Current status of disposal and measurement analysis of radioactive components in linear accelerators in Korea

  • When X-ray energy above 8 MV is used, photoneutrons are generated by the photonuclear reaction, which activates the components of linear accelerator (linac). Safely managing the radioactive material, when disposing linac or replacing components, is difficult, as the standards for the radioactive material management are not clear in Korea. We surveyed the management status of radioactive components occurred from medical linacs in Korea. And we also measured the activation of each part of the discarded Elekta linac using a survey meter and portable High Purity Germanium (HPGe) detector. We found that most medical institutions did not perform radiation measurements when disposing of radioactive components. The radioactive material was either stored within the institution or collected by the manufacturer. The surface dose rate measurements showed that the parts with high surface dose rates were target, primary collimator, and multileaf collimator (MLC). 60Co nuclide was detected in most parts, whereas for the target, 60Co and 184Re nuclides were detected. Results suggest that most institutions in Korea did not have the regulations for disposing radioactive waste from linac or the management procedures and standards were unclear. Further studies are underway to evaluate short-lived radionuclides and to lay the foundation for radioactive waste management from medical linacs.

Arsenite induces premature senescence via p53/p21 pathway as a result of DNA damage in human malignant glioblastoma cells

  • In this study, we investigate whether arsenite-induced DNA damage leads to p53-dependent premature senescence using human glioblastoma cells with p53-wild type (U87MG-neo) and p53 deficient (U87MG-E6). A dose dependent relationship between arsenite and reduced cell growth is demonstrated, as well as induced ${\gamma}H2AX$ foci formation in both U87MG-neo and U87MG-E6 cells at low concentrations of arsenite. Senescence was induced by arsenite with senescence-associated ${\beta}$-galactosidase staining. Dimethyl- and trimethyl-lysine 9 of histone H3 (H3DMK9 and H3TMK9) foci formation was accompanied by p21 accumulation only in U87MG-neo but not in U87MG-E6 cells. This suggests that arsenite induces premature senescence as a result of DNA damage with heterochromatin forming through a p53/p21 dependent pathway. p21 and p53 siRNA consistently decreased H3TMK9 foci formation in U87M G-neo but not in U87MG-E6 cells after arsenite treatment. Taken together, arsenite reduces cell growth independently of p53 and induces premature senescence via p53/p21-dependent pathway following DNA damage.

Value of Repeat Brain Computed Tomography in Children with Traumatic Brain Injury (소아 두부외상 환자에서의 반복적인 두부 CT 검사의 유용성)

  • Purpose: Traumatic brain injury (TBI) is the most common cause of pediatric trauma patients came to the emergency department. Without guidelines, many of these children underwent repeat brain computed tomography (CT). The purpose of this study was to evaluate the value of repeat brain CT in children with TBI. Methods: We conducted a retrospective study of TBI in children younger than 19 years of age who visited the emergency department (ED) from January 2011 to December 2012. According to the Glasgow Coma Scale (GCS) and Pediatric Glasgow Coma Scale score of the patients, study population divided in three groups. Clinical data collected included age, mechanism of injury, type of TBI, and outcome. Results: A Total 83 children with TBI received repeat brain CT. There were no need for neurosurgical intervention in mild TBI (GCS score 13-15) group who underwent routine repeat CT. 4 patients of mild TBI group, received repeat brain CT due to neurological deterioration, and one patient underwent neurosurgical intervention. Routine repeat CT identified 12 patients with radiographic progression. One patient underwent neurosurgical intervention based on the second brain CT finding, who belonged to the moderate TBI (GCS score 9-12) group. Conclusion: Our study showed that children with mild TBI can be observed without repeat brain CT when there is no evidence of neurologic deterioration. Further study is needed for establish indication for repetition of CT scan in order to avoid unnecessary radiation exposure of children.

  • Radiation and radioactive materials serve man in many beneficial ways. Diagnostic X-ray, radiation therapy, and other nuclear medicine uses of radioactivity save thousands of lives each year. Industrial application of radiation, such as radiography, make many manufactured products more reliable and less expensive. Nuclear power plants are producing more electrical power each year and reducing our dependence on imported oil. However, radiation can and dose produce harmful effects particularly as the reault of a radiation accident in which a victim receives as the result of a radiation accident in which a victim receives a large dose. Fortunately such accidents are very rare and recently we need more electric power produced by nuclear power plants. Considering increase of use of radiation or radioactive materials, we have to establish the radiological emergency response system prepared for radiation accidents.

Practical Approach to the Diagnosis of Pediatric Nasal Bone Fractures (소아 환자의 코뼈 골절 진단을 위한 실제적 접근)

  • Purpose: Nasal bone fractures are the most common childhood facial bone fractures, with an incidence of about 39%. While taking a nasal bone x-ray is a common modality used in the emergency department, reports have expressed concerns with its low sensitivity and low specificity. Our study was aimed at comparing accuracy of physical and x-ray examination with that of facial bone computed tomography (CT). Methods: Electronic medical records (EMR) were retrospectively reviewed for patients under the age of 15 who visited our emergency department from January 2010 to December 2011with a chief complaint of nasal pain due to trauma and who had also undergone a nasal bone x-ray and facial bone CT. Patients who had not taken facial bone CT, who had been transferred, and who did not have EMR were excluded. We divided the patients into 2 groups, those who had nasal bone fractures and those without a fracture on their facial bone CT. We analyzed other parameters such as age, sex, and type of fracture to find statistical differences between the two groups. Results: A total of 209 patients were included. The patients with nasal bone fractures on their facial bone CT were older, and their traumas were more violent. Ten patients who had apparent nasal bone fractures on their facial bone CT had no definite signs of a fracture on their plain x-ray. Conclusion: Though facial bone CT is an effective modality in detecting nasal bone fractures, in evaluating younger patients suspected of having nasal bone fractures, prudent use of facial bone CT is needed to reduce unnecessary exposure to radiation.

Bias-corrected Hp(10)-to-Organ-Absorbed Dose Conversion Coefficients for the Epidemiological Study of Korean Radiation Workers

  • Background: The effects of radiation on the health of radiation workers who are constantly susceptible to occupational exposure must be assessed based on an accurate and reliable reconstruction of organ-absorbed doses that can be calculated using personal dosimeter readings measured as Hp(10) and dose conversion coefficients. However, the data used in the dose reconstruction contain significant biases arising from the lack of reality and could result in an inaccurate measure of organ-absorbed doses. Therefore, this study quantified the biases involved in organ dose reconstruction and calculated the bias-corrected Hp(10)-to-organ-absorbed dose coefficients for the use in epidemiological studies of Korean radiation workers. Materials and Methods: Two major biases were considered: (a) the bias in Hp(10) arising from the difference between the dosimeter calibration geometry and the actual exposure geometry, and (b) the bias in air kerma-to-Hp(10) conversion coefficients resulting from geometric differences between the human body and slab phantom. The biases were quantified by implementing personal dosimeters on the slab and human phantoms coupled with a Monte Carlo method and considered to calculate the bias-corrected Hp(10)-to-organ-absorbed dose conversion coefficients. Results and Discussion: The bias in Hp(10) was significant for large incident angles and low energies (e.g., 0.32 for right lateral at 218 keV), whereas the bias in dose coefficients was significant for the posteroanterior (PA) geometry only (e.g., 0.79 at 218 keV). The bias-corrected Hp(10)-to-organ-absorbed dose conversion coefficients derived in this study were up to 3.09- fold greater than those from the International Commission on Radiological Protection publications without considering the biases. Conclusion: The obtained results will aid future studies in assessing the health effects of occupational exposure of Korean radiation workers. The bias-corrected dose coefficients of this study can be used to calculate organ doses for Korean radiation workers based on personal dose records.

Integrating Machine Learning with Data Envelopment Analysis for Enhanced R&D Efficiency & Optimizing Resource Allocation in the Specialized Field

  • Enhancing the efficiency of research and development (R&D) is crucial for organizations to remain competitive and generate innovative solutions. Data Envelopment Analysis (DEA) has emerged as a powerful tool for evaluating R&D efficiency. However, traditional DEA models heavily rely on the selection of input and output variables, which can limit their effectiveness. To overcome this dependency and improve the robustness of DEA, this study proposes a novel methodology that integrates machine learning techniques with DEA for determining the most suitable input and output variables. The proposed approach is particularly relevant for specialized R&D fields, such as Radiation Emergency Medicine (REM). REM is a critical domain that deals with the medical and public health consequences of nuclear emergencies. The selection of REM as the focus of this study is motivated by several factors, including the unique challenges posed by the field, the potential for significant societal impact, and the need for efficient resource allocation in emergency situations. By leveraging machine learning algorithms, such as Support Vector Machines (SVM), the proposed methodology aims to identify the most relevant input and output variables for DEA in the context of REM. The integration of machine learning enables the DEA model to capture complex relationships and non-linearities in the data, leading to more accurate and reliable efficiency assessments. The effectiveness of the proposed methodology is demonstrated through a comprehensive evaluation using real-world REM data. The results highlight the superior performance of the machine learning-integrated DEA approach compared to traditional DEA models. This study contributes to the advancement of R&D efficiency assessment in specialized fields and provides valuable insights for decision-makers in REM and other critical domains.

Measurements of Neutron Activation and Dose Rate Induced by High-Energy Medical Linear Accelerator

  • Purpose: During the treatments of cancer patients with a linear accelerator (LINAC) using photon beams with energies ≥8 MV, the components inside the LINAC head get activated through the interaction of photonuclear reaction (γ, n) and neutron capture (n, γ). We used spectroscopy and measured the dose rate for the LINAC in operation after the treatment ended. Methods: We performed spectroscopy and dose rate measurements for three units of LINACs with a portable high-purity Germanium (HPGe) detector and a survey meter. The spectra were obtained after the beams were turned off. Spectroscopy was conducted for 3,600 seconds, and the dose rate was measured three times. We identified the radionuclides for each LINAC. Results: According to gamma spectroscopy results, most of the nuclides were short-lived radionuclides with half-lives of 100 days, except for 60Co, 65Zn, and 181W nuclides. The dose rate for three LINACs obtained immediately in front of the crosshair was in the range of 0.113 to 0.129 µSv/h. The maximum and minimum dose rates measured on weekends were 0.097 µSv/h and 0.092 µSv/h, respectively. Compared with the differences in weekday data, there was no significant difference between the data measured on Saturday and Sunday. Conclusions: Most of the detected radionuclides had half-lives <100 days, and the dose rate decreased rapidly. For equipment that primarily used energies ≤10 MV, when the equipment was transferred after at least 10 minutes after shutting it down, it is expected that there will be little effect on the workers' exposure.

Emergency Cranial Irradiation Effects in Adult Leukemia with Extremely High Leukocytosis (극심한 백혈구 증다증이 동반된 성인 백혈병에 있어서 응급 두개부 방사선 조사의 효과)

  • We have treated adult acute leukemia 64 patients between January 1990 and October 1991 at the Chungnam National University Hospital. They were examined for the impact of presenting WBC count on the initial course and from them we have chosen twenty patients whose leukocyte count is over one hundred thousands per cubic milimeter, We divided the twenty patients into 4 groups on the base of treatment modalities: conservative therapy only, chemotherapy only, cranial irradiation only, and chemotherapy with cranial irradiation. Early sudden death rate is lower in cranial irradiation with/without chemotherapy groups than the conservative only or chemotherapy only patients. Also the remission rate is high in cranial irradiation with chemotherapy patients. Therefore we suggest that the rapid intervention of cranial irradiation in adult acute leukemia could be helpful in reducing the early sudden death rate and perhaps in increasing the remission rate.
