A STUDY ON THE ACTUAL CONDITIONS OF CHILDREN′S REHABILITATION CENTERS IN SEOUL (서울지구 소아재활원 실태조사)

  • The social understanding of the rehabilitation and welfare work for the handicapped children is nearly devoid in Korea. Especially the medical rehabilitation or nursing for them is left ign. ored as well as the literature preferential to this study is wanted. For the purpose of conducting the study of this thesis Sam Yook Children's Rehabilitation Center, Inc. and Crippled Children's Center, Yonsei University Medical center sampled out; covering three months from Aug. 1 to Oct. 30, 1973, the problems of children's rehabilitation, the characteristics of the children, the medical rehabilitation, nursing, education, and vocational rehabilitation were interviewed and widely grasped. This thesis aimed at developing the rehabilitation, the professional nursing and especially aimed at contributing to the improvement of welfare for the handicapped by analysing, observing the result of this study. The result is as follows: 1. Sam Yook Children's Rehabilitation Center, Inc. has, since its establishment, contributed to the advancement of the crippled children and its purpose consists in implementing services, medical rehabilitation, nursing, education and vocational training. Crippled Children's Center, Yonsei University Medical Center is, being based on Christianity, aiming at the medical treatment and education for the crippled children. 2. All of the children admitted in the children's rehabilitation centers are the crippled children. The ratio of boys io girls is three to two. Boys are more than girls. Orphan formed 55.2 per sent of them. About 60 per cent of them is receiving medical attention free of charge. But there is no orphan in Crippled Children's Center, Yonsei University Medical Center. 3. 15.7 per cent of them have received the previous medical attention before their admitting in the centers; in Sam Yook Children's Rehabilitation Center 8.6 percent, Crippled Children's Center, Yonsei University Medical Center 50 percent; there is remarkable difference between the two. 4. On the standpoint of period of being in the centers, the children who have been over three years in Sam Yook Children's Rehabilitation Center formed 48.7 per cent; in Crippled Children's Center, Yonsei University Medical Center 2.6 percent; there is also considerable difference between the two; they couldn't discharge from the centers owing to the economic conditions and being orphan. 5. Among the diagnosis of the crippled children, poliomyelitis formed highest 51.7 percent of them ; cerebral palsy formed 30 percent secondly in order. Environmental factors (67.8 percent formed about three times of congenital factors (23.7 percent). 6. The children who are capable of doing independently activity of daily living formed 87.9 percent; 73.2 percent can walk about 300 m by wearing brace and so on. The ratio of wearing brace or leg prosthesis formed 47.4 per cent: crutch 44.3 per cent. The medical rehabilitation service and education for the crippled, are comparatively well carried out. But it is desired to improve and cultivate the vocational training, vocational guidance and special nursing to the insufficiency of their implementation. In the tendency that the rehabilitation and welfare for the handicapped are today emphasized, International Handicapped Rehabilitation Association, declaring that 1970s are the decade of rehabilitation, urged to improve positively the policy of rehabilitation and welfare for the handicapped. But here in korea the handicapped, being the object of social prejudice, ignorance, disdain lives in obscurity. Therefore the government or the community should draw up countermeasure on social under-standing, financial support, and rehabilitation services for the handicapped as well as should endeavor to make them participate in social activities as the productive total- person though they are physically imperfect.

The Effects of Scapular Stabilization Exercising on Dynamic Standing Balance in Stroke Patients (견갑골 안정화 운동이 뇌졸중 환자의 동적 서기 균형에 미치는 영향)

  • Purpose: We investigate to evaluate the effects of scapular stabilization exercise on dynamic standing balance in stroke patients. Methods: Thirty hemiplegic patients participated was divided into control group and training group randomly. Control group(n=15) had only general rehabilitation training and training group (n=15) had both the general rehabilitation training and intensive scapular stabilization exercise. They were treated for 15minute/5 times/4 week. We measured PALM (palpation meter) to measure affected side's distances from spine to scapular. We measured FRT (functional reach test), FSST (four squared step test), and BPM (balance performance monitering) to evaluate dynamic standing balance. Results: Training group showed a significant difference in the distances from spine to scapular, FRT, FSST, and variables of BPM comparing control group (p<0.05). But control group was not significant difference. Conclusion: These results suggest that scapular stabilization exercises have positive effects on dynamic standing position.

Neurofeedback Training for Anxiety: A Systematic Review (불안 감소를 위한 생기능자기조절 훈련(뉴로피드백) 임상연구: 체계적 문헌고찰)

  • Objectives: The purpose of this systematic review was to investigate the clinical effects of neurofeedback training on reducing anxiety. Methods: Eight databases were used to extract clinical reports on neurofeedback intervention for anxiety reduction published until 2016. We analyzed the characteristics of selected studies and evaluated biases using the Risk of Bias (RoB) assessment. Results: A total of 22 clinical trials were extracted for the analysis. The risk of bias in most studies was high or unclear. The Chinese Classification of Mental Disorders-3 (CCMD-3) was the most frequently used diagnostic criteria, the Hamilton Rating Scale for Anxiety (HAMA) was the most frequently used assessment tool, and the alpha wave activity increase, sensorimotor rhythm (SMR), and theta wave training were the most frequently used intervention methods. All papers showed a statistically significant decrease of anxiety symptoms; however, significant adverse events were not reported. Conclusions: Neurofeedback intervention might be beneficial for reducing anxiety. However, the quality of the studies used in the analysis was low, and the heterogeneity of the population and interventions was revealed. Therefore, more scientifically designed clinical studies regarding neurofeedback training are required.

Preliminary Study of Ambulation Training on Treadmill in Patient with Incomplete Spinal Cord Injury (트레드밀을 이용한 불완전 척수손상자 보행훈련의 사전 연구)

  • The purpose of this study was to investigate the effect of Treadmill Training on WISCI level, walking velocity, walking endurance, motor score and gait cycle of spinal cord injury patient with incomplete. Four subjects with spinal cord injury participated in this study. They took walking excercise 5 times per week for 8 weeks. One time excercise spent 30minutes. The theraputic effect was evaluated by WISCI level, walk 10 meters test, walk for 12 minutes test, motor score and gait cycle. Four subjects were examined before, after 8 week, walking training. Collected data were statistically analyzed by SPSS PC for Wilcoxon signed rank test. The results of this study are as follows; 1) In WISCI level, walking velocity, walking endurance and motor score, post - treatment score were higher compared to pre-treatment score with statistical significance(p<0.05). 2) In Rt SLS, DLSII and Lt SLS, post-treatment percentage were higher compared to pre-treatment percentage with statistical significance(p<0.05). but DLSI were not statistical significance(p>0.05). The findings suggest that spinal cord injury patients with incomplete can improve their WISCI level, walking velocity, walking endurance, motor score and gait cycle through Treadmill gait training.

Influence of Memory Intensive Training Program on Cognitive Function, Memory Performance, and Self-Esteem in Elderly People (기억강화프로그램이 노인의 인지기능, 자아존중감과 기억수행에 미치는 영향)

  • Purpose: This study was to confirm the influence of memory intensive training program on the elderly people's cognitive function, memory performance, and self-esteem. Method: Using a quasi-experimental or experimental design, 60 elderly aged over 60 years randomly assigned the experimental and control groups completed pretest-post evaluation. The experimental group participated in the memory intensive training program was offered to the participants in the experimental group for three weeks (2times/week). The t-test and $X^2$-test using SAS program. Results: 1) The cognitive function was significantly higher in the experimental group compared to that in the control group (t=3.26, p=.002). 2) The memory performance that included immediate word recall tasks, word recognition tasks and delayed word recall tasks was significantly higher in the experimental group than in the control group (t=5.30, p<.001). The experimental group showed significantly higher scores for memory performance than the control group (t=5.30, p<.001). 3) The self-esteem was higher in the experimental group than in the control group, but there was no significant difference between the two groups (t=1.94, p=.058). Conclusion: The Memory Intensive Training Program could be an effective intervention for improving cognitive function, and memory performance of the elderly people.

Effects of Treadmill Training with Kinesio Taping of Tibialis Anterior on Muscle Function, Tibialis Anterior Muscle Strength, and Gait Ability in Poststroke Patients

  • Objective: Stroke patients generally have problems with motor function, muscle weakness, and gait.This study was purposed toevaluate the effect of the treadmill training with kinesio taping of tibialis anterior (TKT) on muscle function, tibialis anterior, muscle strength, and gait ability in poststroke patients. Design: A randomized controlled design Methods: The participants were randomly divided in the TKT group (experimental group) and treadmill training with sham kinesio taping (control group), with 21 patients assigned to each group. Both groups receive treadmill training with kinesio taping and sham kinesio taping for 30 minutes per day, five days per week, for four weeks. The motor function was measured using the Fugl-Meyer assessment. A disital manual muscle test and G-walk were used to evaluate ankle dorsiflexor and gait ability.Evaluation was performed baseline and 4 weeks after the experiment. Results: Both groups showed significantly more improvement in muscle function, tibialis anterior muscle strength, cadence, gait velocity, and stridelength in pre-post intervention change(p<0.05).The experimental group showed significantly more improvement in motor function, muscle strength, cadence, gait velocity, and stridelength ability comparedto the control group(p<0.05). Conclusions: These finding show the benefits of treadmill training with kinesio taping for functional recovery in poststroke patients

Comparison of Feasibility of Touch-Based Cognitive Training Games in Community Elderly and Elderly with Minor Dementia

  • Objective: The purpose of this study is to select a cognitive training game that can evaluate five cognitive domains and to study their validity with existing cognitive evaluation tools. Design: Methods: Delphi survey was conducted through the 2nd questionnaire for 30 experts to select games suitable for training 5 cognitive domains. Five cognitive training games and Mini Mental State Examination - Korea(MMSE-K), and cognitive impairment screening test(CIST) were conducted for 82 elderly in the community. Pearson correlation analysis was performed to find out the correlation of the three tests. The ROC curve was used to calculate the selection criteria for the game results for the screening evaluation of the presence or absence of mild cognitive impairment. Results: The coefficient of variation to evaluate the stability of the Delphi survey was less than 0.50 in most game items. The 'correct answers' and 'level' of the five final selected game items showed a statistically significant positive correlation with the CIST and MMSE-K scores. CIST score and 'time' of all game items except 'number making_time' showed a statistically significant negative correlation. Conclusions: The validity of the cognitive training program using smart devices was evaluated, and the criteria for classifying the cognitive domain and distinguishing the presence or absence of cognitive impairment were confirmed.

  • The purpose of this study is to announce the present condition, walking training, and adaptable training of a limb amputated patient. The study is a successful report of the limb amputated patient through the medical treatment of the physical therapy. A cause of the limb amputated patient, a truck driver, was that the patient was hit by a train when the driver alight from the truck. Then, the driver was surgical operated on left AK (Above Knee) amputation and left AE (Above Elbow) amputation by orthopedics at the Young Dong Severance Hospital on Dec.7, 1996. Two weeks after the operation (Dec., 22, 1996), the patient was trained at the Yonsei Medical Center Physical Therapy Hospital for the walking and temper adjust training. It was possible to do a flat surface walking and a slope surface walking without helping due to the patients optimistic personal character and motivation. However, the patient struggled to a dull surface walking and his weak endurance. the patient has several problems when the patient wears artificial legs and hands, fears on fall down, and mentally worries on noise when he walks. It is necessary to approach for this problems by many fields of the helpers, such as Rehabilitation medical doctor, physical therapist, occupational therpist, artificial limbs makers, psychologists, and etc. Therefore, in order for recovering from the amputated parts function after the surgical operation, more approved reports have to be for the amputated patients due to increasing traffic accidents, industrial disaster, cancer, diabetes, obstacles of the peripheral nervous system, and etc.

Study on the Improvement of Postural Balance of the Elderly using Virtual Bicycle System (가상 자전거 시스템을 이용한 노인의 자세균형 증진에 관한 연구)

  • In this paper, a new rehabilitation training system was developed to improve equilibrium sense of the elderly by combining virtual reality technology with a fixed exercise bicycle. In order to evaluate the effectiveness of the training system, the elderly participated as test subject in the investigation of the influence of different the parameters on postural balance control. We measured three different running modes of virtual bicycle system with two successive sets. The parameters measured were running time, velocity, the weight movement, the degree of the deviation from the road, and the variables about the center of pressure. The repeated training, our results showed that the running capability of the elderly improve compared. In addition, it was found out that the ability of postural control and the equilibrium sense was improved with the presentation of the visual feedback information of the distribution of weight. From the results of this experiment, we showed that our newly developed system might be useful in the diagnosis of equilibrium sense or in the improvement of the sense of sight and, somatic, and vestibular sense of the elderly in the field of rehabilitation training.

  • Almost of stroke patients who experience aftereffects such as motor, sensory and cognitive disorders have to take some rehabilitation therapies. It is known that the consistent training for rehabilitation therapy in their home is more effective than rehabilitation therapy in hospital. A few home training programs were developed but these programs don't give any appropriate feedback messages to the client. Therefore, we developed a home training program which can provide appropriate feedback message to the clients using the Kinect sensor which can analyze user's 3-D dimensional motion. With this development, the client can obtain some feedback messages such as the knowledges of performance, results and self-controlled feedback. The program will be more effective than any existing programs.
