유즙분비이상에 관한 문헌적 고찰 (Bibliographic Studies on Disorder of Milk Secretion)

  • Breast milk is general term for crude milk and mature that is secrete after two-three days of delivery. Because amount of milk secretion is different, disorder of milk secretion is to be classified into galactostasis and spontanous flow of milk. According to this point, we considered thirty four papers and got the conclusion about the concept, cause, therapeutic method and medication of secreation disorder. Therefore we report the result. galactostasis due to deficiency of the qi and blood, depression of liver qi, blood stasis of postpartum, excessiveness and so on. If the qi and blood is deficient, therapeutic method is enriching qi and benefiting blood, if the liver qi is deprssive, treatment is relieving the deprssive liver and regulating the circulation of qi. The milk is flowing spontanously and continously due to a prosperous condition of qi and blood, too deficiency of the qi and blood, depression of liver qi and so on. If the qi and blood is deficient, therapeutic method is tonifying and arresting the qi and blood, if Liver channel is stagnated fire, puring the liver of pathologic fire, relieving the deprssive liver and regulating the circulation of qi. Clinical study for the 1 case of the recurrent cervical cancer patient.

현대적인 관점인 내장계와 체벽계로 바라본 영기(營氣)와 위기(衛氣)에 대한 연구 (A study of Nutrient Qi(營氣) and Defense Qi(衛氣) from the modern perspective of Visceral body and Somatic body)

  • Objectives: The aim of this study is to modernize Nutrient Qi(營氣) and Defense Qi(衛氣) from the point of view of Visceral Body and Somatic Body. Methods: Investigate the areas of body and function of Nutrient Qi(營氣) and Defense Qi(衛氣) recorded in the 『HuangdiNeijing(黃帝內經)』 and 『Dongeuibogam(東醫寶鑑)』. And investigate Visceral Body and Somatic Body that anatomical structure is divided according to function based on the book 『Life and Rhythm』. Results: Nutrient Qi(營氣) works to nourish the body such as viscera and bowels in the meridian. Defense Qi(衛氣) works to protect the body such as skin, limb joint, eye outside the meridian. The human body is divided into Visceral Body called a vegetable organ and Somatic Body called an animal organ. Visceral body is the organ that manages "nutrition-reproductive" and is divided into the intestine(腸管), blood vessels(血管), and the nephridium(腎管). Somatic Body is an organ that manages 'sensory-motion' and is divided into a three-layer structure: the outer layer covering the body surface, the nerve layer connecting the outer layer and the muscle layer, and the muscle layer controling contraction and movement. Conclusions: It is estimated that Nutrient Qi(營氣) is similar to Visceral Body, and Defense Qi(衛氣) is similar to Somatic Body.

공공병원 의료 질 관리 전담자의 의료 질 향상 활동 경험연구 (Quality Improvement Nurses' Experiences of Working for Healthcare Quality in Public Hospitals)

  • 본 연구는 공공병원 질 관리 전담자들의 병원 진료부문과 전반적 질 향상 활동의 경험 탐색을 통해 공공병원 질 향상 활동 방안을 수립하고자 수행되었다. 공공병원의 질 관리 전담자 10명을 대상으로 질 향상 활동 경험에 대해 포커스 그룹 면담을 수행하였다. 질 향상 경험 면담을 통해 '진료 부문과 전반적 의료질 향상 활동에 대한 성공과 실패', '현재 질 향상 활동의 활성화 정도', '공공병원 질 향상 활동의 특성' 그리고 '향후 질 향상 활동의 활성화를 위해 필요한 것들'의 범주화를 통해, '공공병원 고유의 특성에 따른 질 향상 활동의 성공', '리더십과 역량강화를 통한 공공병원 질 향상 활동의 활성화', '공공병원의 지역 중점 병원의 역할 재정립'의 주제를 도출하였다. 공공병원 진료 부문의 질 향상 활동은 의사의 참여가 중요하며 이러한 의사참여를 이끌어내기 위해서는 의사의 질 향상 활동의 성공적 경험을 공유하고 질 향상 활동에 필요한 지식을 제공하여 질 향상 전문성과 자율성을 보장하는 지원들이 필요하다. 또한 공공병원의 질 관리 전담자는 국가 의료 질 향상을 위한 핵심위치에서 질 향상 활동의 견인역할을 하고 지역 의료기관간 협력적 소통을 통해 의료 질 향상 활동을 이끌어가야 한다.

표본(標本)·근결(根結) 이론과 임상응용에 관한 고찰(考察) (A Literature Study on PyoBon·GeunGyul Theory)

  • PyoBon GeunGyul - one of the twelve regular meridians theory - play a important role on the principle of point selection and point prescription in acumoxibustion. PyoBon explain the connection of the concentration and diffusion of channel qi, GeunGyul explain the relation of both poles of channels flow. So, Geun and Bon means the starting point of channel qi, and Pyo and Gyul means the terminal point of channel qi. But the flow of channel qi on PyoBon GeunGyul different from today's circulation courses of twelve regular channels based on Kyungmaek(經脈) chapter of Youngchu. Thus this study investigate the contents of PyoBon GeunGyul and consider its connection with channel flow. The results are as follows : 1. PyoBon GeunGyul theory explain that the relation of the limbs and trunk at meridian and emphasize that the connection of meridian and the importance of the limb acupoints. 2. PyoBon GeunGyul theory can be understandable in the view of the primordial qi and explain that the primordial qi of twelve regular channels acts from the limbs to the trunk. 3. PyoBon GeunGyul theory is based on the system of primordial qi channel which circulates from fingers and toes facing toward heart or the head, different from today's circulation courses of twelve regular meridians. 4. PyoBon GeunGyul theory act as a basis of principle of a part or distant point selection which applicated widely in acumoxibustion.

기구(氣口), 인영맥(人迎脈)의 진단(診斷)을 통(通)한 내상(內傷), 외감병기(外感病機)에 대(對)한 문헌적(文獻的) 고찰(考察) (Invastigating literatures about mechanism of internal·external damage, through Qi mouth and Renying)

  • Objective : We have studied the part, definition, diagnostic generaliztion and pathological viewpoint of Qi mouth and Renying by refering literatures Methods : I refered to records from anciant to modern. The results are followings : 1. Qi mouth and Renying is ancient diagnostic method and the origin of diagnostic method through the pulsation of radial artery. And Qi mouth is recognized as the right and left Qi mouth and Renying is located on pulsation points of both common carotid arteries. 2. The yin of five viscera, nutritional blood and viscous liquid are diagnosed by Qi mouth which is inclued lung meridian. And the yang of six organs, guarding Qi and the rise and fall of stomach energy are diagnosed by Renying which is inclued stomach meridian. 3. It is possible that the position, process and transformational condition of disease are decided by comparing and examining Qi mouth and Renying.

기수련과 스트레스반응척도 및 행복지수의 관계 고찰 (The Connection between Stress Response Inventory(SRI), Happiness Index(HI), and Qi-training)

  • The purpose of this research is to investigate the connection between Stress Response Inventory(SRI), Happiness Index(HI), and Qi-training to find out the meaning of Qi-training on anti-stress. Method: The study group was consisted of 35 subjects training Qi and 179 subjects not training. We bad all subjects to reply to demographic questimnaire, SRI questimnaire and HI questimnaire. We made the Qi-training group write the kinds and periods of Qi seperately. Results and Conclussions : 1. The higer Happines Index score, the subjects bad lower Stress Response Iuventory total score. Each details were connected significantly. 2. The Qi-training group's mean SRI total scores and ill scores were higer than the non-training's significantly. 3. In Qi-training group, mean HI scores get higer as training longer.

환자 급식서비스의 질 향상 사례 연구 - Back-up 처방 중심으로 - (A Case Study on Quality Improvement of the Food Services for Patients - Focused on back-up orders -)

  • The purpose of this research which, surveyed target hospitals, was to evaluate job operations by surveying the influences of Quality Improvement (QI) activities in various divisions related to a decrease in their back-up orders. Statistical data analyses were completed using the SPSS 11.0 program. The results can be summarized as follows Before QI there were 147 cases of back-up orders ; after QI there were 83 cases, decrease of 64 cases. This was 44%, less than the projected goal of 50%. For each item, there was a decrease of 40 nurse cases, 9 patients and patron cases,9 test surgery deliverly cases and 5 doctor cases after QI. The registering of midnight meals was not shown after QI, due to the Order Communication System (OCS) settlement. After performing QI, the average manual operation per month was reduced from 840 minutes to 498 minutes, of which the difference was 342 minutes, and the average of 342 minutes per month could be used for the peculiar operation of each division. This QI activity provided a good opportunity for establishing cooperation among divisions in providing meals to patients through interactions among divisions. It was recognized that these interactions were effective only when medical services were achieved through organized cooperation among divisions. Among the 7 items included on the patient satisfaction questionnaire, “satisfaction with offered menus” (p<0.01) showed significantly higher scores before QI. However “satisfaction with meal times” (p<0.01) showed significantly lower scores before QI.

기 개념의 이해와 간호 실무 적용 (Understanding Qi: Its Development and Clinical Application to Nursing Practices)

  • Purpose: This paper analyzed the historical origins of the Qi concept and assesses its possible contribution to the development of complementary therapy and new nursing practices. Methods: In order to clarify the Qi concept measure its theoretical/clinical potentials, this study analyzed both historical data and experimental research that adopt and apply the concept. These include modern/contemporary research measuring its effects in promoting mental and physical health. Using the method of cross-cultural comparison, this study analyzed diverse approaches to the Qi concept and sought to find common features among the approaches. Results: A historical, cross-cultural analysis revealed several fundamental similarities between Qi theories that have developed in the east and the holistic concepts that have evolved in the western traditions. Especially, the analysis of the more recent research on the Qi concept shows ample possibilities of its future contributions to the development of new diagnostic applications and the promotion of overall human health. Conclusion: The historical study of the Qi concept found some key common factors in the diverse philosophical traditions in the east and the west. Considering the growing popularity of complementary therapy among health professionals and the general public, the Qi concept and its clinical applications are expected to promote human health. In this context, this research contributes to developing new nursing practices based on the concept by clarifying its philosophical origins and theoretical backgrounds.

QI부서근무자들의 의료기관인증제관련 인식이 직무스트레스 및 이직의도에 미치는 영향 (Perception related to the Healthcare Accreditation Effects on Turnover Intention and Job Stress of employees' Quality Improvement Department)

  • 본 연구는 QI부서 근무자들의 의료기관인증제관련 인식이 직무스트레스 및 이직의도에 미치는 영향을 파악하기 위해 시도된 서술적 조사연구이다. 연구기간은 2016년 6월1일부터 2016년 6월15일까지 이루어졌으며, 조사대상자는 의료기관 인증평가에 참여한 경험이 있는 QI부서 근무자 154명이다. 자료분석은 SPSS Win 21.0 프로그램을 이용하였고 영역별 분석은 평균과 표준편차, t-test와 ANOVA, 다중회귀분석으로 하였다. 연구결과에서 의료기관인증제에 대한 QI부서 근무자들의 인식은 5점 만점에 3.35점, 직무스트레스는 3.66점, 이직의도는 3.32점으로 나타났으며, 일반적 특성에 따른 의료기관인증제에 대한 인식도는 성별, 연령, QI부서 근무기간, 직위, 직무 만족도에 따라 유의한 차이가 있었다. 직무스트레스는 직위에서 유의한 차이가 나타났으며, 이직의도는 연령, QI부서 근무기간, 직위, 직무만족도에서 유의한 차이가 있었다. 의료기관인증제에 대한 인식과 직무 스트레스간의 관계는 유의한 양의 상관관계를 보인 반면 이직의도는 유의한 음의 상관관계를 보였다. 따라서 조사대상자의 의료기관인증제 관련 인식이 높을수록 직무스트레스는 높아지고, 이직의도는 감소하였으나 직위와 연령이 낮은 군과 QI부서 근무기간이 10년이상인 군, 직무만족도가 낮은 군일수록 이직의도에서 더 큰 영향을 받는 것으로 나타났다. 그러므로 QI부서 일부 근무자들의 이직의도를 낮추고, 직무 스트레스를 감소시키기 위해서는 업무 부담감을 줄이고 다양한 지원을 위한 노력이 필요할 것으로 사료된다.

  • Jiu Qi(九氣) was shown at Ju Tong Lun(擧痛論) in Shao Wen(素問) Huang Ti Nei Ching(黃帝內經), and is nine important factors that affect the function of human body. Jiu Qi concludes endogenous, exogenous, non-endo-exogenous factors. I do the bibliographical study on the Jiu Qi, the results were as follows; 1. The Qi of Jiu Qi has two opposite meanings. one is genuine vital energy(正氣), and the other is the factors causing abnormal state in vital energy. Jiu Qi is nine factors concluding coldness-heat(exogenous factors). six emotional factors(endogenous factor), overworking(non -endo-exogenous factor). 2. Anger may lead to abnormal rising of vital energy. Anger causes Qi of the liver to go perversely upward, and perverted flow of exuberant Qi of the liver lead to dysfunction of the spleen, so resulted in hematemesis, diarrhea, indigestion. 3. Joy can promote the harmony of vital energy and blood, so do the circulation of nutrient and defensive energy in physiological state. But an excessive joy may lead to the sluggishness of vital energy. 4. The lung keeps the pathway of air unconstructed, disseminates vital energy, cleanses the inspired air and keeps vital energy flowing downward. Sorrow affects on the function of the lung and the heart, so could result in obstruction of the circulation of nutrient and defensive energy. An excessive sorrow after stagnation may lead to the consumption of vital energy. 5. Fear makes vital energy and essence of the kidney sink to inward and downside, makes Yang-Qi can't go upward, so causes obstruction of triple wanner. An excessive fear can obstructs the ascending of Yang-Qi, so may lead to the abnormal falling of vital energy. 6. Coldness makes the sweat pore be contracted, so obstructs the circulation of triple warmer, causes sluggishness of defensive energy or Qi of the internal organ. 7. Heat makes the sweat pore be open, much amount of sweat is excreted with Yang-Qi, defensive energy, vital energy. Heat may consume vital energy. 8. Sudden fright affects on spirits of the heart and liver, causes disorder of the mental faculties and separation of blood and vital energy. Fright may lead to disorder of Qi. 9. Overwork concludes overfatigue and exhaustion caused by intemperance in sexual life. Overwork renders vital energy consumed, and hence results in lassitude and listlessness. 10. Thinking affects on the function of the heart and the spleen. Over thinking may lead to depression of vital energy. Through the bibliographical study on Jiu Qi, I got smallest amount of it, and this must be more investigated correlating with clinical study.
