcus equi in foals between 3 and 5 months of age. The disease histories were not fully taken from local veterinarians. At least 1 sick foal has been treated with cephalothins followed by penicillins during approximately 1 week, but died without effectiveness and other foals rearing with the animal have been suffering from severe pneumonia which show high fever, laboring respiration, cough and/or nasal discharge. There were many abscessations into lungs of 2 foals in postmortem examination and another 1 sample was pus collected from abscess around the shoulder, indicating the osteomyelitis. Those bacteria were grampositive coryneform and were identified as a R. equi by a polymerase chain reaction (PCR) using primers for R. equi-specific vapA gene. The pathogens were usually resistant to penicillin, ampicillin, amoxycillin/clavulanic acid, cefazolin, clindamycin, sulfamethoxazol/trimethoprim, kanamycin, and tetracycline, while were sensitive to ciprofloxacin, norfloxacin, orfloxacin, gentamicin, erythromycin, neomycin, and vancomycin. Some more foals with respiratory symptoms in 1 horse farm were treated by orally administration with erythromycin during 2 weeks. Because the combination of erythromycin and rifampin has recommended as the treatment for R. equi infections in foals, the local equine veterinarian can choose those antibiotics for the treatment of this disease in future. However, another antimicrobial agent may be necessary if R. equi resistant to both agents is isolated.

Multifocal interstitial nephritis of pigs slaughtered in Jeju (제주지역 도축돈의 간질성 신염)

  • Total 160 head of porcine kidneys were examined for gross and histopathological lesions and polymerase chain reaction (PCR) for porcine circovirus type 2 (PCV-2), porcine parvovirus (PPV), Leptospira species and porcine reproductive and respiratory syndrome virus (PRRSV). Grossly, 137 kidneys (85.6%) had lesions characterized by the presence of the scattered white foci. Microscopically, multifocal interstitial nephritis, which classified into 4 grades such as, no lesion (Score 0), mild lesion (Score 1), moderate lesion (Score 2) and severe chronic lesion (Score 3) with fibrosis, was observed in 159 cases (99.4%). The histopathologic mean score for multifocal interstitial nephritis was significantly different (P<0.05) between the cases of PCV-2 single infection and the cases of co-infection with PCV-2 and PPV. According to PCR evaluation, PCV-2 were detected in 73.8% (118 cases), PPV were in 66.9% (107 cases), however Leptospira spp. and PRRSV were negative in all kidneys. Both PCV-2 and PPV were detected in 52.5% (84 cases). In 84 cases co-infected with PCV-2 and PPV, the occurrence of lymphoid follicle and vasculitis were observed as 65.5% (55 cases) and 26.2% (22 cases), respectively. These results revealed that PCV-2 and PPV were major infectious agents for interstitial nephritis in slaughtered pigs, Jeju. And the histopathologic lesions of multifocal interstitial nephritis were more severe in the case co-infected with PCV-2 and PPV.

Molecular Prevalence of Cryptosporidium spp. among Companion Birds Kept in Pet Shops in Japan

  • Cryptosporidium is the most common protozoan that can infect a wide range of animals, including mammals and birds. Avian Cryptosporidium spp. can cause enteric and respiratory diseases which can be fatal in birds and some species are zoonotic. Companion birds have the potential as reservoir due to their close contact with humans. Pet shops are the major source of companion birds. However, few reports are available regarding Cryptosporidium spp. infection among companion birds kept in pet shops. The present study reports the prevalence and molecular characteristics of Cryptosporidium spp. among companion birds kept in pet shops in Japan. A total of 265 fresh fecal samples were obtained from birds kept in 4 pet shops; these birds belonged to 41 species in 3 bird orders. A nested polymerase chain reaction (PCR) assay targeting the small subunit rRNA gene was employed for the detection of Cryptosporidium spp. A total of 24 samples (9.1%) were positive, and Cryptosporidium spp. were detected from all pet shops. The prevalence of Cryptosporidium spp. in each of the bird orders was 6.5% (10/153) in Psittaciformes, 14.4% (13/90) in Passeriformes, and 4.5% (1/22) in Galliformes. Based on sequence analysis, 13 (54.2%) isolates were classified to C. galli, 8 (33.3%) were avian genotype III, and the remaining 3 (12.5%) were C. baileyi. No infection with zoonotic C. meleagridis and no coinfection with multiple Cryptosporidium spp. and/or genotypes were observed. The zoonotic potential of Cryptosporidium spp. infecting companion birds kept in pet shops in Japan is likely to be low.

PCR technique for detection of toxigenic Pasteurella multocida in mixed bacterial cultures from pigs (Polymerase chain reaction을 이용한 독소생산성 Pasteurella multocida의 검출)

  • Pasteurella multocida is kind of commensal bacteria in the upper respiratory tract of pigs. It is classified toxigenic and nontoxigenic strains based on the production of dermonecrotic toxin. Toxigenic strain is most associated with atrophic rhinitis which brings great economical loss in swine industry. However, toxigenic and nontoxigenic strains do not differ by diagnostic biochemical reaction or morphology. One of recently developed techniques, PCR detects the toxigenic P multocida. Amplification of an 846-nucleotide fragment of toxA gene was developed. The fragment amplified by PCR was detected in P multocida type D not type A. The PCR amplification was as sensitive as it could detect 1 pg of P multocida DNA. We compared the result of the PCR with the enzyme linked immunosorbent assay (ELISA) in a test for 40 swine nasal swabs. All of these isolates were toxin negative based on the ELISA while 2 isolates were detected in the PCR technique. in addition to accuracy, as required for rapid detection from contaminated nasal swabs, toxigenic P multocida was recovered efficiently from contaminated culture without inhibition of the PCR. The results show that the PCR detection of toxigenic P multocida directly form nasal swabs are feasible.

Efficacy of atropic rhinitis vaccine in pigs (돼지 위축성 비염백신의 효과에 관한 연구)

  • Atropic rhinitis (AR) is one of major respiratory diseases in pigs. AR causes a great economic losses and is considered to be a multifactorial disease in which herd management, heredity, and environment. Several vaccines against have been developed commercially and used in pig farms but the efficacy of each vaccine is still questionable. In this study, one of commercial AR vaccines, which contains inactivated Bordetella bronchiseptica, Pasteurella multocida type D and their toxoid was evaluated for vaccine efficacy by challenge test. Twenty piglets were divided into four groups as follows; group I was piglets from vaccinated sows (twice before parturition); group II was piglets from vaccinated sows (same as group I) and were vaccinated at 1 day old; group III and IV were piglets without any vaccination. Groups I, II, and III were challenged by intranasal instillation of $5.3{\times}10^7$ CFU of B bronchiseptica twice and $1{\times}10^9$ CFU of P multocida five times. Group IV was control group without any vaccination and any challenge. We compared serological results, recovery rate of P multocida by polymerase chain reaction, clinical signs and pathological findings between vaccinated groups and unvaccinated groups for efficacy of the vaccine, Serological responses against B bronchiseptica and toxigenic P multocida type D were not showed evident discrepancy between vaccinated groups and unvaccinated groups assuming that the antibody responses against the vaccine is very delayed. However, growth rate, clinical signs and snout lesion grading in vaccinated groups showed more favorable than those in unvaccinated group. Therefore, AR vaccination in this study is considered to be effective in the prevention of AR in pigs.

Development of DNA-Based Assessment Method for Mold in Floor Dust of Dwellings in Korea (바닥 먼지내에서 DNA 기반 곰팡이 분석기법 개발)

  • Objectives: Much scientific evidence indicate a positive association between moldy environments and respiratory illnesses and/or symptoms. However, few comprehensive assessments of mold have been performed for such settings. Spore counts or microscopic enumeration only may not be sufficient for evaluating fungal exposure. Recently, Mold Specific QPCR technology developed by the US EPA (Environmental Relative Moldiness Index, ERMI) has been widely used worldwide and great performance for assessing fungal exposure has been shown. Methods: We aimed to develop a Korean version of ERMI suitable for the distribution of fungal flora in Korea. Thirty dwellings in the Seoul and Incheon area were selected for sampling, and each was classified as 'Flooded, 'Water-damaged' or 'Non-water-damaged'. Results: Dust on the floor and airborne sampling were collected using an MAS100 and a 'Dustream' collector. Samples were analyzed by quantitative polymerase chain reaction(QPCR) for the 36 molds belonging to ERMI. Student t-test and ANOVA tests were carried out using SAS software. The median ERMI values of flooded, water damaged, and non-water damaged dwellings were 8.24(range: -5.6 to 27.9), 5.47(-25. 4 to 32.7), and -15.30(-24.6 to 14.8), respectively. Significant differences were observed between flooded and non-water damaged dwellings (P=0.001) and between water-damaged and non-water damaged dwellings (P=0.032). Conclusion: Our findings indicate that ERMI values attributed to dust samples in Korea could be applicable for the identification of flooded or water damaged buildings. However, much data is needed for continuously developing the Korean version of ERMI values.

A Patient with Mitochondrial Myopathy who Experienced Diabetic Ketoacidosis with Auto-antibody (사립체 근병증 환자에서 발생한 자가항체 양성의 당뇨병성 케톤산증 1례)

  • Mitochondrial myopathy results from a primary dysfunction of the respiratory chain and is frequently accompanied with endocrine manifestations. Among the endocrine manifestations of mitochondrial disease, diabetes mellitus is relatively common. Diabetes mellitus in the mitochondrial myopathy is usually insulin dependent due to the defect in insulin secretion resulted from mitochondrial dysfunction. But it is seldom manifested as diabetes ketoacidosis and doesn't usually have an auto-antibody. We report a patient with mitochondrial myopathy who was diagnosed as having diabetes mellitus by presenting as diabetes ketoacidosis and had both of the auto-glutamic acid decarboxylase (GAD) antibody and anti-insulin auto-antibody.
