Scarf(Z) osteotomy of the First metatarsal shaft in the Treatment of Hallux Valgus (무지 외반증 환자에서 스카프 절골술을 이용한 치료)

  • Purpose: The purpose of this study was to the evaluate the clinical and radiographic results and complication of the Scarf osteotomy which is one of the surgical treatment of Hallux valgus. Material and Method: Fifty feet (30 patients) of moderate to severe hallux valgus deformity were operated by Scarf osteotomy procedure from January 2002 to July 2002. Patients were asked about their satisfaction in terms of pain, appearance and motion. The AOFAS clinical ratio scales and radiographic results for hallux valgus were evaluated. Result: The mean Hallux valgus angle improved from an average of 30.6 degrees to 9.0 degrees and the first intermetatarsal angle improved from an average 13.3 degrees to 5.2 degrees respectively. The preoperative mean AOFAS score was 58 points and on last follow up AOFAS score improved to 88 points. Complication which including troughing, nonunion, malunion was not developed. Conclusion: We found that Scarf osteotomy is a reliable operation for mild to moderate Hallux valgus deformity as regards firm fixation and early postoperative ambulation and large contact area for union.

  • Purpose: The palatogingival groove is a developmental anomaly of the incisor teeth, which often presents severe localized periodontal disease. The purpose of this study was to evaluate the clinical outcome of palatogingival groove-associated periodontal lesion following flap operation with glass ionomer filling. Methods: Four patients with periodontal lesion associated with the palatogingival groove were chosen for this case study. Clinical indices were taken and radiographic exam was performed at the baseline of the study and four patients were treated by flap operation with GI filling. Post-surgical visits were scheduled at regular intervals to check clinical and radiographic changes. Results: Symptoms and signs of periodontal lesion were almost completely resolved with improvement of periodontal indices. Conclusions: Flap operation with direct glass ionomer restoration is thought to be an acceptable method which can produce favorable results in the treatment of periodontal lesion caused by palatogingival groove on the maxillary lateral incisor.

Characterization of the Spatial Variability of Paper Formation Using a Continuous Wavelet Transform

  • In this investigation, a wavelet transform analysis was used to decompose beta-radiographic formation images into spectral and spatial components. Conventional formation analysis may use spectral analysis, based on Fourier transformation or variance vs. zone size, to describe the grammage distribution of features such as flocs, streaks and mean fiber orientation. However, these methods have limited utility for the analysis of statistically stationary data sets where variance is not uniform with position, e.g. paper machine CD profiles (especially those that contain streaks). A continuous wavelet transform was used to analyze formation data arrays obtained from radiographic imaging of handsheets and cross machine paper samples. The response of the analytical method to grammage, floc size distribution, mean fiber orientation an sensitivity to feature localization were assessed. From wavelet analysis, the change in scale of grammage variation as a function of position was used to demonstrate regular and isolated differences in the formed structure.

  • Purpose: An oroantral communication (OAC) is an abnormal space between the maxillary sinus and oral cavity. The causes, complications, treatment, and radiographic features of OAC in 2-dimensional and 3-dimensional imaging modalities are discussed. Materials and Methods: This pictorial review presents a broad spectrum of imaging findings of OAC. Representative radiographs depicting OAC were chosen from our database. PubMed was used to conduct a comprehensive literature search of OAC. Results: Characteristic features of OAC include discontinuity of the maxillary sinus floor, thickening of the maxillary sinus mucosa, or a combination of both. Two-dimensional imaging modalities are the method of choice for identifying discontinuities in the maxillary sinus floor. However, 3-dimensional imaging modalities are also essential for determining the status of soft tissue in the maxillary sinus. Conclusion: The integration of 2-dimensional and 3-dimensional imaging modalities is crucial for the correct diagnosis and comprehensive treatment of OAC. However, the diagnosis of OAC must be confirmed clinically to prevent unnecessary mental and financial burdens to patients.

  • Objectives: There have been few studies to date on the residual effect of bisphosphonate. This study investigated the radiographic changes of mandibular cortical thickness upon bisphosphonate drug holiday. Materials and Methods: This retrospective study includes 36 patients diagnosed with MRONJ (medication-related osteonecrosis of the jaw) at Ajou University Dental Hospital in 2010-2021. All patients stopped taking bisphosphonate under consultation with the prescribing physicians. Panoramic radiographs were taken at the start of discontinuation (T0), 12 months after (T1), and 18 months after (T2) discontinuation of bisphosphonate, respectively. Mental index and panoramic mandibular index were calculated using Ledgerton's method. Paired t-tests were used to analyze differences over time. Results: The difference in indices (mental index and panoramic mandibular index) between T0 and T1 was not statistically significant (paired t-test, P>0.05). However, the difference in these indices between T1 and T2 was statistically significant (paired t-test, P<0.05). Conclusion: The cortical thickness of the mandible decreased in the late stage (after 18 months) as observed by panoramic radiograph.

Outcome of open reduction and Kirschner wire fixation in pediatric radial neck fracture

  • Background: Radial neck fracture in children is rare. This study attempted to evaluate the outcome of surgically treated patients and any associated complications. Methods: This study evaluated 23 children under 15 years of age with radial neck fracture who were treated with open reduction between 2006 and 2016 to determine their range of motion, postoperative complications, and radiographic outcomes. The results were assessed clinically using the Mayo clinic elbow performance score. Results: The mean follow-up duration for patients was 34.6 months. The average postoperative angulation was 3.6°. Hypoesthesia was reported in only 9% of patients, and none of the patients complained of postoperative pain. The postoperative X-ray results were excellent in 60% and good in 40%. No radiographic complications were identified. The elbow score was excellent in 87% and good in 13% (mean score, 96.74). There was a statistical relationship between range of motion limitations and age, degree of fracture, initial displacement, and surgical pin removal time. Conclusions: Although most patients accept the closed reduction method as a primary treatment, the present study suggests that an open-reduction approach has been associated with optimal therapeutic outcomes for patients in whom closed reduction was not satisfactory or indicated.

ALVEOLAR BONE LOSS & BONE DENSITY CHANCES DURING THE FIRST YEAR A COMPARATIVE STUDY ON FOUR TYPES OF ROOT-FORM ENDOSSEOUS DENTAL IMPLANT SYSTEMS (치근형 골내 임플란트의 초기 1년간 치조골 소실 및 골 밀도 변화에 관한 연구)

  • Regular radiographic examination has been considered an essential diagnositic method for osseointegrated dental implantation. This study investigated marginal bone loss through the measurement on periapical radiographs and changes in bone density through digital subtraction image radiographic method around 88 endoseous root-form dental implants in 43 human subjects. Four types of endosseous dental implants were investigated : Standard series, Mini series and Hex-lock system of Steri-Oss Dental implant system, and $Br{\aa}nemark$-type implant from 3i dental implant system, in a 3 month interval for a total period of 12 months. The results were as follows : 1. Rapid bone loss occurred in the first 3 months in all 4 groups, and the bone level stabilized at the first thread of the implant fixtures. Amount of bone loss for 12 months showed correlation with the length of the polished neck portion. 2. Most of the implant systems showed resorption of alveolar bone up to the polished neck portion although a long polished neck could delay the resolution. 3. Alveolar bone loss apical to the polished neck portion stabilized at the first thread of the fixtures with no correlation to either the time of exposure of the polished neck or types of implant systems. 4. No changes in bone density around the implant threads were observed throughout the experimental period. Bone density decreased at the marginal bone, and increased at the newly-formed alveolar crest. These results indicate that most of the alveolar bone loss occur within the first 12 months after installation of endosseous root-form dental implants resulting in the exposure of polished neck portions, and the bone level stabilizes thereafter at the first thread portions of the implant fixtures. The experimental period of 12 months seems insufficient for observing changes in bone density, and a long-term observation should be needed.

RADIOLOGIC STUDY OF THE EXPERIMENTALLY PRODUCED BONE DESTRUCTION AND BONE FORMATION (실험적 골파괴와 골형성에 대한 방사선학적 연구)

  • Bone destruction was induced experimentally by the insertion of a bit of the arsenic compound into the pulp chambers of the right premolars and the artificial bone defects were produced in the periapical regions of the left premolars in 7 dogs. The serial standardized periapical radiographs using aluminum stepwedge attached to the XCP instruments, and resin bite blocks were taken following insertion of arsenic compound and at 2, 4, 7, 10, 14, 17, 21, 24 and 28 days in case of bone destruction and following bone injury and weekly thereafter for a total of 14 weeks in case of bone formation. The errors of the method were determined with error estimators described by the Duinkerke. All radiographs were evaluated by the visual examination after joint evaluation by three dental radiologists and analysed with densitometer. The following results were obtained; 1. Analysis of the bone destruction process 1) The error of the method in estimating two distances proved to be small (S.D. for the measuring error; 0.04㎜, S.D. for the over-all error; 0.06㎜, S.D. for the positioning error; 0.05㎜) 2) The radiographic changes were observed after 7 days in 6 cases, 4 days in 1 case and 10 days in 1 case by the visual examination. 3) Aluminum equivalent values were diminished after 2 days and the diminution of 0.58±0.19㎜ was demanded to be detected by the visual examination. 2. Analysis of the bone formation process 1) The error of the method in estimating two distances proved to be small (S.D. for the measuring error; 0.03㎜, S.D. for the over-all error; 0.04㎜ S.D. for the positioning error; 0.04㎜) 2) The radiographic changes were observed after 2 weeks in 5 cases and 3 weeks in 2 cases by the visual examination. 3) Aluminum equivalent values were increased after 1 week and the increase of 0.45±0.15㎜ was demanded to be detected by the visual examination. 4) Aluminum equivalent values were increased continuously for 7 or 9 weeks but there as only extremely small change after 10 weeks.

Radiographic Evaluation of Hip Dysplasia of the Sapsaree (삽사리 고관절 이형성의 방사선 평가)

  • This study was carried out to estimate the range of the hip score with 3 radiographic evaluation techniques and to identify the relationship among these methods in 71 Sapsarees (35 males and 36 females). Mean age of dogs was ranged from 4 to 73 months (averaged $24{\pm}18$ months). Hip joints were evaluated with OFA method, BVA/KC scoring scheme, and $PennHIP^{(R)}$ method. Hip joints were graded by the OFA grading system(only used for dogs over 24 month old age). The 64 joints evaluated were graded as good (17.2%), fair (46.9%), mild dysplastic (12.5%), moderate dysplastic (7.8%), and severe dysplastic (15.6%), respectively. In BVA/KC scoring scheme, the mean score of the 86 joints was recorded as $14.5{\pm}12.8$ (range from 2 to 52). The mean of Norberg angle in the 78 joints was $92.5{\pm}8.5$ degree (ranged from 63 to 110.5 degree). In $PennHIP^{(R)}$ method on the 126 joints, the mean DI was resulted as $0.60{\pm}0.16$ (ranged from 0.08 to 1.0). BVA/KC scores were gradually increased with age, positively correlated with OFA grade (r = 0.92, P < 0.001), but inversely proportional to Norberg angle (r = -0.79, P < 0.001). However, there was no correlation between BVA/KC and DI (r = 0.11). It was suggested that Sapsarees have high prevalence of cannine hip dysplasia, so selective breeding among sapsarees having normal hip joints might be necessary for preventing or controlling hip dysplasia.

COMPARATIVE STUDY OF DIRECT DIGITAL RADIOGRAPHIC SYSTEM WITH FILM-BASED DIGITAL IMAGING SYSTEM USING EKTASPEED AND EKTASPEED PLUS FILM (직접 디지탈 방사선 촬영시스템과 Ektaspeed 및 Ektaspeed Plus 필름을 이용한 방사선 사진용 디지탈 영상시스템과의 비교 연구)

  • The purpose of this investigation was to compare the direct digital radiographic system with film-based digital imaging system using Ektaspeed and Ektaspeed Plus film with respect to image characteristics and detectability and evaluate the sensor noise with the use of subtraction method. Direct digital radiographic system which used was Sens-A-Ray system(Regam Medical Systems, Sundsvall, Sweden) and film-based digital imaging system was composed of Macintosh II ci computer, high resolution Sony XC-77 CCD camera and intraoral x-ray film(Kodak Ektaspeed film, Kodak Ektaspeed Plus film). Images were taken by using CCD sensor of Sens-A-Ray system, Ektaspeed film and Ektaspeed Plus film with variable exposure time(0.06s, 0.1s, 0.16s, 0.2s, 0.3s, 0.4s, 0.5s, 0.6s, O.8s, LOs), 5 times at each exposure time. And then ektaspeed films and ektaspeed plus films were digitized using CCD camera. Image groups were divided into 3 groups; Sens-A-Ray group(direct digital radiographic system), Ektaspeed group and Ektaspeed Plus group (film-based digital imaging system) They were assessed by the following three aspects; image density, image contrast and detectability and sensor noise of Sens-A-Ray system was also evaluated. The results were as follow : 1. S group showed higher density than E , EP group except at the low exposure time(p<0.01). 2. S group showed higher contrast than E,EP group except at the high exposure time(p<0.01). 3. All groups showed good detectability at the each proper exposure time. Lowest exposure time which shows maximum detectability in S,EP group(0.5s) was lower than that in E group(0.6s). 4. Sensor noise of Sens-A-Ray system generally increased according to exposure time. On the basis of the above results, it was considered that Sens-A-Ray system could show higher speed, higher contrast than Ektaspeed, Ektaspeed Plus film except at too high and low exposure time and the same detectability as the conventional intraoral film.
