Effects of Systemic and Intrathecal AMPA/KA Receptor Antagonist LY293558 in a Rat Model for Postoperative Pain (절개통증모델에서 복강 및 척수강내로 투여된 AMPA/KA 수용체 길항제 LY293558의 효과)

  • Background: Intraperitoneal (IP) and intrathecal (IT) administration of $\alpha$-amino-3-hydroxy-5-methyl-4-isoxazole-propionic (AMPA) and kainate (KA) receptor antagonist attenuate hyperalgesia in various models of persistent pain. The purpose of this study was to assess the effects of IP and IT LY293558, a novel AMPA/KA receptor antagonist on mechanical hyperalgesia after incision. Methods: Sprague-Dawley rats were anesthetized with halothane and underwent plantar incision. Two hours later, responses to mechanical stimuli were assessed using the response frequency to a nonpunctate mechanical stimulus and withdrawal threshold to calibrated von Frey filaments. One group of rats received vehicle, 5 or 10 mg/kg of LY293558 IP. In the other group, vehicle, 0.2, 0.5 or 2 nmol of LY293558 was administered IT. Ataxia and motor function were also evaluated. Results: Hyperalgesia was persistent in both the vehicle and 5 mg/kg group. IP administration of 10 mg/kg of LY293558 increased withdrawal threshold at 30 and 60 min after incision; deficits in rotorod performance were observed at 30, 60, 90 and 150 min. IT administration of 0.5 nmol of LY293558 increased the median withdrawal threshold at 30 and 60 min. Motor function was only impaired at 30 min. IT administration of 2 nmol produced hemiparesis. Again, inhibition of pain behaviors outlasted the effects on motor function. Conclusions: These data further suggest AMPA/KA receptors are important for the maintenance of pain behaviors caused by incisions. IT administration of LY293558 was more effective than systemic administration and reducing pain behaviors caused by a surgical incision.

Immediate Changes of Pure Tone Audiogram Results Following Stellate Ganglion Block in Sensory Neural Hearing Loss (돌발성난청에서 성상신경절 차단 직후 순음청력치는 즉각적으로 변화되는가?)

  • Background: Vascular occlusive event is one of the etiologies of sudden sensorineural hearing loss (SNHL). Stellate ganglion block (SGB) induces dramatic and intense vasodilatation in head and neck. Based on this principle, SGB has used as one of the treatment modalities in SNHL. This study was performed to evaluate immediate response of SGB on pure tone audiogram (PTA) in SNHL. Methods: Forty patients were studied. Each patient received daily ipsilateral SGB in paratracheal approach using 0.2% bupivacaine for 2 weeks. On first, third, and fifth day of treatment, we checked their PTA twice 1 hour before (Pre-PTA) and after (Post-PTA) SGB. Pre- and Post-PTA were compared. Several factors were analyzed as a prognostic factor of therapeutic results. Results: Eleven of 40 patients revealed decreased PTA after SGB. Degree of decreased PTA were insignificant ($2.5{\pm}1.6$ dB). Initial and final PTA results was $76.2{\pm}22.5$ and $49.8{\pm}28.3$ dB, respectively. Thirty-one of 40 patients were improved their PTA over 10 dB. The recovery was mainly influenced by the severity of initial hearing loss (P<0.001) and slightly by age (P<0.05). However, the change of PTA after SGB, time interval to receive SGB, sex, site, and number of SGB were not correlated to therapeutic outcome. Conclusions: These results suggest that vasodilatation by SGB has no immediate improvement in SNHL. Therefore, we question whether SGB is beneficial to all patients with SNHL as a therapeutic modality.

Effects of SosokMyeongTang extract on the Regional Cerebral Blood Flow and Blood Pressure in Rats (소속명탕(小續命湯)이 백서(白鼠)의 국소뇌혈류량(局所腦血流量)과 혈압(血壓)에 미치는 영향(影響))

  • SosokMyeongTang(SMT) have been used in oriental medicine for many centuries as a therapeutic agent for cerebral disease. The effects of SMT on the vascular system is not known. The purpose of this study was to investigate the effects of SMT on the changes in blood pressure(BP) and regional cerebral blood flow(rCBF) of rats. SMT consists of the following components : Radix Ledebouriellae(防風), Radix Cocculi or Stephaniae(防己), Semen Armeniacae(杏仁), Cortex Cinnamomi(肉桂), Radix Scutellariae(黃芩), Radix Paeoniae Lactiflorae(白芍藥), Radix Ginseng(人蔘), Rhizoma Cnidll(川芎), Herba Ephedrae(麻黃), Radix Glycyrrhizae(甘草), Radix Aconiti(附子?), Fructus Zizyphi Jujubae(大棗), Rhizoma Zingiberis(生薑) and the changes of BP and rCBF were tested Leser-Doppler Flowmetry(LDF) The experimental results were as follows ; BP was not affected by SMT in rats, but rCBF was increased significantly by SMT in a dose dependent manner. SMT increased previous decreasd rCBF due to pretreatment methylene blue, but did not increased previous decreasd rCBF due to pretreatment L-NNA, indomethacin. Pretreatment with indomethacin decreased BP compaired with control group. These results suggest that SMT causes a diverse response of blood pressure and regional cerebral blood flow(rCBF). The increased rCBF is mediated by nitric oxide synthease.

  • Objectives : The progression of renal disease can be identified as a glomerulosclerosis by histological examination, and the basic mechanism of glomerulosclerosis is mesangial cell proliferation and mesangial matrix accumulation. ICAM-1, ${\beta}1-integrin$ and MHC-class II are known to attribute to the progression of glomerulosclerosis. They mediate cell-cell or cell-matrix interactions and are expressed in response to injury and inflammation. Up to now, there have been few satisfactory regimens to treat glomerular diseases except minimal change nephrotic syndrome, which can be improved by steroid therapy. Studies were performed in order to investigate whether Jinmu-tang has suppressive effects on some factors associated with the progression of glomerular disease, mesangial cell proliferation, fibronectin synthesis, ICAM-1, ${\beta}1-integrin$ and MHC-class II expression. Methods : Studies were performed with the method of surface enzyme immunoassays or flow cytometry after addition of peripheral blood mononuclear cells(PBMC) supernatants treated with Jinmu-tang, using the cultured human mesangial cells. Results : 1. The suppressive effect of Jinmu-tang on mesangial cell proliferation was higher than that of hydrocortisone. 2. Jinmu-tang has some suppressive effects on fibronectin synthesis, ICAM-1, expression, ${\beta}1-integrin$ expression and MHC-class II expression of mesangial cells, but was lower than hydrocortisone. Conclusions : Jinmu-tang generally shows some immunosuppressive effects. We carefully suggest that the above prescription may be applied to prevent the progression of renal disease or can be used as an adjuvant of or a substitute for steroid therapy.

Treatment of Intractable Cancer Pain by Stereotactic Bilateral Anterior Cingulotomy (난치성 암성 통증 제어를 위한 뇌정위적 대상회 절개술)

  • Objective : Although cingulotomy has been applied to patients with affective disorders more frequently, there are numerous reports of its use for the control of severe pain. The goal of this study was to investigate the role of stereotactic bilateral anterior cingulotomy for intractable cancer pain. Method : Between January and June, 2000, we underwent stereotactic bilateral anterior cingulotomy in 6 patients for intractable cancer pain with poor response to opioids. The patients were suffering from widespread musculoskeletal or visceral pain. We made four lesions along the two tracks on either side of the cingulate cortex. Result : In all patients, pain reliefs after cingulotomy were dramatic and immediate. Five out of six patients did not require any opioids and one patient could reduce dose of opioids. There were no deaths or serious complications related to the procedure. Conclusion : These results suggested that a bilateral anterior cingulotomy might be useful method to control intractable cancer pain associated with the widespread metastatic disease. To provide rationale of bilateral anterior cingulotomy in intractable cancer pain, the theoretical mechanisms and role of bilateral anterior cingulotomy are discussed, along with our surgical techniques and the course of our patients.

Identification of M-1, S-1 Cortex Using Combined Intraoperative SEP and Cortical Stimulation - A Case Report - (수술중 체성감각 유발전위 및 대뇌피질 자극을 이용한 일차 운동피질영역과 일차 감각피질영역의 확인 - 증례보고 -)

  • In the removal of small subcortical lesion in the eloquent area like sensory-motor cortex, the prevention of neurologic deficit is important. We present our technique of identification of M-1, S-1 cortex in a case of subcortical granuloma located in sensorymotor cortex. To accurately localize mass, stereotactic craniotomy was planned. At the beginning of procedure, functional MRI of motor cortex was done with stereotactic headframe in place. Next, the stereotactic craniotomy about 4 cm was done under propofol anesthesia for cortical mapping. After reflection of dura, central sulcus was identified with phase-reversal response of intraoperative SEP(somatosensory evoked potential) of contralateral median nerve. Then the patient was awakened, and direct cortical stimulation was done. We observed the muscle contractions of elbow, hand and fingers and the paresthesia over forearm, hand, fingers on the M-1 and S-1 cortex. Through cortical mapping and stereotactic guidance, we concluded that the mass lie immediately posterior to central sulcus, then the mass was carefully removed through small transsulcal approach, opening about 1 cm of rolandic sulcus.

A Study of Environmental Characteristics of a Group Room and Teacher's Recognition of Child Care Centers (보육시설의 보육실 환경 특성 및 교사인식에 관한 연구)

  • The purpose of this study is to provide basic data that is necessary for planning an adequate environment for various child-care activities and programs by recognizing the environmental characteristics of a group room, where children's activities take place. A survey was conducted of 257 childcare centers and 544 teachers all over the country, including only those of which that have opened after year 2000 with more than 20 children capacities. The survey period was from January 18th to 28th of 2010. The data was analyzed using SPSS WIN 15 software, calculating the data's frequency, percentage, $x^2$, multiple response analysis, F-test, and factor analysis. The results of this study led us to the following conclusions. 1) We would like to propose a group room space design varying by the room size, so that interesting and diverse activities can take place after taking account of the child's development and childcare centers program. 2) Group room space and facilities should be made of proper finishing materials, various materials with differing textures, and lighting for the kinds of childcare activities that gives various sensational experiences and comfortable feelings to the children. 3) Teachers with higher age and education tend to believe that child's competence and a homelike atmosphere are important factors in a group room environment.

Performance Analysis of Sequential and Parallel Interworking Model for Connection Control of VB5.2 Interface (VB5.2 인터페이스의 연결 제어를 위한 순차형 및 병렬형 연동 방식의 성능 분석)

  • A broadband access network in B- ISDN subscriber network, plays a role of concentration or multiplexing of various subscribers into a service node. To establish an end-to-end ATM connection, interworking functions between \'B5.2 interface's connection protocol and the signalling protocol, are perfonned at the service node. A sequential interworking model and a parallel interworking model were proposed for these interworking functions. The sequential interworking model proceeds the signalling protocol of network node after the response of VB5.2 interface protocol. On the other hand, the parallel interworking model proceeds the signalling protocol and the VB5.2 interface protocol simultaneously. In this paper, we analyze and simulate above two interworking models in tenns of a connection setup delay and completion ratio, when call requests with eBR and VBR transfer capability arrive at the service node. We observed that the completion ratios of the two interworking models are quite closed. However the connection setup delay of parallel interworking model is improved by about 33% compared with that of the sequential interworking model.

  • This paper intends to improve the problems of the slack stealing algorithm scheduling for periodic tasks with fixed priority and aperiodic tasks which occur dynamically. The Slack stealing algorithm reduces unnecessary waiting time by making the service possible immediately when the slack stealing server gives suitable priority to aperiodic tasks according to the status of aperiodic tasks arrivals at runtime. But no performs the slack stealing, we must calculate execution time of periodic tasks till the point of random. And, execution time of periodic tasks is being repeatedly every hours while the slack algorithm is applied. We show time complexity hen is used as to O(n) if the nubmer of tasks which is applied to the calculation is n. In this appear, due to stored in tables slack times and the execution times of the scheduled periodic tasks, the complexity of aperiodic tasks which is occurring dynamically reduced to O(log n) and improves the responses times. We prove the algorithm proposed in this paper through the simulation.

  • The development of molecular biology has brought many changes in psychiatry. Molecular biology makes us possible to know the cause of mental disorders that provide the way to prevent the disorders, and to develop various accurate diagnostic and treatment methods for mental disorders. The author discusses the concept, cause, and treatment of mental disorders in the aspect of molecular biology. Importing the methods of molecular biology into psychiatry, we can anticipate to get a number of the goals of psychiatric genetics, including identification of specific susceptibility genes, clarification of the pathophysiological processes whereby these genes lead to symptoms, establishment of epigenetic factors that interact with these genes to produce disease, validation of nosological boundaries that more closely reflect the actions of these genes, and development of effective preventive and therapeutic interventions based on genetic counseling, gene therapy, and modification of permissive or protective environmental influences. In addition to their capacity to accelerate the discovery of new molecules participating in the nervous system's response to disease or to self-administered drugs, molecular biological strategies can also be used to determine how critical a particular gene product may be in mediating a cellular event with behavioral importance. Molecular biology probably enables us discover the environmental factors of mental disorders and allow rational drug design and gene therapies for mental disorders, by isolation of gene products that facilitate a basic understanding of the pathogenesis of these disorders. A specific genetic linkage may suggest a novel class of drugs that has not yet been tried. With respect to gene therapy, the hypothetical method would use a gene delivery system, most likely a modified virus, to insert a functional copy of a mutant gene into those brain cells that require the gene for normal function.
