  • The aim of this paper is to describe a relatively recent international agreement on the widely debated concepts of: (i) attributing effects to low dose radiation exposure situations that have occurred in the past and, (ii) inferring radiation risk to situations that are planned to occur in the future. An important global consensus has been recently achieved on these fundamental issues at the level of the highest international intergovernmental body: the General Assembly of the United Nations. The General Assembly has welcomed with appreciation a scientific report on attributing health effects to radiation exposure and inferring risks that had been prepared the United Nations Scientific Committee on the Effects of Atomic Radiation (UNSCEAR) following a formal request by the General Assembly.

  • The purpose of this study is to build a database of patient information for efficient radiotherapy management. Microsoft Office Access was used to build the database owing to its convenience and compatibility. The most important aspect when building the patient database is to make the input and management of patient information efficient at every step of radiotherapy process. The information input starts from the patient's first visit to the radiation therapy department and ends upon completion of the radiotherapy. The forms for each step of radiotherapy process include the patient information form, the radiotherapy schedule form, the radiotherapy information form, the simulation order form, and the patient history form. Every form is centrically connected to the radiation oncology department's patient information form. A test revealed that the database was found to be efficient in managing patient information at each step. An important benefit of this database is improved efficiency in radiotherapy management. Information on patients who received radiotherapy is stored in a database. This means that this clinical data can be found easily and used in future, which will be helpful in research studies on the radiation oncology department. Benefits such as these will potentially contribute to improved radiotherapy quality.

Comparative Analysis of Terminology and Classification Related to Risk Management of Radiotherapy

  • We analyzed the terminology and classification related to the risk management of radiation treatment overseas to establish the terminology and classification system for Korea. This study investigated the terminology and classification for radiotherapy risk management through overseas research materials from related organizations and associations, including the IAEA, WHO, British group, EC, and AAPM. Overseas risk management commonly uses the terms "near miss", "incident", and "adverse event", classified according to the degree of severity. However, several organizations have ambiguous terminologies. They use the term "near miss" for events such as a near event, close call, and good catch; the term "incident" for an event; and the term "adverse event" for the likes of an accident and an event. In addition, different organizations use different classifications: a "near miss" is generally classified as "incident" in most cases but not classified as such in BIR et al. Confusion might also be caused by the disunity of the terminology and classification, and by the ambiguity of definitions. Patient safety management of medical institutions in Korea uses the terms "near miss", "adverse event", and "sentinel event", which it classifies into eight levels according to the severity of risk to the patient. Therefore, the terminology and classification for radiotherapy risk management based on the patient safety management of medical institutions in Korea will help in improving the safety and quality of radiotherapy.

몬테카를로 시뮬레이션을 이용한 보호복용 방사선 차폐 소재 연구 (A Study on Radiation Shielding Materials for Protective Garments using Monte Carlo Simulation)

  • Purpose: Lead has been widely used in radiation shielding for its low price and high workability. Recently in several europe countries, use of lead was banned for environmental issues. Also lead can cause health problems like alergies. Alternative materials for lead are highly required. The purpose of this study was to propose lead free radiation shielding material. Methods: Research of radiation shielding in Korea is not easy for certain limits such as radiation materials, experimental facilities and places. The collected data through the research were simulated using MCNPX. The simulation tools used for this study were utilized Monte Carlo method. Results: we suggest new design of lead free radiation shielding material using MCNPX code comparing shielding performance of new composite materials to lead. Conclusion: This newly introduced nano-scale composite of metal and polymer makes new chance for highly lightened radiation protective garments with endurable shielding performance.

  • C-arm fluoroscopy is useful equipment in interventional pain management because it helps to guide correct needle targeting for the accurate injection and drug delivery. However, due to increased use of C-arm fluoroscopy in various pain procedures, the risk of radiation exposure is a significant concern for pain physicians. The harmful biological effects of ionizing radiation on the human body are well known. It is therefore necessary to strive to reduce radiation exposure. Lead aprons with thyroid shields are the most fundamental radiation protective devices for interventional procedures, and are very effective. However, the operator's radiation safety cannot be guaranteed because pain physicians seem to lack sufficient interest, knowledge, and awareness about radiation safety. Also, inappropriate care and use of radiation protective devices may result in a higher risk of radiation exposure. The purpose of this article was to review the literature on radiation safety with a focus on lead aprons and thyroid shields and present recommendations related to those devices during C-arm fluoroscopic-guided interventions by pain physicians.

방사선치료 환자관리를 위한 컴퓨터 프로그램의 실험적 제작 (Experimental Computer-Based Management System of Patients in Radiation Oncology)

  • 근래에 와서 치료방사선학 영역에서의 전산화가 급속히 이루어지고 있으나 그 전산화의 대부분은 치료계획용 계산에 치우쳐져 있고 환자정보관리, 퇴원환자의 추적검사, 환자관리사무에 있어서의 전산화에 필요한 프로그램 등 환자관리에 필요한 프로그램은 매우 적으며, 상업적으로 개발된 프로그램은 일반화하기에는 많은 문제점을 갖고 있다. 이에 저자들은 16비트 개인용 컴퓨터를 이용하여 환자 현황 관리 프로그램을 시험 제작하였다. 1. 환자정보의 입력은 특별한 부호나 숫자를 사용하지 많고 현재 우리가 사용하는 영어나 한글을 그대로 쓸 수 있었다. 2 환자정보의 분석은 간단한 명령이나 키 동작으로 이루어질 수 있으며 특히 환자 현황에 대한 도표를 즉시 묘출할 수 있었다. 3. 환자관리정보의 내용은 기존 프린터를 이용하여 쉽게 문서화할 수 있으며, 학술발표, 강의재료 및 교육자료로 사용할 수 있었다.

  • Radiation synovetomy with various radiopharmaceuticals has been used to alleviate pain and swelling of rheumatoid arthritis and related joint diseases for more than 40 years. It is an attractive alternative to the surgical synovectomy for the management of the various joint diseases. Recently, the development of new radiopharmaceuticals labeled with $^{90}Y,\;^{32}P,\;^{186}Re,\;^{188}Re,\;^{153}Sm,\;^{165}Dy$ and $^{166}Ho$, for the effective management of synovial inflammation and related arthritic problems are gaining attention. In this article the general concepts and the clinical application of radiation synovectomy are reviewed.

원전 가동중 ECT 검사 방사선 작업시간 고찰 (Review on the Working Hours of Radiation Work Plan for ECT through In-service Inspection)

  • 원자력발전소 가동중 검사의 일부로서 계획예방정비 기간중 수행되는 ECT 검사작업은 작업자의 과피폭 우려로 인해 방사선안전관리 대상으로 비중을 두고 관리감독이 이루어지고 있다. 정기적으로 반복되는 검사수행경험 사례 분석의 결과, ECT 검사작업 완료후 피폭결과가 방사선안전관리 목표선량을 상회하는 경우가 상당수 나타나고 있어서 개인별 피폭선량관리가 쉽지않고 방사선작업허가서 발급에도 지장을 초래하는 것으로 나타났다. 검사자의 피폭선량 저감을 위한 세심한 방사선작업계획 수립과 최적화된 방사선방호대책마련을 위한 지속적인 방사선작업 관련 자료 축적도 중요하다고 판단되나, ECT검사 방사선작업계획의 기본요소인 작업내용 및 작업소요시간, 작업인원에 대한 검토와 논의는 미미한 실정이다. 방사선작업계획 수립의 근간을 이루는 예상피폭선량표에 포함될 검사작업내용과 검사자 작업소요시간에 대하여 반복시험과 사례분석 결과를 제시하였다.

Development of a Web-Based Program for Cross-Calibration and Record Management of Radiation Measuring Equipment

  • Purpose: To manage radiation measurement equipment, a web-based management program has been developed in this study. Materials and Methods: This program is based on a web service and Java Server Pages (JSP) and employs compatibility and accessibility. Results: The first step in the workflow has been designed to create accounts for each user or organization and to log in. The program consists of two parts: fields for listed instruments, and measurement information. The instruments for measuring radiation listed in this program are as follows: ionization chambers, survey meters, thermometers, barometers, electrometers, and phantoms. Instrument properties can be put in the recording fields and browsing for associated instruments can be performed. The main part of the program is the cross-calibration for each ion chamber. For instance, the ionization chamber to be used as a relative dosimeter can be registered by cross-calibration data with a reference chamber calibrated by an accredited laboratory. This program supports methods using the central axis transfer theory for cross-calibration for the ionization chambers. The reference and field ionization chambers were placed in a solid water phantom along the beam central axis at two different depths, and then the positions were switched. Each measured value was used for calculating the cross-calibration factor. Conclusions: Because many instruments are used and managed in radiation oncology departments, systematic, traceable recording is very important. The web-based program developed in this study is expected to be used effectively in the maintenance of radiation measurement instruments.
