Evaluation of the Quality of the Case Reports in Journal of Korean Medicine from Year 2015 to 2020 : Using CARE Guidelines (CARE 지침에 따른 대한한의학회지의 증례 보고에 대한 질 평가 연구: 2015년~2020년 증례 보고를 중심으로)

  • Objectives: The purpose of this study is to assess the quality of the Journal of Korean medicine case reports. Methods: Case reports published in the Journal of Korean Medicine from January 2015 to March 2020 were selected by using Oriental Medicine Advanced Searching Integrated System (OASIS) and the journal search system of the Korean Medical Association (https://www.jkom.org). The quality of the case reports was assessed using the Consensus-based Clinical Case Reporting Guideline Development (CARE) guidelines. Results: 33 case reports were selected for the assessment. Based on the CARE guidelines, 61.54% of the case reports included necessary information, but the quality level was uneven. More than 60% of the reports were missing data regarding 'Discussion of the strengths and limitations in your approach to this case', 'Intervention adherence and tolerability', 'Timeline', 'Medical, family, and psychosocial history including relevant genetic information', 'Patient perspective or experience', 'Adverse and unanticipated events', 'Administration of intervention', and 'De-identified demographic information and other patient specific information'. In most reports of over 90%, data regarding 'Diagnostic challenges', 'Intervention adherence and tolerability', and 'Key word' were not included. Conclusions: Efforts are needed to improve the quality of case reports in the Journal of Korean Medicine, and it is necessary to develop appropriate guidelines for case reporting for the Journal of Korean Medicine. In addition, all articles submitted to the Journal of Korean Medicine are to be complied with submission instructions and CARE guidelines.

  • A software configuration management(SCM) is essential for processing large scale project. The scope of SCM involves each developer's work space management, software building management, and development process control as wet] as version control. In this paper we focus on what parts should be controlled systematically for the optimized software build that is an important part of the SCM. We also analyze that to increase the efficiency of software build, what kind of configuration management information should be reported. Based on the analysis, we have actually designed and implemented a new reporting system and evaluated it. The evaluation includes comparative evaluation in efficiency based on the analysis about the related functions provided by existing tools and some additional qualitative evaluation through the questionnaires from stakeholders.

Near Misses Experienced at a University Hospital in Korea

  • Objectives: This study aimed to investigate how many healthcare professionals experienced near misses, what types of near misses occurred most often, and healthcare professionals' opinions about near misses at one university hospital in Korea. Methods: The authors developed a questionnaire including 26 core types of near misses and 4 questions about preventability and reporting barriers. The survey was conducted from Oct. 31st to Nov. 18th 2011, about 3 weeks, using a self-administrated questionnaire that was administered to 697 healthcare professionals (registered nurses, pharmacists, technicians, and nurses aides) who worked at a university hospital. Medical doctors and employees working in the department of administration were excluded. Results: About half of hospital workers experienced at least one or more near misses during the past one year. The drug dispensing process was the most common subcategory of near misses. Among the 26 items, patient falls was highest. Over 95% of respondents reported that the near miss they experienced was preventable. Also, more than half of respondents did not report the near miss and the main reason for omission was fear of blame. Conclusion: Regarding patient safety issues, a near miss is a very significant factor because it can be a potential adverse event. Therefore, we should grasp the size of the problem through tracking and analyzing near misses and should make an effort to reduce them. To do so, we should check whether our reporting system is well designed and functioning.

The Surveillance of Adverse Drug Reactions (ADR) Reported in Samsung Medical Center (삼성서울병원에서 보고된 의약품 부작용의 현황 조사)

  • Adverse drug reactions (ADR) may result in increased hospital admissions, morbidity and mortality, adding extra cost to healthcare expenditures. Thus, it is critical to activate ADR monitoring and reporting program in tertiary hospitals in developing countries such as Korea. This study was performed to identify the types of ADR being reported in a tertiary hospital, Samsung Medical Center, and to find out the ways to improve current ADR monitoring system. Of 464 ADR reports submitted to the pharmacy department during the 6-month survey period, $97.8\%$ of the reports were from out patient and $48.5\%$ were from patients aged between 50 and 60. The medical department with the highest frequency in ADR reporting was Internal Medicines $(35.6\%)$. The most common ADR manifestations were gastrointestinal complaints $(43.4\%)\;and\;75\%$ of the reported cases were mild in their severity. The most common drugs suspected of causing ADR were CNS drugs which accounted for $32.8\%$. In terms of causality assessment, $85.1\%$ of the reports were probable cases by WHO causality assessment criteria. In regards to sources of report, $75.6\%$ of ADR were reported by physicians and $24.4\%$ by nurses. There were no ADR reported by pharmacists. In conclusion, there is an urgent need to improve ADR monitoring system for inpatient and to motivate pharmacist involvement in ADR monitoring and reporting in Korea.

Fentanyl PCA Monotherapy and Fentanyl TTS Combination Therapy in Post-Operative Pain Management: Analyses of Spontaneous Adverse Drug Reaction Reports (자발적 약물 이상반응 보고 분석을 통한 수술 후 통증 조절에 사용된 Fentanyl의 약물사용적정성)

  • Objective: There have been many cases of spontaneous adverse drug reactions to fentanyl at a regional pharmacovigilance center in the hospital. To assess the factors causing the adverse drug reactions reported in patients receiving fentanyl patient-controlled analgesia (PCA) monotherapy or in combination with fentanyl transdermal therapeutic system (TTS) for acute post-operative pain management. Methods: We conducted a retrospective cohort study with all patients prescribed fentanyl PCA for pain management after orthopedic surgery at a single university hospital from June 2012 to May 2013. We analysed the factors causing adverse drug reactions reported by a spontaneous reporting system in patients receiving fentanyl PCA monotherapy and those receiving fentanyl TTS in combination with fentanyl PCA. Results: Based on the spontaneous adverse drug reaction reporting, the risk ratio for the incidence rate of adverse drug reaction in the fentanyl TTS combination therapy group was 3.04 (95 % CI: 2.4-4.00, P < 0.0001), which was approximately 3-fold higher than that reported for fentanyl PCA monotherapy. Only 60 % of the adverse drug reactions were reported. Conclusion: It is inappropriate to add fentanyl TTS to fentanyl PCA to manage post-operative acute pain. There is a need to improve adverse drug reaction reporting. We expect that regular analysis of adverse drug reactions reported at regional pharmacovigilance centre would aid in appropriate drug utilization by patients.

Detection of Adverse Drug Reactions Using Drug Reviews with BERT+ Algorithm (BERT+ 알고리즘 기반 약물 리뷰를 활용한 약물 이상 반응 탐지)

  • In this paper, we present an approach for detection of adverse drug reactions from drug reviews to compensate limitations of the spontaneous adverse drug reactions reporting system. Considering negative reviews usually contain adverse drug reactions, sentiment analysis on drug reviews was performed and extracted negative reviews. After then, MedDRA dictionary and named entity recognition were applied to the negative reviews to detect adverse drug reactions. For the experiment, drug reviews of Celecoxib, Naproxen, and Ibuprofen from 5 drug review sites, and analyzed. Our results showed that detection of adverse drug reactions is able to compensate to limitation of under-reporting in the spontaneous adverse drugs reactions reporting system.

Improvement Strategies for the Management and Status Inspection System of National Survey Control Point Markers (국가 측량기준점 표지 관리 및 조사체계의 개선방안)

  • The objective of this study is to improve the survey rate and reliability of survey control point marks, which serve as the foundation for location-based services. Within the scope of this research, we have inspected the current status and reporting systems for national control point marks, public control point marks, and cadastral control point marks, identifying various issues. To tackle these challenges, we have prepared proposals for new unit cost calculations for national control point mark inspections, enhancements to the inspection and reporting systems for survey control point marks, and preliminary reporting procedures for surveyors using these control point marks. Additionally, we have established a workflow to refine the process of survey control point mark inspections and have proposed an automated system for calculating the unit cost for unreported targets. The study anticipates that these changes will improve the efficiency of future survey control point mark inspection tasks.

  • This study aims to analyze safety reports received through the recently added SMS safety self-report in the Air Force's "integrated air control management" system, identify hazards, present improvements by region (base), and lay the foundation for future data-based safety management. To identify risk factors, it was first classified by base based on data classified into 16 groups in the autonomous reporting system, and second classified in detail based on the type and description. Risk factors were analyzed for the most reported control cooperation (306) items, and improvements were derived by dividing risk factors into information sharing, regulations, procedures, education, training, and equipment items based on the analysis results. It was confirmed that risk factors and specific improvements vary by base (12), which is important data that can present statistical analysis and the direction of safety management in the flight control field by base (region). In addition, since there is no data-based risk factor analysis study for each specific base (region), it can be used in the future as basic research data for data-oriented safety management.

  • 컴퓨터에서 사용자의 usage정보는 Accounting의 기본 자료가 될 뿐만 아니라 그리드 컴퓨팅과 같은 환경에서도 교환되어지는 중요한 자료이다. 본 논문에서는 다수의 컴퓨팅 시스템으로부터 usage정보를 수집하여 제공하는 시스템을 구축하였다. 이 서버에서의 특징은 정보관리를 위하여 일반적으로 사용하는 DB를 사용하지 않고 XML파일로 처리한 것이다. 이렇게 함으로써 서버는 보다 단순해 질 수 있으며, 개인이나 중소 규모의 전산센터에서도 쉽게 usage정보 제공이 가능해 질 수 있다.
