Preliminary study of presumptive intradural-intramedullary intervertebral disc extrusion in 20 dogs

  • Background: Intradural-intramedullary intervertebral disc extrusion (IIVDE) is a rare condition of intervertebral disc disease. However, the diagnosis of IIVDE is challenging because the prognosis and imaging characteristics are poorly characterized. Objectives: We aimed to describe the clinical and imaging characteristics of tentatively diagnosed IIVDE in dogs to assess the prognostic utility of neurological grade and magnetic resonance imaging (MRI) findings. Methods: Twenty dogs were included in this retrospective cohort study. Results: Nonchondrodystrophic breeds (n = 16) were more predisposed than chondrodystrophic breeds. Most dogs showed acute onset of clinical signs. Neurological examination at admission showed predominant non-ambulatory paraparesis (n = 9); paresis (n = 16) was confirmed more frequently than paralysis (n = 4). Follow-up neurological examination results were only available for 11 dogs, ten of whom showed neurological improvement and 8 showed successful outcomes at 1 month. The characteristic MRI findings include thoracic vertebra (T)2 hyperintense, T1 hypointense, intramedullary linear tracts with reduced disc volume, and cleft of the annulus fibrosus. None of the MRI measurements were significantly correlated with neurological grade at admission. Neurological grade did not differ according to the presence of parenchymal hemorrhage, parenchymal contrast enhancement, and meningeal contrast enhancement. Neurological grades at admission showed a statistical correlation with those observed at the 1-month follow-up (r = 0.814, p = 0.02). Conclusions: IIVDE is a rare form of disc extrusion commonly experienced after physical activity or trauma and most frequently affects the cranial-cervical and thoracolumbar regions of nonchondrodystrophic dog breeds. Neurological score at admission emerged as a more useful prognostic indicator than MRI findings in dogs with suspected IIVDE.

Estimation of $T_2{^*}$ Relaxation Times for the Glandular Tissue and Fat of Breast at 3T MRI System (3테슬러 자기공명영상기기에서 유방의 유선조직과 지방조직의 $T_2{^*}$이완시간 측정)

  • Purpose : $T_2{^*}$ relaxation time which includes susceptibility information represents unique feature of tissue. The objective of this study was to investigate $T_2{^*}$ relaxation times of the normal glandular tissue and fat of breast using a 3T MRI system. Materials and Methods: Seven-echo MR Images were acquired from 52 female subjects (age $49{\pm}12 $years; range, 25 to 75) using a three-dimensional (3D) gradient-echo sequence. Echo times were between 2.28 ms to 25.72 ms in 3.91 ms steps. Voxel-based $T_2{^*}$ relaxation times and $R_2{^*}$ relaxation rate maps were calculated by using the linear curve fitting for each subject. The 3D regions-of-interest (ROI) of the normal glandular tissue and fat were drawn on the longest echo-time image to obtain $T_2{^*}$ and $R_2{^*}$ values. Mean values of those parameters were calculated over all subjects. Results: The 3D ROI sizes were $4818{\pm}4679$ voxels and $1455{\pm}785$ voxels for the normal glandular tissue and fat, respectively. The mean $T_2{^*}$ values were $22.40{\pm}5.61ms$ and $36.36{\pm}8.77ms$ for normal glandular tissue and fat, respectively. The mean $R_2{^*}$ values were $0.0524{\pm}0.0134/ms$ and $0.0297{\pm}0.0069/ms$ for the normal glandular tissue and fat, respectively. Conclusion: $T_2{^*}$ and $R_2{^*}$ values were measured from human breast tissues. $T_2{^*}$ of the normal glandular tissue was shorter than that of fat. Measurement of $T_2{^*}$ relaxation time could be important to understand susceptibility effects in the breast cancer and the normal tissue.

The Correlation between Cross-sectional Area of Lumbar Paraspinal Muscles and Sponylolisthesis; A Retrospective Study (요추 주변 근육 단면적과 척추전방전위증의 상관성에 대한 후향적 연구)

  • Objectives To investigate correlation between slip percentage (SP) of spondylolisthesis and cross-sectional area (CSA) of lumbar paraspinal muscles; psoas major (PM), multifidus (MU) and erector spinae (ES). Methods A retrospective study was carried out in 120 spondylolisthesis patients who had visited the Spine center of Kyung Hee University Hospital at Gangdong and had taken lumbar MRI. CSA of lumbar paraspinal muscles was measured from axial T2-weighted MRI and divided by CSA of vertebral body to avoid weight's influence. SP was also measured from sagittal MRI. Results SP increase has significant correlation with decreased CSA-MU (r=0.37, p<0.01) and increased CSA-ES (r=0.19, p<0.05). There was no significant correlation between SP and CSA-PM. Conclusions MU atrophy and ES hypertrophy have significant correlation with SP of spondylolisthesis. CSA of lumbar paraspinal muscles can be a risk factor of progression of spondylolisthesis and compensation for the instability.

Magnetic resonance imaging texture analysis for the evaluation of viable ovarian tissue in patients with ovarian endometriosis: a retrospective case-control study

  • Background: Texture analysis has been used as a method for quantifying image properties based on textural features. The aim of the present study was to evaluate the usefulness of magnetic resonance imaging (MRI) texture analysis for the evaluation of viable ovarian tissue on the perfusion map of ovarian endometriosis. Methods: To generate a normalized perfusion map, subtracted T1-weighted imaging (T1WI), T1WI and contrast-enhanced T1W1 with sequences were performed using the same parameters in 25 patients with surgically confirmed ovarian endometriosis. Integrated density is defined as the sum of the values of the pixels in the image or selection. We investigated the parameters for texture analysis in ovarian endometriosis, including angular second moment (ASM), contrast, correlation, inverse difference moment (IDM), and entropy, which is equivalent to the product of area and mean gray value. Results: The perfusion ratio and integrated density of normal ovary were 0.52±0.05 and 238.72±136.21, respectively. Compared with the normal ovary, the affected ovary showed significant differences in total size (p<0.001), fractional area ratio (p<0.001), and perfusion ratio (p=0.010) but no significant differences in perfused tissue area (p=0.158) and integrated density (p=0.112). In comparison of parameters for texture analysis between the ovary with endometriosis and the contralateral normal ovary, ASM (p=0.004), contrast (p=0.002), IDM (p<0.001), and entropy (p=0.028) showed significant differences. A linear regression analysis revealed that fractional area had significant correlations with ASM (r2=0.211), IDM (r2=0.332), and entropy (r2=0.289). Conclusion: MRI texture analysis could be useful for the evaluation of viable ovarian tissues in patients with ovarian endometriosis.

Role of Chemical Exchange Saturation Transfer and Magnetization Transfer MRI in Detecting Metabolic and Structural Changes of Renal Fibrosis in an Animal Model at 3T

  • Objective: To investigate the value of combined chemical exchange saturation transfer (CEST) and conventional magnetization transfer imaging (MT) in detecting metabolic and structural changes of renal fibrosis in rats with unilateral ureteral obstruction (UUO) at 3T MRI. Materials and Methods: Thirty-five Sprague-Dawley rats underwent UUO surgery (n = 25) or sham surgery (n = 10). The obstructed and contralateral kidneys were evaluated on days 1, 3, 5, and 7 after surgery. After CEST and MT examinations, 18F-labeled fluoro-2-deoxyglucose positron emission tomography was performed to quantify glucose metabolism. Fibrosis was measured by histology and western blots. Correlations were compared between asymmetrical magnetization transfer ratio at 1.2 ppm (MTRasym(1.2ppm)) derived from CEST and maximum standard uptake value (SUVmax) and between magnetization transfer ratio (MTR) derived from MT and alpha-smooth muscle actin (α-SMA). Results: On days 3 and 7, MTRasym(1.2ppm) and MTR of UUO renal cortex and medulla were significantly different from those of contralateral kidneys (p < 0.05). On day 7, MTRasym(1.2ppm) and MTR of UUO renal cortex and medulla were significantly different from those of sham-operated kidneys (p < 0.05). The MTRasym(1.2ppm) of UUO renal medulla was fairly negatively correlated with SUVmax (r = -0.350, p = 0.021), whereas MTR of UUO renal medulla was strongly negatively correlated with α-SMA (r = -0.744, p < 0.001). Conclusion: CEST and MT could provide metabolic and structural information for comprehensive assessment of renal fibrosis in UUO rats in 3T MRI and may aid in clinical monitoring of renal fibrosis in patients with chronic kidney disease.

  • A new quadrature RF coil is designed for Breast MR Imaging. Quadrature RF coils for MRI have been useful to improve the signal-to-noise ratio (SNR) by "$\sqrt{2}$" using two orthogonal RF coils in combination. A modified Breast Quadrature coil is designed. It is a modified type of the high-pass birdcage coil. To reduce the field distortion, by using current feeding, the field pattern is optimized to achieve a quadrature circularly-polarized field pattern. The coil has been implemented for receive-only mode, and tested by phantom imaging. The experimental results show the utility of the proposed RF coil.

Correlation Between Vertebral Marrow Fat Fraction Measured Using Dixon Quantitative Chemical Shift MRI and BMD Value on Dual-energy X-ray Absorptiometry (Dixon 정량 화학적 변위 자기공명영상을 이용한 척추 골수 지방함량과 이중에너지 방사선 흡수법의 BMD 값의 비교)

  • Purpose : The purpose of this study was to determine whether there is a significant correlation between vertebral marrow fat fraction measured using Dixon quantitative chemical shift MRI (QCSI) and BMD on dual-energy X-ray absorptiometry (DXA). Materials and Methods: This retrospective study included 68 healthy individuals [mean age, 50.7 years; range, 25-76; male/female (M/F) = 36/32] who underwent DXA of the L-spine and whole body MRI including QCSI of the L-spine and chemical shift MRI of the liver. The enrolled individuals were divided into subgroups according to sex and T-score [i.e., normal bone density (M/F=27/23) and osteopenia (M/F=9/9)]. Vertebral marrow (Dixon QCSI, TR/TE 10.2/4.8 ms) and hepatic fat fractions (chemical shift technique, TR/TE 110/4.9 and 2.2 ms) were calculated on MRI. We evaluated whether there were significant differences in age, body mass index (BMI), vertebral marrow fat fraction, or hepatic fat fraction among the subgroups. Whether or not the participant had reached menopause was also evaluated in females. The correlations among variables (i.e., age, BMI, vertebral marrow and hepatic fat fractions, BMD) were evaluated using Spearman's correlation method. Results: There were no significant differences in age, BMI, or vertebral marrow and hepatic fat fractions between the two male subgroups (normal bone density vs. osteopenia). In female subjects, mean age in the osteopenic subgroup was greater than that in the normal subgroup (p=0.01). Presence of menopause was more common in the osteopenic subgroup [77.8% (7/9)] than the normal subgroup [26.1% (6/23), p<0.05]. The other variables showed no significant difference between female subgroups. The only significant correlation with marrow fat fraction after partial correlation analysis was that with age in the female subjects (r=0.43, p<0.05). Conclusion: The vertebral marrow fat fraction calculated using the Dixon QCSI does not precisely reflect the mild decrease in BMD for either sex.

  • A new type of DTPA-bis-amides (L1-L4) and their Gd(III)-complexes of the type $[Gd(L)(H_2O)]{\cdot}nH_2O$ (5: L1; 6: L2; 7: L3; 8: L4) have been prepared and characterized by analytical and spectroscopic techniques. The X-ray crystal structure of 8 has been determined for structural confirmation. The coordination geometry adopts a tricapped trigonal prism geometry with L4 acting as a chelate octadentate and a water molecule in the coordination sphere. Crystals are monoclinic, $P2_1$, a = 14.468(3), b = 19.235(4), c = 13.527(2) $\AA$ $\beta$ = $107.245(3)^{\circ}$, V = 3595.2(11) $\AA^3$, Z = 4, $D_{calc}$ = 1.570. Significant increases in relaxivities are observed with 6 and 7 as compared with that of $Omniscan^{(R)}$, a commercial MR agent: R1 = 12.46 $mM^{-1}\;s^{-1}$, R2 = 8.76 $mM^{-1}\;s^{-1}$ for 6; R1 = 12.77 nm-1 s-1, R2 = 7.60 mM-1 s-1 for 7; R1 = 4.9 $mM^{-1}\;s^{-1}$, R2 = 4.8 $mm^{-1}\;s^{-1}$ for $Omniscan^{(R)}$. In the case of 5, however, both R1 and R2 are found to be lower to show 2.09 $mM^{-1}\;s^{-1}$, and 1.82 $mM^{-1}\;s^{-1}$, respectively.

Evaluation of MR-SENSE Reconstruction by Filtering Effect and Spatial Resolution of the Sensitivity Map for the Simulation-Based Linear Coil Array (선형적 위상배열 코일구조의 시뮬레이션을 통한 민감도지도의 공간 해상도 및 필터링 변화에 따른 MR-SENSE 영상재구성 평가)

  • Parallel imaging technique can provide several advantages for a multitude of MRI applications. Especially, in SENSE technique, sensitivity maps were always required in order to determine the reconstruction matrix, therefore, a number of difference approaches using sensitivity information from coils have been demonstrated to improve of image quality. Moreover, many filtering methods were proposed such as adaptive matched filter and nonlinear diffusion technique to optimize the suppression of background noise and to improve of image quality. In this study, we performed SENSE reconstruction using computer simulations to confirm the most suitable method for the feasibility of filtering effect and according to changing order of polynomial fit that were applied on variation of spatial resolution of sensitivity map. The image was obtained at 0.32T(Magfinder II, Genpia, Korea) MRI system using spin-echo pulse sequence(TR/TE = 500/20 ms, FOV = 300 mm, matrix = $128{\times}128$, thickness = 8 mm). For the simulation, obtained image was multiplied with four linear-array coil sensitivities which were formed of 2D-gaussian distribution and the image was complex white gaussian noise was added. Image processing was separated to apply two methods which were polynomial fitting and filtering according to spatial resolution of sensitivity map and each coil image was subsampled corresponding to reduction factor(r-factor) of 2 and 4. The results were compared to mean value of geomety factor(g-factor) and artifact power(AP) according to r-factor 2 and 4. Our results were represented while changing of spatial resolution of sensitivity map and r-factor, polynomial fit methods were represented the better results compared with general filtering methods. Although our result had limitation of computer simulation study instead of applying to experiment and coil geometric array such as linear, our method may be useful for determination of optimal sensitivity map in a linear coil array.

Evaluation on Protrusion of the Imaginary Prostate Volume Using Three-Dimensional Volume Rendering (3차원 볼륨 렌더링을 이용한 가상 돌출형 전립선 부피 평가)

  • This study is to compare the accuracy of evaluation regarding the volume of the prostate, which three-dimensional volume rendering was produced the shape of protrusion, by measuring two kinds of craniocaudal length from the top of the protrusion and from the exclusion of the protrusion as the starting points. For the imaginary protrusion prostate models, total of 10 models were roughly made by using devils-tongue jelly and changing each of the 10 ml of capacity from 10 ml to 100 ml. For the protrusion prostate models aimed at estimating the real volume, through 64 cannel computed tomography (CT) and 3.0 tesla magnetic resonance image (MRI) were conducted by planimetry technique from three-dimensional volume rendering. And then we performed to evaluate on significance of these volumes by wilcoxon signed rank test. Also the obtained volumes data by ellipsoid volume formula were measured the volume of protrusion prostate models two times with each method using the two kinds of craniocaudal length from top of the protrusion and from exclusion of the protrusion as the starting points. Finally, the significance of differences using wilcoxon signed rank test was evaluated between the real volume by planimetry technique and the measured volume by ellipsoid volume formula from three-dimensional volume rendering. The average of the protrusion length on the models was $0.90{\pm}0.18\;mm$ in CT and was $0.75{\pm}0.11\;mm$ in MRI. There were not statistically significant difference between MRI and CT from the volume of protrusion prostate models (p=0.414). In MRI (p=0.139) and CT (p=0.057), there were not statistically significant difference between the real volume by planimetry technique and the measured volume by ellipsoid volume from exclusion of the protrusion as the starting points. While, there were statistically significant difference between the real volume by planimetry technique and the measured volume by ellipsoid volume from top of the protrusion as the starting points in MRI (p=0.005) and CT (p=0.005). For the accurate measurement of the protrusion prostate models, the craniocaudal length of the prostate should be measured from the exclusion of the protrusion as the starting points.
