Bilateral vocal cord palsy is uncommon but is serious because of airway obstruction. Treatments of bilateral vocal cord palsy are initially tracheotomy, vocal cord lateralization and vocal cord reinnervation. Recently, we experienced nerve-muscle pedicle reinnervation in 3 cases of bilateral vocal cord palsy, so reported it with a review of literature.

The purpose of this study was to investigate the revascularization and reinnervation of calcitonin gene-related peptide immunoreactive nerves in immediately replanted rat molars. First maxillary right molars in 56 rats(35days old) were extracted and immediately replanted. The rats were killed 1, 2, 3, 7, 14, 28 and 42 days after replantation and revascularization of pulpal blood vessels were examined microangiogram with korean traditional ink and reinnervation of pulpal nerve were examined immunohistochemical method using calcitonin gene-related peptide(CGRP) antiserum. The results were as follows; 1. Revascularization and reinnervation of CGRP immunoreactive nerve fibers were observed mesial side whole pulp tissue of replanted teeth. Revascularization and reinnervation of CGRP immunoreactive fiber were made at 2days after replantation in entire pulp of replanted teeth and the distribution density of blood vessels were gradually increased according time elapsed, but did not achieve the density of control. 2. Postoperative dentin formation in replanted teeth revealed at 1week after replantaton and gradually increased according to time elapsed. 3. Revascularization and Reinnervation of CGRP immunoreactive nerve fibers were established at the same time and it seems to be closed relatationship between revascularization and reinnervation.

In unilateral vocal fold paralysis (UVFP) patients, we try to improve their symptoms such as hoarseness or aspiration by restoring nerve functions or medialization laryngoplasty (ML), etc. Until now, ML (thyroplasty and/or arytenoid adduction) is considered as gold standard of treatment for UVFP. However, if recurrent laryngeal nerve (RLN) is damaged and use of RLN is feasible during operation, laryngeal reinnervation (LR) would be a good option. Anastomosis with ansa cervicalis to RLN is most common reinnervation method. Delayed LR may be considered in young patients when the RLN denervation period is not long (less than 2 years) for the treatment of surgery-related UVFP. Injection laryngoplasty and laryngeal framework surgery showed great voice outcomes in UVFP. Combination therapy (neuromuscular pedicle innervation with ML) also showed good post-operative voice outcomes even in longer periods (over 2 years). In pediatric patients, LR would be considered as a good treatment option because all procedures need to general anesthesia.

Background and Objectives : Until now, various attempts have been made fir reinnervating paralyzed vocal cord. Nevertheless, the most cases did not produce satisfactory outcome due to occurrence of synkinesis of larynx secondary to misdirected axonal regeneration. Accordingly, the purpose of this investigation is to learn the effect of chronic electrical stimulation on regeneration of the recurrent laryngeal nerve. Material and Methods : Using 20 healthy male Sprague-Dawley rats(250-300g) with normal vocal functions, transections were made on their left recurrent laryngeal nerves and then primary anastomosis were performed under the operating microscope and they were divided into an experimental group and a control group each having 10 rats. After the procedure, for the experimental group, chronic electrical stimulation was carried out until vocal cord movement was functionally recovered. for the control group, only chronic electrical stimulation was not given. Result : In experimental group, the number of functionally recovered rats was two and in control group, that of functionally recovered rate was same. The reorganization of posterior cricoarytenoid muscle motoneuron in nucleus ambiguus appeared in the case of directed reinnervation of recurrent laryngeal nerve. Conclusion : The chronic electrical stimulation does not a direct beneficial effect on the early functional recovery in rats with injured recurrent laryngeal nerve.
Face transplantation has been reported over the last six years, and it started in 2004 with the announcement of Cleveland Clinic granting the world's first IRB approval to proceed with human face transplantation. Composite tissue allografts (CTAs) in the facial region are considered to be more immunogenic than other solid organ transplants, so these kinds of animal experiments were also considered as challengeable activities to the facial reconstructive surgeons. For the better understanding of CTAs in the oral and maxillofacial fields, we reviewed several recent articles about facial composite transplantation animal model, and summarized some knowledges of composite midface allotransplantation model with sensory and motor reinnervation in this review article.

Background and Objectives : Laryngeal Evoked Electromyography(EEMG) is a objective, quantitative technique to determine innervation status of larynx. The possible applications of this technique are to confirm the etiology of impaired vocal fold motion and monitor perioperative vagus nerve trauma. The purpose of this study is to develop a novel method for determining the amount of reinnervation of recurrent laryngeal nerve with accurate, inexpensive, and minimally invasive technique in human. Materials and Methods : Laryngeal EEMG was performed for 16 adults with intact vocal folds motion and 2 patients diagnosed as unilateral vocal fold paralysis. for the purpose of searching what is the optimal and noninvasive technique for laryngeal EEMG, we used 2 types of stimulation configurations(transcutaneous vs percutaneous) and 2 types of recording configurations(intramuscular vs. surface). Results and Conclusions : Percutaneous needle stimulation and surface recording of laryngeal EEMG was reliable and comparable to standard needle stimulation and invasive intramuscular needle recording. But the laryngeal EEMG by the surface recording and transcutaneous surface stimulation was not reliable and repeatable. Therefore we recommended that laryngeal EEMG by surface recording and percutaneous needle stimulation would be minimally invasive, reliable technique to know the status of reinnervation in e patients with vocal fold paralysis.

Spasmodic dysphonia is a voice disorder characterized by involuntary voice breaks during speech. Adductor spasmodic dysphonia is most common and characterized by strained and strangled voice breaks. The current standard of treatment of therapy for adductor spasmodic dysphonia is chemodenervation of thyroarytenoid muscle with botulinum toxin(Botox). However, Botox is a temporary treatment with each injection lasting approximately 3 months on average and require repeated injections. In this study, we report our experience with surgical treatment for adductor spasmodic dysphonia. In this procedure, the thyroarytenoid branch and lateral cricoarytenoid branch of recurrent laryngeal nerve is selectively denervated unilaterally, and its distal nerve stump of thyroarytenoid branch is reinnervated with branch of the usa cervicalis nerve. And lateral cricoarytenoid muscle partial myotomy was done unilaterally. After 6 months of treatment, voice fluency had improved and no period of breathiness or dysphagia was noted.

The incidence of extremity amputation is estimated at about 200,000 cases annually. Over 25% of patients suffer from terminal neuroma or phantom limb pain (TNPLP), resulting in pain, inability to wear a prosthetic device, and lost work. Once TNPLP develops, there is no definitive cure. Therefore, there has been an emerging focus on TNPLP prevention. We examined the current literature on TNPLP prevention in patients undergoing extremity amputation. A literature review was performed using Ovid Medline, Cochrane Collaboration Library, and Google Scholar to identify all original studies that addressed surgical prophylaxis against TNPLP. The search was conducted using both Medical Subject Headings and free-text using the terms "phantom limb pain," "amputation neuroma," and "surgical prevention of amputation neuroma." Fifteen studies met the inclusion criteria, including six prospective trials, two comprehensive literature reviews, four retrospective chart reviews, and three case series/technique reviews. Five techniques were identified, and each was incorporated into a targetbased classification system. A small but growing body of literature exists regarding the surgical prevention of TNPLP. Targeted muscle reinnervation (TMR), a form of physiologic target reassignment, has the greatest momentum in the academic surgical community, with multiple recent prospective studies demonstrating superior prevention of TNPLP. Neurorrhaphy and transposition with implantation are supported by less robust evidence, but merit future study as alternatives to TMR.
