A Study on the Carotid Artery Ultrasonography for the Metabolic Syndrome (대사증후군에서 경동맥 초음파 검사에 대한 연구)

  • The primary goal of this study was to ascertain the primary factors to the affect for the carotid artery intima-media thickness (IMT), the prevalence of metabolic syndrome and other risks can possibly influence the carotid artery IMT. All patients data (total specimens: 289, male: 197, female: 92) including the carotid artery ultrasonography examination. The all data were analyzed by the use of SPSS software, version 21.0 (SPSS, Chicago, IL USA), with the descriptive statistics method. The Results of this study was found to be highly increased in the males than the females. The prevalence of metabolic syndrome in all of the participants was 30.5 percentages. The carotid artery IMT in the subjects with metabolic syndrome was significantly high in both genders, compared to the rest, who were without metabolic syndrome. The Pearson's correlation coefficient of metabolic syndrome and CIMT was 0.378(p<0.01). In conclusions, the present study also supports the association between the carotid artery IMT and the metabolic syndromes with cardiovascular risk factors. Usage of B-mode ultrasonography to measure the carotid artery IMT was found to be highly effective in the current analysis.

The result of Radiotherapy in Malignant Thymona (흉선암의 방사선치료 성적)

  • Twenty one patients of malignant thymoma treated with curative aim at the Department of Therapeutic Radiology of Seoul National University Hospital from 1979 to 1987 were analysed retrospectively. The 3 year overall and relapse free survival rate was $80.5\%\;and\;78.6$, respectively. Myasthenia gravis (MG) was seen in $43.5\%$ at presentation and disappeared in $40\%$ (4/10) after radiotherapy with or without operation. The 3 year cumulative survival rate with and without MG was $90\;and\;78.8\%$, respectively. We could consider that MG was no longer adverse prognostic factor. The complete response rate after partial resection was $100\%$ (3/3), and that after biopsy was $20\%$(3/15). The overall local control rate including complete and partial response rate ($33\;vs\;56\%$)was$89\%$ and the 3 year actuarial survival rate by the response rate was $88.9\%\;and\;81.7\%$, respectively. There was no statistically significant survival difference between two groups. The crude rate of relapse at 3 years was $23.8\%$ (5/21), and $80\%$ (4/5) were locoregional failures. All failures were observed in biopsy only group, while no failure was observed in resected group. The major pattern of the treatment failure was the locoregional failure and the distant metastases was rarely observed.

Development of Central Nervous System in Scuttle Fly (Scuttle Fly에서 중추신경계 발달 연구)

  • The scuttle fly central nervous system (CNS) is unobservable during egg and larvae instar stage 1. During days 2~3 of larvae instar stage 2, the left and right hemisphere of the brain can be observed. Below the brain, the subesophageal ganglion (SOG) connects to the ventral nerve cord (VNC). During days 3~5 of larvae instar stage 3, the CNS enlarged slightly with no other changes. During days 1~3 of the pupal stage the CNS moved to the head with no distinguishable changes from the previous stage. During days 4~6 of the pupal stage, the left and right hemisphere of the brain had fused into one mass and the optic lobe (OL) located on the side of the brain completed its development. During days 7~9 of the pupal stage, the OL began to show eyeball pigment. The SOG was connected to the brain and the VNC began to separate, which was accompanied by an increase in nerve fibers. During days 10~12 of the pupal stage, the brain of the CNS and VNC was clearly distinguished and the brown pigmentation of OL became darker. During days 13~15 days of the pupal stage, the separated brain and VNC became connected by thin nerve fiber. The VNC began to separate into two with a greater increase in nerve fibers. The adult fly showed similar features to the previous stage, but the brain was located in the head and the VNC in the chest.

The Relevance of Caregiver Burden, Depressive symptoms and Mental Related Quality of Life in a Stroke Patient's Caregiver (뇌졸중 환자 보호자의 부양부담감 및 우울감과 정신건강관련 삶의 질과의 관련성)

  • Objective: This study examined the caregiver burden, depressive symptoms, and mental-related quality of life of 226 caregivers of stroke patients, who had been hospitalized in 7 general hospitals located in Cheongju and Daejeon Metropolitan city. Methods: Data were collected from August 5, 2014 to October 5, 2014 and a structured self-administered questionnaire was used. The results were analyzed using a t-test, ANOVA for different comparisons of the mental related quality of life in the sociodemographic characteristics, care-related characteristics, health-related behavioral characteristics, caregiver burden, and depressive symptoms. Hierarchical multiple regression was conducted to determine the explanatory power of the independent variables on the dependent variables, with the variables showing significant differences in univariate analysis as independent variables. Results: According to the results of hierarchical multiple regression analysis, the relevant factors that influenced the mental-related quality of life were the relationship with a patient, burden by 'care', burden by sacrifice of 'personal life', and depressive symptoms. Conclusion: To enhance health-related quality of life, not only is a systematic complement on such factors needed, but the development and implementation of an intervention program to the caregiver burden and depressive symptoms is also urgently required.

Relationship between the depth of cingulate sulcus on neonatal high resolution cranial ultrasound and gestational age (신생아 고해상 뇌 초음파검사에서 대상고랑의 깊이와 재태기간과의 관계)

  • Purpose:It is critical that the exact gestational age of a newborn baby, especially premature baby, be determined to evaluate the status of a disease and its management and to estimate the prognosis of a patient. This study aimed to investigate an easy and accurate method to estimate gestational age on cranial ultrasound, requiring minimal additional time and equipment. Methods:A high-resolution coronal sonographic image was obtained via the anterior fontanel with a 5-12 Mhz linear probe after the usual cranial sonographic examination. We measured the depth of cerebral hemisphere, thickness of corpus callosum, and depth of cingulate sulcus and obtained the correlations between these factors and gestational age. Results:Depth of cingulate sulcus had the highest correlation coefficiency with gestational age (r=0.878, P=0.000). All the cases, except for 2 cases, had a gestational age of more than 37 weeks, when the depths of cingulate sulcus were more than 0.55 cm, and had a gestational age less than 34 weeks, when the depths of cingulate sulcus were less than 0.35 cm. Conclusion:Measurement of the depth of cingulate sulcus was a simple and accurate method to estimate the gestational age on cranial ultrasound. The gestational age is more than 37 weeks, when the depth of cingulate sulcus is more than 0.55 cm, and is less than 34 weeks, when the depth of cingulate sulcus is less than 0.35 cm.

Analysis on Developmental Factors of the Liver Diseases in Ultrasound Diagnosis of Healthcare (종합검진 초음파검사에서 나타난 간질환과 건강관련 요인분석)

  • The study found out developmental factors of the liver diseases in 29, 531 cases of the healthy adults who were diagnosed by using ultrasound at domestic healthcare centers in 6 cities. The results are as follows. Based on the result of the study, the liver diseases diagnosed by using ultrasound was revealed to show 43.1% of prevalence, and the occurrence was significantly higher in male (23.3%) than in female (19.8%). The prevalence of hepatic diseases related to the BMI was revealed to show highest prevalence of the fatty liver in obese group (BMI $\geqq$ 25) by recording 44.3%. Smoking contributed to the high prevalence of all liver diseases. Although the fatty liver was the most frequently occurred form of liver diseases by recording the prevalence of 49.1% (22.2% in male, 26.9% in female), the significant difference was found only in female (p < 0.05), but male group did not show significant difference (p > 0.05). The prevalence of hepatic diseases related to the hypertension was revealed to show highest prevalence of the fatty liver in hypertension group by recording 67.7%. The prevalence of hepatic diseases related to the diabetes was revealed to show highest prevalence of the fatty liver in diabetes group by recording 66.2%. The high prevalence of all hepatic diseases was related to diabetes mellitus with statistical significance (p < 0.001). The multiple regression analysis for the related factors which affect the prevalence of the liver diseases showed the higher prevalence by age. Sex, obesity and diabetes mellitus were positively related to the prevalence (p < 0.05) while hypertension and smoking showed no significant relationship to the prevalence of the disease (p > 0.05).

The Combined Effect of Fast Neutron and Hyperthermia according to the Sequence and Interval in MKN-45 Cells (MKN-45 세포에서 속중성자와 온열치료의 순서 및 간격에 따른 병용효과)

  • Purpose : It has been well established that the response of cells and tissues to low LET radiations (X- or gamma-ray) can be enhanced by combining with hyperthermia. However, there has been relatively little work of hyperthermia on the possible modification of either cellular or tissue responses to other types of radiation. So, we investigated the combined effect of fast neutron irradiation and hyperthermia according to the sequence and time interval of the two. Materials and Methods : In MKN-45 cells, a human stomach cancer ceil line, suwiving fractions were measured according to the sequential treatment of 0, 4, 2, 0 hour-intewal for fast neutron irradiation (1.5 Gy) combined with hyperthermia (41 $^{\circ}C$ for 30 min or 43$^{\circ}C$ for 30 min). Results : D$_{0}$ and n of MKN-45 for neutron were 0.8 Gy and 2.5, respectively. The surviving fraction by 1.5 Gy of neutron was 0.36$\pm$0.34. Interacting powers were mostly ranged between 1 and 2, but they were 3.0 and 2.7, respectively for hyperthermia (41 $^{\circ}C$ for 30 min) fellowed by neutron irradiation 6 and 4 hours later. Conclusion : The combined effect of fast neutron (1.5 Gy) and hyperthermia (41 $^{\circ}C$ or 43$^{\circ}C$ for 30min) is largely independently additive. Preceding mild hyperthermia (41 $^{\circ}C$ for 30 min) 4 or 6 hours before neutron may cause decreased sensitivity to subsequent neutron irradiation.

  • Objectives Firefighters and rescue workers are likely to be exposed to a variety of traumatic events; as such, they are vulnerable to the risk of post-traumatic stress disorder (PTSD). The psychometric properties of the Korean version of the PTSD Checklist (PCL), a widely used self-report screening tool for PTSD, were assessed in South Korean firefighters and rescue workers. Methods Data were collected via self-report questionnaires and semi-structured clinical interviews administered to 221 firefighters. Internal consistency, item-total correlation, one-week test-retest reliability, convergent validity, and divergent validity were examined. Content validity of the PCL was evaluated using factor analysis and receiver operating characteristic (ROC) analyses were used to estimate the optimal cutoff point and area under the curve. Results The PCL demonstrated excellent internal consistency (${\alpha}=0.97$), item-total correlation (r = 0.72-0.88), test-retest reliability (r = 0.95), and convergent and divergent validity. The total score of PCL was positively correlated with the number of traumatic events experienced (p < 0.001). Factor analysis revealed two theoretically congruent factors: re-experience/avoidance and numbing/hyperarousal. The optimal cutoff was 45 and the area under the ROC curve was 0.97. Conclusions The Korean version of the PCL may be a useful PTSD screening instrument for firefighters and rescue workers, further maximizing opportunities for accurate PTSD diagnosis and treatment.

Lipid accumulation product is a predictor of nonalcoholic fatty liver disease in childhood obesity

  • Background: Lipid accumulation product (LAP) is associated with the presence and severity of nonalcoholic fatty liver disease (NAFLD) in adults. Purpose: Here we evaluated the ability of LAP to predict NAFLD in obese children. Methods: Eighty obese children (38 girls; age 6-18 years) were included. Anthropometric measurements and biochemical values were obtained from the patients' medical records. LAP was calculated as [waist circumference (WC) (cm) - 58]×triglycerides (mmol/L) in girls; [WC (cm) - 65]×triglycerides (mmol/L) in boys. The minLAP and adjLAP were described (3% and 50% of WC values, respectively) and the total/high-density lipoprotein cholesterol index (TC/HDL-C) was calculated. NAFLD was observed on ultrasound, and patients were divided into 3 groups by steatosis grade (normal, grade 0; mild, grade 1; moderate-severe, grade 2-3). The area under the curve (AUC) and appropriate index cutoff points were calculated by receiver operator characteristic analysis. Results: LAP was positively correlated with puberty stage (rho=0.409; P<0.001), fasting insulin (rho= 0.507; P<0.001), homeostasis model assessment of insulin resistance (rho=0.470; P<0.001), uric acid (rho=0.522; P<0.001), and TC/HDL-C (rho=0.494; P<0.001) and negatively correlated with HDL-C (rho=-3.833; P<0.001). LAP values could be used to diagnose hepatosteatosis (AUC=0.698; P=0.002). The LAP, adjLAP, and minLAP cutoff values were 42.7 (P=0.002), 40.05 (P=0.003), and 53.47 (P= 0.08), respectively. For LAP, the differences between the normal and mild groups (P=0.035) and the normal and moderate-severe groups were statistically significant (P=0.037), whereas the difference between the mild and moderate-severe groups was not (P>0.005). There was a statistically significant difference between the normal and mild groups for adjLAP (P=0.043) but not between the other groups (P>0.005). There was no significant intergroup difference in minLAP (P>0.005). Conclusion: LAP is a powerful and easy tool to predict NAFLD in childhood. If LAP is ≥42.7, NAFLD should be suspected. This is the first study to assess LAP diagnostic accuracy for childhood obesity.

Factors Affecting the Turnover Intention for Radiologic Technologists Working in Private Clinic and University Hospital in Daejeon City, Korea - (병.의원에 근무하는 방사선사들의 이직의사에 관련된 요인 - 대전광역시를 중심으로 -)

  • This study was executed to find out the intensity of turnover intention and its associated factors of radiologic technologists who worked for private clinic and university hospitals in Daejoen. Such variables as social and demographic characteristics, job-related features, job stress, job satisfaction, job performance, organizational commitment and turnover intention were investigated based on the survey of randomly sampled 400 radiologic technologists. The results are summarized as follows. 1. College graduates of private clinic workers were significantly higher than junior college graduates of them in the mean score for turnover intention. And low age groups, unmarried group, no religion group, low job career groups and low income groups of university hospital workers were significantly higher than high age groups, married group, religion group, high job career groups and high income groups of them respectively in the mean score for turnover intention. 2. Private clinic and university hospital workers' low and high groups in the variables of job demand, job control, supervisor support, job satisfaction, organizational commitment showed significant difference in the mean score for turnover intention. specially private clinic workers' low and high groups in the variables of coworker support, job performance showed significant difference in the mean score for turnover intention. 3. A hierarchial multiple regression analysis was conducted to catch the influence of related variables on turnover intention according to private clinic workers and university hospital workers. As a result of the analysis, the higher the degrees of education and job demand were and the lower the degree of job satisfaction was for private clinic workers, the higher the degree of turnover intention was. And the lower the degrees of age, job career, job satisfaction and organizational commitment were for university hospital workers, the higher the degree of turnover intention was. In conclusion, it was found that job demand and job satisfaction for private clinic workers and job satisfaction and organizational commitment for university hospital workers influenced on turnover intention.
