Ubiquitous services are high duality and differentiated services which are provided for users by recognizing the context of users and environmental conditions actively. In this case, context-aware middleware is one of the most important technologies required to implement the ubiquitous services. In this paper, we propose a method for providing ubiquitous services in a specific user space effectively and monitoring human bio-signal sensors. That is, the design and implementation of intelligent home service middleware and monitoring human bio-signal sensor based on context awareness is discussed here. Context information from various sensors is gathered, and suitable services are inferred and provided to users by the middleware system. In our approach, user services can be modelled easily by using facts and rules, and the system can be extended easily to support various ubiquitous services other than intelligent home services also. The system can be integrated with external applications and legacy systems effectively by using various protocols such as RMI, socket and HTTP, XML and Zigbee etc. We have designed and evaluated various facts and rules for intelligent home services in real environments. Functionality evaluation with the system shows that ubiquitous services can be provided to users effectively in a home environment.

Background: Percutaneous cardiopulmonary support (PCPS) provides passive support of gas exchange and perfusion, allowing the use of other methods of care for organ recovery, and saves lives of patients with severe cardiopulmonary failure in a wide variety of clinical settings with a minimal risk of bleeding and need for chest re~ exploration. We summarized a single center's experiences with PCPS in patients with cardiogenic shock or cardiac arrest due to the ischemic heart disease. Material and Method: Among the 20 consecutive patients with cardiogenic shock or cardiac arrest from May 1999 to June 2005, Biopump (Medtronic, Inc, Minneapolis, MN) was used in 7 patients and the self-priming, heparin-coated circuit of EBS (Terumo, Japan) was applied to remaining 13 patients. Most of cannulations were performed percutaneously via femoral arteries and veins. The long venous cannulas of DLP (Medtronic inc. Minneapolis, MN) or the RMI (Edwards's lifescience LLC, Irvine, CA) were used with the arterial cannulae from 17 Fr to 21 Fr and the venous cannula from 21 Fr to 28 Fr. Result: The 20 consecutive patients who were severely compromised and received PCPS for the purpose of resuscitation were comprised of 13 cardiac arrests and 7 cardiogenic shocks in which by-pass surgery was performed in 11 patients and 9 ongoing PCls under the cardiopulmonary support. The mean support time on the PCPS was 38$\pm$42 hours. Of the 20 patients implanted with PCPS, 11 patients ($55\%$) have had the PCPS removed successfully; overall, 8 of these patients ($40\%$) were discharged from the hospital in an average surviving time for 27$\pm$17 days after removing the PCPS and survived well with 31$\pm$30 months of follow-up after the procedure. Conclusion: The use of PCPS appears to provide the hemodynamic restoration, allowing the survival of patients in cardiac arrest or cardiogenic shock who would otherwise not survive, and patients receiving PCPS had a relatively long-term survival.

최근 MOM 기술은 비즈니스 로직을 수행하는 애플리케이션 서버의 필수적인 구성요소로서 자리잡고 있으며, 보통 수백에서 수천의 클라이언트 요청을 처리할 수 있는 능력을 제공한다. MOM 은 이러한 대용량의 클라이언트 요청을 효과적으로 처리하기 위해서 효율적이고 확장성있는(스케일러블) 네트워크 모듈이 필요하며, 다양한 네트워크 프로토콜을 지원해야 한다. MOM이 기본적으로 지원하는 메시징 기능은 PTP(Point-To-Point)와 publish/subscribe 메시징 도메인으로 나뉘는데 이 논문에서는 두 가지 메시징 도메인과 그룹통신 메시징 서비스 기능을 동시에 지원하는 MoIM-Message 시스템의 하부 통신 모듈의 설계에 대해 기술한다. The CT number corresponds to electron density and its influence on dose calculation was studied. Five kinds of CT scanners were used to obtain Images of electron density calibration phantom (Gammex RMI 467), Then the differences between CT numbers for each scanners were ${\pm}2\%$ In homogeneous medium and $9.5\%$ in high density medium. In order to Investigate the influence of CT number to dose calculation, patients' thoracic CT images were analyzed. The maximum dose difference was $0.48\%$ for each organ. It acquired the phantom Images inserted high density material in the water phantom. Comparing the doses calculated with CT Images from each CT scanner, the maximum dose difference was $2.1\%$ in 20 cm in depth. The exact density to CT number conversion according to CT scanner is required to minimize the uncertainty of dose depends on CT number Especially the each hospital with various CT scanners has to discriminate CT numbers for each CT scanner. Moreover a periodic quality assurance is required for reproducibility of CT number.
Background: A peripheral extracorporeal membrane oxygenator (p-ECMO) has been developed to support patients who are dying due to a serious cardiopulmonary condition. This analysis was planned to define the clinical situation in which the patient benefits most from a p-ECMO. Material and Methods: Between June 2007 and Aug 2009, a total of 41 adult patients used the p-ECMO. There were 23 males and 18 females (mean age $54.4{\pm}15.1$ years). All patients had very unstable vital signs with hypoxia and complex cardiac problems. We divided the patients into 4 groups. In the first group, a p-ECMO was used as a bridge to cardiac operation. In the second group, patients did not have the opportunity to undergo any cardiac procedures; nevertheless, they were treated with a p-ECMO. In the third group, patients mostly had difficulty in weaning from CPB (cardiopulmonary bypass) after cardiac operation. The fourth group suffered from many complications, such as pneumonia, bleeding, infections, and LV dysfunction with underlying cardiac problems. All cannulations were performed by the Seldinger technique or cutting down the femoral vessel. A long venous cannula of $DLP^{(R)}$ (Medtronic Inc, Minneapolis, MN) or $RMI^{(R)}$ (Edwards Lifesciences LLC, Irvine, CA) was used together with a 17~21 Fr arterial cannula and a 21 Fr venous cannula. As a bypass pump, a Capiox emergency bypass system ($EBS^{(R)}$; Terumo, Tokyo, Japan) was used. We attempted to maintain a flow rate of $2.4{\sim}3.0\;L/min/m^2$ and an activated clotting time (ACT) of around 180 seconds. Results: Nine patients survived by the use of the p-ECMO. Ten patients were weaned from a p-ECMO but they did not survive, and the remainder had no chance to be weaned from the p-ECMO. The best clinical situation to apply the p-ECMO was to use it as a bridge to cardiac operation and for weaning from CPB after cardiac operation. Conclusion: Various clinical results were derived by p-ECMO according to the clinical situation. For the best results, early adoption of the p-ECMO for anatomical correction appears important.

The point is that these stores are threatening at small and medium-sized, community-based private supermarkets. Private supermarkets and retailers, who are using existing old operation systems and their dilapidated facilities, are losing a competitive edge in business. Recent the social effects of large series of corporate supermarkets for traditional markets has been very controversial, and commercial media, academia, and industry associated with it have been held many seminars and public hearings. This may slow down the speed in accordance with the regulations, but will not be the crucial alternative. The reason for this recent surge of enterprise-class SSM up, one of the reasons is a stagnation in their offline discount mart, so they are finding new growth areas. Already in the form of large supermarkets across the country got most of the geographical centre point and is saturated with stages. Targeting small businesses that do not cover discount Mart, in order to expand business in the form of SSM is urgent. By contrast, private supermarkets are going to lose their competitiveness. The vulnerability of individual supermarkets, one of the vulnerabilities is price which economies of scale can not be realized so they are purchasing a small amount of products and difficult to get a quantity discount. The lack of organization and collaboration, and education which is not practical, caused the absencer of service-oriented situations. As a first solution, making specialty shops which are handling agricultures, fruits and vegetables and manufactured goods is recommended. Second, private supermarkets franchisees join the organization for the organization and collaboration is recomaned. It can be meet the scale of economy and can be formed a alternative business formats to a government. Third, the education is needed as a good service will get consumer's awareness. In addition, a psychological stores operating is also one way to stimulate consumer sentiment as SSM can't operate. Japan already has a better conditions of their lives through small chain expression. This study includes the vulnerabilities of private supermarkets, and suggests a competitiveness reinforcement strategies.
