A Case report on the Biceps Tendinitis Treatment with Fire needling Therapy (보존적 치료로 호전되는 않은 상완이두근 건염에 대한 화침을 포함한 한방 치료 1례)

  • Objective : The purpose of this study is to evaluate the effect of the fire needling therapy treatment on the biceps tendinitis. Methods : Fire needling therapy was administered five times to the patient complaining with biceps tendinitis. The improvement of clinical symptom was evaluated by McGill Pain Questionnaire -Short Form(SF-MPQ), Visual Analogue Scale(VAS) and Shoulder Pain, and Disability Index (SPADI). Result : After fire needling therapy to the biceps tendinitis patient five times, the patient's SF-MPQ score, VAS, and SPADI improved from 24 to 11, from 9 to 4, and from 33.6 to 14.8 respectively. Conclusions : Fire needling for biceps tendinitis should be tested in future randomized clinical trials.

The Effect of Using a Two Step Verbal Cue to a Visual Target above Eye Level on the Parkinsonian Gait (파킨슨병 환자 보행에서 눈 높이 위수준의 시작 목표에 대한 두 걸음 구두 암시의 효과)

  • It is well known that visual cues can improve the motor performance of Parkinsonian patients. Previous laboratory studies have examined the effects of visual cueing to the floor. This case study examined the effects of using a visual cue above eye level on the gait of a Parkinsonian man. It was found that cueing the patient to a target above eye level while waking not only improved the kinematic parameters of the gait cycle but also facilitated a more functional gait pattern with re-intergration of arm swing, rhythm, heel strike and a more erect posture. Visual targeting above eye level may serve as an important clinical tool for physiotherapists treating Parkinsonian patients.

A Study of Perception of Caregiver in Rehabilitation exercise of Companion Animal (반려동물의 재활운동에 대한 보호자의 인식도 조사)

  • Background: This study attempted to find out the degree of awareness of owners who have the most influence on companion animal's rehabilitation exercise, and the purpose of the treatment effect, and the expectations of rebilitation treatment. The purpose of this study is to find out about the awareness, comprehension, and expectations of caregiver for rehabilitation exercise in companion animal medical services, and to develop them into a field of animal medical services. Design: The tool used in this study used a questionnaire, and the composition consists of the subject's general characteristics, recognition of companion animal rehabilitation, comprehension and expectation. Methods: A questionnaire was given to 103 companion animal's owners, and all were collected and analyzed. Analysis of the frequency analysis method, Fisher's exact test were analyzed. Results: It was confirmed that the difference was significant in the question of whether the caregivers had heard companion animal rehabilitation (p<.05). When asked that rehabilitation is necessary to improve physical function, the difference was confirmed to be significant (p<.05). The significance was confirmed when asked if the caregiver was willing to participate in animal rehabilitation education (p<.05). When asked if caregiver was willing to recommend rehabilitation exercise for companion animal of other caregiver who need rehabilitation exercise, it was confirmed that the difference was significant (p<.05). Conclusion: There was significant difference between gender. This study confirmed that the definite concept caregivers' perception in animal rehabilitation was not established. Therefore, in order to improve perception of caregivers, interdisciplinary exchanges and publicity for animal rehabilitation are needed, and the development of training and education programs for rehabilitation experts was suggested.

A Phenomenological Study on Developmental Rehabilitation Service Providers' Group Art Therapy Experiences (발달재활서비스 제공 치료사의 집단미술치료 경험에 대한 현상학적 연구)

  • This study aimed to clarify the meanings of developmental rehabilitation service providers' experiences of participation in a group art therapy. The 5 participants in this study were composed of cognitive, language and art therapists working at children developmental rehabilitation center in G city. The group art therapy was provided to participants from October to December, 2018. And the data were collected using in-depth interviews from January to February, 2019. Collected data was analyzed using the phenomenological method by Colaizzi(1978). As a result, four categories were derived: "Purification of emotions", "New discovery", "Encouragement and comfort", and "Close relationship". In conclusion, therapists participating in a group art therapy experienced purification of emotions and newly discovered about themselves and colleagues. And through encouraging and comforting in the group, they became closer. These results may serve as basic data on using art therapy for giving psychological support to therapists who provide developmental rehabilitation service.

Effects of GaAIAs Diode Laser for the Expression of TGF-$\beta$ on Lumbar Spinal Cord after Extensor Digitorum Muscle Crush Injury (GaAlAs 다이오드 레이저 조사가 장지신근 압좌손상 후 요수분절의 TGF-$\beta$ 발현에 미치는 영향)

  • Low intensity laser irradiation is potential physical agent that triggers the muscle regeneration by previous study. In muscle regeneration, a number of growth factors also promotes that is triggered in response to muscle damage. The transforming growth factor(TGF)-$\beta$ is involved in the activation of cell proliferation and the inhibition of cell differentiation in muscle regeneration. This is secreted not only autocrine system but also paracrine and endocrine. Therefore, We investigated that effects of Gallium aluminum arsenide(GaAlAs) diode laser for the expression of TGF-$\beta$ on lumbar spinal cord after extensor digitorum muscle crush injury. After laser irradiation, the immunoreactivity of TGF-$\beta$ was increased bilaterally in gray mater of spinal cord. Especially, in 1 day, experimental group was highed than control, and in 3 day, lateral motor nucleus were storong immunoreactivy of TGF-$\beta$. Also, in 1 and 2 day, TGF-$\beta$ was showed in white mater as well as gray mater, but in 3 day, only showed in gray mater. These data may suggests to the establishment of laser irradiation on spinal cord for skeletal muscle injury.

Factors related to the treatment duration of infants with congenital muscular torticollis

  • Objective: Congenital muscular torticollis (CMT) is a disease with abnormal head and neck posture in infants. It affects the child's movement and development and can lead to complications. Therefore, this study aims to find out what factors influence the rehabilitation treatment duration of infants with CMT. Design: Cross-sectional study. Methods: The subjects were 63 infants under 90 days of age who were diagnosed with CMT. Age, thickness of the sternocleidomastoid muscle (SCM) on the affected and non-affected side, head tilt angle, and head rotation angle of the affected head in infants were collected. The ratio of muscle thickness was calculated from the thickness of the affected SCM and the thickness of the non-affected SCM (A/N ratio). All subjects underwent conservative physical therapy twice a week for 30 minutes, and the end of the treatment was when the angle of head tilt was normal or less than 5 degrees, and the treatment duration was calculated. Results: Age, thickness of affected SCM, and head tilt were significantly correlated with treatment duration (p<0.05). The thickness of the non-affected SCM, A/N ratio, and head rotation angle did not show any correlation with treatment duration. The factors affecting the treatment duration were head tilt and age, showing 21% explanatory power of adjusted R2. Conclusions: The main factors affecting the treatment duration of infants with CMT are head tilt and age. Therefore, more attention should be directed to the infant's head tilt and age for effective physical therapy of infants with CMT.

The effect of single trial transcutaneous electrical nerve stimulation on balance and gait function in elderly people with dementia: a pilot study

  • Objective: Elderly people with dementia experience not only cognitive dysfunction but also motor function deficits, such as balance and gait impairments. Recently, transcutaneous electrical nerve stimulation (TENS) has been used to reduce pain as well as to control muscle spasm, spasticity and motor performance in various types of subjects. The purpose of this study determined the effect of a single trial TENS on balance and gait function in elderly people with dementia. Design: A randomized controlled trial. Methods: Twenty-two subjects with dementia were assigned to the TENS group (n=12) and ten subjects were assigned to the control group (n=10) randomly. Subjects were classified into two groups: the TENS group (n=12) and the placebo-TENS group (n=10). The TENS group had electrical stimulation applied on the calf muscle for 15 minutes, while the placebo-TENS group had not received real electrical stimulation. The timed up and go test (TUG) and functional reach test (FRT) were used to measure the balance function, and the 10 meters walk test (MWT) and 6MWT were used to assess gait ability. All tests were performed before and after intervention under a single-blinded condition. Results: After intervention, there were significant improvements in TUG, FRT, 10MWT, and 6MWT results in the TENS group (p<0.05), while the placebo-TENS group did not show significant changes in all outcome measurements. There were also differences in all tests between the two groups at post-measurements (p<0.05). Conclusions: This study demonstrated that a single trial TENS application on the calf may be used to improve balance and gait function in elderly people with dementia.

A Comparison of EMG Activity for Middle and Lower Trapezius Muscle According to Shoulder Abduction Angles (견관절 외전 각도에 따른 중간 및 하부 승모근의 근 활성도 비교)

  • The purpose of this study was to compare electromyography (EMG) activity for the middle and lower trapezius muscle according to various shoulder abduction angles. Thirty healthy male subjects were recruited for this study. Each subject performed three repetitions of horizontal abduction at $75^{\circ}$, $90^{\circ}$, $125^{\circ}$, $160^{\circ}$, and $180^{\circ}$ of shoulder abduction angle in a prone position. Surface EMG activity was recorded from the middle and lower trapezius while the subjects held for five seconds at a predetermined position. The EMG activity was normalized using the maximal voluntary isometric contraction (MVIC) elicited using a manual muscle testing technique. A repeatedly measured ANOVA was performed by Bonferroni's post hoc test. The EMG activity of the middle and lower trapezius was significantly different among shoulder abduction angles (p<.05). The greatest level of muscle activity for the middle and lower trapezius were demonstrated at $90^{\circ}$ and $160^{\circ}$ of shoulder abduction angle, respectively. These results suggest that middle and lower trapezius strengthening exercise and manual muscle testing should be performed at different shoulder abduction angles.

The Clinical Effects of Heating-conduction Acupuncture Therapy with General Acupuncture Therapy for Lateral Ligament Injury Induced by Ankle Sprain Grade II (전열침(傳熱鍼)과 일반 침의 병행치료가 발목 염좌로 인한 외측인대 2도 손상에 미치는 임상적 효과)

  • Objectives: The purpose of this study is to investigate the clinical effects of heating-conduction therapy on lateral ligament injury induced by ankle sprain grade II. Methods: The 67 outpatients who were diagnosed as ankle sprain grade II were performed heating-conduction acupuncture therapy on lateral ligament injury. To evaluate the efficiency of heating-conduction acupuncture therapy, pain threshold with pressure algometer and visual analogue scale(VAS) were applied pretreatment and after 1st, 2nd treatment. Results: 1. The pain threshold and VAS score after 1st and 2nd treatment with an isolated injury of the anterior talofibular ligament showed statistically significant improvement compared with pretreatment. 2. The VAS score after 1st and 2nd treatment with an combined injury to the anterior talofibular and the calcaneofibular ligament showed statistically significant improvement compared with pretreatment. 3. The VAS score after 1st and 2nd treatment with an combined injury to the anterior talofibular, calcaneofibular and posterior talofibular ligament showed statistically significant improvement compared with pretreatment. 4. After 1st and 2nd treatment on the posterior talofibular ligament and after 2nd treatment on the anterior taofibular and calcaneofibular ligament with an combined injury to the anterior talofibular, calcaneofibular and posterior talofibular ligament the pain threshold showed statistically significant improvement compared with pretreatment. 5. Between isolated injury and combined injury of lateral ligament, the difference in pain threshold and VAS score was not statistically significant. Conclusions: Heating-conduction acupuncture therapy has clinical effects of pain reduction on patient with an isolated injury and combined injury of lateral ligament induced by ankle sprain grade II.

Chuna Manual Therapy for Adult Constipation : A Systematic Review (성인 변비에 대한 추나요법의 효과 : 체계적 문헌 고찰)

  • Objectives : This study aimed to determine the evidence of effectiveness and safety of Chuna manual therapy for adult constipation patients. Methods : We searched 10 electronic databases(Ovid-MEDLINE, Pubmed, EMBASE, Cochrane Library, CAJ, Oasis, KISS, NDSL, KMBASE, KISTI) and related 2 journals up to October 2016. We included randomized controlled trials(RCTs) of testing Chuna manual therapy for adults constipation patients. The methodological quality of RCTs related assessed by the Cochrane risk of bias tool. Results : Nine RCTs were eligible in our inclusion criteria. The meta-analysis of 6 studies showed positive results for the use of Chuna manual therapy for constipation. Conclusions : There is favorable evidence of Chuna manual therapy for treating adult constipation with meta-analysis. However, our systematic review has limited evidence to support Chuna manual therapy for constipation because of low quality of original articles and further well-designed RCTs should be encouraged.
