중년 여성의 신체정신적 건강, 건강행태가 낙상에 미치는 영향 (Effects of physical & mental health and health behaviors on falls in middle-aged women)

  • 본 연구는 만 40세-64세의 중년 여성의 낙상에 영향을 주는 요인을 알아보고 낙상 예방 프로그램 개발에 기초자료로 활용하기 위하여 시도되었다. 연구방법은 2019년도 지역사회건강조사 자료를 이용하여 43,917명 중년 여성을 연구대상으로 낙상, 신체 건강, 정신건강 및 건강행태를 조사하였다. 대상자의 낙상에 영향을 미치는 요인을 알아보기 위하여 descriptive statistics, chi-square test, multiple logistic regression을 하였다. 연구 결과 중년 여성의 낙상 경험은 11.9%로 보고되었고 연령이 증가할수록, 배우자가 없는 여성에서 낙상을 많이 경험하였다. 높은 우울감과 지각된 스트레스, 당뇨병을 진단받은 경우, 폭음하는 여성에서 낙상 경험이 높게 나타났다. 중년 여성의 낙상을 감소시키기 위해서 본 연구에서 밝혀진 변수를 고려하여 다양한 간호 중재의 근거자료로 적용될 수 있을 것이다.

입원 아동의 낙상영향요인 -퇴원손상심층조사 자료를 이용하여- (Factors Affecting Hospitalized Children's Falls - Using Data in the National Hospital Discharge In-depth Injury Survey)

  • 본 연구는 2008년부터 2017년까지의 질병관리본부의 퇴원손상심층조사 자료를 이용하여 입원 아동 중 낙상을 경험한 환자를 대상으로 유·아동·청소년 낙상의 특성과 낙상영향 요인을 규명하고자 하였다. 이를 위하여 입원 중 낙상(한국표준질병사인분류, W00-W19)으로 손상(한국표준질병사인분류, S00-S99)을 경험한 18세 이하 아동 환자 116건의 데이터를 수집·분석하였다. 수집된 자료는 SPSS 23을 이용하여 빈도분석, 교차분석, 다중회귀분석을 실시하였다. 연구결과, 입원 중 낙상을 경험한 아동은 남자가 여자 보다 많았고, 1세 이상 6세 이하의 연령에서 낙상이 가장 많이 발생하는 것으로 나타났다. 낙상을 경험한 아동의 질병유형으로는 호흡기 질환이 가장 많았고 낙상으로 인한 상해유형은 열린 상처나 타박상의 빈도가 가장 높았으며, 낙상 사고 원인으로는 침대와 관련된 낙상이 가장 많았다. 낙상 관련 요인분석 결과 성별을 제외한 연령(β=.318), 주진단명(β=.231), 손상(β=.169)의 순으로 낙상에 영향을 미치는 것으로 나타났다. 이와 같은 연구결과는 아동에 대한 낙상예방 교육이 연령, 질병 유형, 아동의 발달단계에 따른 특성과 연령대를 고려하여 실시되어야 함을 시사하는 결과라 할 수 있다. 또한 비교적 활동에 제약이 적은 경미한 질환의 아동이나 과다행동을 보이는 유아기 아동 등 질병의 유형과 연령별 아동 발달 단계특성을 고려한 낙상 사고 위험군에 대한 스크리닝 및 이와 관련한 섬세한 낙상예방 매뉴얼 개발에 힘써야 할 것이다. 마지막으로, 본 연구결과가 입원을 필요로 하는 아동 환자, 보호자, 그리고 관련 의료진에게 연령별 아동 발달단계의 특성에 따른 낙상예방 교육 및 매뉴얼 개발의 단초로 사용되기를 기대한다.

ICF 모델에 기반한 독거노인과 동거 노인의 자살 생각 관련 요인에 대한 비교분석: 2023년 지역사회건강조사 자료 활용 (Comparative Analysis of Factors Related to Suicidal Ideation between Elderly Living Alone and Living with Families Based on the ICF Model: Using Data from the 2023 Community Health Survey)

  • This study utilized the raw data from the Community Health Survey (2023) to compare differences in body function, activity, participation, environment, personal factors, and suicidal ideations between elderly individuals aged 65 and older living alone and those living with families in Korea. The study subjects consisted of 80,618 elderly individuals aged 65 and older. Cross-tabulation analysis χ2-test), independent sample t-test, and logistic regression analysis were conducted. The results indicated that for both elderly individuals living alone and those living with families, age(p<.001), smoking(p<.001), stress(p<.001), depression experience(p<.001), and falls experiences(p<.001) were risk factors that increased suicidal ideations. Conversely, educational level(p<.05), income level(p<.001), subjective health status(p<.001), social participation(p<.001), economic activity(p<.001), community environment(p<.001), and social relationships were protective factors that reduced suicidal ideations. These findings demonstrate that demographic characteristics and individual health status significantly influence suicidal ideation in the elderly, while social factors contribute to a decrease in suicidal ideation. Furthermore, considering the differences found between elderly individuals living alone and those living with families regarding hypertension and gender, differentiated interventions for suicide prevention are necessary.

Relationships between Metabolic Syndrome Component and Depression, Stress

  • The purpose of this study is to provide an academic basis regarding the necessity of managing depression and stress among metabolic syndrome patients by understanding 5 components of metabolic syndrome, perceived stress, and degree of depression, and by investigating their association using the national nutrition survey reference. This study was conducted by using mental health surveys and health screening test data of the 5th (2010~2012) primitive data of the national health and nutrition survey. A total of 19,599 respondents over 19 years of age were selected for the final analysis. The level of depression and stress was set as the dependent variable to identify its connection with 5 components of the metabolic syndrome. For the stress recognition, none of the metabolic syndrome components showed a significant correlation. For experiencing the depression symptom, the fasting glucose among the factors showed a significant correlation (p<0.05) among the metabolic syndrome factors. When it falls within the criteria of fasting glucose of metabolic syndrome, it has a great probability of falling under the group who experienced greater depression symptoms. As a result of the analysis by controlling cardiovascular and cerebrovascular disease which is tightly related with metabolic syndrome and depression, this study observed that glucose out of 5 metabolic syndrome components is related with depression.

폐경여성의 낙상 위험과 관련요인 (Fall Risk related Factors in Postmenopausal Women)

  • Purpose: The purpose of this study was to understand the risk of falling associated with postmenopausal women and to identify the relationships between this risk and factors such as lifestyle, metabolic syndrome, and bone mineral density. Methods: The sample was 128 postmenopausal women between 50 and 65 from one menopausal clinic in an urban city. The Risk Assessment for Falls Scale II, developed by Glydenvand and Reinboth (1982) and adapted by Park Young-Hye (2003), was modified and used for this study. Results: The average fall-risk score in postmenopausal women was 7.2 out of 33, the fall-risk score associated with lifestyle was higher in women exposed to stress frequently or who favored spicy or salty foods. The fall-risk score associated with metabolic syndrome was higher in groups with HBP or with a waist circumference of 80cm or greater. The fall-risk score in groups with three or more factors of metabolic syndrome was the highest. Conclusion: The risk of fall in post-menopausal women was higher in groups with only elementary education, unemployed, reported two or more chronic diseases or reported frequent exposure to stress and for women who preferred spicy or salty foods or exhibited three or more factors of metabolic syndrome.

일 지역 재활병원의 낙상 정보에 근거한 낙상예방 간호중재 프로그램 개발을 위한 문헌분석 연구 (A Literature Review for Fall-Prevention Nursing Program Development based on the Fall Information of a Rehabilitation Hospital)

  • 본 연구목적은 일지역 중소 재활병원 입원환자의 낙상실태 및 위험요인을 규명하고, 낙상예방 간호중재 연구 트랜드를 문헌고찰하여 실제적인 낙상예방 간호중재프로그램을 구상하기 위함이다. 연구방법으로는 낙상실태 및 위험요인을 규명하기 위한 서술적 연구와 실제적 낙상예방 간호중재프로그램 구상을 위한 통합적 문헌고찰을 병행하였다. 연구결과 해당병원의 임상현장 특징이 반영된 낙상실태와 낙상위험요인을 확인하였으며, 문헌고찰을 통해 노년 인구에 민감도가 높은 낙상위험사정도구의 활용과 보다 효과적인 방식의 낙상예방 간호중재를 제안하였다. 결론적으로, 본 연구는 낙상예방 간호중재의 실무근거를 마련하기 위해 통합적 문헌고찰 결과를 기반으로 실제에 맞는 낙상사정도구와 낙상예방 중재프로그램의 선택을 제안하고 이들의 활용 전망을 비춰주었다. 이는 병원낙상 간호에서 비판적사고를 통한 근거기반 간호실무 향상에 기여할 것이다.

재가 노인의 체위성 저혈압 발생빈도와 위험요인 (Prevalence and Risk Factors of Orthostatic Hypotension among the Community-Dwelling Aged)

  • Purpose: This study was to identify the prevalence of orthostatic hypotension and its association with risk factors of orthostatic hypotension aged over 60 in Seoul and Chungju, Korea. Method: The data were collected from the 22th of August, 2000 to the 7th May 2001. The participants were 74 community-dwelling aged who could stand up from sitting position without assistance. Subjects were interviewed with structured questionnaire in order to ask experience of previous falls, hours in per day, symptoms related orthostatic hypotension and demographic characteristics. Orthostatic hypotension was assessed at 1 minute after the subjects standing from sitting position and defined as 20mmHg or greater decrease in systolic blood pressure after standing. Result: The prevalence of orthostatic hypotension was 17.1%. The mean drop of systolic blood pressure was 27.46mmHg among orthostatic hypotension subjects. The significant variables which explain the occurrence of orthostatic hypotension was the basal systolic blood pressure, the hit ratio of discriminant function with basal systolic blood pressure was 69.7%. Conclusion: Finding indicate that this study will contribute to develop nursing strategies to identify risk factors and to prevent orthostatic hypotension for the aged.

뇌졸중 성인의 낙상 감소를 위한 1대 1 낙상 예방 교육의 효과 (Effects of One-to-one Fall Prevention Education on Decrease in Falls of Adults with Stroke)

  • 1대 1 낙상 예방 교육이 뇌졸중 성인의 낙상과 낙상 요인에 미치는 영향을 알아보고자 하였다. 뇌졸중 성인 17명을 대상으로 낙상에 대한 4개의 다른 주제(낙상의 소개, 낙상 예방 운동, 낙상 예방 보조도구, 낙상예방 환경 수정)에 대하여 주 1회, 총 4회기에 걸쳐서 하는 뇌졸중 성인의 인지 능력을 고려한 1대 1 낙상예방 교육 프로그램을 개발하여 시행하였다. 종속변인으로 최근 한 달간의 낙상횟수 및 낙상위험횟수, 버그균형척도, 낙상공포, 낙상 효능감, Beck 우울증 척도를 측정하였다. 4회의 1대 1 낙상예방교육에 참여한 결과, 대상자들은 버그균형척도를 제외한 모든 종속변인에서 통계학적으로 유의미한 수준의 긍정적 변화가 있었다(p<.05). 본 연구의 결과로 뇌졸중 성인에게 1대1 낙상예방교육을 적용하였을 때, 낙상횟수 및 낙상위험횟수, 낙상공포, 우울증세가 감소하고 낙상 효능감의 긍정적인 변화를 일으킨다는 것을 알 수 있었다. 뇌졸중을 가진 성인들이 지역사회 및 병원에서 낙상의 위험에서 보다 더 안전하기 위해서는 그들의 인지수준을 고려해서 개발된 1대 1 낙상예방교육이 필요하다.

입원환자 낙상 발생 실태와 원인에 관한 분석 연구 (An Analysis of Fall Incidence Rate and Its Related Factors of Fall in Inpatients)

  • Background: The purpose of this research was to examine the fall incidence rate and its related factors of fall in inpatients. Methods: The data were collected from the 138 fall incident reports in one tertiary hospital in Seoul from April 1st 1999 to September 30th 2001. The Fall Incident Report Form was originally developed based on that of Massachusetts General Hospital revised in 1995. And this was modified for this survey by the collaborating work of QI team including researcher and department of nursing service of this particular hospital. The contents of Fall Incident Form were general characteristics of patient. factors related to fall. types and places of fall. circumstances, nursing interventions. and outcome. Results: 1) The incidence rate of fall was 0.08% of total discharged patients and 0.081 per 1000 patient-day. This incidence rate is much lower than that of several hospitals in USA. This finding might result from the different incidence report system of each hospital. 2) The characteristics of fall-prone patient were found as follows. They were mostly over 60 years old, in alert mental status, ambulatory with some assistance, and dependent on ambulatory device. The types of diseases related high incidence rate were cerebrovascular disease(3.2), hypertension(1.6), cardiovascular disease(1.4), diabetes(1.3) and liver disease(0.6). 3) The majority of fall events usually occurred m bed. bedside(walking or standing) and bathroom in patient room. Usually they were up on their own when they fell. And there were more falls of elderly occurred during night time than day or evening. 4) 63.8% of fall events resulted in physical injuries such as fracture and usually the patients had diagnostic procedures and some treatment(ex. suture) which caused additional cost to the patients and their families. 5) The found risk factors of fall were drugs(antihypertensive drug, diuretics) and environmental factors like too high bed height, long distance of bedside table and lamp switch, and slippery tile of bathroom floor. Conclusion: Considering these results, every medical and nursing staff should be aware of the risk factors of patients in hospital, and should intervene more actively the preventive managements, specially for the elderly patients during night. Therefore, it is recommended that the development of Fall Prevention Programs based on these results.

Occupational Hazards in Firefighting: Systematic Literature Review

  • Background: Firefighting involves exposure of firefighters to risks related to this activity, serious injuries, and occupational diseases are recorded. There are other consequences such as thermal and emotional stress. This systematic review is proposed in order to analyze the risks and consequences faced by these workers and thus provide elements to improve safety management systems in institutions. Method: A descriptive observational study of systematic literature review on the risks and consequences of exposure to firefighters' activity was proposed, and the information was analyzed and described based on the available data and according to the variables determined. Results: The studies showed data on mechanical, physical, chemical, psychosocial risks, workers' perception and resilience, and epidemiological data. Information related to firefighters' activity on falls and slips, exposure to noise, and high concentrations of carbon monoxide is detailed. In addition, the relationship between burnout, cognitive, and physical fatigue as adverse effects on health and performance is mentioned. Conclusions: Among the preventive measures, the use of personal protective equipment is suggested, incorporation in prevention programs of information on exposure to risk factors, as well as the implementation of models that can predict the perception of workers, additionally, the generation of management systems with safety climate models for fire departments.
