Pulmonary mucormycosis is very rare but has a devastating opportunistic fungal infection in immunocompromised hosts. The infection usually occurs in patients with hematologic malignancy, chronic renal failure, diabetes mellitus, or in solid organ transplant recipients. We experienced a case of pulmonary mucormycosis associated with renal cadeveric allograft recipient who had uncontrolled diabetes mellitus. The patient was successfully treated by surgical resection with Amphotericin B therapy.

Pulmonary fungal infection has been investigated in patients with healed or active pulmonary tuberculosis or other lung diseases by demonstrating serum precipitating antibodies to the various fungal antigens and by isolating the related fungi from sputums or other clinical specimens. Out of 1,192 suspected patients, 405(34.0%) showed precipitin bands on immunodiffusion tests and the related fungi have been demonstrated in sputums or other specimens of 79.5% of them(327) whose specimens had been cultured. Three patients did not have precipitating antibodies to any fungal antigen, but the same fungus was repeatedly isolated from sputums of two patients for over one year of period and from open lung biopsy specimen in the other patient. Most commonly involved species in pulmonary infection were A. fumigatus(70.3%) and C. albicans (at least 23.8%), followed by A. flavus, P. boydii, A. nidulans, etc. Out of fungi isolated from individuals(459), who were apparently not infected with fungi, molds were 66.0% and the rest, yeasts. Most commonly encountered molds were aspergilli(31.7%), followed by penicilli(16.3%), Cladosporium spp.(2.8%), Fusarium spp.(2.2%), etc. C. albicans(16.6%) was of course most common yeast in human sputums and the other species were seen in few.

Background : Central venous catheters(CVCs) area major source of nosocomial infection. Chlorhexidine-silver sulfadiazine coated catheters (CHSS) were developed to reduce the rate of CVC infection. However, the clinical effectiveness of CHSS in comparison wth non-coated catheter (NCC) remains to be evaluated. Methods : From January 2004 to December 2004 in medical intensive care unit (ICU) of Asan Medical Center, CVCs were inserted in 446 cases. We retrospectively analyzed characteristics of patients and catheterization, the catheter-related infection rate and colonization, microbiologic findings, and insertion sites (subclavian, jugular, femoral) according to the type of inserted CVCs (NCC: 187 cases, CHSS: 259 cases). Catheter related infection is defined as catheter related bacteremia and catheter related non-bacteremic sepsis. Results : 1) The mean age of the patients in each group was $62{\pm}16$ years, $63{\pm}15$ years (p=0.42), and sex ratio 94:50, 141:69 (p=0.9) in NCC and CHSS. Duration of ICU admission ($29{\pm}37$, $26{\pm}44$ p=0.42), duration of mechanical ventilation ($17{\pm}22$, $15{\pm}19$ p=0.17), and APACHE III score at the time of CVC insertion ($81{\pm}34$, $82{\pm}37$ p=0.61) were not different between both groups. 2) Mean duration of catheterization was 118 in NCC and 119 in CHSS (p=0.98). Number of catheter-days was 2176 days in NCC and 3035 days in CHSS. Catheter-related infection occurred in 9 (4.8%) cases receiving NCC and 4 cases (1.5%) receiving CHSS. Catheterrelated infection incidence per 1000 catheter-days was 4.1 and 1.3, respectively (p=0.04). CHSS was associated with a significant reduction of infection in jugular catheters regarding to insertion sites (p=0.01). 3) Microorganisms causing infection were Staphylococcus aureus (n=3), Candida (n=3), coagulase-negative Staphylococci (n=2), and Klebsiella (n=1) in NCC, and Candida species (n=2), coagulase-negative Staphylococci (n=2), Proteus (n=1) in CHSS. Conclusion : CHSS has significantly reduced the episodes of infection compared to NCC in jugular catheterization in medical ICU.
A 13-year-old neutered male Pomeranian, weighting 3 kg, presented with respiratory distress and depression. Radiographic examination revealed calcified ring-like opacities in the main pulmonary artery, mimicking thoracic foreign bodies. Additionally, right heart and main pulmonary artery enlargement and notable lung infiltrations were also observed. Echocardiography showed coil shaped structures in the main pulmonary artery with increased echogenicity compared to other nearby heartworms, which is consistent with calcified Dirofilaria immitis (heartworms). The dog was diagnosed with caval syndrome, which is the advanced and severe manifestation of heartworm infection. This report presents a rare case of calcified heartworm infection observed during a radiological examination, which resemble foreign bodies. Therefore, chronic heartworm disease should be considered as a differential diagnosis when radiopaque ring-like opacities are observed in the pulmonary artery on thoracic radiographs.

We report a case of disseminated Mycobacterium intracellulare infection in a 31-year-old man who had been diagnosed as having dermatomyositis and systemic lupus erythematosus 3-years prior. The patient developed a left pleural effusion M. intracellulare was repeatedly isolated from the pleural fluid. After antimycobacterial treatment, the patient's pleural effusion resolved, but a left knee joint effusion developed newly and M. intracellulare was cultured from the joint fluid. At present, the patient has been taking antimycobacterial medication for 15 months but his left knee joint fluid remains positive for M. intracellulare. To our knowledge, this is the second reported case of disseminated NTM infection in a non-HIV infected patient in Korea.
A unilateral pulmonary artery agenesis, which develops occasionally as one of the associated anomalies in congenital cardiovascular defect like as tetralogy of Fallot, is very rare anomaly as an isolated congenital defect. The diagnostic approach for the pulmonary artery agenesis is first suggested by the unique appearance of the involved lung on a routinely checked chest roentgenogram, because most patients are asymptomatic unless pulmonary infection had been superimposed. We have recently experienced a case of left pulmonary artery agenesis, which was diagnosed by perfusion scan, digital subtraction angiogram and then treated by left pneumonectomy in a 9 year-old boy, and presented hereby with the review of relevant literature.
Objectives: The object of this study was to compare perceived stress and quality of life among patients with HIV infection, patients with pulmonary tuberculosis and normal controls. Methods: Stress response inventory(SRI) and Symptom checklist 90-Revised(SCL-90-R) were used to measure perceived stress responses and psychopathology. Smithklein Beecham quality of life scale was used to measure quality of life. Results: Patients with HIV infection scored significantly higher on scores of tension, anger, depression, fatigue and frustration subscale of the SRI than those with pulmonary tuberculosis and normal controls. Scores of the SCL-90-R, somatization, depression, anxiety, hostility, phobic anxiety, paranoid ideation and psychoticism subscale were also significantly higher in patients with HIV infection than those with pulmonary tuberculosis and normal controls. Patients with HIV infection scored significantly lower in quality of life than those with pulmonary tuberculosis. In patients with HIV infection, age had a significantly negative correlation with scores of somatization, obsessive-compulsive, interpersonal sensitivity, phobic anxiety, paranoid ideation and psychoticism subscale of the SCL-90-R. but the level of education had a significantly positive correlation with somatization, obsessive-compulsive, interpersonal sensitivity, phobic anxiety, paranoid ideation and psychoticism subscale of the SCL-90-R. Conclusion: The results suggest that patients with HIV positive were likely to have higher levels of perceived stress response and psychopathology, and lower quality of life than those with pulmonary tuberculosis and normal controls.

During the last 10 years of period, one hundred patients with various pulmonary diseases were pneumonectomized upon at the Department of Chest Surgery of Pusan University Hospital. This paper is concerned with the clnical results of these patients along with the serious postoperative complications such as postoperative intrapleural infection and hemorrhage. The results were obtained as follows. 1.Left pneumonectomy was done in sixty-six of 100 patients [66 %] and the right one was done in the rest thirty-four[34 %]. The ratio between left and right was nearly 2:1. 2.Of all oostoperative complications, the intrapleural infection was most common, and these were 53 % in empyema thoracis and 12.7 % in pulmonary tuberculosis respectively. 3.More postoperative complications could be seen after right pneumonectomy than the left one. 4.It was thought that the postoperative intrapleural infection was closely correlated with the methods of pleural dissection at pneumonectomy, postoperatlve tube drainage, time of operation, massive hemorrhage during operation, prolongation of bleeding time, and dysfunction of the liver. 5.The repeated thoracenteses with infusion of neomycin into the infected thoracic cavity and intravenous administrations of the high units of penicillin were effective in treatment of the postoperative intrapleural infection, however, the refractory cases have to be cured by thoracoplasty with open window. 6.Immediate secondary open thoracotomy appears to be the method of choice in life saving who developed massive intrathoracic hemorrhage after pneumonectomy. 7.The mortality rate was 10 % in our cases and the main causes of death were postoperative respiratory insufficiency, pulmonary edema, hemorrhage and sudden cardiac arrest.
Objective: To assess induction effects of Chlamydia pneumoniae (Cpn) on lung cancer in rats. Methods: A lung cancer animal model was developed through repeated intratracheal injection of Cpn (TW-183) into the lungs of rats, with or without exposure to benzo(a)pyrene (Bp). Cpn antibodies (Cpn-IgA, -IgG, and -IgM) in serum were measured by microimmunofluorescence. Cpn-DNA or Cpn-Ag of rat lung cancer was detected through polymerase chain reaction or enzyme-linked immunosorbent assay. Results: The prevalence of Cpn infection was 72.9% (35/48) in the Cpn group and 76.7% (33/43) in the Cpn plus benzo(a)pyrene (Bp) group, with incidences of lung carcinomas in the two groups of 14.6% (7/48) and 44.2% (19/43), respectively (P-values 0.001 and <0.000 compared with normal controls). Conclusions: A rat model of lung carcinoma induced by Cpn infection was successfully established in the laboratory for future studies on the treatment, prevention, and mechanisms of the disease.
