보육시설 내 여성노인 자원봉사활동 도입을 위한 탐색적 연구 -보육시설 종사자를 대상으로- (A Study on the Elderly Women's Voluntary Activities for Child Nursing Facilities -Centering around workers of child nursing facilities-)

  • The purpose of this study is to develop a program that allows older women to provide voluntary services for child nursing facilities, making those women have more social participation, effectively their spare time and ultimately improve the quality of their life. Results of the study can be described as follows. First, social requests about the use of older women as volunteers for child nursing services were analyzed to show that most people working at child nursing facilities perceived lack in the number of child nursing teachers. Those people recognized the need of using older women as service volunteers for the facilities. They were very positive about such use. Second, it is recommendable that volunteers of child nursing service should be not too much older women, or below 65. Voluntary activities that those women participate include taking care of younger children, reading interesting stories for children, helping works of child homes, teaching manners and living attitudes, instructing dietary etiquettes and cooking. Voluntary service activities should be made 1 to 3 times a week, 1 to 2 hours a time, considering psychological and physical states of older people. Third, such older volunteers should be educated about 6 areas, 'the operation and management of child homes, 'practices', 'child health and safety', 'the quality of child nursing', 'child development and counseling' and 'voluntary service training'. Out of these six areas, 'child health and safety' and 'voluntary service training' are more required to be instructed. such training needs to focus on activities in which older women can actually engage as volunteers. Fourth, it is advisable that the recruitment and management of such volunteers are led by the city and that the development of training programs for voluntary service activities and education of the volunteers are referred to universities.

간병인의 구강보건지식 및 태도가 노인환자의 구강건강관리행위에 미치는 영향 (Effect of Oral Health Knowledge and Attitude of Caregivers on Oral Health Management of Elderly Inpatients)

  • Objectives: Republic of Korea has entered an aged society, recently. As chronic diseases increase, elderly inpatients has been increasing and they have used caregivers for convenience. Accordingly, this study aims to investigate the oral health knowledge and attitude of caregivers on oral health management of elderly inpatients, which would affect the general health and quality of life. Methods: A survey questionnaire was distributed to the caregiver(n=165), at a neurosurgery or orthopedic hospital inpatient ward in Seoul, Gyeonggi-do from April 1 to 14, 2017. We analyzed the data with descriptive statistics analysis, chi-squared analysis and logistic regression analysis by using SPSS 23.0. Results: The response rate to the questionnaire was 92%. Oral health management provided by caregivers to inpatients in the elderly was prevention of oral diseases(92.8%), tooth brushing (89.5%), use of oral care products(68.4%), denture cleaning(90.8%) and oral cleaning(90.8%). Frequency analysis of oral health management in accordance with the educational needs of caregivers was all confirmed statistically significant(p<0.05). In the logistics regression model, oral health knowledge was significantly associated with low oral cleaning(OR=1.58) and oral health attitude was significantly associated with high denture cleaning(OR=1.29) and oral cleaning(OR=1.28) after adjustment for other covariates including gender, age, certification, education level, ward, working years. Conclusions: It is necessary to expand the scope and improve the quality of education that can change the attitudes and behaviors of the caregivers on the oral health management of the elderly on the basis of the current oral health education.

영국의 시간제 근로: 기혼 여성의 일에서 보편적 유연근로로의 변화? (Part-time Work in the UK: From Married Women's Work to Universal Flexible Work?)

  • 이 글은 영국의 시간제 근로의 특징과 그 제도적 맥락을 분석하고 있다. 노동시장에 대한 규제와 여성의 노동시장 참여에 대한 지원이 낮은 영국에서는 시장주도의 시간제 활용이 일찍부터 발달했다. 시간제 근로는 영국 취업 여성의 40 퍼센트를 차지할 정도로 특히 기혼 여성들의 주요한 일자리이다. 시장의 역할과 개인의 선택이 강조되는 영국에서 시간제 일자리는 고용주 주도의 시장의 유연성에 기여하는 방식으로, 그리고 여성들이 일과 가족 사이에서 시간제 근로를 통해 여성들 자신의 노동시간을 조정하는 주요 수단으로서 작동했다. 그러나 1997년 이후 영국에서는 제도적 개입을 강화하는 변화가 있었고, 시간제 근로를 기혼 여성을 위한 제한적 일자리로부터 보편적 유연근로로 변화시키려는 법적, 제도적 노력이 있었다. 이러한 변화는 시간제 근로에 대한 불이익은 원칙적으로 시정할 수 있게 되었으나, 여전히 이해당사자들 사이의 집합적 해결방식보다는 개별 근로자와 고용주 사이의 개인화된 해결방식으로 영국이 접근함으로써 여성들의 고용패턴, 그리고 시간제 근로의 질적 성격에 큰 변화는 나타나지 않았다.

노인전문체육지도자의 리더십이 조직몰입 및 직무만족에 미치는 영향 (The Influence of Senior Citizens' Professional Sports Leader's Leadership on Organizational Immersion and Job Satisfaction)

  • 본 연구는 노인전문체육지도자의 리더십이 조직몰입 및 직무만족에 미치는 영향을 알아보기 위하여 2019년 4월 1일부터 4월 30일까지 서울, 경기, 인천 지역 6곳의 스포츠센터 199명의 노인전문체육지도자를 중심으로 연구하였으며, 다음과 같은 결과를 도출하였다. 첫째, 노인전문체육지도자의 리더십은 조직몰입에 유의한 영향을 미치는 것으로 나타났다. 둘째, 노인전문체육지도자의 리더십은 직무만족에 유의한 영향을 미치는 것으로 나타났다. 상기 연구의 결과를 토대로 지도자의 리더십은 구성원들에게 조직에 대한 몰입을 높여주고 직무에 만족감을 높이는 중요한 요인임을 확인 하였다. 따라서 노인들의 생활체육 참여를 향상시키고 즐거움을 높여 건강한 노년기의 삶을 영위하기 위하여 리더십 교육을 통한 지도자의 올바른 인성과 역량의 개발이 필요할 것이며, 이러한 지도자의 역할은 노인들의 삶의 질을 높여줄 것으로 기대된다.

임상간호사의 공감피로, 신체화 증상 및 침묵반응의 관계: 침묵반응에 대한 매개효과를 중심으로 (A Study of the Relationship between Compassion Fatigue, Somatization and Silencing Response among Hospital Nurses: Focusing on the Mediating Effects of Silencing Response)

  • Purpose: The purpose of this study was to identify Compassion Fatigue (CF), Somatization, and Silencing Response (SR) among nurses and understand intermediate effects between the variables. Methods: The sample of 240 nurses who were working three shifts in medical and surgical wards, and emergency room were recruited in three hospitals with over 700 beds. A structured questionnaire was used which included CF, Somatization and SR scales. The data were analyzed using descriptive statistics, ANOVA, Pearson's correlation coefficients and stepwise multiple regression. Results: There were statistically significant differences in CF, Somatization and SR depending on perceived personal health condition, experience of turnover, co-worker support. There were significant correlations between those study variables. The result also indicated that burnout (${\beta}$=.81, p<.001) which is a part of Secondary Traumatic Stress and Somatization (${\beta}$=.79, p<.001) have the role of partial mediator in the relationship between Secondary Traumatic Stress and Silencing response. Conclusion: The results of study show that an intermediary role by Burnout and Somatization in Silencing response of nurses is important for effective human resource management in hospital nursing staffs. Effective human resource management which includes mentoring and social support system can enhance the professional quality of life of nurses, which will eventually contribute to the quality of care by those care providers and counselors.

Association Between Insomnia and Constipation: A Multicenter Three-year Cross-sectional Study Using Shift Workers' Health Check-up Data

  • Background: Although insomnia and constipation are highly prevalent worldwide, studies examining a possible association between them are lacking. We examined the relationship between insomnia and constipation in shift workers who have a high prevalence of insomnia and other diseases. Methods: This study had a multicenter cross-sectional design and conducted using health examination data including self-reported questionnaires. In total, 12,879 and 4,650 shift workers were enrolled in Severance Hospital and Wonju Severance Hospital, respectively, during 2015-2017. Multivariate logistic regression models and subgroup analysis were performed in each center with the same protocol, using a common data model. Results: The mean age of the total population was 44.35 (standard deviation = 8.75); the proportion of males was 56.9%. Female sex, being underweight and non-smoker were strongly associated with an increased risk of constipation symptom (p < 0.001). Pooled odds ratios (ORs) were calculated using ORs of both centers with weights; there was a significant dose-response relationship (sub-threshold 1.76 [95% confidence interval [CI] 1.62-1.91]; moderate 2.28 [95% CI 2.01-2.60]; severe 4.15 [95% CI 3.18-5.41] in the final model, p for trend < 0.001). Subgroup analysis performed by stratifying sex and pooled ORs showed a similar trend to that of the entire group. Conclusion: We observed a strong correlation between insomnia and constipation in this population. Our findings may help in formulating guidelines and policies to improve quality of life in shift workers through the management of sleep quality and proper bowel function. This study is the first to report this relationship among people working in shifts.

작업치료사의 임파워먼트, 소진 및 직무만족도에 관한 연구 (A Study on Occupational Therapists' Empowerment, Burnout, and Job Satisfaction)

  • Purpose : This study investigated the empowerment, burnout, and job satisfaction levels among occupational therapists, and to ascertained the effects of empowerment and burnout on job satisfaction. The study aimed to provide foundational data to help increase job satisfaction among occupational therapists. Methods : We surveyed 180 occupational therapists working in the Jeollabuk-do. The survey included 49 questions covering general characteristics, empowerment, burnout, and job satisfaction. Differences in empowerment, burnout, and job satisfaction levels based on general characteristics were analyzed using independent sample t-tests and one-way ANOVA. We performed multiple regression analyses to ascertain the effects of empowerment and burnout on job satisfaction. Results : The average scores for job satisfaction, burnout, and empowerment were 40.32, 58.40, 41.82, respectively. Job satisfaction according to general characteristics showed statistically significant differences based on the annual income and the most difficult treatment. We found that empowerment factors such as perceived importance of the job confidence in one's abilities, and independence and autonomy in job performance, significantly affected job satisfaction. However, the most significant factor influencing job satisfaction was daily fatigue and stress. Conclusion : Job empowerment and burnout of significantly affected job satisfaction among occupational therapists. Occupational therapists have a effect on the daily lives of their clients through interventions, playing a crucial role for each individual client. Thus, increasing the empowerment and job satisfaction levels of occupational therapists and reducing their burnout levels can help improve the quality of life of the clients and enable occupational therapists to provide high-quality services.

중소도시 아파트단지 방범계획의 CPTED 요소 적용에 관한 연구 (A Study on Crime Prevention Design in Urban Apartment Complex by Application of a CPTED -focused on the Medium sized City-)

  • 도시의 인구집중 결과로 공동주택의 비중이 지속적으로 증가하는 추세에 있다. 이러한 도시화의 진전과 함께 과밀화되고 있는 공동주택단지를 대상으로 한 여러 유형의 범죄가 증가하고 있다. 공동체 의식의 소멸과 익명성의 증가 및 출입통제의 곤란은 물론 주거동의 배치계획 등에서 방범에 관한 점이 거의 고려되고 있지 않은 등의 결과로 대부분의 공동주택단지가 범죄에 대한 방어에 매우 취약한 환경에 놓여 있는 실정이다. 본 연구에서는 이러한 점에 주목하여 원천적인 범죄예방의 방안을 모색하기 위해 실제 주거단지를 대상으로 단지의 환경설계를 통한 범죄예방의 적용가능성을 모색해 보고자 하였다. 이를 위해 CPTED의 개념과 내용을 분석하고, 해외 선진사례를 조사하였다. 또한 현 주거단지의 방범상 문제점의 파악과 물리적 기법의 적용가능성을 분석해 보았다. 그 대상은 중소도시에 입지한 공동주거단지 중 근래 가장 수요가 많은 30평형대의 규모가 위주인 단지를 선정하였다. 건축과 도시설계의 단계에서 CPTED의 기법을 적용하는 범죄예방 환경설계 측면의 물리적 수단을 모색해보는 것은 추후 공동주택단지의 계획에 기초자료로서 의미를 가질 수 있을 것이며, 유지관리의 측면에서는 공동체의 강화가 안전한 주거환경의 지속가능성을 제고하는 기반이 된다는 점도 중요한 계획요소로 고려되어야 할 것이다.

응급전문간호사의 교육과정안 개발 (Development of Curriculum for the Emergency Clinical Nurse Specialist)

  • Various accidents and injuries are currently occurring in Korea at increasingly high rates. Good quality emergency care service is urgently needed to cope with these various forms of accidents and injuries. In order to develop a sound emergency care system, there need to be a plan to educate and train professionals specifically in emergency care. One solution for the on going problem would be to educate and train emergency clinical nurse specialists. This study on a strategy for curriculum development for emergency clinical nurse specialist was based on the following five content areas, developed from literature related to the curriculum of emergency nursing and emergency care situation : 1. Nurses working in the emergency rooms of three university hospitals were analyzed for six days to identify categories of nursing activities. 2. Two hundreds and eleven nurses working in the emergency rooms of 12 university hospitals were surveyed to identify needs for educational content that should be included in a curriculum for the clinical nurse specialist. 3. Examination of the environment in which emergency management was provided. 4. Identification of characteristics of patients in the emergency room. 5. The role of emergency clinical nurse specialist was identified through literature, recent data, and research materials. The following curriculum was formulated using the above mentioned process. 1. The philosophy of education for emergency clinical nurse specialist was established through a realistic philosophical framework. In this frame, client, environment, health, nursing, and learning have been defined. 2. The purpose of education is framed on individual development, social structure, nursing process and responsibility along with the role and function of the emergency clinical nurse specialist. 3. The central theme was based on human, environment, health and nursing. 4. The elements of structure in the curriculum content were divided to include two major threads, I, e., vertical and horizontal : The vertical thread to consist of the client, life cycle, education, research, leadership and consultation, and the horizontal thread to consist of level of nursing (prevention to rehabilitation), and health to illness based on the health care system developed by Betty Neuman system model. 5. Behavioral objectives for education were structured according to the emergency clinical nurse specialist role and function as a master degree prepared in various emergency settings. 6. The content of the curriculum consisted of three core courses(9 credits), five major courses(15 credits), six elective courses(12 credits) and six prerequisite courses (12 credits). Thus 48 credits are required. Recommendations : 1. To promote tile quality of the emergency care system, the number of emergency professionals, has to be expanded. Further the role and function of the emergency clinical nurse specialist needs to be specified in both the medical law and the Nursing Practice Act. 2. In order to upgrade the qualification of emergency clinical nurse specialists, the course should be given as part of the graduate Program. 3. Certification should be issued through the Korean Nurses Association.

여성 작업치료사의 직무 스트레스와 근무 환경에 관한 연구 (A Study on Job Stress and Working Environment of Female Occupational Therapist)

  • 여성 작업치료사의 직무와 관련된 문제를 파악하여 근무환경 개선, 직무 스트레스 인식, 성희롱 예방교육 및 여성의 작업치료사의 장기간 경제활동 참여 향상에 필요한 자료를 제공하며, 환자에게 보다 질 높은 서비스 제공에 기여하기 위해 진행되었다. 현재 작업치료사로 근무하고 있는 여성을 대상으로 200부의 설문지를 배포하여 회신된 185부를 분석하였으며 결과는 다음과 같다. 직무스트레스의 주요 항목은 '하는 일에 비해 보수가 적다고 느낄 때가 있다.', '근무로 인해 피로를 느낀다.', '치료 이외의 것까지 신경써야할 때가 있다.' 순으로 나타났다. 근골격계 통증은 손목과 손(90%), 어깨(86%), 목(69.7%), 허리(68.6%)순으로 나타났으며, 손목과 손(63%), 어깨(62.2%), 허리(51.9%) 통증으로 인하여 일상생활에 지장이 있는 상태였다. 언어적 성희롱 경험은 71.4%에 이르고 있었으며, 신체적 시각적 성희롱은 48.6%가 경험하였다고 응답하였다. 가족친화적 조직문화 항목에서는 '개인과 일의 사이에서 곤란하지 않다.'가 61.1%로 가장 많았으며, 직장상사의 긍정적인 지원(54.6%), 직장동료와의 유대관계유지와 감정 공유(73.%)등의 긍정적 응답이 많았다. 여성 작업치료사들이 겪고 있는 직무스트레스, 근골격계 통증, 성희롱 등의 효과적인 대처방법 마련 및 업무 환경 개선이 시급 할 것이라 판단된다. 또한 이를 위해 참고할 수 있는 자료를 제공하는 것에 의의가 있다.
