Clinical Analysis of Arterial Bypass on the Atherosclerotic Occlusive Disease in Lower Extremities (폐쇄성 하지동맥 경화증에서 동맥 우회술의 임상적 고찰)

  • From Dec. 1992 to Dec. 1995, 20 patients with atherosclerotic occlusive disease in the lower extremities underwent arter al bypass surgery. The age of the patients ranged between 46 and 77 years(mean .60.8 years) and the most prevalent incidence was in the 7th decades. The mean follow up period was 18.8 months ranging from 4 to 36 months. Associated diseases were diabetus mellitus(25%), hypertension (35%), cerebrovascular accident(25%), and acute myocardial infarction(5%). Disabling intermittent claudifcation(35%), resting pain(20%) or ischemic gangrene(or ulceration)(45%) were operative indications. Aorto-bifemoral bypass in 4 cases, aorto-single femoral bypass in 2 cases, Aorto-bifemoral bypass and femoropopliteal bypass in 3 cases, aorto-single femoral bypass and femoropopliteal bypass in 1 case, femoropopliteal bypass in 4 cases, femorotibial bypass in 1 case, popliteotibial bypass 4 cases and femorofemoral bypass in 1 case were the surgical approches. Early thrombosis(2 cases) and wound infection(2 case) were main complication. Postoperative complication rate was 20% . Postoperative patency rates re 92.6% at 6 months, 84.2% at 1 year, 75.4% at 2 years and 69.4% at 3 years.

The Promoting Effect of Pleuropterus cilinervis Extracts Fermented with Lactobacillus rhamnosus on Hair Growth (백하수오 발효액의 모발성장 효과)

  • The bacterial growth and pH of Pleuropterus cilinervis extracts and animal milk, fermented with Lactobacillus rhamnosus during the fermentation process, were evaluated. The results indicated that the bacterial count after fermentation always remained higher than 6 log CFU/mL, with a constant pH of approximately 4. In order to evaluate the effects of Pleuropterus cilinervis extracts and animal milk, fermented with Lactobacillus rhamnosus, on hair growth promotion, C57BL/6 mice were chosen as experimental subjects. Six week old males with similar body weights were divided into four groups; a normal group (saline), a negative control group (essence base), a positive control group and an experimental group (Pleuropterus cilinervis extracts and animal milk, fermented with Lactobacillus rhamnosus, mixed with negative control). The substances and test materials were applied topically on the back skin of the mice for 8 days. Other external conditions and variables such as food intake were kept at the same as the four groups. At the end of the experiment it was noted that hair re-growth in the experimental group, using gross and histological examinations, was higher than that in the positive control group. This study therefore provides an empirical evidence that Pleuropterus cilinervis extracts and animal milk fermented with Lactobacillus rhamnosus promotes hair growth, and suggests that applications could be found in hair loss treatments.

Fishing Experiment for Development of Unused Fishery Resources on the Deep Sea Bed of Korean East Sea (동해구 심해 미이용 자원의 어획 시험 연구)

  • In accordance with a rapid growth of demani on aquatic animals, researches of the unused fishery resources On the deep sea b~d in the Korean Waters has been and will be required. The authors carried out a series of fishing experiments to investigate the available resources and to find the effective fishing method on the deep sea bed of the Korean East Sea. In the experiments, 19 kinds of traps which are different from each other in shape, mesh size and entrance diameter were used. The fishing experiments w~r;; carried out in four areas of 20Dm, 600.'11, 800m and 1000.'11 deep respectively, by the Pusan 402 (30:) GT) and the Pusan 403 (279GT), the training ships of National Fisheries University of Pusan, during August, 1986. The catc~ were analyzed with the size, the depth and the construction of traps. The results obtained can be summarized as follows: 1. Main species of the catch w~re pink shrimp, Pandalus bolelis, a kind of welks, Buccimum striatissimum and a kind of larg~ crabs. Chiono8cetes japonicus and the another species were few. 2. The CPUE value (expressed by the number of catch per trap in this paper) of pink shrimp was the highest in the depth of 20J-n around, and the value in the depth of 600.'11 or more decreased gradually with an increase of the depth. But, the value of Buccimum straitissimum was much higher in the depth of 6:J:)!1I or more than that in the depth of 200m around. On the other hand, the value of Chion:Jecetes japonjcus was very low in general. 3. The iniividual body size of the catch differed with the depth. Pink shrimps caught in the depth of 200m around were smaller than those in the depth of 600.'11 or more. In contrast with this, Baccimum striatissim:t.m caught in the depth of 200m around were larger than those in the depth of 600.'11 or more. 4. Depending on the selection curve in Ishida's method for the mesh size of trap webbing, the carapace length of pink shrimp and the shell length of Buccimum striatissimum which are equivalent to 100% relative catching efficiency can be estimated about 3.5cm and 6.5cm or so respectively. 5. The number of catch of pink shrimp and Buccimum striatissimum by the 60.'1'1.'11 entrance diameter of trap were less than that by the 90mm, 120mm and 150mm, even thogh the diffierence am~r, g 9:Jmm, 120.'11.'11 and 150:1'.'11 are not so large.

A Study on the Classification of 500m×500m Mesh Level by the Combinations of Building Needs in Busan for the Feasibility Evaluation of Ocean Energy Plant Introduction (해양에너지 활용지역 선정을 위한 부산시 500m 메시 레벨에서의 건물용도구성에 의한 유형화 연구)

  • On the view point of renewable energies as energy sources of district heating and cooling plant, the purpose of this study is to develop, classify and map the 500m${\times}$500m mesh, of which is treated as normal size in DHC regulations for evaluation process. Followings are the results. Various building and geographical informations including 13 districts and 108 counties are re-defined to create 500m${\times}$500m meshes, and it is find out that 3,289 meshes among 8,463 meshes have meaningful floor areas. Only 59 meshes(1.8%) are evaluated as mesh which has more than 50% of building volume ratio per mesh. 5 clusters classified by principal analysis and cluster analysis with building needs' characteristics are defined. Gwang-an Dong is representative of cluster 1 characterized as commercial area, and the cluster 4, 5 which has mainly residential needs are distributed in Yong-ho dong. Because there are a lot of cluster 3 meshes, which has complex needs area based on residential, cluster 3 could be defined as representative of Busan metropolitan city.

Metabolomes and transcriptomes revealed the saponin distribution in root tissues of Panax quinquefolius and Panax notoginseng

  • Background: Panax quinquefolius and Panax notoginseng are widely used and well known for their pharmacological effects. As main pharmacological components, saponins have different distribution patterns in the root tissues of Panax plants. Methods: In this study, the representative ginsenosides were detected and quantified by desorption electrospray ionization mass spectrometry and high-performance liquid chromatography analysis to demonstrate saponin distribution in the root tissues of P. quinquefolius and P. notoginseng, and saponin metabolite profiles were analyzed by metabolomes to obtain the biomarkers of different root tissues. Finally, the transcriptome analysis was performed to demonstrate the molecular mechanisms of saponin distribution by gene profiles. Results: There was saponin distribution in the root tissues differed between P. quinquefolius and P. notoginseng. Eight-eight and 24 potential biomarkers were detected by metabolome analysis, and a total of 340 and 122 transcripts involved in saponin synthesis that were positively correlated with the saponin contents (R > 0.6, P < 0.05) in the root tissues of P. quinquefolius and P. notoginseng, respectively. Among them, GDPS1, CYP51, CYP64, and UGT11 were significantly correlated with the contents of Rg1, Re, Rc, Rb2, and Rd in P. quinquefolius. UGT255 was markedly related to the content of R1; CYP74, CYP89, CYP100, CYP103, CYP109, and UGT190 were markedly correlated with the Rd content in P. notoginseng.

The Effectiveness of Accounting Information in Military Construction Contracts under the Qualification Assessment System (적격심사낙찰제 계약에서 회계정보의 유용성에 관한 연구)

  • This paper re-evaluated the research on the accounting information's effectiveness in military construction contracts, questioning the former research results on the central army's construction contract that was under the Qualification Assessment System, especially concerning the relations between the accounting information and the bidding ratios as well as the relations between the bidding rates and the defect rates, which targeted between 1 to 10 billions civil engineering, construction, electricity, telecommunication, and environment work types. Unlike the previous results, the relationship between the accounting information and the bidding rates was shown to be considerably limited that the bidding ratio could not be identified through the accounting information, while it could discern the relations with the defect rates. Furthermore, the proven results did not support the hypothesis that the differences in bidding rates could affect the defect ratio. However, through the prediction model of bidding and defect ratios, we could identity the accounting variables that influenced the ratios. Additionally through the results regarding the non-financial indexes in the Pre-Qualification items, the weight on these indexes could be adjusted. In conclusion, the research results has given us new understanding of the problems in the Qualification Assessment System which accounts for the majority of the current military construction contracts and provide validity on the government's expanded implementation of the Lowest Price Award System.

Efficacy and Safety of Herb Medication According to Cold-heat Tendency of Knee Osteoarthritis Patients (퇴행성 슬관절염 환자의 한열 성향에 따른 한약 제제의 유효성과 안전성 비교)

  • Objectives : To demonstrate the importance of syndrome differentiation in clinical research of herb medication, through the comparative study on efficacy and safety of herb medication according to cold-heat tendency of OA knee patients. Methods : During December 2010 to July 2011, 138 knee OA patients were randomly assigned to WIN-34B 600mg(300mg, b.i.d.), 1,200mg(600mg, b.i.d.) and placebo b.i.d. for 8 weeks. Patient were re-classified into cold-heat tendency group according to cold-heat questionnaires. To investigate efficacy and safety, we assessed the 100mm pain VAS at baseline and 8 weeks later, and we monitored adverse event of patients during treatment period. Results : 1. Efficacy study : In WIN-34B 1,200mg group, VAS mean changes of heat tendency group showed slightly increase than those of cold tendency, but no significant difference within two groups. In heat tendency group, WIN-34B 1,200mg group showed a significant decrease of VAS compared to placebo group. but there were no significant difference in cold tendency group. 2. Safety study; In WIN-34B 600mg group, incidence of adverse events of cold tendency group was higher than those of heat tendency, but not in WIN-34B 1,200mg group. Conclusions : This study suggests that WIN-34B tend to have more efficacy in heat tendency-knee OA patients and WIN-34B is safe drug relatively, regardless of cold-heat tendency. In further clinical research on efficacy and safety of WIN-34B, stratification using syndrome differentiation is required.

The Effects of the Q-Ray View on Reliability of Assessing a Tooth Status for Dental Hygiene Process (Q-Ray View 활용이 치위생과정을 위한 치아 검사의 신뢰도 향상에 미치는 영향)

  • The objective of this study was to evaluate the impact of the Q-ray view, a novel optical device on reliability of assessing a tooth status by dental hygiene students in the training for dental hygiene process. Twenty patients were enrolled in this study. Oral examinations were conducted by both seventeen third-year dental hygiene students and a trained faculty member. Traditional visual inspection was performed in phase I and then re-examined with Q-ray view in phase II. Restoration codes and lesion codes for each tooth were recorded separately according to the predefined criteria. As a measure of reliability, percent agreement and Cohen's kappa were determined. Agreements for each intraoral regions and types of lesion and restoration were calculated. Paired t-test and Pearson chi-square test for two proportions were used to compare mean Cohen's kappa and percent agreement at each phase. For the lesion code, mean kappa values of phase II for intraoral regions were significantly greater than that of phase I (p=0.017). For the both of the lesion code and restoration code, percent agreements of phase II for each types of lesion and restoration were significantly greater than that of phase II (p<0.001 and p<0.001, respectively). Especially difference of percent agreements between phase I and II for incipient caries, caries and fracture were significant for the lesion code (p=0.046, p<0.001, and p=0.029, respectively) and for not restored or sealed, tooth-colored restoration were significant for the restoration code (p<0.001 and p=0.011, respectively). The reliability of assessing a tooth status was improved when the Q-ray view used in dental hygiene student with beginner level of expertise. Q-ray view can be a promising device for conducting and educating the dental hygiene process better.

Needs of Home Care Nursing Services for Hospital Patients (재원환자의 질환군에 따른 가정간호 요구도)

  • Purpose: The purpose of this study was to investigate the needs of home care nursing services in relation to the patients in hospital. Method: Subjects were 129 patients who admitted hospital and were selected through convenience sampling. Data were collected using the Home Health Care Need Assessment Questionnaire constructed by the Korea Health Industry Development Institute. Data were analyzed using SAS 8.12 program by applying percentage. Results: 1. According to the diagnosis of the subjects, the majority had cancer (25.0%), followed by musculoskeletal disease (15,6%), neuro/cerebral vascular disease (14.1%), digestive (10.9%) and respiratory disease (10.9%). 2. With regard to fundamental nursing service, subjects wanted to receive home care services for the following reasons: Problem identification and diagnosis (77.5%), vital sign check (49.6%); and intake and output measure (20.9%). 3. With regard to clinical laboratory tests, 62.8% wanted to receive blood tests, and followed by urine tests 26.4%, and wound drainage 26.4%. 4. With regard to medication and treatment service, 40.3% of the subjects wanted to receive intravenous fluid therapy, 26.4% intravenous antibiotics, and 26.4% the monitoring of fluid therapy. 5. With regard to therapeutic nursing service, 33.3% wanted to receive wound care, 26.4% ROM exercise, and 27.9% foley catheter change and care. 6. With regard to educational needs, 42.6% wanted education on infection monitoring, 41.4% on medication, and 34.9% on diet. 7. With regard to counseling needs, 65.9% wanted to receive telephone counseling about patient condition, 52.7% counseling about re-admission and 51.9% direct counseling about patient condition. In the group of injury and toxicity, and cardiovascular/circulatory diseases, 100% wanted telephone and direct counseling about the patient condition. Conclusion: Therefore, in order to improve the quality of hospital based home health care services, various factors that affect to the need of home health care should be analyzed and specified nursing care should be looked into.

  • The purpose of this study is to analyze the factors affecting the school health teachers' self-confidence. This study was conducted with 87 school health teachers working in Jeonbuk province, from September to December, 1986, The results are follows; 1. Demographic characteristics of school health teachers 1) Age mean ; 34, range; 23-54 2) School Nursing experience mean; 11 years, range; 0-24 3) Clinical experience mean; 1.5 years, range: 0-13 2. Status of school health resources & nursing activities 1) Personnel resource school health teacher: pupils : 1 : 1,436 'classes=1:31 'general teacher=1:39 2) Budget Total school operating budget: School health budget : 100 : 4.2 (52.2 thousand Won) Half of the school health budget expend on medicine. 3) Clinic 80% of all schools have health clinic seperately. 71.32 of all schools have less than $35m^2$, 23.9%, $36-66m^2$. 4) Only 20% of all schools have organization for health 5) Average of clinic visitor for 1 year; 2,084 Major problem is on digestive system. And other problem: respiratory, skin, musculo - skeletal system, dental problem, etc... 6) Literal message for 1 year; 12 times.
