  • In medically vulnerable areas in Korea, public health doctors play a significant role in providing not only general medical care but also emergency medical services to the local residents. However, it has been observed that public health doctors generally lack field experience, resulting in insufficient ability to handle emergency patients and to effectively use medical equipment. This study confirmed the effectiveness of education after conducting remote education using smart glasses on how to use medical equipment necessary for public health doctors. Specifically, real wear was used for smart glasses for medical equipment utilization education, and 10 public health officials in 10 islands in Shinan-gun were targeted. After the training, both the effect of using the equipment and the level of satisfaction were 3 or higher. Therefore, it was confirmed that remote education using smart glasses can be usefully used for public health doctors in medically vulnerable areas.

  • Objectives: To investigate the state of medical care around health sub-centers, public health doctors' participation and opinion in the process of district public health programs. Methods: The study included 1,036 public health doctors who worked at health sub-center all over the country. The data were collected for Feb, 2002 using self-administered questionnaire by mail. Results: One or two doctors were working at health sub-center and 33.5% of health sub-centers was located in the region of the separation of prescription and dispensing. There were another medical facilities in 45.9% of the administrative district(eup or myon) where health sub-centers were located. The count of medical utilization went down to 14.8${\pm}$14.8 per a day in Nov, 2001 from 18.0${\pm}$15.6 in May, 2000, and the decline was much more in the region of the separation of prescription and dispensing. Among public health programs in health sub-centers, public health doctors participated mostly in preliminary medical examination for vaccination and least in health education. They participated in implementation rather than planning or evaluation of health program. Over a half of public health doctors were found to be positive that health programs implemented in their health sub-centers would promote the level of health in community people and they were willing to participate in district public health program if community people were in need. Conclusions: Recently health sub-centers are required to turn into health promotion facilities rather than medical practice facilities. Health program in health sub-centers will be advanced in both quality and quantity by turning the role of public health doctors who have provided medical services mainly into managing health program. Persistent education about managing health program and the policy to motivate participation in health program should be provided for public health doctors.

Methods : Data were collected from 222 of 996 public health oriental medical doctors(PHOMD) using the structured questionnaire by e-mail. Collected data were analyzed through frequency analysis, T-test, and ANOVA using SPSS 12.0 and significant level was 0.05. Results : 59.5% of the respondents said that the TKM-PHP are not efficiently executed because there are not enough motivations to entice oriental medical doctors in charge of the programs and because PHOMD and the government officials in charge lack in relevant experiences and skills. A majority of the PHOMD recognize a need for activating the TKM-PHP but less actively participate in the programs since there is not a good rewarding system and there are neither standard manuals nor methodological guidelines for the programs. In order to activate the TKM-PHP, it is urgent to employ full-time oriental medical doctors and to continually and systematically appoint the professional manpower in charge. And it is also needed for the state to secure an adequate budget and prepare schemes for persistently train such professionals. Conclusion : Activating the TKM-PHP will lead to the development of TKM, but there are still such problems as lack of the professional manpower in charge, an excessive burden of the duties of PHOMD, lack of program manuals, and a poor system for evaluation. To solve these problems, it is advised to construct foundations for administerial supports, draw up a standard manual, prepare a system for evaluation in consideration of the characteristics of TKM, and hire professionals to ensure sustainable programs.

  • Background: This study aims to contribute to the adjustment of the appropriate doctor manpower by analyzing the distribution, supply and demand, and estimation of the doctor manpower. Methods: This study utilized the medical personnel data of the Ministry of Health and Welfare, population trend data of the National Statistical Office, and health insurance benefit performance data of the National Health Insurance Service. Based on 2021, we compared the number of doctors in actual supply and the number of doctors in demand according to the amount of medical use by gender and age for 250 regions. Logistic regression analysis and scenario analysis were performed to estimate the future medical workforce by considering the demand for doctors according to the future demographic structure, the size of the quota in medical schools, and the retirement rate. Results: There were 186 regions in which the supply of doctors was below average, and the average ratio of the number of doctors in supply to demand in the region was 62.1%. Conclusion: In order to increase the number of active doctors nationwide to at least 80%, 7,756 people must be allocated. The number of doctors in demand is estimated to decrease after increasing to 1.492 times in 2059. The future projected number of doctors is expected to increase to 1.349 times in 2050 and then decrease taking into account the doctor quota and the retirement rate.

  • Korea's quarantine response to the novel coronavirus (coronavirus disease 2019 [COVID-19]) pandemic is based on the sacrifice of health professionals, especially public health doctors (PHDs) who were called out first and put in the first place in the country. PHDs performed major quarantine tasks, such as collecting samples from screening clinics and epidemiological investigations, in various parts of the country, including the Daegu area, where the first large-scale COVID-19 confirmed cases in Korea and explosively increased. Because of their position as fixed-term civil servants, however, PHDs' professionalism as doctors was ignored, and they were not properly compensated for their work. They were also exposed to problems such as a high risk of infection, mental suffering, and various human rights violations. We must prepare concrete measures to improve the fundamental treatment of PHDs and protect their human rights in order to prepare for a possible infectious disease pandemic in the future.

This issue is so important that it raises a question of accessibility, availability, continuity of care and equity of rural area people's health care utilization. To solve these problems and to satisfy the basic demand of oriental medical service in rural areas, the oriental public health doctors were placed in rural health centers since 1998. The main objectives of this study are twofold: to measure the cognition and attitude of health center directors on the strategies for utilizing oriental public health doctors and to provide basic data for improving the health manpower management program. Data have been collected by way of the self-administrative questionnaires. Developing the questionnaire, the literature review on the previous studies and delphi method were carried out. The response rate was 38.7%. The results of this study are summarized as follows; 1. community people respond positively on the oriental medical service activity in health center. 2. In regard to workloads of oriental public health doctor, 'appropriate' was 81.1% and 'burdensome' was 18.2%, respectively. 3. The 94.0% of respondents thought that the oriental medical service will be continued. 4. To activate oriental medical service in health center, the sufficient budget and provision of aid workers is a necessity. 5. The 75.5% of health center directors respond positively on the allocation of oriental public health doctor to health sub-centers. 6. Health center directors agreed that oriental public health doctor should perform the clinical service as well as prevention and health promotion activity. These results recommend that oriental medical service in health center should be continued gradually, and oriental public health doctors working at health center perform their work efficiently. Undoubtedly, their activity should be more focused on health promotion and disease prevention than daily patient care. For achieving this objective, more support of governmental policy is essential for utilizing oriental public health doctor and better health of the rural area community people.

  • Recently, several social issues have raised public concerns about a deficiency of doctors. The government says it will solve the problem by increasing the number of doctors, saying essential medical service is collapsing. However, the medical community argues that there is a fundamental problem with Korea's health care delivery system. The government needs to take advice from the medical community to make policies in the right direction. And the press should no longer provide the public with misinformation that doctors and the public are opposed to. In this article, I would like to mention problems among current medical services and suggest healthcare policies for them.
