Quality Assurance of Leaf Speed for Dynamic Multileaf Collimator (MLC) Using Dynalog Files (Dynalog file을 이용한 동적다엽조준기의 Leaf 속도 정도관리 평가)

  • Purpose : The purpose of this study is to analyze the mechanical and leaf speed accuracy of the dynamic multileaf collimator (DMLC) and determine the appropriate period of quality assurance (QA). Materials and Methods : The quality assurance of the DMLC equipped with Millennium 120 leaves has been performed total 92 times from January 2012 to June 2014. The the accuracy of leaf position and isocenter coincidence for MLC were checked using the graph paper and Gafchromic EBT film, respectively. The stability of leaf speed was verified using a test file requiring the leaves to reach maximum leaf speed during the gantry rotation. At the end of every leaf speed QA, dynamic dynalog files created by MLC controller were analyzed using dynalog file viewer software. This file concludes the information about the planned versus actual position for all leaves and provides error RMS (root-mean square) for individual leaf deviations and error histogram for all leaf deviations. In this study, the data obtained from the leaf speed QA were used to screen the performance degradation of leaf speed and determine the need for motor replacement. Results : The leaf position accuracy and isocenteric coincidence of MLC was observed within a tolerance range recommanded from TG-142 reports. Total number of motor replacement were 56 motors over whole QA period. For all motors replaced from QA, gradually increased patterns of error RMS values were much more than suddenly increased patterns of error RMS values. Average error RMS values of gradually and suddenly increased patterns were 0.298 cm and 0.273 cm, respectively. However, The average error RMS values were within 0.35 cm recommended by the vendor, motors were replaced according to the criteria of no counts with misplacement > 1 cm. On average, motor replacement for gradually increased patterns of error RMS values 22 days. 28 motors were replaced regardless of the leaf speed QA. Conclusion : This study performed the periodic MLC QA for analyzing the mechanical and leaf speed accuracy of the dynamic multileaf collimator (DMLC). The leaf position accuracy and isocenteric coincidence showed whthin of MLC evaluation is observed within the tolerance value recommanded by TG-142 report. Based on the result obtained from leaf speed QA, we have concluded that QA protocol of leaf speed for DMLC was performed at least bimonthly in order to screen the performance of leaf speed. The periodic QA protocol can help to ensure for delivering accurate IMRT treatment to patients maintaining the performance of leaf speed.

Evaluation of Factors Used in AAPM TG-43 Formalism Using Segmented Sources Integration Method and Monte Carlo Simulation: Implementation of microSelectron HDR Ir-192 Source (미소선원 적분법과 몬테칼로 방법을 이용한 AAPM TG-43 선량계산 인자 평가: microSelectron HDR Ir-192 선원에 대한 적용)

  • Currently, the dose distribution calculation used by commercial treatment planning systems (TPSs) for high-dose rate (HDR) brachytherapy is derived from point and line source approximation method recommended by AAPM Task Group 43 (TG-43). However, the study of Monte Carlo (MC) simulation is required in order to assess the accuracy of dose calculation around three-dimensional Ir-192 source. In this study, geometry factor was calculated using segmented sources integration method by dividing microSelectron HDR Ir-192 source into smaller parts. The Monte Carlo code (MCNPX 2.5.0) was used to calculate the dose rate $\dot{D}(r,\theta)$ at a point ($r,\theta$) away from a HDR Ir-192 source in spherical water phantom with 30 cm diameter. Finally, anisotropy function and radial dose function were calculated from obtained results. The obtained geometry factor was compared with that calculated from line source approximation. Similarly, obtained anisotropy function and radial dose function were compared with those derived from MCPT results by Williamson. The geometry factor calculated from segmented sources integration method and line source approximation was within 0.2% for $r{\geq}0.5$ cm and 1.33% for r=0.1 cm, respectively. The relative-root mean square error (R-RMSE) of anisotropy function obtained by this study and Williamson was 2.33% for r=0.25 cm and within 1% for r>0.5 cm, respectively. The R-RMSE of radial dose function was 0.46% at radial distance from 0.1 to 14.0 cm. The geometry factor acquired from segmented sources integration method and line source approximation was in good agreement for $r{\geq}0.1$ cm. However, application of segmented sources integration method seems to be valid, since this method using three-dimensional Ir-192 source provides more realistic geometry factor. The anisotropy function and radial dose function estimated from MCNPX in this study and MCPT by Williamson are in good agreement within uncertainty of Monte Carlo codes except at radial distance of r=0.25 cm. It is expected that Monte Carlo code used in this study could be applied to other sources utilized for brachytherapy.

Suitability Assessment of Domestic Natural Waters as a Culture Medium for Daphnia magna (Daphnia magna의 배양배지로서 국내 자연수의 적절성 평가)

  • The present study was conducted to assess the suitability of domestic natural waters as a Daphnia magna culture medium. In order to assess survival rate and reproductive output, young female daphnids (parent animals), aged less than 24 hours at the start of the test and produced in the Elendt M4 medium, were exposed to Elendt M4 medium, de-chlorinated tap water, and natural mineral water for 21 days. D. magna cultured in Elendt M4 medium (reference medium) and natural mineral water met the criteria of OECD No. 211, Daphnia magna Reproduction Test Guidelines in terms of percent adult survival, first day of reproduction, and average young production. However, the mortalities of adult daphnids observed in de-chlorinated tap water were more than 20% in two reproduction tests for 21 days. Mortality was observed on exposure days 13, 15, and 18 in de-chlorinated water. The use of D. magna is recommended in water of hardness >80 mg $CaCO_3\;L^{-1}$. However, the hardness of de-chlorinated tap water used in the present study was 50~53 mg $CaCO_3\;L^{-1}$. Therefore, it is judged that the delayed mortalities observed in de-chlorinated tap water were caused by a rapid decreased in hardness when the medium was changed from Elendt M4 to de-chlorinated tap water. When D. magna is cultured using domestic natural waters (underground water, surface water, and de-chlorinated water), the quality-control (QC) data should be maintained through a standardization for health assessment method, toxicity test method using reference chemical, test intervals of reference toxicant toxicity test, and data treatment and interpretation. In the long term, national research programs are needed for the development of test species which are representative of domestic aquatic environmental conditions among indigenous daphnids.

Factors Affecting Wet-Paddy Threshing Performance (탈곡기의 제작동요인이 벼의생탈곡성능에 미치는 영향)

  • Threshing operation may be one of the most important processes in the paddy post-production system as far as the grain loss and labor requirement are concerned . head-feeding type threshers commercially available now in Korea originally were developed for threshing dry paddy in the range of 15 to 17 % in wet basis. However, threshing wet-paddy with the grain moisture content above 20 % has been strongly recommended, especially for new high-yielding Indica -type varieties ; (1) to reduce high grain loss incurred due to the handling operations, and (2) to prevent the quantitative and qualitative loss of milled -rice when unthreshed grains are rewetted due to the rainfall. The objective of this study were to investigate the adaptability of both a head-feeding type thresher and a throw-in type thresher to wet-paddy , and to find out the possiblilities of improving the components of these threshers threshing. Four varieties, Suweon 264 and Milyang 24 as Tongil sister line varieties, minehikari and Jinhueng as Japonica-type varieties, were used at the different levels of the moisture content of grains. Both the feed rate and the cylinder speed were varied for each material and each machine. The thresher output quality , composition of tailing return, and separating loss were analyzed from the sampels taken at each treatment. A separate experiment for measurement opf the power requirement of the head-feeding type thresher was also performed. The results are summarized as follows : 1. There was a difference in the thresher output quality between rice varieties. In case of wet-paddy threshing at 550 rpm , grains with branchlet and torn heads for the Suweon 264 were 12 % and 7 % of the total output in weight, respectively, and for the Minehikari 4.5 % and 2 % respectively. In case of dry paddy threshing , those for the Suweon 264 were 8 % and 5% , and for the Minehikari 4% and 1% respectively. However, those for the Milyang 23 , which is highly susceptable to shattering, were much lower with 1 % and 0.5% respectively, regardless of the moisture content of the paddy. Therefore, it is desirable to breed rice varieties of the same physical properties as well as to improve a thresher adaptable to all the varieties. Torn heads, which increased with the moisture content of rall the varieties except the Milyang 23 , decreased as the cylinder speed increased, but grains with branchlet didnt decrease. The damaged kernels increased with the cylinder speed. 3. The thresher output quality was not affected much by the feed rate. But grains with branchlet and torn heads increased slightly with the feed rate for the head-feeding type thresher since higher resistance lowered at the cylinder speed. 4. In order to reduce grains with branchlet and torn heads in wet-paddy threshing , it is desirable to improve the head-feeding type thresher by developing a new type of cylinder which to not give excess impact on kernels or a concave which has differenct sizes of holes at different locations along the cylinder. 5. For the head-feeding type thresher, there was a difference in separating loss between the varieties. At the cylinder speed of 600 rpm the separating losses for the Minehikari and the Suweon 264 were 1.2% and 0.6% respectively. The separating loss of the head-feeding type thresher was not affected by the moisture content of paddy while that of the Mini-aged thresher increased with the moisture content. 6. From the analysis of the tailings return , to appeared that the tailings return mechanism didn't function properly because lots of single grains and rubbishes were unnecessarily returned. 7. Adding a vibrating sieve to the head-feeding type thresher could increase the efficiency of separation. Consequently , the tailing return mechanism would function properly since unnecessary return could be educed greatly. 8. The power required for the head-feeding type thresher was not affected by the moisture content of paddy, but the average power increased linearly with the feed rate. The power also increased with the cylinder speed.

Elevated Temperature Treatment Induced Rice Growth and Changes of Carbon Content in Paddy Water and Soil (온도상승 환경 처리가 논토양과 용수에서 탄소량 변화와 벼 생육에 미치는 영향)

  • BACKGROUND: The global mean surface temperature change for the period of 2016~2035 relative to 1986~2005 is similar for the four representative concentration pathway (RCP)'s and will likely be in the range of $0.3^{\circ}C$ to $0.7^{\circ}C$. Climate change inducing higher temperature could affect not only crop growth and yield, but also dynamics of carbon in paddy field. METHODS AND RESULTS: This study was conducted to evaluate the effect of elevated temperature on the carbon dynamics in paddy soil and rice growth. In order to control the elevated temperatures, the experiments were set up as the small scale rectangular open top chambers (OTCs) of $1m(width){\times}1m(depth){\times}1m(height)$ (Type 1), $1 m(W){\times}1m(D){\times}1.2m(H)$ (Type 2), and $1m(W){\times}1m(D){\times}1.4m(H)$ (Type 3). The average temperatures of Type 1, Type 2, and Type 3 from July 15 to October 30 were higher than the ambient temperatures at $0.4^{\circ}C$, $0.5^{\circ}C$, and $0.9^{\circ}C$, respectively. For the experiment, Wagner's pots (1/2,000 area) were placed inside chambers. The pots were filled with loamy soil, and chemical fertilizer and organic compost were applied as recommended after soil test. The pots were flooded with agricultural water and rice (Shindongjin-byeo) was planted. It was observed that TOC (total organic carbon) of the water increased by the elevated temperatures and the trend continued until the late growth stage of the rice. Soil TOC contents were reduced by the elevated temperatures. C/N ratios of the rice plant decreased by the elevated temperature treatments. Thus, it was assumed that the elevated temperatures induced to decompose soil organic matter. Elevated temperatures significantly increased the culm length (P<0.01) and culm weight (P<0.05) of rice, but the number and weight of rice panicle did not showed significant differences. CONCLUSION: Based on the results, it was suggested that the elevated temperatures had an effect on changes of soil and water carbons under the possible future climate change environment.

A Cohort Study of Incidence Rate and Causes of School Accidents in a Boys' Middle School in Taegu (남자중학교 학생의 학교사고 발생률과 사고원인에 관한 코호트 연구)

  • To determine the incidence rate of school accidents and its associated factors, a cohort observation was carried out in a boys' high school of a total of 2,324 students for an academic year from March 1986 to February 1987. Data for accidents were collected by the author (school nurse) as students came to the school health station for treatment of accidental injury. Data for school activities were obtained from the diary of academic affairs. A total of 603 students made 1,126 visits to the station for accidental injury during the study period that gave an average 1.9 accidents per student. The average number of visits for accidental injury per day was 5.1 and the incidence rate of accidental injury was 2.2 per 1,000 student days. Each class had about 30 accidents on the average in a year. However there were two classes that had exceptionally high incidence rate of accidents; 54 accidents in one class where 10 athletes were assigned and 58 accidents in the other class where the teacher in charge was On vacation for two months. The highest incidence rate of accident was observed in June (3.4 per 1,000 student days) and the lowest rates were in December and February (1.5 and 0.7 per 1,000 student days, respectively) and there were no appreciable difference by day of week. Accidents were caused by mischief or carelessness of students in 62.2% and by the inappropriate tools and facilities of school in 18.6% . The accidents caused by tools and facilities were mainly due to the antiquated chairs and tables. Referral rate to hospital for severe injury was three times higher in cloudy days and five times higher in rainy days than in clear days. There was almost no accident during the examination period but increased after the examination. Based on these findings, following measures are recommended to prevent school accident: disperse the athletes throughout the classes: should the teacher in charge leave the class for a long period, replace with another teacher who is familiar with the class to keep order; replace or repair the inappropriate tools and facilities; and warn the students for the risk of accident or broadcast music or let the students have light physical exercise to relieve the tension in cloudy or rainy day and after examination.

Effects of Intensive Weight Bearing Treadmill Training and Electrical Stimulation on Skeletal Muscle Properties in Hindlimb Suspended Rats (흰 쥐의 뒷다리 현수 후 집중 체중부하 트레드밀 훈련과 전기자극이 골격근 특성에 미치는 효과)

  • The purpose of this study was to investigate the treatment effect of three interventions on the disuse atrophy of rat hindlimb after two weeks suspension. Forty-eight 11~12 weeks old female Sprague-Dawley white rats were divided into four intervention groups: 1) suspension only (S; n=10), 2) intensive weight bearing treadmill (IWBT; n=10), 3) electrical stimulation (ES; n=9), 4) 2)+3) (ES/IWBT; n=9). Another 10 rats received no intervention or hindlimb suspension and served as controls (C). After the interventions, 1) the cross-sectional area (CSA), 2) the ratio of white muscle fiber composition (WMFC), 3) isometric tetanic tension (ITT), and 4) muscle weights (MWs) were measured from the four calf muscle specimens. The results were as follows: 1. In all intervention groups, the CSAs of medial and lateral gastrocnemius (MG LG), soleus (SOL), and flexor digitorum superficialis (FDS) decreased when compared to the control (C) group (p<.05). The CSA increased in FDS and LG for the IWBT group, in SOL for the ES/IWBT group compared to the S only group (p<.05). 2. The ratios of WMFC in MG, LG, SOL, and FDS increased compared to the C group for all interventions (p<.05). The ratios of WMFC decreased in SOL and LG for the IWBT group, in SOL for the ES/IWBT group compared to the S only group, and decreased in SOL for the ES/IWBT group compared to the IWBT group (p<.05). 3. The ITT in the MG, LG, SOL, and FDS decreased compared to the C group for all interventions (p<.05). The ITT increased in MG LG/FDS, SOL, and the whole calf muscles (WCMs) in the IWBT, ES and ES/IWBT groups compared to the S only group (p<.05). 4. The MWs in MG LG/FDS, SOL, WCMs decreased compared to the C group for all interventions (p<.05). The MWs increased in MG LG/FDS and WCMs for the IWBT group, in SOL for the ES group, and in SOL for the ES/IWBT group compared to the S only group (p<.05). 5. In atrophied muscles, the IWBT group showed the best recovery and the ES/IWBT and ES groups followed in decreasing order. The most susceptible muscle to disuse atrophy was the SOL. But conversely, it showed the best recovery in the ES/IWBT group. After two weeks of hindlimb suspension, the calf muscles of rats atrophied and their isometric tension decreased. These changes were best reversed by hindlimb-focused treadmill activity. The next best results were achieved by electrical stimulation combined with the treadmill followed by only electrical stimulation. These findings indicate that full weight bearing treadmill activity alone or in combination with electrical stimulation are effective treatments for non-weight bearing induced muscle atrophy. Further study of the effect of different intensities of electrical stimulation and variations in the duration period of full weight bearing treadmill activity on disuse atrophy is recommended.

Association of food intake with serum levels of phosphorus and potassium in hemodialysis patients (혈액투석 환자의 일상식품군 섭취량에 따른 혈청 인, 칼륨 농도의 상관성 분석)

  • Purpose: Elevated serum phosphorus and potassium levels are a major problem for hemodialysis (HD) patients. Hyperphosphatemia and hyperkalemia are closely related to intake of dietary phosphorus and potassium. Methods: This study was conducted in order to investigate the effects of food consumed on serum phosphorus and potassium levels in 48 HD patients (20 males and 28 females). We collected anthropometric data, biochemical parameters, and dietary data of the subjects. Dietary data for usual intake were obtained by use of a food-frequency questionnaire (FFQ) consisting of 21 food items. Results: The mean body mass index (BMI) was $22.2{\pm}3.0kg/m^2$, mean serum phosphorus level was $4.50{\pm}1.52mg/dl$, and mean serum potassium level was $4.74{\pm}0.73mEq/l$. Hyperphosphatemia (> 4.5 mg/dl) was found in 45.8% of subjects, and hyperkalemia (> 5.0 mEq/l) in 35.4%. Subjects who took medication only were 56% of total, and those who took medication with dietary therapy were 27%. Patients with medication and dietary therapy showed significantly lower serum phosphorus levels compared to patients with medication only (p < 0.05). Mean duration of HD was $7.9{\pm}7.3$ years and it showed positive correlation with serum potassium levels (p < 0.05). Serum phosphorus levels showed positive correlation with intake of mixed grains and soybean milk (p < 0.05). Serum potassium levels showed positive correlation with intake of mixed grains (p < 0.01), potatoes, fish, and high-potassium vegetables (p < 0.05). On the other hand, intake of white rice showed negative correlation with serum potassium levels (p < 0.05). Conclusion: The results of our study suggest that intake of white rice rather than mixed grains is an important factor in sustaining normal serum phosphorus and potassium levels. In addition, limiting intake of soybean milk, potatoes, and fish to under three serving per week is recommended. Finally, conduct of a strict dietary therapy along with medical treatment is desirable because inappropriate food intake increases serum phosphorus and potassium levels to a higher than normal range.

Effect of Pine Silage Feeding on Nutrient Digestibility, Feed Conversion and Carcass Traits of Korean Native Cattle (잣나무 生枝葉사일리지 급여가 한우의 영양소 소화율, 사료요구율 및 도체성적에 미치는 영향)

  • The wastes (the needle leaves and branches) produced from thinning the forest were fermented under an anaerobic condition (pine silage) to utilize as a forage source of ruminants. An in situ trial was conducted with two ruminally fistulated Korean native cows by 4 replicates (2 bags per cow), and in vivo digestibility of pine silage was estimated with five Korean native steers by 5 replicates in incomplete double turn-over design. In order to investigate feed efficiency and carcass traits, forty eight Korean native bulls were assigned to four treatments (0%, 25%, 50%, 75% of pine silage) with a completely randomized design in 12 pens accommodating 4 animals per pen. The amounts of concentrate and roughage allowed to experimental animals were in the range of 2.5% and 0.6% of body weight, respectively. Animals had freely accessed to mineral block and water in stanchion barn.An in situ crude protein digestibility of the roughage sources when suspended for 48 hrs in the rumen was higher for feeding the pine silage than the rice straw, whereas NDF digestibility was vice versa. No differences between the treatments were observed in in vivo digestibilities of dry matter, crude protein and NDF. Daily body weight gain was significantly (P<0.01) higher for the pine silage substitutions compared with the rice straw feeding except for the 25% silage feeding. The pine silage feeding did not affect feed efficiency.In carcass traits, marbling score tended to be higher for the 50% pine silage treatment than the other treatments while back fat thickness tended to be the thinnest for the rice straw feeding. From these results, it may be recommended to substitute pine silage, as a forage source, for rice straw by 50 percent.

Effect of Inhomogeneity correction for lung volume model in TPS (Lnug Volume을 모델로 한 방사선치료계획 시 불균질 조직 보정에 따른 효과)

  • Introduction : The phantom that includes high density materials such as steel was custom-made to fix lung and bone in order to evaluation inhomogeneity correction at the time of conducting radiation therapy to treat lung cancer. Using this, values resulting from the inhomogeneous correction algorithm are compared on the 2 and 3 dimensional radiation therapy planning systems. Moreover, change in dose calculation was evaluated according to inhomogeneous by comparing with the actual measurement. Materials and Methods : As for the image acquisition, inhomogeneous correction phantom(Pig's vertebra, steel(8.21g/cm3), cork(0.23 g/cm3)) that was custom-made and the CT(Volume zoom, Siemens, Germany) were used. As for the radiation therapy planning system, Marks Plan(2D) and XiO(CMS, USA, 3D) were used. To compare with the measurement value, linear accelerator(CL/1800, Varian, USA) and ion chamber were used. Image, obtained from the CT was used to obtain point dose and dose distribution from the region of interest (ROI) while on the radiation therapy planning device. After measurement was conducted under the same conditions, value on the treatment planning device and measured value were subjected to comparison and analysis. And difference between the resulting for the evaluation on the use (or non-use) of inhomogeneity correction algorithm, and diverse inhomogeneity correction algorithm that is included in the radiation therapy planning device was compared as well. Results : As result of comparing the results of measurement value on the region of interest within the inhomogeneity correction phantom and the value that resulted from the homogeneous and inhomogeneous correction, gained from the therapy planning device, margin of error of the measurement value and inhomogeneous correction value at the location 1 of the lung showed $0.8\%$ on 2D and $0.5\%$ on 3D. Margin of error of the measurement value and inhomogeneous correction value at the location 1 of the steel showed $12\%$ on 2D and $5\%$ on 3D, however, it is possible to see that the value that is not correction and the margin of error of the measurement value stand at $16\%$ and $14\%$, respectively. Moreover, values of the 3D showed lower margin of error compared to 2D. Conclusion : Revision according to the density of tissue must be executed during radiation therapy planning. To ensure a more accurate planning, use of 3D planning system is recommended more so than the 2D Planning system to ensure a more accurate revision on the therapy plan. Moreover, 3D Planning system needs to select and use the most accurate and appropriate inhomogeneous correction algorithm through actual measurement. In addition, comparison and analysis through TLD or film dosimetry are needed.
