Usefulness Evaluation of HRCT using Reconstruction in Chest CT (흉부CT 검사 시 HRCT 영상 재구성의 유용성)

  • Purpose : Skip the repetitive HRCT axial scan in order to reduce the exposure of patients during chest HRCT scan, Helical Scan Data into a reconstructed image, and exposure of the patient change and visually evaluate the usefulness of the HRCT images. Materials and method : Patients were enrolled in the survey are 50 people who underwent chest CT scans of patients who presented to the hospital from January 2015 to March 2015. 50 people surveyed 22 people men and 28 people women people showed an average distribution of 30 to 80 years age was 48 years. 50 patients to Somatom Sensation 64 ch (Siemens) model with 120 kVp tube voltage to a reference mAs tube current to mAs (Care dose, Siemens) as a whole, including the lungs and the chest CT scan was performed. Scan upon each patient CARE dose 4D (Automatic exposure control, Siemens Medical Solution Erlangen, Germany) was to maintain the proper radiation dose scan every cross-section through a device that automatically adjusts the tube current of. CT scan is the rotation time of the Tube slice collimation, slice width 0.6 mm, pitch factor was made under the terms of 1.4. CT scan obtained after the raw data (raw data) to the upper surface of the axial images and coronal images for each slice thickness 1 mm, 5 mm intervals in the high spatial frequency calculation method (hight spatial resolution algorithm, B60 sharp) was the use of the lung window center -500 HU, windows were reconstructed into images in the interval -1000 HU to see. Result : 1. Measure the total value of DLP 50 patients who proceed to chest CT group A (Helical Scan after scan performed with HRCT) and group B (Helical Scan after the HR image reconstruction to the original data) compared with the group divided, analysis As a result of the age, but show little difference for each age group it had a decreased average dose of about 9%. 2. Conclusion : Scan was confirmed a continuous, rapid motion video to get Helical scan is much lower lobe lung reduction in visual blurring, Helical scan data to not repeat the examination by obtaining HRCT images reorganization reduced the exposure of the patient.

Diagnosis of Rib Fracture Using Artificial Intelligence on Chest CT Images of Patients with Chest Trauma (외상 환자의 흉부 CT에서 인공지능을 이용한 갈비뼈 골절 진단)

  • Purpose To determine the pros and cons of an artificial intelligence (AI) model developed to diagnose acute rib fractures in chest CT images of patients with chest trauma. Materials and Methods A total of 1209 chest CT images (acute rib fracture [n = 1159], normal [n = 50]) were selected among patients with chest trauma. Among 1159 acute rib fracture CT images, 9 were randomly selected for AI model training. 150 acute rib fracture CT images and 50 normal ones were tested, and the remaining 1000 acute rib fracture CT images was internally verified. We investigated the diagnostic accuracy and errors of AI model for the presence and location of acute rib fractures. Results Sensitivity, specificity, positive and negative predictive values, and accuracy for diagnosing acute rib fractures in chest CT images were 93.3%, 94%, 97.9%, 82.5%, and 95.6% respectively. However, the accuracy of the location of acute rib fractures was low at 76% (760/1000). The cause of error in the diagnosis of acute rib fracture seemed to be a result of considering the scapula or clavicle that were in the same position (66%) or some ribs that were not recognized (34%). Conclusion The AI model for diagnosing acute rib fractures showed high accuracy in detecting the presence of acute rib fractures, but diagnosis of the exact location of rib fractures was limited.

Evaluated Absorbed Dose According to Prescribed Dose and Therapeutic Technique in Radiation Therapy (방사선치료 시 처방선량과 치료기법에 따른 흡수선량 평가)

  • In this study, evaluated absorbed dose of moving target using PLD according to prescribed dose and therapeutic technique. First, result of MCNPX when target was deviated from exposure field was reduced dose in proportion to distance. According to prescribed dose, absorbed dose of 3D CRT was better than IMRT in low dose and IMRT was more better in high dose. Absorbed dose of 3D CRT was highest according to therapeutic technique. Therefore, 3D CRT was technique of irradiated highest dose to moving target. But, considered protective effect of normal tissue and patient condition that therapeutic technique was selected to maximized treatment efficiency.

How Computed Tomography Contrast Media and Magnetic Resonance Imaging Contrast Media Affect the Changes of Uptake Counts of 201Tl

  • The purpose of the study is to investigate how uptake counts of $^{201}Tl$ of radioisotopes in the human body could change, when taking computed tomography and magnetic resonance imaging right after injecting contrast media. $^{201}Tl$ radioisotope substances of iodine contrast medium, which is a computed tomography contrast medium, and paramagnetic contrast medium, which is an magnetic resonance imaging contrast medium, were used as study materials. First, $^{201}Tl$ was put into 4 cc of normal saline in test tube, and then a computed tomography contrast medium of Iopamidol$^{(R)}$ or Dotarem$^{(R)}$, was put into 2 cc of normal saline in test tube. An magnetic resonance imaging contrast medium of Primovist$^{(R)}$ or Gadovist$^{(R)}$ was also put into 2 cc of normal saline in test tube. Each contrast medium was distributed to make $^{201}Tl$ as 3 mCi, with a total of 4 cc. Gamma camera, low energy high resolution collimator, and pinhole collimator were used to obtain images. The uptake count of $^{201}Tl$ was measured with 1000 frames of images, and obtained after 10 times of repetition. This study revealed that the use of Gadovist$^{(R)}$, which is an magnetic resonance imaging contrast medium, showed the smallest number of uptake count, after measuring $^{201}Tl$ uptake count by low energy high resolution collimator. On the other hand, the use of Iopamidol$^{(R)}$, which is a computed tomography contrast medium, showed the biggest difference in uptake count, when measuring $^{99m}Tc$ uptake count by Pinhole collimator. When examining with gamma camera, using contrast medium and $^{201}Tl$, identifying the changes of uptake count is very important for improving the value of diagnosis.

The Usefullness of Percutaneous Transarterial Embolization in Patients with Severe Multiple Traumas (다발성 손상에 의한 중증외상환자의 경피적 동맥색전술의 유용성)

  • The study was conducted in patients who had undergone angiography and arterial embolization after being diagnosed with arterial bleeding among those registered with severe trauma at the admission to the department of emergency due to accidents. In the study, comparison was done between the survived group and the nonsurvived group to investigate the usefulness and the optimal performance timing of angiography and embolization as the survival-affecting factors and treatment methods for patients. The study was performed as a retrospective analysis in 45 patients who had undergone angiography and arterial embolization among those categorized as severe trauma patients from the emergency department of H hospital located in Gyeonggi-do from July, 2006 until December, 2010. The Injury Severity Score (ISS), the Revised Trauma Score (RTS), the hematocrit were the main outcome measurements. Arterial embolization was technically successful in all cases, transcatheter arterial embolization of arterial bleeding is a useful procedure in lowering the mortality and complication in severe trauma patients. and RTS, ISS, Hematocrit were the predictable early mortality factors. So bad vital sign patients must be progressed rapidly.

  • Accuracy control of Linear accelerator installed in OBI is done daily and weekly and importance of accuracy multiplies exponentially at that moment. Purpose of this experiment is everyday and twice a week over a four month period (march~june) 2009 year to confirm maintenance of accuracy through Quality control of OBI. In short, measurement of exponentially multiplying accuracy of OBI and regular accuracy control was able to maintain accuracy from the center of treatment within 0.1 cm. Therefore, evaluation of exponentially multiplying accuracy using OBI accuracy control linear accelerator phantom on daily, weekly basis was confirmed.

A Survey of the Radiographic Cassettes Disinfection of University Hospitals in Seoul (서울지역 대학병원의 방사선과 카세트 소독에 관한 연구)

  • The purpose of this study is to prevent nosocomial infection in patients through contact of radiographic cassettes. Data were collected from radiographers working in 29 university hospitals in Seoul in February and March 2001. Radiographic cassettes were disinfected daily in 5 hospitals, weekly in 4 hospitals, monthly in 5 hospitals, bimonthly in 1 hospital and once every three months in another hospital. 12 other hospitals do not practice regular disinfections of radiographic cassettes. Gauze soaked in disinfectant solution is used in 7 hospitals while 11 hospitals used cotton and cloth soaked in disinfectant solution to clean the radiographic cassettes. 26 hospitals used 99% alcohol based disinfectant solutions while 3 hospitals used 75% alcohol based disinfectant. 26 hospitals use of intercourse cassettes outpatients and in patients. In 26 hospitals, all patients shared the same set of radiographic cassettes used in the hospitals, or in 26 hospitals, separate sets of radiographic cassettes are used for outpatients and inpatients. Separate sets of cassettes are used for ICU and inpatients in 6 others hospitals. 23 hospitals used the same sets of radiographic cassettes for all their patients. radiographic cassettes are cleaned in wash area in the study room of the radiographic department in 17 hospitals. 12 other hospitals do not have designated cleaning areas for the cassettes. All radiographers practiced hands washing with soap. All 29 hospitals surveyed have infection control committee. However, only 9 out of the 29 hospitals surveyed provided Infection disinfections control education to radiographers. Only 3 hospitals have radiographers sitting in the infection control committee. Infection management education is conducted in 63 hospitals annually, twice a year in 1 hospital and once every 3 months in 2 hospitals.

Analysis on Fluorine-18 Shielding Efficiency of Double Shield Apron using Acrylic (아크릴을 활용한 이중 차페 Apron의 F-18 차폐 효율 분석)

  • Fluorine-18 used in PET/CT scans is a radioactive isotope that emits positrons, and high energy annihilation gamma rays and beta rays cause exposure to radiation workers. In this study, as part of a plan to reduce the exposure dose of radiation workers working in the Department of Nuclear Medicine, the cause of the low shielding efficiency of Apron for F-18 was identified, and the effectiveness of the Apron double-shielded with acrylic was evaluated. L-Block, Apron+acrylic, Apron, Acrylic+Apron, and Acrylic five shields are used to measure the dose, and the tendencies were compared by performing a Monte Carlo simulation. As a result, it was found that the shielding rate of Apron double shielded with acrylic was about 4 to 8% higher than that of Apron single shielded. To the extent that it does not significantly affect the user's activity, double-shielded personal protective clothing with an appropriate acrylic thickness could help reduce radiation workers' exposure.

Cost-Effective Neuro-Modulation Device for Medical Imaging (의료영상 촬영을 위한 비용-효율적인 신경조절 장비)

  • In this study, we aimed to develop a cost-effective neuro-modulation device for use in neuroscientific and clinical medical imaging applications. To achieve this, we designed and fabricated a brain neuro-modulation device with a material cost of less than $500, and subsequently evaluated its performance. The measured performance was found to be comparable to existing medical devices (TENS), with the developed device being compact in size, measuring less than 3 cm by 3 cm. The outcomes of this study are expected to be applicable in accelerating research and development in related fields, as well as in the rapid commercialization of neuro-modulation technology. Furthermore, it is anticipated that this work will contribute to advancements in functional radiological medical imaging technology and enhance accessibility to brain science and brain stimulation technology.

Microwave and RF Heating for Medical Application under Noninvasive Temperature Measurement Using Magnetic Resonance

  • Recent development of magnetic resonance imaging (MRI) equipment enables interventional radiology (IVR) as diagnosis and treatment under MRI usage. In this paper, a new methodology for magnetic resonance (MR) scanner to apply not only diagnostic equipment but for treatment one is discussed. The temperature measuring procedure under MR is to measure phase shift of $T_1$, which is the longitudinal relaxation time of proton, for the position inside a sample material with the application of pulsed RF for heating inside the sample as artificial dielectrics. The result shows the possibility to apply MR as temperature measuring equipment and as a heating equipment for applying such as hyperthermia heating modality.
