메타분석을 이용한 임상영양서비스의 비용-효과성 평가 (Evaluation of Cost-Effectiveness of Medical Nutrition Therapy : Meta-Analysis)

  • Objectives: A meta-analysis of the literatures was conducted to evaluate the cost-effectiveness of medical nutrition therapy by dietitians. Methods : The 30 studies were identified from a computerized search of published research on MEDLINE, Science-Direct and the PQD database until May, 2002 and a review of reference lists. The main search terms were“dietitian”,“dietary intervention”,“nutrition intervention”, “cost”,“cost-effectiveness”and“cost-benefit analysis”. The subgroup analysis was performed by publication year, study design, intervention provider, type of patient (in/out-patient) and type of cost (total cost/direct cost). Two reviewers independently selected trials for inclusion, assessed the quality and extracted the data. Results : The 30 studies were identified using the electric database search and bibliographies. The 17 trials were eligible for inclusion criteria, then the systematic review and a meta-analysis were conducted on effectiveness and cost-effectiveness of medical nutrition therapy. The quality of the studies was evaluated using the quality assessment tool for observational studies. The quality score was 0.515 $\pm$ 0.121 (range : 0.279-0.711, median : 0.466). The meta-analysis of 17 studies based on the random effect model showed that medical nutrition therapy was highly effective in treating the diseases (effect size 0.3092 : 95% confidence interval 0.2282-0.3303). The vote-counting method, one of meta-analysis methods, was applied to evaluate the cost-effectiveness of medical nutrition therapy conducted by dietitians. Two criteria (method 1, method 2) for voting were used. The calculated p-values for method 1 (more conservative method) and method 2 (less conservative method) were 0.1250 and 0.0106, respectively. Medical nutrition therapy by dietitians was significantly cost-effective in the method 2. Conclusion. This meta-analysis showed that the effectiveness of medical nutrition therapy was statistically significant in treating disease (effect size 0.3092), and that the cost-effectiveness of medical nutrition therapy was statistically significant in the method 2 (less conservative method) of vote counting. 프로젝트 일정에서 선행활동 품질이 후행활동의 시간과 비용에 미치는 문제 - 자원제약이 존재하는 프로젝트 일정문제 - (The Problem of the Quality of the Predecessor Activity on the Time and Cost of the Successor Activity in the Project Schedule - Project Schedule with Resource Constraints -)

  • Purpose: The time and cost of a project activity exists in a selected mode and there is a quality level for the selected mode, and the time and cost of the current activity is determined by the quality level of the preceding activity. When an activity is a predecessor activity of an activity, it is characterized as a trade-off problem in which the time and cost of the activity are determined according to the quality level of the activity. Methods: A neighbor search heuristic algorithm obtains a solution by (1) randomly determining the mode, quality level, and assignment order for each activity. (2) get a solution by improving the solution by changing the possible modes and quality levels; (3) to find a solution by improving the solution from the point where it is feasible to advance the start time. Here, Case[1] is a method to find the optimal solution value after repeating (1). Case [2] is a method for finding a solution including (1) and (2). Case [3] refers to a method for finding solutions including (1), (2), and (3). Results: It can be seen that the value of the objective function presented by the algorithm changes depending on how the model of the heuristic algorithm is designed and applied. In other words, it suggests the importance of algorithm design and proves the importance of the quality problem of activities in the project schedule. Conclusion: A study significance of the optimization algorithm and the heuristic algorithm was applied to the effect of the quality of the preceding activity on the duration and cost of itself and the succeeding activity, which was not addressed in the project schedule problem.

Efficient Operation Policy in a Closed-loop Tire Manufacturing System with EPR

  • This paper deals with a closed-loop remanufacturing system with one manufacturer and one remanufacturer. The manufacturer sells new products bearing the 'Extended Producer Responsibility (EPR).' It is assumed that the manufacturer's collection rate of used products depends only on the buy-back cost, while that of the remanufacturer depends on the minimum allowed quality level of used products in addition to the buy-back cost. Through the development of mathematical models with the objective function of maximizing profit, we study an efficient operation policy of each party. The decision variables are the unit selling price of new products and remanufactured products, the unit buy-back cost of the used products of the manufacturer and remanufacturer, and the minimum allowed quality level. The validity of the model is examined through numerical examples and sensitivity analysis.

Optimal Production Planning for Remanufacturing with Quality Classification Errors under Uncertainty in Quality of Used Products

  • This paper discusses a green supply chain with a manufacturer and a collection trader, and it proposes an optimal production planning for remanufacturing of parts in used products with quality classification errors made by the collection trader. When a manufacturer accepts an order for parts from a retailer and procures used products from a collection trader, the collection trader might have some quality classification errors due to the lack of equipment or expert knowledge regarding quality classification. After procurement of used products, the manufacturer inspects if there are any classification errors. If errors are detected, the manufacturer reclassifies the misclassified (overestimated) used products at a cost. Accordingly, the manufacturer decides to remanufacture from the higher-quality used products based on a remanufacturing ratio or produce parts from new materials. This paper develops a mathematical model to find how quality classification errors affect the optimal decisions for a lower limit of procurement quality of used products and a remanufacturing ratio under the lower limit and the expected profit of the manufacturer. Numerical analysis investigates how quality of used products, the reclassification cost and the remanufacturing cost of used products affect the optimal production planning and the expected profit of a manufacturer.

A cost model for determining optimal audit timing with related considerations for accounting data quality enhancement

  • As society's relience on computerized information systems to support a wide range of activities proliferates, the long recognized importance for adequate data quality becomes imperative. Furthermore, current trends in information systems such as dispersal of the data resource together with its management have increased the difficulty of maintaining suitable levels of data integrity. Especially, the importance of adequate accounting (transaction) data quality has been long recognized and many procedures (extensive and often elaborate checks and controls) to prevent errors in accounting systems have been introduced and developed. Nevertheless, over time, even in the best maintained systems, deficiencies in stored data will develop. In order to maintain the accuracy and reliability of accounting data at certain level, periodic internal checks and error corrections (internal audits) are required as a part of internal control system. In this paper we develop a general data quality degradation (error accumulation ) and cost model for an account in which we have both error occurrences and error amounts and provide a closed form of optimal audit timing in terms of the number of transactions that should occur before an internal audit should be initiated. This paper also considers the cost- effectiveness of various audit types and different error prevention efforts and suggests how to select the most economical audit type and error prevention method.

식료품비 수준에 따른 식사의 질 (Effects of Food Cost on a Diet Quality)

  • This study was performed to investigate the relationship between diet quality and food cost and identify the effects of food cost on healthy diet among Korean adult. Among the subjects of 2001 National Health and Nutrition Survey 1,641 men and 1,765 women aged from 30 to 49 years were selected and their information of dietary intakes, socio-demo-graphic information, and anthropometric data were analyzed. For the analysis, subjects were classified to quartile based on their daily food cost. Food guidelines provided by Korean Nutrition Society was regarded as a model of healthy diet. Subjects in the lowest quartile of food cost consumed inadequate amount of food from all food categories of the guidelines. Many subjects in the highest quartile not only satisfied the recommended amount of the Food Guide but also seemed to overeat high energy density foods. Even in the highest quartile, about 90% of subjects did not satisfied recommended amount of dairy products. According to the calculated cost of healthy diet, the average cost did not seem to be more expensive than the current mean food cost of the subjects, and most cost-sensitive food intake was observed in the meat fish beans eggs category. The implications of study results were: 1) all the high cost diets did not indicate the high quality of diet, 2) most practical matter of healthy diet in low income group seemed whether they could afford the expense of meat and fish, 3) nutrition education was required to most subjects for healthy diet.

  • In this paper a model is developed for an inventory system in which the number of units of acceptable quality in a replenishment lot is uncertain and the demand. during the stockout period is back ordered and. also under the same condition an inventory model with experdited stockout is developed. It is assumed that the fraction of the acceptable quality in a replenishment lot is a random variable whose probability distribution is known. The optimal replenishment policy is synthesized for such a system. A numerical example is used to illustrate the theory.

  • This paper is concerned with an economic selection of both the process mean and the lower limit for a continuous production process with the quadratic loss function. It is assumed that the quality characteristic is normally distributed with a known variability. A profit model is developed which involves selling price, production cost, reprocessing cost and the cost which is incurred by imperfect quality. Methods of finding optimum values of the process mean and the lower limit are presented, and a numerical example is given.

A(Q, r) Spare-Part Inventory Model with Gamma Leadtime

  • This paper deals with a (Q, r) spare-part inventory model with gamma leadtime. In the model, if the inventory level falls to a reorder point r, a replenishment order quantity Q is ordered. Assumming that the number of operating units is one and the lifetime of a unit follows an exponential distribution, we derive the expected cost rate and suggest a procedure to obtain the optimal pair of (Q, r) minimizing the cost rate. A numerical example is presented to explain the model.

  • 많은 Buyer들이 품질의 기본조건으로 요구하는 완제품 Burn-in 시간은 실제 필요 이상으로 긴 시간일 수 있으며 생산자 입장에서 Cost, Delivery, Capacity 등의 제약으로 이행하기 어려운 경우가 종종 있다. 본 연구는 생산자에게 실무적으로 적용이 용이하고 Buyer에게 설득력있는 최적 Burn-in 시간 근거를 제시할 수 있도록, 구간개념과 Curve Fitting 방법을 적용하여 최적 Burn-in 시간을 추정하고 시간대별 Reliability를 산출 비교해 봄으로서 생산자 입장에서 Burn-in 시간을 결정하고 Reliability 추정 Model을 결정해 갈 수 있도록 하였다. 향후 제시된 Burn-in 시간의 최적여부는 Field Data로부터 검증되어야 할 것이다.
