The Prevalence of Neck Disability Index among Some College Students (일부 대학생의 경부장애지수에 대한 분포)

  • Neck pain was commonly experienced by both adolescents and adults. The neck disability index(NDI) was the most commonly used self-report measure for evaluating status in neck pain. The objective of this study is to research the prevalence of the NDI in their 20's of college students in department of physical therapy. The data were collected from the NDI questionnaire of college students(males: 229, females: 405) for functional disability measures. Statistical analysis was carried out with SPSS 12.0 program, which was used for descriptive statistics, independent samples test and Pearson correlation analysis. The results were : First, 'headache' item was showed 'the highest prevalence' in second choice demonstrating the neck disability and 'pain intensity' in third choice. Second, female(46.4%) was higher distribution than male(36.3%) on score, percentage and grade of NDI(p<0.05). Third, the NDI grade was investigated that it is composed of 'no disability(63.8%)', 'mild disability(35.4%)', and 'moderate disability(0.9%)' in male and 'no disability(53.6%)', 'mild disability(44.7%)', 'moderate disability(1.5%)' and 'severe disability(0.2%)' in female. Fourth, there was a statically significant relationship between NDI grade and NDI items(p<0.05). These results have specific meaning as future clinical and epidemiological studies. Also, it is needed to education and preparation on prevention and management of neck disability among college students.

Effect of a Intentional Scapular Abduction on the Sit-and-Reach Test (의도적인 견갑골 외전이 Sit-and-Reach Test에 미치는 영향)

  • Purpose : This study was done to examine the actual effect of the scapular position in these flexibility tests. The purpose of this study was to examine the differences between the measurement of SRT(Sit-and-Reach Test) without intentional abduction of the scapular (pre-test) and with intentional abduction of the scapular (post-test). The hypothesis is: 1. There is no differences in the SRT result of the pre-test and the post-test. 2. There is no differences in the HJA(Hip Joint Angle) result of the pre-test and the post-test. 3. There is no differences in the Spine Motion Test(SMT) result of the pre-test and the post-test. Method : The total 60 people (30 men, 30 women) were participated in this study. In the pre-test, the subjects were asked to sit on the examination table and try the SRT motion; and then the HJA, SRT and the SMT numerical values were taken. In the post-test. the subjects were instructed to intentionally bring the scapula outward from the midline; and the HJA, SRT and the SMT numerical values were taken. Result : In the pre-test. the SRT result averaged 34.17cm. and in the post-test. the SRT result averaged 36.68cm. The difference was about 2.81cm which showed a significant mean statistically (p<0.01). The measurement increased by 8.22%. The HJA was $85.9^{\circ}$ in pre-test, and it was $85.5^{\circ}$ in post-test. giving the $0.4^{\circ}$ differences. Therefore, it didn't have a statistical mean (p>0.05). The SMT value was 69.56cm in the pre-test. and it was 69.28cm in the post-test, about 0.28cm decreased. Therefore, it didn't have a statistical mean (p>0.05). SRT values and HJA values were correlated (p<0.01). SMT values and SRT values (p<0.01), and SMT values and HJA values(p<0.05) were each in counter correlation. Conclusion : The result of the SRT without intentional scapular abduction (34.17cm) and with intentional scapular abduction (36.68cm) showed a significant increase about 8.22% (p<0.01). In SRT, the effect of the intentional scapular abduction on SMT showed no significant means, the pretest value being 69.56cm and the post-test value being 69.28cm (p>0.05).

The Effects on the Level of $\beta-endorphin$ and Pain Threshold according to each TENS and MENS Application (경피신경 자극과 미세전류신경근 자극이 $\beta-endorphin$ 과 동통역치에 미치는 영향)

  • The purpose of this study was to examine the amount of $\beta-endorphin$ and pain threshold according to time sequences in applying experimented TENS and MENS(pre treatment, posttreatment, 25 minutes after the treatment). This test was to aim at showing the effects of the experimental $\beta-endorphin$ and pain threshold applied to the randomly selected twenty healthy men in their twenties. The subjects were divided into two group(ten for TENS and the other ten MENS). The results obtained are as follows : 1. There was no significant difference in the $\beta-endorphin$ between TENS and HENS(p>0.05). There was no difference in the pain threshold at pretreatment and posttreatment(p>0.05), but there was some difference at 25 minutes after the treatment(p<0.05). 2. The group of TENS in the experimental $\beta-endorphin$ had the highest level of $14.40{\pm}3.098$ at posttreatment, but the level decreased a little according to time passed. And in the experimental pain threshold, the level went to the highest plateau of $2.92{\pm}0.483$ at 25 minutes after the treatment. 3. The group of MENS in the experimental $\beta-endorphin$ had the highest plateau of $14.20{\pm}3.967$ at posttreatment, but the level decreased a bit according to time passed also. And in the experimental pain threshold, the level went to the highest plateau of $2.49{\pm}0.617$ posttreatment. 4. There were some differences of the experimental $\beta-endorphin$ in TENS group at pretreatment and posttreatment(p<0.05). There were some differences in the experimental pain threshold between pretreatment and posttreatment as well as between pretreatment and 25 minutes after the treatment(p<0.05) MENS did not influence the experimental $\beta-endorphin$ and pain threshold. This experiment showed that TENS increased the levels of the experimental $\beta-endorphin$ at posttreatment and increased the levels of the experimental pain threshold untill 25 minutes after the treatment. Therefore, the time of sustaining pain in TENS group was longer than that of MENS group. Also, MENS showed that it increased each level of the experimental $\beta-endorphin$ and pain threshold, but these levels were not statistically meaningful.

Neonatal Late-onset Hypocalcemia: Is There Any Relationship with Maternal Hypovitaminosis D?

  • Purpose: Neonatal late-onset hypocalcemia is defined as hypocalcemia developed after postnatal 3 days and associated with hypoparathyroidism, high phosphate diets and vitamin D deficiency. We experienced the increment of neonatal late onset hypocalcemia over 1 year. We tried to evaluate the relationship between late onset hypocalcemia and maternal hypovitaminosis D. Methods: The medical records in the neonates with late-onset hypocalcemia during January 2007 to July 2008 were retrospectively reviewed. Among those patients, 17 paired sera of mothers and neonates had collected. The levels of 25-OH vitamin D (25OHD) and intact parathyroid hormone (iPTH) were measured and were compared with neonate and the mother. Results: The mean gestational age was $38^{+1}$ weeks, and the mean body weight was 2,980 g. The onset time of hypocalcemia was 5.9 days of age. Most of them (88.2%) were feeding with formula and no one was only breast milk feeding. Of the 17 patients, 13 were born in spring or in winter. The median levels of calcium, phosphorus, alkaline phosphatase, iPTH and 25OHD were 7.0 mg/dL, 8.6 mg/dL, 191.0 U/L, 57.2 pg/mL and 24.0 ng/mL in neonates. The levels of 25OHD of 6 neonates were <20 ng/mL. A total of 16 mothers were considered vitamin D-deficient (<20 ng/mL), and vitamin D insufficient (20<25OHD<30 ng/mL). Conclusion: Neonatal late-onset hypocalcemia in our study seems to be influenced by maternal vitamin D deficiency and insufficiency. Sun tanning and vitamin D supplements from winter to spring would be helpful to prevent maternal vitamin D deficiency, one of the causes of neonatal late-onset hypocalcemia.

Evaluation of Effect of Renal Transplantation on Growth in Children with Chronic Renal Failure (소아 신장이식 후 성장에 대한 평가)

  • Purpose : We aim to identify the clinical and demographic characteristics in children who underwent renal transplantation(RTx) and to evaluate the influence on growth of RTx in children. Methods : We reviewed 17 medical records of chronic renal failure patients who underwent RTx from April 1992 and June 2004 at Busan Paik Hospital. Age and sex distribution, cause of disease, donor analysis, patient and graft survival rate, and the status of growth after RTx were analysed by retrospective study. Results : Eighteen RTx were performed in 17 patients(8 boys, 9 girls). The mean age at the time of RTx was $15.8{\pm}3.5$ years and the mean duration of dialysis therapy before RTx was $22.4{\pm}18.0$ months. The 1 year and 5 year patient survival rate were each 100%, and the 1 year and 5 year graft survival rate were 88%, 36% respectively. The most common cause of graft failure was chronic rejection. The mean final height of male patients was $162.8{\pm}10.0$ cm(143.0-172.5 cm) and of female patients was $154.5{\pm}12.1$ cm(135.8-160.0 cm). The mean height standard deviation score(Ht SDS) increased after RTx from -1.95 to -1.53 but the increment rate was not statistically significant. Similar changes were noted in individual patient analysis. Also there was no significant difference between the living-related donors and cadaveric donors. Conclusion : Our data shows that even successful RTx rarely results in full growth rehabilitation. To overcome retarded growth in children with chronic renal failure, appropriate combined management of metabolic and nutritional problems, correction of anemia, proper use of recombinant growth hormone therapy, early renal transplantation and shortening of the duration of dialysis would be necessary.

Changes of Surface Electromyography on Frontalis Muscle during Electro-Acupuncture Stimulation of Abdomen or Legs - A Pilot Study for Clinical Approach to Autonomic Nervous System Changes - (복부 및 사지 부위의 전기침 자극이 전두근에서 측정한 SEMG 변화에 미치는 영향 - 자율신경계의 변화에 대한 임상적 접근을 위한 사전 연구 -)

  • Objectives : To observe physiological changes during clinical acupuncture treatment. Methods : We recruited 40 healthy volunteers who had experienced an acupuncture treatment at least once within the past three years. The experimental group was divided into four groups according to the needling site and frequency of electrical stimulation. Sites consisted of abdomen and legs. Frequencies consisted of 100 Hz and 2 Hz. The procedures of experimental treatment consisted of seven phases, Resting I phase (Resting I), Needle insertion phase (Insertion), Maintenance of needle insertion I phase (Maintain I), Electrical stimulation phase (ES), Maintenance of needle insertion II phase (Maintain II), Needle removal phase (Removal) and Resting II phase (Resting II). We measured the surface electromygraphy (SEMG) through an electrode on the frontalis muscle during all phases consecutively. Results : When SEMGs of all seven phases were analyzed, they significantly increased or decreased according to phases. SEMGs of Insertion, Maintain I, ES and Maintain II phase significantly increased more than RestingI in abdomen and legs groups. SEMGs of the abdomen group were measured as being $4.78{\pm}0.74{\mu}V$ on Resting I, $16.48{\pm}3.97{\mu}V$ on Insertion, $46.31{\pm}10.56{\mu}V$ on Maintain I, $45.88{\pm}9.72{\mu}V$ on ES, $45.56{\pm}9.69{\mu}V$ on Maintain II, $18.76{\pm}3.05{\mu}V$ on Removal, and $3.75{\pm}0.65{\mu}V$ on Resting II. SEMGs of the legs group were measured as being $3.34{\pm}0.35{\mu}V$ on Resting I, $12.11{\pm}1.76{\mu}V$ on Insertion, $36.74{\pm}6.99{\mu}V$ on Maintain I, $33.57{\pm}6.30{\mu}V$ on ES, $32.66{\pm}6.03{\mu}V$ on Maintain II, $14.08{\pm}2.15{\mu}V$ on Removal, and $2.88{\pm}0.32{\mu}V$ on Resting II. Conclusions : SEMG changed differently according to processes of acupuncture. Electrical stimulation showed different change of SEMG. Thus, acupuncture treatment may change the status of the autonomic nervous system.

Correlation between Instrument on Pattern Identification for Depression and Psychological Tests by Statistical Analysis (통계적 분석을 통한 우울증 변증도구와 심리검사의 상관성 연구)

  • Objectives: This study was performed to determine the correlation between Instrument on Pattern Identification for Depression and Psychological tests by Pearson Correlation Coefficient and Regression analysis.Methods: Two assessors carried out the evaluation using the instrument on pattern identification for depression. They also performed the following psychological tests: Beck Depression Inventory (BDI), State-Trait Anxiety Inventory (STAI), State-Trait Anger Expression Inventory (STAXI), the Temperament and Character Inventory (TCI), Insomnia Severity Index (ISI_Total), Self-disclosure Inventory, subjective well-being Inventory, Health perception Inventory, and Personality Assessment Inventory (PAI). A total of 167 participants who got HAM-D score over 12 were targeted for the evaluation. Our research team carried out Pearson correlation coefficient analyses and regression analysis between pattern identification for depression and Psychological tests. We listed the results by descending order and interpreted the results.Results: Pearson correlation analysis revealed the following results: 1) Stagnation of Liver Gi was associated with BDI (0.60) and STAI (0.55); 2) Dual Deficiency of the Heart and Spleen was associated with BDI (0.60), ISI_Total (0.52), and STAI (0.42); 3) Relieving stagnation of Phlegm-Gi was associated with BDI (0.65), STAI (0.54), and Subjective well-being inventory (−0.52); 4) Gi-deficiency Mingled with sputum was associated with BDI (0.50), ISI_Total (0.40), and STAI (0.395); 5) Stagnant Gi transforming into fire was associated with BDI (0.56), STAI_TR (0.51), and Health perception Inventory (−0.458); 6) Yin-Deficiency with Effulgent Fire was associated with BDI (0.55), ISI_total (0.54), and STAI (0.41).Conclusions: Through correlation analysis between Instrument on Pattern Identification for Depression and Psychological tests, we could suggest a System for Oriental Medical Diagnosis for Depression.

Development of Occupational Therapist Competency Indicators for Occupation-Based Intervention: Delphi Study (작업기반 중재를 위한 작업치료사 역량지표 개발: 델파이연구)

  • Objective : The purpose of this study was to develop a competency indicators of occupational therapists for occupation-based intervention by conducting a Delphi survey to occupation-based intervention experts. Methods : After analyzing the data related to occupational therapists indicators in korea, the results of Delphi survey were collected three times from October to December 2016. Based on the results, we developed occupational therapists competency indicators for occupation-based intervention through expert panel's opinions. Results : Based on the eight competency elements, the competency indicators were examined by 133 primary and 133 secondary competency indicators. Finally, 131 competency indicators were developed based on 8 components as occupational therapist competence indicators for occupation-based intervention. The results of the third delphi showed a high level of content validity of .95, stability of .10, convergence of 0, and consensus of 1. Conclusion : The developed occupational therapist competence indicators for occupation-based intervention could be used in various clinical decision making such as occupation-based intervention, education training, education development and evaluation.

Characteristics of ROM and EMG to Balance Training in Unstable Plate System: Primary Study (균형 훈련 플레이트 시스템을 이용한 생체역학적 특성 연구)

  • The purpose of this study was to investigate the unstable plate system for the advanced balance ability. 7 male volunteers (age $33.7{\pm}1.2$ years, height $174.7{\pm}3.8cm$, weight $86.0{\pm}3.6kg$, BMI $28.2{\pm}2.0kg/m^2$) performed the partial squat motion on the shape of CAP type(${\cap}$) and BOWL type(${\cup}$) plate system. The range of motion (ROM) and muscle activation were acquired by the motion analysis system and the EMG system. Results of ROMs of the CAP type plate system were shown the widely range of the deviation in the ankle joint on the sagittal plane (sagittal plane - hip joint $10.7^{\circ}$ > $5.4^{\circ}$, knee joint $16.3^{\circ}$ > $6.4^{\circ}$, ankle joint $18.8^{\circ}$ > $6.3^{\circ}$ ; transverse plane - hip joint $3.5^{\circ}$ > $1.8^{\circ}$, knee joint $5.3^{\circ}$ > $3.4^{\circ}$, ankle joint $11.3^{\circ}$ > $5.3^{\circ}$ ; frontal plane - hip joint $0.9^{\circ}$ > $0.5^{\circ}$, knee joint $0.8^{\circ}$ > $0.6^{\circ}$, ankle joint $4.8^{\circ}$ > $3.7^{\circ}$). Muscle activation results of the CAP type plate system were indicated higher in major muscles for balance performance than the BOWL type plate system (vastus lateralis 0.90 > 0.62, peroneus longus 0.49 > 0.21, biceps femoris 0.38 > 0.14, gastrocnemius 0.11 > 0.05). These findings may indicate that the CAP type plate system would expect better effectiveness in perform the balance training. This paper is primary study for developing balance skills enhancement training device.

The Effect of Home Exercise Program on an Chronic Low Back Pain (가정 운동 프로그램이 만성요통환자에 미치는 영향)

  • Background: To study the effect of the home exercise program on pain, flexibility, endurance of extensor in chronic back pain patients, and suggest optimal method for home exercise program. Methods: I divided into two groups who has chronic back pain; one is control group who was given a treatment at the hospital only and the other is experimental group who did another exercise after treatment at the hospital, and there were 10 people in each group. The manual therapy were given to all the patients in each group after applying a stupe and an electric treatment, but the experimental group conducted another exercise program at homes. All the exercise programs were applied to patients 12 times for 4 weeks totally. Result: SPSS for win version 12 was used for statistic analysis and independent t-test was used to find changes between two groups. VAS scale was used to show changes in pain between each group. The grade of pain was decreased between pre&post test to -5.60 in control group and -4.80 in experimental group but there wasn't significant difference between each group. Finger tip-to-floor test was used for the flexibility changes and it was increased between pre&post test in both groups but the change of flexibility between each groups didn't show statistical difference. Biering-Sorensen test was used to measure the endurance of extensor and it was increased between pre&post test in both groups but there wasn't significant difference between each group. Conclusion: As you read the results above, for a chronic low back pain patient, application of the manual therapy showed that it has effect on decrease of low back pain, increase of flexibility and endurance of extensor. However, the effect of home exercise treatment was not sure about improvements for chronic low back pain patient. So I think there should need further study about the effect of home exercise treatment except the treatments at hospital and the thorough education for the exercise of lumbar should be done before the study for the accurate experiment.
