Inflammatory bowel disease (IBD) is a well-recognized risk factor for thrombotic events in adults but data on children are scarce. In the great majority of adult patients, thrombotic events are usually deep vein thrombosis and pulmonary embolism. Other sites such as jugular veins are extremely rare. We present a case of Lemierre syndrome in an adolescent girl with active ulcerative colitis and discuss possible risk factors. This is the first reported case of severe Lemierre syndrome with thrombus extension to cranial veins in a patient with ulcerative colitis. Early recognition of Lemierre syndrome in patients who present with rapidly worsening symptoms of neck pain, fever and signs of pharyngitis is imperative because it increases a chance of favorable prognosis. It is important for pediatricians treating IBD patients not to underestimate possible thrombotic events in children with IBD. Recognition of additional risk factors is crucial for prompt diagnosis and adequate treatment.

There has been increasing recognition of pediatric bipolar disorder in the psychiatric field during the past 10 years. The clinical presentation of this disorder in preadolescent is greatly debated and few studies have been conducted in Korea. The authors report 3 cases of children with bipolar I disorder whose clinical symptoms were improved after pharmacotherapy. The authors also review current concepts, debates and treatment of pediatric bipolar disorder.
Henoch-Sch$\ddot{o}$nlein purpura is a systemic vasculitis of unknown etiology that is probably related to an autoimmune phenomenon. Henoch-Sch$\ddot{o}$nlein purpura is characterized by a purpuric rash, arthralgia, nephritis, and gastrointestinal manifestations. We reviewed 169 children hospitalized with a diagnosis of Henoch-Sch$\ddot{o}$nlein purpura between 1989 and 1998. One-hundred thirty-nine (82.2 %) had gastrointestinal findings including abdominal pain, nausea, vomiting and gastrointestinal bleeding. Surgical consultations were obtained for ten children, and laparotomy was performed in five. Three children suspected of having appendicitis underwent appendectomy. None had appendicitis. One child thought to have been intestinal strangulation was found to have hemorrhagic edema of the proximal jejunum and of the distal ileum. Another child underwent resection for an hemorrhagic infarct of the distal ileum. A high index of suspicion of this disease entity in the differential diagnosis of abdominal pain in children can avoid unnecessary laparotomy in most cases. However, life-threatening gastrointestinal complications may occur in low percentage of cases. Prompt recognition and adequate radiologic evaluation of the abdominal manifestation of this entity is necessary for early surgical intervention.
Early recognition and surgical treatment of Hirschsprung's disease prevents serious mortality and morbidity from enterocolitis and obstruction. Usually this disease is characterized by a single aganglionic segment of the colon extending distally to the anal margin. In surgical treatment, the surgeon performs a frozen section biopsy to confirm whether there are ganglion cells. If there are intervening ganglionic sites in aganglionic bowel, there may be confusion in diagnosis and treatment. The authors experienced one case of total colonic aganglionosis with skip area. A transverse loop colostomy was performed on a 7 day-old male baby with colon perforation due to Hirschsprung's disease. But intestinal obstruction persisted and required two more operations to find the true nature of the disease. There were aganglionic segments from the anal margin to the terminal ileum 3.7cm proximal to the ileocecal valve. The entire transverse colon and appendix were normally ganglionated.

Congenital isolated hydronephrosis encompasses a spectrum of physiologic states that spontaneously resolve and pathologic obstruction that necessitates surgical intervention. Distinguishing patients whose condition will resolve, those who will require stringent follow-up, and those who will eventually need surgical intervention present a challenge to clinicians, particularly because no unified guidelines for assessment and follow-up have been established. The recognition of the natural course and prognosis of hydronephrosis and a comprehensive understanding of the currently proposed consensus guidelines may aid in multidisciplinary treatment and in providing proper counseling to caregivers. In this review, we aimed to summarize the literature on the grading systems and management strategies for congenital isolated hydronephrosis.
Gastric perforation of the newborn is a rare and life threatening problem. The pathogenesis of gastric perforation is not clear. Since ischemia is responsible for intestinal perforation, a similar mechanism may result in gastric perforation. Twelve patients with neonatal gastric perforation who were treated at the Department of Pediatric Surgery, Hanyang University Hospital from 1987 to 2002 were reviewed. Eight patients were male and four female. The age of perforation was 1 day to 8 days of life. Ten patients were operated upon and 2 patients were treated nonoperatively. The perforation site was located on the anterior wall along the greater curvature of the stomach in 8 patients and along the lessor curvature of the stomach in 2. The precipitating factors were prematurity, gastroschisis, mechanical ventilation, intestinal obstruction, cyanotic heart disease and indomethacine medication. In 5 cases the cause of perforation was not identified. The mortality rate was 25 % (3 of 12). Earlier recognition and treatment were thought to be crucial prognostic factors.
Objectives: The purpose of this study is to examine the parental recognition and utilizing patterns of oriental medical care in children who live in Seong-Dong district. Methods 802 day-care center children's parents participated in the survey on recognition and utilizing patterns of oriental medical care, and 702 of the surveys were analyzed. Results: 1. 38.33% has received previous oriental medical treatments and most were received at the children's age under 12-24 months. Majority of the treatments took place in the clinical settings. By recommendation, some also received their treatment at the medical centers. One of the reasons why they receive oriental medical treatments was that they prefer to balance their treatment with the orental medical treatments along with the western's. 2. 53.85% of those who received oriental medical treatments were satisfied with their treatments. The effective treatment results were the major reason for the satisfaction. 3. 49.56% of those who had oriental medical treatments chose specific oriental medical facilities for the children's ailment. Most specific oriental medical facilities were local oriental medical clinics. Reputation fame were the main reason in choosing the clinics. 4. In 2009, 55.26% had received care in the specific oriental medical clinics for the diseases, such as respiratory symptoms, systemic symptoms, gastrointestinal symptoms, skin problems, neurological and psychiatric disease for infants, and respiratory symptoms, gastrointestinal symptoms, systemic symptoms, skin problems, neurological and psychiatric symptoms, urinary symptoms and musculoskeletal symptoms for toddlers. 5. The average duration of herbal treatment were 7-14 days, preferably 4-6 days, and preferred number of medical visits were once a week on Saturdays and preferred day for night cares were on Friday nights. Average treatment fee per treatment was less than 10,000 won which coincided with the preferred treatment fee per visit. 6. The very first medical institutions chosen by parents once their children are ill are the following; Western medical institutions, western medical clinics, pharmacies, oriental medical clinics, health centers, and lastly, oriental medical institutions. The major concerns in receiving oriental medical cares include pesticides in herbs and hazardous substances in heavy metals. Preferred form of oriental medicine was decoction, the negative feedback of oriental medicine was mostly due to the bitter taste of the oriental herbal decoctions. Preferred frequency of intake was twice a day with less than 10cc~20cc per single intake. Conclusions: Taken all together, we conclude that the growing recognition of Oriental Pediatric medicine is needed among parents.
Objectives : The aim of this study was to investigate the attitude toward the oriental hospital among 100 pediatric outpatients. Methods : A questionnaire about the general characteristics of patients. the reason of choice or alteration was implemented. Results : 1. In distribution of sex & age, 60% was men and 40% women was between 2 and 6 years old. 2. In determinant factor for using Oriental medicine, 43.8% was tolerance increase of the western medicine and 33.0% was recommened by others. 3. In distribution of disease & therapeutic method, 62.5% was respiratory disease and 11.7% gastrointestinal disease, 64.3% was treated with oriental after Western medical therapy and 20.9% only Oriental medical care. 4. The changing causes for using Oriental after Western medicine, 35.9% was ineffective affect and 26.5% side effect. 5. The preference for using Oriental medicine, 41.6% was respiratory disease and 21.0% gastrointestinal disease. 6. The peculiarity of Oriental medicine compared with western medicine, 36% was effectiveness on the cronic disease and 14% for fundamental remedy. 7. The prodominance disease in Western medicine compared with Oriental medicine, 28% was operation, 18% common cold and 15% inflammatory disease 8. The questions about Oriental medicine, 22% was medical material's sanitation and place of origin, 18% harmness or not administered during long time. 9. The controversial point of Oriental medicine, 33.5% was expensive price and 27.7% difficulty of taking medicine. 10. In satisfaction degree of Oriental medicine, 69% was satisfied and 19% was very satisfied. Conclusions : Considering the above results, we have concluded that proper recognition about Oriental Pediatric medicine will be needed among patients.
The authors report a case of Currarino triad which had a congenital anorectal stenosis associated with a sacral defect and a presacral mass. A 1-year-old female presented with constipation since birth. Neurological deficits were not found on admission. She had had a diverting colostomy due to anorectal stenosis at another hospital before admission. Lumbar X-ray films showed bony defect caudal to the third sacral vertebra. Magnetic resonance image demonstrated a round cystic pelvic mass which was connected with a dural sac via anterior sacral defect. Posterior approach with Intradural removal of the presacral cystic mass was performed and followed by anoplasty by a pediatric surgeon. The cystic mass was verified histologically as mature teratoma with cystic change. Postoperatively, the urinary function and bowel movement remained intact. Currarino triad should be suspected and evaluated physically and radiographically in a case of congenital anorectal stenosis. Prompt recognition and close cooperation between pediatric surgeons and neurosurgeons is advisable to ensure adequate surgical treatment.
목 적 : 열은 소아에서 매우 흔한 증상 중 하나이지만 부모들은 열 자체를 위험한 질환으로 인식하는 경우가 많다. 열에 대해 올바로 아는 것은 부모의 불필요한 염려를 덜고 환아의 질환과 진단 및 치료 과정을 이해하는데 도움을 줄 것이다. 이에 저자들은 소아의 열에 대한 부모들의 인식을 조사하고 과거와 비교하여 변화가 있는지 알아보고자 하였다. 방 법 : 2006년 5월부터 7월까지 3개월 동안 가톨릭대학교 성바오로병원 소아과 외래를 방문한 소아의 보호자들에게 열에 대한 지식, 태도, 대처 방법을 묻는 19문항의 설문조사를 실시하였다. 결 과 : 136명이 답하였으며 10년 전에 비하여 열에 대한 정보, 태도, 대처 방법 등에서 크게 개선된 점이 없었다. 열에 대한 기본 지식인 열의 기준과 측정 방법에 대해서 잘못 알고 있는 보호자가 많았고, 열성경련과 뇌손상의 가능성에 대한 염려는 여전히 지속되고 있었다. 열은 항상 해롭고 위험한 것이라고 생각하고 있었으며, 따라서 치료가 필요하지 않은 체온에서도 실제보다 심각한 결과를 걱정하며 해열제를 투여하고 병원에 방문하고 있음을 보여주었다. 결 론 : 보호자들은 열이 이로운 점을 가진 생리 반응중의 하나라는 사실을 인식하지 못하고 실제보다 심각한 결과를 예상하며 따라서 불필요한 우려와 치료를 택하게 된다. 이런 잘못된 인식은 시간의 경과에도 불구하고 큰 변화가 없었으며 따라서 소아과 의사들의 역할이 중요하다고 할 수 있다. 부모들에게 해열제를 사용하도록 할 때는 열이 해롭지 않으며 해열제의 목적은 열로 인한 아이의 불편감을 덜어주는 것임을 반복하여 알려주고, 더 나아가서는 소아과 의사들이 좀 더 체계적인 정보 및 교육 프로그램을 제공한다면 열에 대한 올바른 인식 형성에 도움이 되리라고 생각된다.
