  • In this study, we allocate bits by quantizing these fractal coefficients through a quantizer which can extract the probability distribution. In the coding process of IFS, a variable size block method is used to shorten the coding time and improve the compression ratio. In the future, it will be necessary to further improve the coding time and the compression rate while maintaining the best image quality in the fractal coding process.

Evaluation of canalis basilaris medianus using cone-beam computed tomography

  • The aim of this report is to present two cases of canalis basilaris medianus as identified on cone-beam computed tomography (CBCT) in the base of the skull. The CBCT data sets were sent for radiographic consultation. In both cases, multi-planar views revealed an osseous defect in the base of the skull in the clivus region, the sagittal view showed a unilateral, well-defined, non-corticated, track-like low-attenuation osseous defect in the clivus. The appearance of the defect was highly reminiscent of a fracture of the clivus. The borders of osseous defect were smooth, and no other radiographic signs suggestive of osteolytic destructive processes were noted. Based on the overall radiographic examination, a radiographic impression of canalis basilaris medianus was made. Canalis basilaris medianus is a rare anatomical variant and is generally observed on the clivus. Due to its potential association with meningitis, it should be recognized and reported to avoid potential complications.

Incidental occurrence of an unusually large mastoid foramen on cone-beam computed tomography and review of the literature

  • The incidental finding of an enlarged mastoid foramen on the right posterior mastoid region of temporal bone is reported, together with a discussion of its clinical significance. A 67-year-old female underwent the pre-implant assessment of a maxillary left edentulous region. A cone-beam computed tomographic (CBCT) image was acquired and referred for consultation. Axial CBCT slices revealed a unilateral, well-defined, noncorticated, low-attenuation, transosseous defect posterior to the mastoid air cells in the right temporal bone. The borders of the osseous defect were smooth and continuous. No other radiographic signs suggestive of erosion or sclerosis were noted in the vicinity. The density within the defect was homogenous and consistent with a foramen and/or soft tissue. The patient's history and physical examination revealed no significant medical issues, and she was referred to a neuroradiologist for a second opinion. The diagnosis of an enlarged mastoid foramen was made and the patient was reassured.

Bisphosphonate-related osteonecrosis of the jaw in a multiple myeloma patient: A case report with characteristic radiographic features

  • A 59-year-old male who had suffered from multiple myeloma for nine years and had been administered bisphosphonates for seven years visited a dental hospital for pain relief due to extensive caries in his left maxillary molars. The molars were extracted, leaving an exposed wound for three months. The radiograph showed sequestra formation and irregular bone destruction in the left maxilla. Sudden pain and gingival swelling in the right mandibular molar area occurred six months later. The interseptum of the right lower second molar was observed to be necrotic during surgery. These findings coincided with the features of bisphosphonate-related osteonecrosis of the jaw (BRONJ). In this case, the long intravenous administration of bisphosphonates and tooth extraction were likely the etiologic factors of BRONJ in a patient with multiple myeloma; moreover, the bilateral occurrence of BRONJ is a characteristic feature.

Absorbed and effective dose from newly developed cone beam computed tomography in Korea (최근 개발된 cone beam computed tomography의 흡수선량 및 유효선량 평가)

  • Purpose: Cone beam computed tomography (CBCT) provides a lower dose and cost alternative to conventional CT, promising to revolutionize the practice of oral and maxillofacial radiology. The purpose of this study was to evaluate the absorbed and effective doses of Implagraphy and VCT (Vatech Co., Hwasung, Korea) and compare them with those of panoramic radiography. Materials and Methods: Thermoluminescent dosimeter (TLD) chips were placed at 27 sites throughout the layers of Female ART Head and Neck Phantom for dosimetry. Implagraphy, VCT units, and Planmeca Proline XC panoramic unit were used for radiation exposures. Radiation weighted doses and effective doses were measured and calculated using 1990 and 2005 ICRP tissue weighting factors. Results: Effective doses in Sv (ICRP 2005, ICRP 1990) were 90.19, 61.62 for Implagraphy at maxillay molar area, 123.20, 90.02 for Implagraphy at mandibular molar area, 183.55, 139.26 for VCT and 40.92, 27.16 for panoramic radiography. Conclusion: Effective doses for VCT and Implagraphy were only about 2.2 to 4.5 times greater than those for panoramic radiography. VCT and Implagraphy, CBCT machines recently developed in Korea, showed moderately low effective doses.

Scalloped border as a possible diagnostic aid for differentiating jaw lesions: A pictorial essay

  • Purpose: The aim of this study was to introduce a category of jaw lesions comprising cysts and tumors associated with scalloped borders. Materials and Methods: General search engines and specialized databases including Google Scholar, PubMed, PubMed Central, and Scopus, as well as an authoritative textbook, were used to find relevant studies by using keywords such as "jaw lesion," "jaw disease," "scalloping," "scalloped border," "scalloped margin," "irregular border," and "irregular margin." Out of 289 articles, 252 records were removed because they were duplicates, did not have a relevant title, or did not mention the frequency of findings described using the term "scalloped border." Finally, 37 closely related articles were chosen. Results: According to the relevant literature, scalloped borders are found most frequently in ameloblastoma, followed by simple bone cyst, central giant cell granuloma, odontogenic keratocyst, and glandular odontogenic cyst. Conclusion: The lesions most frequently reported to have scalloped borders are ameloblastoma, central giant cell granuloma, odontogenic keratocyst, simple bone cyst, and glandular odontogenic cyst.

Diffusion-weighted MR Imaging of Hypoxic-Ischemic Encephalopathy (저산소성-허혈성 뇌증의 확산강조영상 소견)

  • Purpose : The purpose of this study was to determine the characteristics of hypoxic-ischemic encephalopathy (HIE) on diffusion-weighted imaging (DWI) and the role of DWI for the diagnosis of HIE. Materials and Methods : Six patients with HIE underwent MRI including DWI. MR examinations were performed within 4 - 32 days (mean, 11.8 days) after hypoxic brain insult. We assessed the distribution of the lesions and compared the DWI and T2, FLAIR images for the subjective conspicuity of the lesions. Results : In all patients, symmetrical hyperintense lesions were demonstrated in the bilateral basal ganglia on T2, FLAIR, and DWI. On ADC map image, the lesions were hypointense in four of six patients and isointense in other two patients. Lesion conspicuity on DWI was higher than on T2 and FLAIR images in four of six patients and similar in other two patients. For the involvement of the cortex and subcortical white matter, in five of six patients, bilateral symmetric hyperintense lesions were seen on T2, FLAIR, and DWI. Lesion conspicuity on DWI was higher than on T2 and FLAIR images in three of them and similar in other two patients. On ADC map image, the lesions showed hypointensity in three of five patients and isointensity in other two patients. For the involvement of the deep cerebral white matter, T2, FLAIR, and DWI showed bilateral symmetric hyperintense lesions in four of six patients. Among them, Lesion conspicuity on DWI was higher than on T2 and FLAIR images in only one patient. Conclusion : HIE is characterized by symmetrical hyperintense lesions in the bilateral basal ganglia, cerebral cortex, and white matter on DWI and the lesions are more conspicuously demonstrated on DWI than on T2 and FLAIR images.

CT-Derived Deep Learning-Based Quantification of Body Composition Associated with Disease Severity in Chronic Obstructive Pulmonary Disease (CT 기반 딥러닝을 이용한 만성 폐쇄성 폐질환의 체성분 정량화와 질병 중증도)

  • Purpose Our study aimed to evaluate the association between automated quantified body composition on CT and pulmonary function or quantitative lung features in patients with chronic obstructive pulmonary disease (COPD). Materials and Methods A total of 290 patients with COPD were enrolled in this study. The volume of muscle and subcutaneous fat, area of muscle and subcutaneous fat at T12, and bone attenuation at T12 were obtained from chest CT using a deep learning-based body segmentation algorithm. Parametric response mapping-derived emphysema (PRMemph), PRM-derived functional small airway disease (PRMfSAD), and airway wall thickness (AWT)-Pi10 were quantitatively assessed. The association between body composition and outcomes was evaluated using Pearson's correlation analysis. Results The volume and area of muscle and subcutaneous fat were negatively associated with PRMemph and PRMfSAD (p < 0.05). Bone density at T12 was negatively associated with PRMemph (r = -0.1828, p = 0.002). The volume and area of subcutaneous fat and bone density at T12 were positively correlated with AWT-Pi10 (r = 0.1287, p = 0.030; r = 0.1668, p = 0.005; r = 0.1279, p = 0.031). However, muscle volume was negatively correlated with the AWT-Pi10 (r = -0.1966, p = 0.001). Muscle volume was significantly associated with pulmonary function (p < 0.001). Conclusion Body composition, automatically assessed using chest CT, is associated with the phenotype and severity of COPD.

  • This research, used an contrast agent, which weighs about 1mmol/mL, of twice as much amount of gadolinum, per unit, for the test of magnetic-resonance shoulder arthrography. the reasearch which was carried out from January, of the year 2012 to August of the year 2013. consisted, of target of 41, patients including 20 ones, of whom an original, contrast agent of amount of 0.5mmol/mL, 21, ones of whom, a new, of amount of 1mmol/mL was used on, were the test target, in order to figure out the differences, according to the amount of gadolinum, according to the test results, the SNR of the contrast agent, of amount of 1mmol/mL, which is of high amount of gadolinum, was noticeably higher than the one of amount of 0.5mmol/mL(the percentage of joint space 38.01%, the supraspinous muscle 8.40%, head of humerus 12.78%). and CNR of the contrast agent, of amount of 1mmol/mL, which is of high amount of gadolinum, was higher than the one of amount of 0.5mmol/mL(the percentage of joint space and supraspinous muscle 48.96%, the one of joint space and head of humerus 42.00%). In conclusion, one of the methods of increasing the reducing effect of T1, is to use contrast agent of amount of 1mmol/mL, in order to increase the reducing effect of T1, acquire the visual of high testing quality.

Development and Usefulness of Acrylic Needle for Percutaneous Bone Biopsy (경피적 골생검용 아크릴 바늘의 제작과 유용성)

  • To evaluate the usefulness of newly designed percutaneous bone biopsy needle for pumpkin's specimen collection. We manufactured three types of biopsy needle with different internal processing which were made of 10 mm-diameter acrylic material. We made the conventional type (Ct) similar to the clinical type then compared the test group. The type a (Ta) made 1 cm-length internal processing from the distal, type b (Tb) made taper, and type c (Tc) made internal processing like spiral configuration. We performed 20 times biopsy to get an 10 mm length specimen from pumpkin's surface and evaluated the success rate of the biopsy, length of the specimen, and determine internal processing type of the most suitable biopsy needle (ANOVA test). Success rates of Ct, Ta, Tb, and Tc were 55%, 80%, 90%, and 100%, respectively. The lengths of the specimen of Ct, Ta, Tb, and Tc were $5.6{\pm}1.1\;mm$, $5.9{\pm}0.87\;mm$, $3.9{\pm}0.77\;mm$, and $9.4{\pm}0.54\;mm$, respectively. All groups were statistically significant (p < 0.05) except the group between Ct and Ta (p = 0.28). Newly designed bone biopsy needle seems to be useful for obtaining enough specimen. Tc may be more effective than other types.
