PURPOSE: This study was conducted to verify the effects of a functional correction of the pain of patients with chronic low back pain(CLBP), and to examine the effect of dysfunctional factors on health-related quality of life. METHODS: A preliminary survey was first conducted on 90 patients with CLBP after functional orthodontic treatment. Some revised questionnaires were also prepared. The survey was distributed for approximately eight weeks, and 215 copies were used as the final analysis data, except for questionnaires that were inadequate, error or non-response. RESULTS: Path analysis using the structural equation model of CLBP patients showed a positive correlation between all the path coefficients and the potential factors. The multidimensional relationship between pain and dysfunction after orthognathic treatment was confirmed using three subdivisions of the pain variables as independent variables and the dysfunctional variables as the dependent variables. Multiple regression analysis was performed to examine the effects of pain on the dysfunction. To identify the multidimensional relationship between dysfunction and the health-related quality of life, eight sub-factors of dysfunctional variables were set as the independent variables, and multiple regression was analyses were performed with the dependent variables of the health-related quality of life. CONCLUSION: This study examined the structural and influence relationships of the functional correction with pain, dysfunction, and health-related quality of life. The results, suggest that a functional orthodontic treatment can be used as a positive program for the health-related quality of life. In addition, this study is meaningful in that it provieds useful information for intervention such as psychosocial change of patients.
Purpose: We measured the Oswestry Low Back Pain Disability Index (OLBPDI) and related factors in patients with low back pain. Methods: The sample consisted of 50 patients who received physical therapy at the physical therapy units of the Andong Seoul Sintong Clinic, St. Luke Clinic, and Yeongju Seoul Sintong Clinic in Andong and Yeongju city from October, 2007, to February, 2008. The OLBPDI questionnaire was administered by 5 physical therapists as a cross-sectional study. Student's t-test and analysis of variance (ANOVA/Tukey and Scheffe) were used to analyze OLBPDI score differences. We also used nonparametric statistic analysis (Wilcoxon rank sum test, Median test). Pearson correlation analysis (Spearman correlation analysis) was used to analyze the relationship between OLBPDI and the visual analogue scale (VAS). Multiple regression analysis was performed to determine the effects of independent variables on pain scores as defined by the OLBPDI. Results: The average patient age was 37.1 years (range: 18$\sim$78 years old), and time from onset was 21.7 months (1$\sim$180). OLBPD and VAS scores were 12.70 (3.0$\sim$28.0) and 5.14 (1$\sim$8), respectively. OLBPDI scores were 14.4 in patients taking medicine and 11.57 in those who did not. There was a statistically significant relationship between OLBPDI and VAS (r=0.54, p=0.0001; r=0.55, p=0.0001 by Spearman coefficient). Gender ($\beta$=6.14, p=0.0124), age ($\beta$=-2.01, p=0.0324), weight ($\beta$=0.31, p=0.0222), time from onset ($\beta$=1.54, p=0.0044), and VAS score ($\beta$=1.59, p=0.0004) were significantly associated with OLBPD by multiple regression analysis. Conclusion: Variables associated with OLBPD were gender, age, weight, time from onset, and VAS score. Collecting information on the pain index using OLBPDI was acceptable to patients with low back pain. Further research should explore the pain index by using larger sample sizes and longer follow-up periods.
Objective : The purpose of this study was to examine the mood disorder, especially depression and anxiety which have an effect on rehabilitation in stroke patients. Methods : Subjects were 32 stroke patients who were inpatients and outpatients at Woosuk University oriental hospital. The degree of depression and anxiety was measured by the Beck Depression Inventory(BDI) and State-Trait Anxiety Inventory(STAI, State Anxiety Inventory : SAI, Trait Anxiety Inventory: TAI). Other questionnaire included sex, age, onset of stroke, degree of education, econommic class, religion, relationship with family, expectancy on treatment, side of hemiplegia, weakness grade of hemiplegia. Results : Results were as follows. 1. The average scores of SAI, TAI and BDI were 45.7, 46.7 and 19.9. The percentages of patients who had relatively high SAI, TAI and BDI scores were 75%(>36 point), 87.5%(>36 point) and 56.2%(>16 point). 2. Purpose: The purpose of this study was to investigate the level of nausea, vomiting, anorexia, fatigue and quality of life as well as to determine the relationship between those variables and identify the influencing factors on fatigue and quality of life in stomach cancer patients receiving adjuvant chemotherapy. Method: Subjects were 94 stomach cancer patients undergoing postoperational chemotherapy in a general hospital in Seoul. Nausea and vomiting were measured with Rhodes et al(1984) and anorexia with one 5-point item. Fatigue was measured using Lee's tool(1999) except open questions. The tool for quality of life was modified based on the Quality of Life Index by Padilla et al(1983). Result: Subjects reported low level of fatigue(mean=3.86, range=0-10) compared with the results of previous researches and moderate quality of life(mean=2.64, range=1-4). Fatigue was positively correlated with nausea, vomiting and anorexia(r=.21 ~ .55, p<.05). Quality of life was negatively correlated with nausea, vomiting, anorexia(r= -.24 ~ -.45, p<.05) and fatigue (r=-.61, p<.01). Multiple regression analysis revealed that activity level, vomiting before admission, anorexia during chemotherapy and age explained 52.8% of the variance in fatigue. Fatigue, anorexia before admission, age and sex explained 50.5% of the variance in quality of life. Fatigue and quality of life were not influenced by the stage of disease, nor weight change. Conclusion: These results may contribute to a better understanding of how much the side effects of anticancer drugs can affect fatigue and quality of life in cancer patients undergoing chemotherapy. Also it is the remarkable fact that symptoms remaining after discharge such as vomiting or anorexia continued until re-admission, contributing to patients fatigue and lowered their quality of life.
Purpose: In addition to regulating calcium and phosphorus homeostasis and bone metabolism, vitamin D is known as an immune modulator. Recently, there has been increased worldwide interest in the association between low levels of vitamin D and allergic diseases. The purpose of this study was to assess the relationship between serum vitamin D levels and allergic/vasomotor rhinitis (AR/VR) in children. Methods: This study included 164 patients. The sample included 59 patients with AR, 42 patients with VR, and 63 controls. Their ages ranged from 0 to 16 years. We examined the levels of 25-hydroxyvitamin D, Immunoglobulin E, specific IgE, and eosinophil cationic protein; peripheral blood eosinophil count; and the results of a skin prick test. Results: Serum 25-hydroxyvitamin D levels were $19.0{\pm}8.5ng/mL$ in the AR group, $25.5{\pm}10.9ng/mL$ in the VR group, and $26.9{\pm}10.7ng/mL$ in the control group. After adjustment for body mass index and season at the time of blood sampling, vitamin D levels in the AR group were lower than those of the VR group (P=0.003) and control group (P<0.001). Vitamin D levels were inversely correlated with Immunoglobulin E levels (r=-0.317, P<0.001). AR patients with food allergy or atopic dermatitis did not have lower levels of 25-hydroxyvitamin D than AR patients without these diseases. Conclusion: This study demonstrates a possible relationship between vitamin D levels and allergic rhinitis in Korean children.
The purpose of this study was to investigate the relationship of Dampness-phlegm to blood lipid level and second reason of hyperlipidemia in acute stroke patients by case-control study. This study was done over 348 patients hospitalized in the Oriental Medical Hospital of Daejeon University of November 2006 to July 2008. Patients had been interviewed by residents and medical specialists who studied standard operation procedures in Fundamental Study for Syndrome of Oriental Medicine for Stroke. Study subjects consisted of 86 patients who distributed to Dampness-phlegm by medical specialist and discriminating program as the case I group, 157 patients who distributed to Dampness-phlegm by medical specialist or discriminating program as the case II group and 191 patients who distributed to Non-Dampness-phlegm by medical specialist and discriminating program as the control group. For the purpose of obtaining suitable result we analyzed blood lipid level of each group by univariate and multivariate logistic analysis. Dampness-phlegm was not significant correlated with increasing of Total cholesterol, Triglyceride and decreasing of HDL cholesterol. Dampness-phlegm was significant correlated with increasing LDL cholesterol and the independent predictors of hyperlipoproteinemia by multivariate logistic analysis. Dampness-phlegm was not significant correlated with diabetes melitus, liver disease, kidney disease, obesity and abdominal obesity. In this study, we demonstrated new relationship between Dampness-phlegm and LDL cholesterol. Based on these results, it is suggested that Dampness-phlegm would be the independent predictors of hyperlipoproteinemia. And more prospective studies are to be done with more clinical data.
Purpose: This study investigated the mediating effect of perceived stress in the relationship between subjective health and depression in patients with coronary artery disease. Methods: This secondary analysis study used data from the 2016-2018 Korea National Health and Nutrition Examination Survey (KNHANES). Study participants included 466 patients over 20 years of age with coronary artery disease. Measures included questions about general characteristics, subjective health, perceived stress, and depression. The data were analyzed using complex sample design, descriptive analysis, Rao-Scott 2 statistic, and multivariate logistic regression analysis with the SPSS 24.0 program. Results: Subjective health influenced depression, and perceived stress partially mediated it. Conclusion: To prevent depression in patients with coronary artery disease, it is necessary to develop effective stress management strategies.
연구목적 : Alexithymia(감정표현불능증)는 내면적 감정을 올바로 인식하거나 언어적으로 표현하기 어려운 상태를 뜻하는 용어이다. 최근에는 말로 표현하는데 장애가 있는 것만이 아니라 인지과정에서의 결함도 강조하고 있다. 그리고 감정은 신체증상과 전반적인 건강상태에 영향을 준다는 점에서 정신신체연구에 매우 중심적인 역할을 하고 있다. 이에 본 연구는 신체형 장애 환자와 정신신체장애의 하나로 알려져 있는 당뇨병 환자를 대상으로 alexithymia의 척도를 비교하고 이 환자들에서의 정신병리와 성격특성을 알아 보고자하였다. 방법 : 본 연구는 만 18세 이상의 신체형장애 환자 20명, 당뇨병 환자 20명 그리고 정상 대조군 20명을 대상으로 세 군의 alexithymia의 정도, 정신병리 및 성격특성을 알아보기 위하여 각각 Toronto Alexithymia Scale(TAS), 한국판 Symptoms Check List 9G-R(SCL-90-R) 및 Sixteen Personality Factor Questionnaire(16-PF)를 이용하여 세 군간의 차이를 분석하고 TAS와 SCL-90-R 및 TAS와 16-PF와의 상관관계를 분석하였다. 결과 : 1) TAS에 따른 세 군간의 비교에서는 신체형장애 환자군이 당뇨병 환자군과 정상 대조군보다 유의하게 높은 수치를 나타냈다. 2) SCV90-R에 의한 비교에서는 신체화증상과 불안증상 척도에서 신체형장애 환자군이 당뇨병 환자군과 정상 대조군보다 유의하게 높게 나타났다. 강박증상, 우울증상, 공포증상 및 정신증상 척도에서는 선체형장애 환자군이 정상 대조군보다 유의하게 높게 나타났다. 전체지표 중 전체심도지수는 신체형장애 환자군과 당뇨병 환자군이 정상 대조군보다 높았고, 표출증상심도지수는 신체형장애 환자군과 당뇨병 환자군이 정상 대조군보다 높았으며 표출증상합계에서는 신체형장애 환자군이 정상 대조군보다 유의하게 높은 결과를 보였다. 3) 16-PF에 의한 비교에서는 지능요인에서만 정상 대조군이 신체형장애 환자군보다 높게 나타났고 다른 요인에서는 차이가 없었다. 4) TAS와 SCL-90-R과의 상관관계분석에서는 신체형장애 환자군에서 표출증상심도지수를 제외한 모든 척도에서 정적 상관관계를 보였다. 그러나 다른 두 군에서는 유의한 상관관계를 보이지 않았다. 5) TAS와 16-PF와의 상관관계분석에서는 모든 대상군에서 유의한 상관관계를 보이지 않았다. 결론 : 신체형장애 환자군이 당뇨병 환자군이나 정상 대조군에 비하여 높은 alexithymia 척도와 많은 정신병리를 보였다. 그리고 신체형 환자군에서의 높은 alexithymia 척도는 정신병리와 유의한 정적 상관관계를 보이고 있었다. 당뇨병환자들은 정상 대조군과 유의한 차이를 보이지 않았다. 그러므로 신체형장애 환자들에게는 감정을 표현하고 그들의 정신병리를 완화하거나 해소할 수 있는 치료적 접근이 도움이 되겠고, 당뇨병환자들에게는 우선적으로 신체질환의 상태를 호전시킬 수 있는 치료가 더욱 필요로 하겠다.
Background: The rising prevalence of asthma worldwide may be associated with the rising prevalence of obesity in developed nations. Although several studies have suggested a relationship between asthma and obesity, controversy still remains. The aim of this study was to examine the relationship between obesity and asthmatic factors such as atopy, eosinophilia, serum total Ig E and bronchial hyperresponsiveness in chronic cough patients. Methods: This study was a retrospective, observational study in two centers done between January 2007 and June 2008. The subjects included individuals who had a chronic cough. We examined body mass index (BMI) to measure obesity and pulmonary function. We did a metacholine provocation test for airway hyperresponsiveness (AHR), a skin prick test for atopy, and tests for blood eosinophils and serum IgE. Results: A total of 1022 subjects were included. Airway hyperresponsiveness was not related with obesity (p=0.06), and atopy incidence was significant higher in non obese patients (p=0.00). There was no significant difference in serum IgE and blood eosinophil counts between obese and non obese patients. Forced expiratory volue in one second ($FEV_1$)/forced vital capacity (FVC) was significantly reduced in obese patients (p=0.03), but FEV1 and FVC were no significant difference between obese and non obese patients. Conclusion: There is no relationship between obesity and bronchial hyperresponsiveness. The nonobese group appears to have more atopy. The relationship between obesity and bronchial hyperresponsiveness and atopy need further investigation.
As a major cause for postoperative hypoxia, the importance of increased physiological shunting is increasingly emphasized. This study is a review and analysis of postoperative physiological shunting following open heart surgery with the aid of extracorporeal circulation. Sixteen patients were selected from among 21 patients who underwent elective open heart surgery at the Department of Thoracic and Cardiovascular Surgery, Kyungpook National University, School of Medicine, from December, 1975 to September, 1977. The results were as follows: 1. The degree of postoperative physiological shunt was progressively increased from 18.8% mean value one hour after surgery to 22.7% mean value, reaching a peak on the second postoperative days. 2. For up to one week, large physiological shunt[15%] was persisted in one patient. 3. Comparing long[more than 90 minutes] with short[less than 90 minutes] perfusion time group using pump oxygenator, it was found that the physiological shunt increased about 3% in the long as compared with the short perfusion time group. 4. The mean blood pressure was 70-80 mmHg without a remarkable causal relationship between physiological shunt and mean blood pressure. 5. On elevated $PaO_2$[>200 torr], the physiological shunt was decreased less than 20% of cardiac output, but on diminished $PaO_2$[102 torr] after two days, it was 22.7% of cardiac output. From above results, a contrary causal relationship between $PaO_2$ and physiological shunt was obtained. Co Reviewing chest X-rays postperfusion, it was demonstrated that no remarkable causal relationship between roentgen-ray evidence and physiological shunt could be obtained.
