Effect of Saline-Sephadex on the Organ Distribution of $^{99m}TC$-Sephadex (식염수-Sephadex가 $^{99m}TC$-Sephadex의 폐분포에 미치는 영향)

  • The organ distribution study and the whole body scan were done in the albino rats at intervals of 5, 30, 60, 90 and 120 minutes after the intravenous injection of $^{99m}TC$-pertechnetate absorbed in Sephadex beads of $20{\sim}80$ micra in diameter. Effect of additional injection of physiological saline and saline absorbed in Sephadex beads of $20{\sim}80$ micra in diameter on the scan and organ distribution were also studied. The results were as follows. 1. Five minutes after the injection of $^{99m}TC$-pertechnetate absorbed in Sephadex beads of $20{\sim}80$ micra in diameter, Sephadex was well trapped in the lungs, with which the excellent lung scan was obtained. Two hours after the injection, kidneys were well visualized instead of lungs, which suggested that kidney acts as the excretory organ. Five minutes prior to scan, additional injection of physiological saline absorbed in Sephadex above mentioned was done. The bladder was also well visualized together with the kidneys. 2. In the distribution studies, most of radioactivity was detected in the lungs at 5 minutes and was gradually transferred chiefly to the kidneys and bladder and partly to the liver. 3. Additional injection of physiological saline resulted in a rapid transfer of $^{99m}TC$ trapped in the lung to both the kidneys and liver. 4. Additional injection of physiological saline absorbed in Sephadex beads of $20{\sim}80$ micra in diameter resulted in a rapid transfer of $^{99m}TC$ trapped in the lung to only the urinary system. 5. Results of these studies suggested that; a) Other nutrients and therapeutic compounds may be carried into the lungs along with Sephadex beads and then released in high concentration, which would exert greater therapeutic effect locally than that of the usual administration. b) Some radionuclides absorbed in Sephadex could be used as the lung scan agents, the flushing out of which by Sephadex-saline also give satisfactory renal and bladder scans. c) Other potent therapeutic radionuclides could be retained for some time by this method, which can be in the lungs easily flushed out within 2 hours.

Urinary Excretion of Various Urinary Proteins in Children with Vesicoureteral Reflux (방광요관 역류증 환아에서의 다양한 요단백의 배설)

  • Purpose : This study aimed to examine the excretion of various urinary proteins in children with a history of urinary tract infection(UTI), with or without vesicoureteral reflux(VUR) or reflux nephropathy, and to identify means of predicting the severity of VUR or the presence of reflux nephropathy as indicated by these markers, and to know how these markers are changed after resolution of VUR. Methods : We studied 30 children with previous UTI, without VUR and renal scarring(group I), 12 children with VUR, without evidence of renal scarring(group II), and 34 children with VUR and renal scarring(group III). 24-hour or 12-hour urine ${\beta}_2$ microglobulin(${\beta}_2$ MG), microalbumin and N-acetyl-${\beta}$-D-glucosaminidase(NAG) were measured in each child. Urinary protein excretions were analyzed according to the degree of VUR(mild VUR : a grade reflux I-III, severe VUR : a grade reflux IV-V). Cases of bilateral VUR were graded by the higher grade of reflux detected. A total of 46 children with primary VUR were followed. Among these patients, VUR was completely resolved in 16 children. Voiding cystourethrography(VCUG) and DMSA scan were performed every year. Values for urinary markers were estimated every year. Results : 24 or 12 hour urine microalbumin and NAG excretions were significantly increased in group III compared to group I(microalbumin : $27.7{\pm}26.0mg/gCr$ vs $15.0{\pm}10.7mg/gCr$, P<0.05, NAG : $15.2{\pm}18.7U/gCr$ vs $3.4{\pm}2.2U/gCr$, P<0.05). Urinary ${\beta}_2$ MG excretions were not significantly different between groups. Urinary NAG excretions were elevated in the group of children with severe VUR compared to mild VUR($26.8{\pm}27.1U/gCr$ vs $7.6{\pm}3.8U/gCr$, P<0.05). After resolution of VUR, urinary microalbumin and NAG excretions were decreased(P<0.05). Conclusion : Urinary microalbumin and NAG may be useful clinical indicators to predict the presence of reflux nephropathy and the resolution of VUR. Especially, urinary NAG excretions may be used as a possible method to predict the severity of VUR.

Urinary Protein and Enzyme Excretion of Spot Urine in Children with Vesicoureteral Reflux (방광요관 역류증 환아의 단회뇨에서 요단백 및 효소 배설 측정)

  • Purpose : The aim of this study was to evaluate the clinical usefulness of measurement of ${\beta}_2$ microglobulin (${\beta}_2$ MG), N-acetyl-$\beta$-D-glucosaminidase (NAG) of spot urine samples as indices of renal tubular damage and microalbumin of spot urine samples as a parameter of glomerular damage in children with vesicoureteral reflux (VUR) or renal defects. Methods : We studied 91 children with previous UTI. The children were classified as 62 children without VUR and renal defects (group I), 10 children with VUR, without renal defects (group II), and 19 children with VUR and renal defects (group III). Patients having VUR were separated according to the degree of VUR (mild VUR: VUR grade I-III, severe VUR: VUR grade IV-V). Urinary excretion of ${\beta}_$ microglobulin (${\beta}_2$ MG), microalbumin, N-acetyl-$\beta$-D-glucosaminidase (NAG), creatinine were measured in samples of morning urine specimens. Children with VUR or renal defects detected by voiding cystourethrography (VCUG) and DMSA renal scan were investigated. Results : Microalbumin/Cr ratio of spot urine was significantly increased in group III compared group I ($42.3{\pm}27.2\;mg/gCr$ vs $25.2{\pm}10.9\;mg/gCr$, P<0.05). NAG/Cr ratio of spot urine was significantly increased in group II compared group I ($3.70{\pm}23.4\;mg/gCr$ vs $18.7{\pm}12.7\;mg/gCr$, P<0.05). There was no statistically significant difference of ${\beta}_2$ MG/Cr ratio among three groups. Conclusion : Urinary microalbumin excretion of morning urine sample may be a simple and reliable clinical indicators for early identification of renal damage in children with VUR and renal defects. Urinary microalbumin excretion may be useful marker to predict the the severity of VUR.

Urinary Tract Infection and Vesicoureteral Reflux in Children (소아에서의 요로감염과 방광요관역류에 관한 고찰)

  • Urinary tract infection (UTI) in children has been known to be a cause of renal damage, leading to scar formation, hypertension and renal failure. And vesico-ureteral reflex (VUR), frequently accompanying UTI in young children, has been incriminated as the main factor causing scar formation. This retrospective study has been undertaken to see the relationship among UTI, VUR and renal scar formation. Study population consisted of 291 children (boy 134, girl 42) with UTI, who have been admitted to the Pediatric Department of Kyungpook University Hospital during 6 1/2 year period from January 1990 to June 1996. VUR was diagnosed by VCUG and renal scar by ultrasonogram, DMSA scan (or DMSA SPECT) and IVP. The following result were obtained. Sexual difference showed male predominance (male to female, 134:42) below 1 year of age, and female predominance (male to female, 11:35) over 5 years of age were rioted. VUR has been found in 64 children (22%) and the degree of reflux, classfied by the method proposed by 'International Reflux Study in Children', were as follows ; Grade I : 4.0%, Grade II : 3.0%, Grade III : 2.7%, Grade IV : 5.8% and Grade V : 6.2%. There was no sexual difference E.coli was the most predominant infecting agent occurring in 167 children (57%), and end-stage renal failure was diagnosed at the time of first admission in 5 children with Grade V VUR. Renal scar has been noted in 49 out of 582 kidneys (8.4%), and the incidence of scar foramation according to the degree of VUR were as follow ; Grade 0 (No reflux) : 1.2%, Grade I : 6.7%, Grade II 27.3%, Grade III 29.4%, Grade IV : 57.1%, and Grade V : 100%. In summary, present study shows that renal scar formation in UTI has close correlation with the severity of VUR occurring more frequently in severe reflux, so that early diagnosis and proper treatment of UTI and VUR is of paramount importance in preventing renal damage in children with UTI.

  • The mother was 24 years old, primipara, and had been taking carbamazepine 400mg(serum concentration $5.0-8.5{\mu}g/ml$) during pregnancy without any clinical seizures. A male baby with physical malformation was delivered on week 39. The malformation is extradigit(polydactily) on X-ray of right foot and left mild hydronephrosis on ultrasonography and renal scan with radioactive material. We reported this rare case and reviewed related articles about teratogenic effect of carbamazepine, mechanism of action and prevention of teratogenesis.

  • Primary hryperparathyroidism is a relatively rare disease entity in Korea. It's characterized by severe skeletal and renal changes due to hypersecretion of parathyroid hormone, and rarely shows peptic ulceration, hypertension, pancreatitis and impaired mentality. Recently the determination of the serum calcium level has become a routine laboratory test and the awareness of primary hyperparathyroidism has been incerased, the disease is being diagnosed with increasing frequency. Primary hyperparathyroidism is most commonly caused by parathyroid adenoma and rarely hyperplasia, cancer of parathyroid glands. The authors operated sucessfully a case of primary parathyroid adenoma by diagnosed by nuclear medical diagnostic work-up. The patient had anterior neck mass(soft, non tender nodule) on physical examination, multiple goiter on thyroid ultrasonogram and scan. The parathyroid lesion was difficult to find preoperatively.

  • Secondary hyperparathyroidism (SHPTH) occurs commonly in patients with end-stage renal disease (ESRD). Uncontrolled SHPTH is associated with complications of calcium deposition including calciphylaxis and elevated rates of cardiovascular morbidity. Current treatment recommendations for medically refractory disease include total parathyroidectomy, often with autotransplantation (TPTH+AT) of minced parathyroid gland. Surgical intervention is associated with a reduction in cardiovascular mortality. We report a case of a 56-year-old man with ESRD who developed SHPTH and underwent TPTH+AT of parathyroid tissue into the right brachioradialis muscle. Over the course of 7 years he developed a mass at the site of the autotransplanted gland as well as recurrent refractory hyperparathyroidism with increased forearm uptake noted on sestamibi scan. After excision of this mass, pathology demonstrated hyperplasia of the minced gland fragments which were embedded within a mass of fibroadipose tissue rather than the muscle tissue it was originally transplanted in.

Prostate spindle cell neoplasm associated with early voiding difficulty after transurethral resection of the prostate

  • This report presents the case of 75-year-old men with spindle cell neoplasm. The patient underwent percutaneous nephrolithotomy and transurethral resection of the prostate (TURP) for renal stones and benign prostatic hyperplasia. One month postoperatively, the patient was able to void without any difficulty. Five months later, the patient experienced difficulty voiding and presented to the emergency room with severe pelvic pain. Computed tomography (CT) showed regrowth of the prostate mass into the posterior bladder and penile root. The prostate-specific antigen level remained constant at 1.14 ng/mL during the pre-and postoperative periods. Five months before the TURP operation, the patient's CT scan showed a soft and mildly enlarged prostate with no protrusion into the bladder. Biopsy of the prostate, however, showed a protruding mass, indicative of a spindle cell neoplasm. The patient was subsequently treated with the chemotherapeutic drug adriamycin. Unfortunately, treatment was unsuccessful, and the patient died 18 months later.

The difference of Quantitative Analysis According to the Method of Region of Interest Setting in $^{99m}Tc$-DMSA Renal Scan ($^{99m}Tc$-DMSA 신장 검사에서 ROI 설정 방법에 따른 정량분석 차이에 관한 연구)

  • Purpose: The nuclear medicine technology has been changed. The hard ware is developed so much. Also the soft ware performs a meritorious deed for the development of nuclear medicine technology. We could use the automated region of interest (ROI) instead of manual ROI. We want to know that what difference of quantitative analysis is there between automated ROI and manual ROI Materials and Methods: There are three experimental to make results. The first is what comparing the renal automated ROI and manual ROI. The second is that we compared three threshold ROI that size is difference each others with visible decision. The third is that we compared full, half, quarter automated background, and survey relative function. Results: Although the first has statistically not significant difference, the second and third have significant difference. Threshold, setting smaller threshold then renal outline or bigger, has statistically significant difference (p<0.01). The third is performed with the first experimental. Full background has significant difference, comparing each three type background (p<0.05). Conclusion: The results that there is not significant difference between automated ROI and manual ROI will increase objectivity and operator's convenience. We could know that smaller threshold then renal out line has significant difference in the second experimental. And the third experimental has results because of a increased background nearby live and spleen.

Urinalysis and Imaging Studies in Children with Urinary Tract Infection (소아 요로감염증으로 진단된 환아에서 요분석 검사와 신영상 검사)

  • Purpose : We studied the correlation between urinalysis and radiologic findings in infants and children with urinary tract infection. Method : Urine Dipstick test and unstained urine microscopic examination were carried out in 56 infants and children who were hospitallized with the diagnosis of urinary infection by pocitive urine culture at Seoul Adventist from September 1996 through August 1998. Urine was collected by midsream, catheter, urine bag after cleansing or bladder puncture. Renal sonography and $^{99m}Tc-DMSA$ renal scan or voiding cystoureterography were studied. Results : 1)In dipstick analysis, leukocyte esterase(LE) were positive in 30 cases of 56 patients(54%) and Nitrite were positive in 20 cases of 56 patients(36%) and LE or Nitrite were positive in 38 cases of 56 children with UTI(68%). 2)In microscopic analysis, WBC were positive in 38 cases of patients(68%) and bacteria were positive in 23 cases of 56 patients(41%) and WBC or bacteria were positive in 41 cases of 56 children with UTI(71%). 3)Ten cases of 56 UTI patients(18%) showed negative finding in Dipstick and microscopic analysis. 4)There was no significant difference in positive rate of dipstick(71% vs 66%) and microscopia analysis(83% vs 66%) between two group with or without renal scar on $^{99m}Tc-DMSA$ scan (P=0.117). 5)There was no significnt difference in positive rate of dipstick(100% vs 91%) and microscopic analysis(100% vs 100%) between two groups with or without vesicoureteral reflux on VCUG. Conclusion : There was no specific relationships between the abnormal urinalysis and the abnormal findings on $^{99m}Tc-DMSA$ scan or VCUG.
