Modified Anterior Craniofacial Osteotomy Using Partial Nasal Bone Division and Reconstruction in Frontoethmoidal Sinus Meningioma

  • Typical transcranial approaches are insufficient for adequate visualization and resection of skull base tumors. Different approaches with multiple modifications have been attempted. Here, we describe a new approach for a lesion that is central and hard to treat by conventional craniotomy and successful reconstruction with calvarial bone graft and titanium mesh plate. A 69-year-old female patient presented with recurrent meningioma. The tumor had invaded the frontal lobe, right supraorbital rim, and ethmoidal bone. We performed a modified anterior craniofacial approach that fully exposed the tumor and invaded bone. In consideration of the patient's age and cosmetic result, the tumor and invaded bone was resected and the defect area was reconstructed with titanium mesh and calvarial bone graft. At 6 months postoperative the patient had no complications and was satisfied with the esthetic result. We report this case to demonstrate the successful approach and reconstruction using this technique.

  • The closure of a wide alveolar cleft and fistula in cleft patients and the reconstruction of a maxillary dentoalveolar defect in bilateral cleft lip and palate (BCLP) patients are challenging for both orthodontists and oromaxillofacial surgeons. It is due to the difficulty in achieving complete closure by using local attached gingiva (palatal flap) and the great volume of bone required for the graft. In this article, the authors used bifocal distraction-compression osteosynthesis(BDCO) to create a segment of new alveolar bone and attached gingiva for the complete approximation of a wide alveolar cleft/fistula and the reconstruction of a maxillary dentoalveolar defect. Since the alveoli and gingivae on both ends of the cleft were approximated after BDCO, the need for extensive alveolar bone grafting was eliminated. It also could create new alveolar bone and gingiva for orthodontic tooth movement and implant.

Awake intubation in a patient with huge orocutaneous fistula: a case report

  • Mask ventilation, the first step in airway management, is a rescue technique when endotracheal intubation fails. Therefore, ordinary airway management for the induction of general anesthesia cannot be conducted in the situation of difficult mask ventilation (DMV). Here, we report a case of awake intubation in a patient with a huge orocutaneous fistula. A 58-year-old woman was scheduled to undergo a wide excision, reconstruction with a reconstruction plate, and supraomohyoid neck dissection on the left side and an anterolateral thigh flap due to a huge orocutaneous fistula that occurred after a previous mandibulectomy and flap surgery. During induction, DMV was predicted, and we planned an awake intubation. The patient was sedated with dexmedetomidine and remifentanil. She was intubated with a nasotracheal tube using a video laryngoscope, and spontaneous ventilation was maintained. This case demonstrates that awake intubation using a video laryngoscope can be as good as a fiberoptic scope.

  • Background: Galeal or temporalis muscle flaps have been traditionally used to reconstruct skull base defects after tumor removal. Unfortunately, these flaps do not provide sufficient vascularized tissue for a dural seal in extensive defects. This study describes the successful coverage of large skull base defects using anterolateral thigh (ALT) free flaps. Methods: This retrospective study included five patients who underwent skull base surgery between June 2018 and June 2021. Reconstruction was performed using an ALT free flap to cover defects that included the intracranial space and extended to the frontal sinus and cribriform plate. Results: There were no major complications, such as ascending infections or cerebrospinal leakage. Postoperative magnetic resonance imaging showed that the flaps were well-maintained in all patients. Conclusion: Successful reconstruction was performed using ALT free flaps for large anterior skull base defects. In conclusion, the ALT free flap is an effective option for preventing communication between the nasal cavity and the intracranial space.

  • Three dimensional density distributions of impinging and eccentric flames have been investigated by digital speckle tomography. The flames have been ignited by a mixture of butane and air from a circular nozzle and impinged against a plate located at the upper side of the burner exit. For comparison with experimental data, computer synthesized phantoms of impinging and eccentric flames have been reconstructed by a developed three-dimensional multiplicative algebraic reconstruction technique (MART). The advanced reconstruction in the stagnation flow region involved the sharp change of the flow direction and pressure gradient has been developed using a cross-correlation method and new scanning technique for the speckle displacement.

Polarization recording and reconstruction in the chalcogenide As-Ge-Se-S thin films (비정길 칼코게나이드 As-Ge-Se-S 박막에서 편광기록 재생)

  • Chalcogenide glasses are suggested as a candidate for optical recording. In this study, we have investigated the holography recording and reconstruction of the polarization state in chalcogenide As$_{40}$ Ge$_{10}$Se$_{15}$ S$_{35}$ thin films. We have used a He-Ne laser light(633nm) to probe and record of the grating. Also the polarization state of object beam was modulated with a λ/4 wave plate. The polarization state of the +1st order diffracted beam was generated by readout of the grating with a linearly polarized reference beam. It was the same-handed polarization state as the polarization state of the recording beam. The result is shown that the diffraction efficiency of circularly polarized recording represents higher than other polarization state.ate.e.

  • The authors have realized reconstruction of 3D shape using contacting method with pins and TCP/IP communication. In this paper, the working principal of the mechanism is proposed and tested. A special sensor system is designed as 148 matrix form to measure 3D shape. When contact occurs between pin and the plate of the sensor matrix, the position information of pins is sent to computer and raised height data of the pin is obtained by counter which accumlates encoder signal. So, all datum which contain 3-dimensional coordinate, is transferred using TCP/IP communication. Finally, 3D shape is reconstructured by Web browser of remote computer. The measuring result shows that the proposed mechanism is reliable and promising as a remote measuring device through Internet.

  • We present a numerical procedure that compensates for tilt phase aberration in in-line digital holography by computing the period of interference patterns in the reconstructed phase image. This method enables the reconstruction of correct and accurate phase information, even if strong tilt aberrations exist. Example applications of tilt aberration compensation are shown for a tilted plate, a micro-lens array, and a thin film transistor. This method is convenient because it uses only one hologram and no hardware to minimize the tilt aberration.

  • This paper describes the accident analysis by modelling the current transformer mounting in a metal-clad switchgear at 22.9kV. In analyzing the accident, the reconstruction at the current transformer mounting(VCB connecting guide) has to be taken into account. The accident was modelled as a 3-phase ground fault occurring between the end plate of 22.9kV lines and the safety shutter at the current transformer mounting of the VCB inside the metal clad switchgear. Since the outside maintenance of the metal clad switchgear is restricted by the enclosed compartments, Its circumference has to be kept clean. Through the reconstruction results, it was confirmed that the fault of the enclosed switchboard could be reduced when the shutter made of Fe material was chanted into an insulation.
