  • Infection of a median sternotomy incision may result in a large, unsightly, unstable, and potentially fatal wound. During the past 8 years, 5 consecutive patients [ 4 male and 1 female ] had repair of infected sternotomy wound. We describe our current preferred techniques and the results we have achieved with them. As soon as the sternal infection was shown, operative wound was opened and irrigated more than 4 times a day with 0.5% Betadine iodine solution until the exudate became clean with no growth of bacteria. Operation was performed in one-stage, which consisted of aggressive debridement of the infected bone and muscle transposition. Reconstruction was with one-side or both pectoralis flaps in all patients and rectus abdominis in 2 patients. There was no mortality or morbidity within 30 days postoperatively. We conclude that early aggressive debridement and muscle transposition remain the treatment of choice for most patients with infected median sternotomy wounds.

  • Although there has been an upcoming recognition in the repairing reinforcement and remodeling of currently existing buildings with a regard to economic and resource saving effects of buildings roughly exposed to the open air for a long-term period, followed by a number of problems with the construction business operations that support economy-oriented projects within the limits of their short-term period durability, it is true that reasonable decisions of concrete performance are insufficient owing to the lack of proper history management for those existing buildings. This research attempted to comparatively analyze the compression strength together with investigation of carbonization depth and alkali concentration according to the passage of years on the subjects of aged buildings, and to provide basic data for remodeling and/or reconstruction of future construction structures by indirectly estimating durability lifetime expectancy according to carbonization phenomenon.

  • The purpose of this study wasto understand Theraplay interaction and to find out how Theraplay works to bring about change in withdrawn children. In exploring the meaning of Theraplay interaction, I used a phenomenology, which looked reflectively into a child's experience. Examining the situation, I found that the child had changed by "touch" with a therapist, which had been done throughout thewhole process of therapy. The child had changed from "child with horny body" to "playful child." Furthermore, I realized the four meanings (of what?) through research: "encounter", "reconstruction", "responsibility", and "immersion." Those four meaningscould have made the subject of interaction go beyond and look for its own existence. In addition, those four meanings melt together in the phenomena of interaction. There were always those four meanings throughout Theraplay interaction. This study has presented different consequences, compared to Theraplay research that is based on effectiveness, and examined the process of change that includes position tendency.

  • Construction plans for large construction projects are problems such as Incompatibility of information, omissions due to technical limitations of 2D CAD. In addition to, Plan established for the validation such as process, cost and construction technology plan is difficult. By occurring Frequent changes in the course of business, Inefficiency of management has occurred. In the construction process, there is occurred costs and construction delays due to reconstruction or communication error. In this study, It was deemed BIM will complement Existing 2D CAD problem. And, BIM case studies in the construction phase of domestic construction were analyzed, How to apply BIM in the construction phase investigated.

  • A New kinetic scheme for the compressible Navier-Stokes equations is developed. While the conventional approach, such as KFVS scheme, employs the splitting algorithm and computes the numerical flux on the basis of the collisionless equation, the present approach employs the splitting algorithm in the evaluation of the numerical flux, where the collision effect is explicitly taken into account. However, the initial condition employed in the computation is slightly different from the conventional Chapman-Enskog NS distribution function. The present study also reveals the background of the existing kinetic schemes. such as the KFVS scheme and Gas-Kinetic BGK scheme.

  • It is very important to allocate valuable resources efficiently when reconstructing buildings after earthquake damage. This paper proposes the use of a simple seismic retrofitting system to make buildings more resilient than the stiffer systems such as the shear walls implemented in Chile after the earthquake in 2010. The proposal is based on the use of steel chevron-type braces in RC buildings as a dual system to improve the seismic performance of multistory buildings. A case study was carried out to compare the proposal with the shear wall solution for the typical seismic Chilean RC building from the structural and economic perspectives. The results show that it is more resilient than other stiffer seismic solutions, such as shear walls, reduces the demand, minimizes seismic damage, gives reliable earthquake protection and facilitates future upgrades and repairs while achieving the level of immediate occupancy without the costs of the shear walls system.

  • Lately Construction industry tends to prefer remodeling of existing building rather then new construction or reconstruction of buildings due to strengthening of several restriction related to real estates along with prolonged depression. Specially in case of Apartment houses, since vagueness on ownership and management authority on common part exists. So in this study we are to present the procedure of analyzing apartment house remodeling method through user request by approaching to several considerable factors in user request side.

  • Management of posttraumatic enophthalmos can present as a challenge to the reconstructive surgeon, particularly in cases of late presentation. This article reviews the pertinent anatomy of the orbit, diagnostic modalities, indications for surgery, and surgical approaches as they relate to the treatment of posttraumatic enophthalmos. Internal orbital reconstruction has evolved to an elegant procedure incorporating various biologic or alloplastic implants, including anatomical pre-bent implants. Successful repair of late enophthalmos has been demonstrated in multiple recent studies and is likely related to the precision with which orbital anatomy can be restored.

  • Computer aided design and manufacturing (CAD/CAM) technology today is the standard in manufacturing industry. The application of the CAD/CAM technology, together with the emerging 3D medical images based virtual surgical planning (VSP) technology, to craniomaxillofacial reconstruction has been gaining increasing attention to reconstructive surgeons. This article illustrates the components, system and clinical management of the VSP and CAD/CAM technology including: data acquisition, virtual surgical and treatment planning, individual implant design and fabrication, and outcome assessment. It focuses primarily on the technical aspects of the VSP and CAD/CAM system to improve the predictability of the planning and outcome.

  • In medical applications, the cooperation work using computer network is in the spotlight as next generation technology, because it offers sharing of equipments, knowledge of specialists, and data and surmounts the limitations of geographical position and time. We present a prototype of telepresent surgical simulation system in this treatise. It is a telemedicine system using volume image, which is reconstructed from radiological images such as MRI and CT. It is used as a tool for surgical simulation among telepresent doctors. It provides the functions of conference control, volume reconstruction and manipulation, and multimedia database management among one server and multi-clients through high-speed network. It is implemented on Unix workstation using X-windows and C language, TCP/IP protocol and UNiSQL as DBMS.
