Development of and Evaluation Tool for the Quality of Patient Care in Musculo-skeletal Disorder (${\cdot}$ 골격장애 환자의 간호질 평가 도구 개발)

  • We need to continuously evaluate for nursing quality and the outcome to reflect nursing, to repair and improve of nursing. The first problem for nursing quality improvement is development of evaluation tool for the quality of nursing care. We need a system for quality assurance evaluation that is, development and framework of evaluation tool for nursing care quality. The purpose of this study was attempted to develop an evaluation tool for the quality of nursing care in musculoskeletal disorder patients. The approach method of nursing quality for the development of the tool in this study was process evaluation framework. The study were devided dvelopment process and reliability, validity verification process. The subjects of development process of this tool are three nursing export groups, and the subject of reliability, validity verification process are 20 samples of two university hospital in seoul, who were within discharge 3 months after admission treatment in musculo-skeletal disorder. Data for this study was collected from March 10 to April 13, 1995. The development process of the tool were as follows : 1. Make preliminary list of the tool by focal group were constituted 12 clinical nurses. 2. Modify and add preliminary list by 4 nursing expert panel. 3. Calculate content validity of the tool by 25 nursing expert panel of judge. 4. Verify reliability and validity of the tool. 5. Finalize an evaluation tool for the quality of the nursing care in musculo-skeletal disorder. The results of this study were as follows : 1) Development an evaluation tool for the quality of nursing care in musculo-skeletal disorder. (1) The evalution tool of this study was developed 5 standards, 33 criterias and 133 indicators. (2) 5 standards were divided according to Nursing Process. from standard 1 to standard 5, involved criterias were each 6(18%), 3(9%), 3(9%), 15(46%), 6(18%). 2) Verify reliability and validity of the tool. (1) Score of adequate degree for content validity of 33 criterias and 133 indicators were every average 2.82. (2) Inter-rater reliabilities(consentaneity score) of the tool by pearson correlation coefficient between three raters were : r=.7506, r=.8934, r=.6695. and Inter-rater reliabilities by single-facet crossed design were : r=.7464. (3) The alpha coefficient relating to internal consistency was .8524 over all 30 items of 33 criterias of developed tool. (4) Score of the quality of nursing care following to generaal characteristics of this study subjects were stastically significant differences according to educational level (F=2.93, p=.029)and diagnosing classification (F=2.50, p=.042). Through this study, I'm sure that the developed tool for the quality of patient care in musculo-skeletal disorder will show the way of more improvement of the quality of nursing care and effective nursing interventions.

  • Preventing the forgery and alteration of test reports is important, in order to maintain the reliability of military supplies. Based on a survey of reports on testing institutions and a case study of established electronic document systems in the public sector, we propose a system architecture, which helps prevent the forgery and alteration of test reports with legal force. The proposed architecture takes advantage of both the time stamp that records the time a document is received and a server synchronized with the testing institution. Using the proposed system architecture, the user is able to request a test and receive reports from the testing institution without delay. Also, a defense agency is able to conveniently prove the authenticity of the test reports and utilize the statistical data collected by the architecture. Therefore, we expect that the proposed system architecture will help defense agencies to prevent the forgery and alteration of test reports and ensure their reliability and quality.

Characteristics of dose distribution for virtual wedge (가변형 쐐기필터의 선량분포에 관한 특성)

  • We was investigate the dosimetric characteristics of the virtual wedge and it compared to the conventional fixed wedge. Also we was evaluate the quality factor of the experimental multi-channel dosimetry system for virtual wedge. Recently virtual wedge technique and wedge fraction methods are available through the computer controlled asymmetric collimator or the independent jaw in medical linear accelerator for radiation therapy. The dosimetric characteristics are interpreted by radiation field analyzer RFA-7 system and PTW-UNIDOS system. Experimental multi-channel dosimetry system for virtual wedge was consists of the electrometer, the solid detector and array phantom. The solid detectors were constructed using commercially diodes for the assessment of quality assurance in radiotherapy. And it was used for the point dose measuring and field size scanning. The semiconductor detector and ion chamber were positioned at a dmax, 5 cm, 10 cm, 20 cm depth and its specific ratio was determined using a scanning data. Wedge angles in fixed and virtual type are compared with measurements in water phantom and it is shown that the wedge angle 15$^{\circ}$, 30$^{\circ}$, 45$^{\circ}$were agree within 1$^{\circ}$ degree in 6, 10 MV photon beams. In PDD and beam flatness, experimental multi-channel disimetry system was capable of reproduceing the measured values usually to within $\pm$2.1% the statistical uncertainties of the data. It was used to describe dosimetric characteristics of virtual wedge in clinical photon beams. Also we was evaluate optimal use of the virtual wedge and improve the quality factor of the experimental multi-channel dosimetry system for virtual wedge.

The History and Development of the Marine Environment QA/QC (Quality Assurance/Quality Control) Management System (해양환경 정도관리제도 운영에 대한 고찰)

  • The Marine Environment QA/QC management system has been operated since 2010 to secure the reliability of data and improve the analysis capabilities of measurement and analysis institutions. From 2010 to 2020, the cumulative number of measurement and analysis institutions participated in the QA/QC management system was 266. And the number of certificates issued by the ministry of oceans and fisheries is 182. A total of 32 reference materials for proficiency testing and interlaboratory comparisons have been developed. They were first developed focusing on items (Nutrients, COD) commonly analyzed in marine environmental measuring network, marine pollution impact surveys, sea area utilization impact assessment, deepsea water surveys, and information network on fishing ground environments. In addition, it is time to expand the filed of the QA/QC management system, such as seawater temperature, salinity, PCBs and PAHs in sediments, which are mainly analyzed in most monitoring programs. On-site assessment has been conducted for 162 laboratories according to ISO/IEC 17025 to evaluate their conformity of the quality management system and deficiency. In terms of management and technology requirements, about 4.2% of organizations showed insufficient division of duties among employees 8.7% of them revealed the lack of employee training. By test item, about 6.3% of organizations showed the lack of standard substance management and the state of the cleaning glassware was pointed out in about 5.4% of them. The QA/QC management system should be continuously supplemented by identifying the causes of nonconformities and area for improvement.

Improvement of the Marine Environmental Assessment for Dredging and Ocean Disposal of Coastal Sediment in Korea (연안준설 및 준설토 해양투기 해양환경평가 개선방안)

  • We studied improvement in marine environmental impact assessment and related management systems of coastal sediments that are dredged inshore but disposed offshore. After reviewing and diagnosing the existing assessment procedures and problems, we recommend to design the core assessment items and improve the reliability of assessment byenhancing the quality assurance/quality control (QA/QC) and verification processes. We proposed eco-friendly disposal plan for dredging sediment such as reuse system in land development was explored. A marine environmental database system was established to support the assessment processes. Guidelines for marine research and modelling were proposed for improving assessment of dredging and disposal of coastal sediment. Also, applying of screening and scoping for marine environmental assessment was reviewed.

Verification of Mechanical Leaf Gap Error and VMAT Dose Distribution on Varian VitalBeamTM Linear Accelerator

  • The proper position of a multi-leaf collimator (MLC) is essential for the quality of intensity-modulated radiation therapy (IMRT) and volumetric modulated arc radiotherapy (VMAT) dose delivery. Task Group (TG) 142 provides a quality assurance (QA) procedure for MLC position. Our study investigated the QA validation of the mechanical leaf gap measurement and the maintenance procedure. Two $VitalBeam^{TM}$ systems were evaluated to validate the acceptance of an MLC position. The dosimetric leaf gaps (DLGs) were measured for 6 MV, 6 MVFFF, 10 MV, and 15 MV photon beams. A solid water phantom was irradiated using $10{\times}10cm^2$ field size at source-to-surface distance (SSD) of 90 cm and depth of 10 cm. The portal dose image prediction (PDIP) calculation was implemented on a treatment planning system (TPS) called $Eclipse^{TM}$. A total of 20 VMAT plans were used to confirm the accuracy of dose distribution measured by an electronic portal imaging device (EPID) and those predicted by VMAT plans. The measured leaf gaps were 0.30 mm and 0.35 mm for VitalBeam 1 and 2, respectively. The DLG values decreased by an average of 6.9% and 5.9% after mechanical MLC adjustment. Although the passing rates increased slightly, by 1.5% (relative) and 1.2% (absolute) in arc 1, the average passing rates were still within the good dose delivery level (>95%). Our study shows the existence of a mechanical leaf gap error caused by a degenerated MLC motor. This can be recovered by reinitialization of MLC position on the machine control panel. Consequently, the QA procedure should be performed regularly to protect the MLC system.

Monitoring of Brominated Flame Retardants (BFRs) for the management of Their Contamination in Environments (브롬화 난연제의 환경오염도 관리 방안)

  • Brominated flame retardants have the market share of 40%, comparing others because of their low cost and highly effective retardation against the flame. However, their toxic effects in human and properties of the accumulation in the environments have been issued among the international organization such as EU, OECD and etc. It, therefore, was surveyed the classification, toxic effects, and the usage of Brominated flame retardants, the trends for their managements in the world and Korea, and their contaminated levels in Korean Peninsula. In addition, the management directions for them were proposed. Penta, octa, and deca-BDE among brominated retardants will seem to be prohibited by the regulation as a flame retardants for plastics in Europe because of their toxic effects. Although Penta and Octa BDEs was used marginally in Korea, deca-BDE was 27% of brominated flame retardants (49,050 ton) which had been used in 2002. However, risk assessment for brominated retardants might not launched in Korea, yet. These reports demonstrate that toxic brominated retardants such as PBDEs will be assessed for their usage and the level of contamination in the environment in the area of the point sources like the industrial areas, incinerators and etc. However, the law to regulate the hazardous chemicals seems not to be dictated the monitoring of their contamination in the environment. We, therefore, suggest how to evaluate and to monitor the toxic contaminants with EIA (Environmental Impact Assessment) and LCA (Life Cycle Assessment) system. Further, to establish the management system of BFRs (such as the monitering of contamination levels in environments, life cycle assessment, and risk assessment for the toxic chemicals), It can be recommended the law to deal with the method analyzing chemicals will be established, which contains QA/QC (Quality Assurance and Quality Control) to evaluate the analytic capability of the companies to prepare EIS (Environment Impact Statement) or other institutes for analyzing chemicals.

A Study on Factors Affecting the Continued Usage Intention of Ubiquitous Services (유비쿼터스 서비스의 지속적 이용의도에 영향을 미치는 요인에 관한 연구: 유비쿼터스 시스템적 특성과 서비스 품질 요인을 중심으로)

  • Ubiquitous computing is enhancing computer use by making many computers available throughout the physical environment without being noticed to the users. To facilitate successful adoption and diffusion of ubiquitous systems, it is important to find out the factors affecting their uses. Although researches related to ubiquitous computing have been vigorously conducted from the aspect of system and service provider, there have been very few studies that focus on the user's perspective. This study attempts to find out major factors which are dedicated to the development of ubiquitous systems and services and the relationships between these factors and user satisfaction factors. This study derived the specific factors that characterize ubiquitous services, such as ubiquity, contextual offer, user affinity, privacy, reliability, responsiveness, assurance, and empathy. We also considered service quality factors of ubiquitous services. Then these factors were combined with the Expectation Confirmation Theory (ECT) model and the path analysis was applied to find out the factors affecting user satisfaction and the intention to use the systems. The research findings indicate that ubiquity, contextual offer, privacy, reliability, and responsiveness affect the perceived usefulness. The perceived ease of use and confirmation were found to affect the perceived usefulness. The perceived usefulness, perceived ease of use, and confirmation fumed out to influence the satisfaction. It turned out that the result of this study on the relationship between the perceived usefulness and user satisfaction was identical with the findings in previous related ECT researches. We think this study makes two significant contributions to the development of ubiquitous services. First, this study provides basic implications for better ubiquitous services by considering important characteristics of ubiquitous systems as services. Second, this study also suggests important factors that need to be considered when ubiquitous service providers develop new ubiquitous services to improve user satisfaction and the intention to use the services.

Inpatient Cost Variation among Hospitals in Some Tracer Diseases (일부 다빈도 상병에서 입원진료비의 변이 정도와 요인에 대한 연구)

  • Variation in the utilization of medical services is a very important issue in cost containment and quality assurance of health care. Practice variation directly affects health care expenditure especially in fee-for-service system, which is the payment system of health insurance in Korea. In addition to cost issue it is generally accepted that variations in medical practice and the cost of inpatient care suggest the possibility of inappropriate quality of care. This study is to closely examine the patterne and degrees of variation in cost structure of inpatient care among types of hospital and individual hospitals in some tracer diseases, and also to inquire into the service items which contribute much to the variation of total medical care cost. Foru common diseases, i.e. Cesarean Section, appendectomy, cataract extraction and pediatric pneumonia, were selected as tracer diseases. In most tracer diseases there were statistically significant differences in total medical care cost among hospitals in same type of hospital as well as among types of hospital(p<0.01). When total medical care cost were subdivided into the types of service, cost of medication and diagnostic examination varied the most prominenly. When the cost of medication were subdivided again, cost of parenteral antibiotics showed the most prominent variation. Of total medical care cost, medication was most contributory to the variation of total medical care cost(58.1~82.3%), and cost of antibiotics was most contributory to the variation of medication cost(63.9~92.2%). The results of study implicated that reducing the variation of medication may plays a significant role in containing the cost of inpatient care. In order to sort out the factors affecting practice variations including drug prescription pattes further researches are required.

Measurement of Image Quality According to the Time of Computed Radiography System (시간에 따르는 CR장비의 영상의 질평가)

  • The regular quality assurance (RQA) of X-ray images is essential for maintaining a high accuracy of diagnosis. This study was to evaluate the modulation transfer function (MTF), the noise power spectrum (NPS), and the detective quantum efficiency (DQE) of a computed radiography (CR) system for various periods of use from 2006 to 2015. We measured the pre-sampling MTF using the edge method and RQA 5 based on commission standard international electro-technical commission (IEC). The spatial frequencies corresponding to the 50% MTF for the CR systems in 2006, 2009, 2012 and 2015 were 1.54, 1.14, 1.12, and $1.38mm^{-1}$, respectively and the10% MTF for 2006, 2009, 2012, and 2015 were 2.68, 2.44, 2.44, and $2.46mm^{-1}$, respectively. In the NPS results, the CR systems showed the best noise distribution in 2006, and with the quality of distributions in the order of 2015, 2009, and 2012. At peak DQE and DQE at $1mm^{-1}$, the CR systems showed the best efficiency in 2006, and showed better efficiency in order of 2015, 2009, and 2012. Because the eraser lamp in the CR systems was replaced, the image quality in 2015 was superior to those in 2009 and 2012. This study can be incorporated into used in clinical QA requiring performance and evaluation of the performance of the CR systems.
