일일 3교대 간호활동시 호르몬분비 및 신장기능의 변화에 관한 연구 -간호학생을 대상으로- (A Study on the Changes of Urinary Hormonal Excretion and Renal Function During Three-shift Nursing Practice)

  • The sympathico-adrenergic system and the hypophyseal-adrenocortical system mediates the regulation of the internal milieu. And the kidneys regulate both water and electrolyte balance of the body fluid. The kidneys are the sites of production of renin which participate indirectly in maintaining renin. angiotensin-aldosterone system. These system de-serve special attention in the context of adjustment the effects on the body function. And so, maximal exercise and work load are associated with home-osthetic function. The nurses working in the hospital have been complained of fatigue and stress by frequent duty changes and overload. In order to define this, the possible changes of hormonal excretion during three-shift nursing practice were investigated. Urine samples were collected at pre-duty and post-duty, and were measured with chemical assay and radioimmunoassay in 30 nursing students, in nursing practice and 43 nursing students, in studying. The results obtained were as follows. 1. In nursing practice, urinary norepinephrine concentration showed a marked increase during day duty, urinary cortisol concentration showed a marked increase during evening duty, and urinary renin concentration was increased in night duty, 2. Corrected ratio of urinary sodium excreted by the urinary excretion of creatinin (UNa/UCr) and UCl/UCr showed a marked decrease during night duty. Nursing practice did not affect on the UK/UCr and urinary concentrating ability. From these results, it is suggested that further studies the define the effects on some physiological function of the three-shift nursing practice against circadian rhythm are needed for better working condition of nurses.

Protective Effect of Korean Red Ginseng Against Dichromate Toxicity

  • The metabolic disturbance and nephrotoxicity induced by sodium dichromate (20 mg/kg, SC) have been diminished by the administration of Korean red ginseng extract (100 mg/kg, PO). Red ginseng has a powerful potency on the blood urea nitrogen (BUN) increment shown in the early 2h after dichromate intoxication. It normalized the dichromate induced hepatic glycogenolysis. The effect of red ginseng on dichromate induced nephrotoxicity was investigated by hematological analysis, and urinalysis. Ginseng treatment significantly reduced the increases in the urinary excretion of protein and glucose. These effects were dose dependent. Ginseng protected the accumulation of BUN and cretonne in the blood, caused by dichromate intoxication. Unlike CaEDTA, ginseng did not change the urinary excretion chromium. And it could not convert htxavalent chromium to trivalent chromium. These results suggest that ginseng treatment is effective in decreasing the metabolic disturbance, one of the earliest signs of dichromate toxicity, resulting in the protective effect of dichromate induced renal damage.

식이 단백질 수준이 어린쥐와 나이든 쥐의 골격의 대사에 미치는 영향 (Effect of dietary protein level on bone metabolism of young and aged rats)

  • To investigate the effect of levels of dietary protein and age on bone metabolism 40% and 5% casein were fed to the rats of 2 & 13 months of age for 12 weeks. High protein groups showed higher bone weight and Ca content than low protein groups and urinary Ca loss was increased in high protein groups but the difference disappeared gradually. A significant increase in urinary hydroxyproline excretion was noted in high protein groups of both age. Another short term study was undertaken to study if the above effect was related with renal function or PTH. Extremely high and low protein diets(60%, 6%) were fed to the rats of different ages(6wks, 6mos.) for 2 weeks, Urinary Ca excretion was significantly increased in high protein groups of young and aged rats and GFR was increased as well. There was no difference in serum iPTH levels between low and high protein groups, but it was elevated in aged rats. Alkaline phosphatase activity was higher in young rats, reflecting faster bone formation. The observed hypercalciuria in high protein groups, especially in aged rats, seems to be related to higher GFR, and PTH dose not appear to be a major mediator.

  • The influence of prazosin (0.1 mg/kg i.v.) on the excretion and diuretic action of furosemide (2mg/kg i.v.) in rabbits was studied to investigate an interaction between ${\alpha}-adrenergic$ blocking agent, prazosin and furosemide. The results were as follows; 1) With the combined administration of prazosin and furosemide, the plasma concentration of furosemide was increased, the urinary excretion rate and renal clearance of furosemide were reduced, and tile biological half-life of furosemide was increased. 2) The diuretic action of furosemide was significantly reduced with the combined administration of prazosin: maximal decrease in urine volume, urinary electrolytes, clearance of $Na^+$ and $Cl^-$, and GFR and RPF, as well as maximal increase in $Na^+$ reabsorption rate were noted 10 minutes after administration of furosemide (2mg/kg i.v.)

Determination of Carnitine Renal Threshold and Effect of Medium-Chain Triglycerides on Carnitine Profiles in Newborn Pigs

  • Colostrum deprived, newborn pigs (N=12, $1.64{\pm}0.05kg$) were used to study the renal threshold of carnitine, and effects of emulsified medium-chain triglyceride (MCT, tri-8:0) feeding on kinetics of plasma carnitine and urinary carnitine excretion. An arterial catheter was inserted through an umbilical artery, and a bladder catheter was inserted via the urachus. Piglets were oro-gastrically gavaged with one of six carnitine levels (0, 60, 120, 180, 240, $480{\mu}mol/kg\;W^{0.75}$) with (+MCT) or without medium-chain triglycerides (-MCT) in 0.9% NaCl solution. Blood was sampled into heparinized tubes at 0, 1, 2, 4, 6, 8, 14, and 20 h after gavage, and urine was collected and pooled into 1 h or 2 h composite samples to determine free- and short-chain carnitine concentrations. Plasma from the 12 newborn piglets before gavage contained $10.6{\pm}1.2{\mu}mol/L$ free carnitine and $7.2{\pm}0.6{\mu}mol/L$ acid-soluble acyl carnitine. The renal threshold for carnitine was similar between the MCT and the +MCT group (42.6 13.1 and $46.4{\pm}2.0{\mu}mol/L$, respectively), but the correlation between plasma free carnitine and urinary excretion was altered. Plasma free carnitine linearly increased with increasing carnitine dosage (-MCT group, $R^2=0.95$, p<0.001; +MCT group, $R^2=0.91$, p<0.001), but was decreased by 50% when medium-chain triglycerides were fed. The peak in plasma free carnitine concentration was depressed by medium-chain triglycerides feeding also. Therefore, the plasma and urinary short-chain/free carnitine ratio of the +MCT group was increased by 100% and 40%, respectively (p<0.01). Feeding of medium-chain triglycerides may delay plasma carnitine elevation via altering the kinetics of absorption. Similarly, the plasma and urinary short-chain/free carnitine ratio were affected by interaction between medium-chain triglycerides and time (p<0.01). The present study suggests that an oral carnitine dose over $480{\mu}mol/kg\;W^{0.75}$ may be needed to reach the free carnitine renal threshold within a short period, especially when provided together with medium-chain triglyceride.

Hydrocortisone의 신동맥 주입이 acetazolamide 및 aminophylline의 이뇨작용에 미치는 영향 (Effect of Hydrocortisone infused into a Renal Artery on the Diuretic Actions of Acetazolamide and Aminophylline)

  • This study was carried out to observe the direct effect of hydrocortisone on renal function by infusing it into a renal artery. Hydrocortisone (5mg/kg) or saline (0.5 ml/kg) was infused directly into the left renal artery of the rabbit, the right kidney was left intact to serve as a control for general action of acetazolamide (10 mg/kg) or aminophylline (10 mg/kg), which was administered intravenously 30 minutes after the direct infusion of pretreated drugs (hydrocortisone or saline). The changes of urine volume, pH, urinary excretion rates of $Na^+,\;K^+\;and\;Cl^-$, and the clearances of inulin and PAH were measured at an interval of 10 minutes for half an hour after the direct infusion of hydrocortisone or saline, and for one hour after intravenous administration of acetazolamide or aminophylline. The results of the experiment were as follows: 1. Significant changes in urine volume and urinary electrolytes (excreted rates of $Na^+,\;K^+\;and\;Cl^-$) were observed in the hydrocortisone-infused group 10 minutes after the administration of acetazolamide, compared with the saline-infused group. Especially, the effect was more potent on the infused (left) side than on the contralateral (right) side. 2. Significant changes in urine volume and urinary electrolytes were also observed in all the aminophylline-treated groups, but no remarkable difference was noticed between the hydrocortisone-infused group and the saline-infused group, nor between the left and right sides. 3. No signicant changes in the clearances of inulin and PAH were in the infused (left) side of all the experimental groups, as compared with the contralateral (right) side. From the above results, it is obvious that hydrocortisone infused into a renal artery exerts diuretic action when administered in combination with acetazolamide, and the mechanism of action rests not on its hemodynamic change for renal blood flow, but on the potentiation of carbonic anhydrase inhibiting action. However, the exact mode of action remains yet to be clarified.

만성신부전 모델쥐에서 인의 함량은 같고 단백질 수준이 다른 식이가 신기능과 골격 대사에 미치는 영향 (Effect of Dietary Protein Level on Renal Function and Bone Metabolism in Rats with Reduced Renal Mass When Dietary P Level is Equal)

  • Studies were carried out to explore the influence of dietary protein level on bone metabolism in uninephrectomized rat (experimental renal failure model) when dietary Ca and P contents were equal. Male rats were uninephrectomized or sham operated and fed 8%, 15% and 40% casein diets for 24 weeks. Ca and P contents of the all diet were 0.4% and 0.6% respectively. The results are summarized as follows. We did not found any significant difference in PTH and Ca level of the serum, Ca intake and Ca excretion among the experimental groups. There was significant positive correlation between the PTH and phosphate level. There was significant inverse correlation between serum Ca and creatinine level. The effect of the dietary protein level and renal mass loss on density and Ca contents of the bone were small and different according to the kinds of the bone. Low protein diet was associated with a significant enhancement of scapular density. Femur and vertebra density, however, were not influenced by dietary protein level and uninephrectomy. Light microscopic examination showed several calcified foci in the kidney in all experimental groups. Low protein diets have been used for a long time in the conservative management of chronic renal failure as they have a beneficial effect in preventing the appearance of symptoms. This study elucidated that part of beneficial effects of the low protein diet related to the suppression of the hyperphosphatemia. And these results, even though uninephrectomized rats fed high protein diet, the secondary hyperparathyroidism is supressed by the regulation of the P level. Therefore this study emphasized the need to pay more attention to the regulation of dietary P level as well as dietary protein content in chronic renal failure. N-3계 지방산과 단백질 수준이 나이가 다른 흰쥐에서 신장 기능에 미치는 영향 (Effect of N-3 Fatty Acids and Dietary Protein Levels on Renal Function in Rats of Different Ages)

  • This study was performed to investigate the effect of n-3 fatty acids and dietary protein levels on renal function. Fifteen-month old male Sprague-Dawley rats were divided into 4 diet groups. Two-month old rats were used as a control group. The experimental diets contained either a% or 25% casein and lipid levels of the diets were 20% by weight. For the control group, the lipid was composed of beef tallow and corn oil on a 1:1 basis, and fish oil was comprised 75% of the fat mixture for the fish oil group. Rats were fed the diets ad libitum for 8 weeks. GFR and urinary protein excretion were higher in high protein groups, while fish oil exhibited no effects. Renal medulla TXB$_2$and PGE$_2$ concentrations tended to be higher in high protein groups and lower in fish oil groups. Light microscopic examinations showed that glomerulosclerosis, tubular atrophy, tubular cast, interstitial inflammation and interstitial fibrosis fended to be higher in aged rats and in high protein groups and lower in fish oil groups. Serum levels of total lipid, triglyceride and total cholesterol were higher in aged rats and lower in fish oil groups while serum HDL-cholesterol level was higher in young rats and in fish oil groups. However, dietary protein level had no effect on serum lipid levels. Serum TBARS concentration was higher in aged rats and in fish oil groups. In conclusion, fish oil caused changes in serum lipid concentrations and eicosanoids metabolism. The effect of fish oil on renal function was less obvious than dietary protein. However, fish oil seemed to be effective in lessening deterioration of renal function due to aging and/or high protein diets through changes in lipid and eicosanoids metabolism.

Pyrazinamide의 신배설기전에 관한 약동학적 연구 (Pharmacokinetic Study of Pyrazinamide Related to the Mechanism of the Renal Excretion)

  • 가토에서 체중 kg당 40mg 용량을 15분간 단기 주입 또는 일정 혈중농도 유지를 위한 지속 주입 방법으로 pyrazinamide를 투여 후 혈청농도 및 뇨중 배설량을 축정하여 이의 신장 배설 기전을 약동학적으로 분석하고 분포기 이후의 조직농도 측정으로 다음과 같은 결과를 얻었다. 1. 가토에서 pyrazinamide의 약동학적 분석을 compartmental model에 적용시 분포기의 반감기는 $2.38{\pm}0.57$시간이었으며 Vc(central compartment의 용적)는 체중 kg당 $349.96{\pm}70.99m{\ell}$이었고 총 분포 용적 $(V_{dss})$$907.00{\pm}255.61m{\ell}$이었다. 2. Non-compartmental model parameter는 총 분포 용적$(V_{dss})$은 체중kg당 $953.34{\pm}362.02m{\ell}$이었으며 평균 체내 잔류 시간$(MRT_{iv})$$3.30{\pm}0.84$시간이었고 신 청소율은 112.26ml/kg 로 총 청소율의 39.5%에 해당하였다. 3. pyrazinamide의 혈청 단백 결합율은 $5{\sim}100{\mu}g/m{\ell}$의 범위에서 12.50-1.80%의 결합율을 보였다. 4. pyrazinamide의 분포기 이후 조직/혈청 농도비는 약물 투여 방법에 따라 지속 정맥주입에 따른 steady-state 유지시 15분 정주 방법시 보다 낮은 값을 보였으며 steady-state 상태의 조직 분포율은 신장에서 가장 높았고$(1.68{\pm}0.53)$, 근육에서 가장 낮은 값$(1.10{\pm}0.46)$을 보였다. 5. 가토에서의 pyrazinamide의 신장 청소율은 혈청 농도 변화에 비직선적인(non-linear) 관계를 보였다. 6. pyrazinamide의 신장 현성 운반량(apparent transport)은 혈청 Pyrazinamide농도 $10{\mu}g/m{\ell}$까지는 양성 균형을 보였으나 이후 혈청 농도의 증가에 따라 현저한 음성균형을 보였다. 7. pyrazinamide의 신장을 통한 분비율(excretion ratio, ER)은 혈청농도 증가에 따라 현저히 감소하는 양상을 보이며 $60{\mu}g/m{\ell}$의 농도에서 약 0.3의 ER치를 보여 70% 이상의 재흡수율을 나타냈다. 8. 혈청 pyrazinamide농도를 일정히 유지하고 뇨량 변화시 pyrazinamide의 신장 청소율은 뇨량 증가에 의존적인 상승을 보였다. 이상의 실험 결과로 pyrazinamide의 신장 배설은 사구체 여과와 더불어 세뇨관 분비 기전이 존재하고 비교적 빠른 확산속도를 보이는 세뇨관 재흡수 기전에 의해 신장 배설이 조절됨을 추정할 수 있었다.
