Four Cases of Shoulder Pain Treated by Ligamentum Pharmacopuncture on Kyonu(LI15), Gyeollyo(TE14), Geogol(LI16) and Nosu(SI10) (견우(LI15), 견료(TE14), 거골(LI16), 노수(SI10)에 시행한 인대 약침으로 치료된 견통 환자 4례)

  • Objectives : The purpose of this study is to report 4 cases of shoulder pain treated by Ligamentum Pharmacopuncture. Methods : This study on shoulder pain was carried out on 4 patients who were treated at Department of Acupuncture & Moxibusition, Dongshin University, Korean Medicine Hospital from May 2016 to July 2016. All patients were treated 2~3 times based on the patient's condition. To evaluate change of pain and function, we used Shoulder Pain and Disability Index(SPADI), Numeric Rating Scale(NRS) and Range Of Motion(ROM). Evaluations were made at baseline and one week and two weeks after treatment. Results : There was remarkable improvement in SPADI, NRS and ROM. Conclusions : This study suggested the possibility that the ligamentum pharmacopuncture therapy may be useful for the treatment of shoulder pain. Follow-up study is needed.

A Case Report of Foot Drop in Soyangin Improved with Sukjiwhanggosam-tang (숙지황고삼탕(熟地黃苦蔘湯)으로 호전된 소양인(少陽人) 족하수(足下華) 치험 1례)

  • 1. Objectives The purpose of this case study is to report the effects of Sukjiwhanggosam-tang which is based on the Sasang Constitutional Medicine for the Foot drop caused by peroneal nerve palsy. 2. Methods This patient was treated by Sukjiwhanggosam-tang according to the result of Sasang Constitutional diagnosis. We used Visual analogue scale(VAS), Range of motion(ROM) and Manual Muscle Testing(MMT) to evaluate the improvement of the Foot drop. 3. Results After 20 days treatment, the VAS of Rt. leg & foot hypoesthesia decreased from 10 to 2. The ROM of dorsiflexion of the ankle joint increased from $10^{\circ}$ to $30^{\circ}$ and the grade of MMT increased from 3 to 4. 4. Conclusions This case study shows an efficient result of using Sukjiwhanggosam-tang in the treatment of the Foot drop caused by peroneal nerve palsy.

Effect of Eight Vacuity Theory Adopted Complex Acupuncture Therapy on Patients with External Epicondylitis (주관절 외측상과염 환자에 대한 팔허(八虛) 이론에 근거한 복합침구치료 치험 2례)

  • Objectives : The objective of this study was to report two cases that patients with external epicondylitis were improved by based on 'eight vacuity' theory bloodletting therapy, acupuncture therapy and bee venom pharmacopuncture. Methods : In this study, patients were treated with bloodletting therapy, then acupuncture therapy and bee venom pharmacopuncture to Ashi point on eight vacuity of elbow in the patients without retention of needles. Subsequently, electroacupuncture and Dong-qi acupuncture treatment were implemented in order to strengthen the effect of acupuncture. In terms of evaluation methods, visual analog scale(VAS) score, range of motion(ROM), grip strength and physical examination were used to assess external epicondylitis-related symptoms. Results : VAS score decreased, ROM and grip strength were increased and physical examination was improved in all cases. Conclusions : It was suggested that the symptoms of intractable external epicondylitis could be improved by eight vacuity theory adopted bloodletting therapy, acupuncture therapy and bee venom pharmacopuncture.

Comparative Analysis of Nordic Walking and Normal Gait Based on Efficiency (노르딕 워킹과 일반 보행의 효율성 비교 분석)

  • The purpose of this study were to analyze the changes in kinematic and kinetic parameters and to find biomechanical benefits of Nordic Walking and normal gait performed under the same velocity. Nine participants(age: $26.73{\pm}3.28$ year, height: $182.45{\pm}4.62\;cm$, weight: $76.59{\pm}6.84\;kg$) was chosen. The velocity of gait was set by 5.75 km/h which was made by a Nordic Walking professional. The data were collected by using VICON with 8 cameras to analyze kinematic variables with 200 Hz and force platform to analyze kinetic variables with 2000 Hz. The results of this study were as follows. First, when compared with Normal gait, Nordic Walking group showed decreased Plantarflexion angle and ROM. Second, Nordic Walking group showed decreased knee flexion angle and ROM. Third, Nordic Walking group showed increased hip joint movement. Fourth, Nordic Walking group showed higher active GRF but decreased loading rate from delayed Peak Vertical GRF time and increased impulse. Fifth, Nordic Walking group showed longer ground contact time. Through this study, we found that Nordic Walking showed higher stability and efficiency during gait than normal gait and that Nordic Walking may help people who have walking difficulties.

Effects of Therapeutic Exercise on Pain, Physical Function, and Magnetic Resonance Imaging Findings in a Patient with Multilevel Lumbar Disc Herniation: A Case Report

  • Background: In some clinical guidelines followed in clinical practice, nonsurgical treatments are recommended as the primary intervention for patients with lumbar disc herniation (LDH). However, the effect of a therapeutic exercise program based on stabilization of the lumbar spine for treatment of multilevel LDH has not been evaluated thoroughly. Objective: To investigate the effects of therapeutic exercise on pain, physical function, and magnetic resonance imaging (MRI) findings in a patient with multilevel LDH. Design: Case Report Methods: A 43-year-old female presented with low back pain, radicular pain and multilevel LDH (L3-L4, L4-L5, L5-S1). The therapeutic exercise program was conducted. in 40-min sessions, three times a week, for 12 weeks. Low back and radicular pain, lumbar disability, and physical function were measured before and after 6 and 12 weeks of the exercise program. MRI was performed before and after 12 weeks of the program. Results: After 6 and 12 weeks of the therapeutic exercise, low back and radicular pain and lumbar disability had decreased, and lumbar range of motion (ROM) was improved bilaterally, compared with the initial values. Also improved at 6 and 12 weeks were isometric lumbar strength and endurance, and the functional movement screen score. The size of disc herniations was decreased on MRI obtained after 12 weeks of therapeutic exercise than on the pre-exercise images. Conclusions: We observed that therapeutic exercise program improved spinal ROM, muscle strength, functional capacity, and size of disc herniation in LDH patient.

Design of Electronic Control Unit for Parking Assist System (주차 보조 시스템을 위한 ECU 설계)

  • Automotive ECU integrates CPU core, IVN controller, memory interface, sensor interface, I/O interface, and so on. Current automotive ECUs are often developed with proprietary processor architectures. However, demends for standard processors such as ARM and RISC-V increase rapidly for saftware compatibility in autonomous vehicles and connected cars. In this paper, an automotive ECU is designed for parking assist system based on RISC-V with open instruction set architecture. It includes 32b RISC-V CPU core, IVN controllers such as CAN and LIN, memory interfaces such as ROM and SRAM, and I/O interfaces such as SPI, UART, and I2C. Fabricated in 65nm CMOS technology, its operating frequency, area, and gate count are 50MHz, 0.37㎟, and 55,310 gates, respectively.

  • Background: Arm swing plays a role in gait by accommodating forward movement through trunk balance. This study evaluates the biomechanical characteristics of arm swing during gait. Methods: The study performed computational musculoskeletal modeling based on motion tracking in 15 participants without musculoskeletal or gait disorder. A three-dimensional (3D) motion tracking system using three Azure Kinect (Microsoft) modules was used to obtain information in the 3D location of shoulder and elbow joints. Computational modeling using AnyBody Modeling System was performed to calculate the joint moment and range of motion (ROM) during arm swing. Results: Mean ROM of the dominant elbow was 29.7°±10.2° and 14.2°±3.2° in flexion-extension and pronation-supination, respectively. Mean joint moment of the dominant elbow was 56.4±12.7 Nm, 25.6±5.2 Nm, and 19.8±4.6 Nm in flexion-extension, rotation, and abduction-adduction, respectively. Conclusions: The elbow bears the load created by gravity and muscle contracture in dynamic arm swing movement.

A Study on Change of Plantar Fascia Thickness in Chronic Stroke Patient Based on Spasticity (만성 뇌졸중 환자에서 경직에 따른 족저근막의 두께 변화에 관한 연구)

  • The purpose of this study was to investigate the imbalance of muscle tone and frequent exposure to excessive gait training in patients with chronic spasticity due to stroke, the thickness of the plantar fascia to change and to evaluate. The subjects of this study was in 54 patients with chronic stroke from 18 patients Group I(MAS G0), 18 patients Group II(MAS G1), and 18 patients Group III(MAS G2) were selected. Measurement of clinical symptoms and physical examination, MAS(Modified Ashworth Scale), ultrasonographic, ROM(Range of Motion), VAS(Visual Analogue Scale), TUG(Timed Up and Go test) was measured. The study results were each group between the unaffected side and the affected side on plantar fascia thickness was statistically significantly thicker(p<.001). Each group between the unaffected side and the affected side on ankle dorsiflexion ROM was statistically significantly decrease(p<.001), VAS(p<.001), TUG(p<.001) statistically significantly increase(p<.001). In this study, the plantar fascia pathokinesiology ever presented by the contents of gait training in stroke patients is one of the information that you need to consider when presented.

Economic Evaluation of Eastern, Western and Collaborative Treatments for Patients with Frozen Shoulder Pain (견비통의 한${\cdot}$양방 진료 및 협진의 경제성 평가)

  • Objectives : The purpose of this study was to evaluate and compare the cost-effectiveness of Eastern, Western, and collaborative treatments and suggest a cost-effective approach for patients with frozen shoulder pain. Methods : Using the data of fifty-two patients, treatment effectiveness was measured by CSA, SPABI, and ROM scales and changes from the baseline score were evaluated. Data source for cost estimation was based on the national health insurance (NHI) payment system. Because the price in NHI was differentiated by health care institutions, five collaborative types were considered in assessing costs. Cost-effective ratios were computed for economic evaluation. Results : Compared with Eastern treatment, collaborative and Western treatments showed better effects on CSA scale after 4 weeks' treatment. The collaborative approach was also the most effective treatment on SPADI and ROM scales. The direct cost per patient receiving Eastern treatment was less than other treatments. In general, collaborative treatment dominated Eastern and Western treatments in cost-effectiveness an analysis. However, the cost-effectiveness ratio of Eastern treatment resulted in \9,000 compared to \29,000 of collaborative treatment on SPADI. Four different indicators of ROM scales resulted in different approaches as the cost-effective treatment. Conclusions : Considering cost-effectiveness ratios, collaborative treatment was the best treatment on CSh and SPADI scales after 4 weeks' treatment. As for ROM scales, the recommended alternatives were Eastern treatment for patients with abduction and adduction disabilities, Western treatment for those with flexion disability, and collaborative approach fir those with extension disabiliry.

Development and application of an English Electronic Textbook for Teaching and Learning based on XML (XML기반 영어과 교수학습용 전자교과서 개발 및 적용)

  • The purpose of this paper is to develop and apply an electronic English textbook for the teaching and learning of 3rd grade students. For this purpose, the textbook was based on XML to maximize the advantages of the electronic textbook format, and avoid the disadvantages of the CD-ROM that $3^{rd}$ grade classes currently use. After electronic textbook was developed, focus group interview was implemented to confirm the effectiveness of that based on interaction, satisfaction, and interests. As a result of this study, interaction, satisfaction, and interests of students will be improved through the use of electronic textbook and it is anticipated self-directed learning ability will be improved. Also, we anticipate that the students' learning effectiveness will be enhanced.
