This study was designed to investigate the effects of an aquatic exercise program on the physical function and depression among the community elderly. The subjects were 41 cases of aged over 65 who went to elderly college in Hawsung-Kun, Kyeongkee province. Seventeen of them were assigned to control group, while twenty four subjects to the experimental group. The period of data collection was from June., 1999 to November., 1999. The aquatic exercise program was developed by the author with the assistance of rehabilitation medicine doctor and physical therapist in the public health center. The program was carried out once a week for 6 months in regular swimming pool. Data were analyzed with t-test, $X^2$ test, ANCOVA. paired t-test using SAS 6.12. Results were obtained as follows: 1. The increments in the range of motion of the shoulder and knee joints in experimental group after exercise was significantly greater than those in the control group. 2. The physical pain score decreases significantly after the exercise in the experimental group only. 3. The experimental group revealed lower level of depression after exercise than the level before the exercise, whereas control group showed no significant changes in this level. These findings may indicate that the aquatic exercise program is effective in increasing the range of motion of shoulder and knee joints and also effective in decreasing pain, the level of depression. Accordingly. the aquatic exercise program can be adopted as an health promotion program for community elderly.
A purpose of this study is to study on Rene Descartes's mind-body theory in medical aspect. Though Rene Descartes was not so much a doctor as a philosopher, he had health and medical science at heart. When he came into the world in 1596, he was in poor health. Therefore, he suffered from his bad health. Descartes's ideas absolutely colored Western thought for three hundred years, especially, his mind-body theory, mechanistic life-view, and reductionism had important effect on medical study and science of public health. As a rule, we know that his mind-body theory was applicable to mind-body dualism, and his mind-body dualism was connected with biomedical model of medicine. But by this study, his mind-body theory was not only mind-body dualism but also mind-body monoism. And he asserted mind-body interaction too. In other words, he advocated mind-body dualism in scientific aspect, but he knew mind-body monoism from his experence. He confessed this fact to Princess Elizabeth of Bohemia, he wrote mind-body interaction in $\boxDr$Discours de la methode$\boxUl$, $\boxDr$Meditationes de prima philosophia$\boxUl$, and $\boxDr$Traite des passions de 1'ame$\boxUl$ etc. However, only mind-body dualism of his mind-body theories was written in our medical text book, morever mental realm was excluded from the persuit of learning Descartes advocated a mechanistic world-view and mechanistic life-view, he regarded human body as a machine part. And a paticent corresponds to a troubled machine, a doctor deserves a repairman. But this point of view made holistic understanding of man impossible. Descartes divide the whole into basic building blocks, we named the approach Reductionism. Reductionism led to ontological concept in medical science, bacteriology established 'specific cause-specific disease-specific therapy'. We examined medical influence of Descartes's thought, we need to draw out a philosophic basis of medical science and science of public health by a close study of his records.
Objectives: The purpose of this study was to investigate the factors contributing to intentions of quitting smoking, and use of smoking cessation services for daily smokers, and willingness of using smoking cessation services by the public health centers. Method: A total of 1,403 cases of male smokers were collected by a telephone survey. The age of smokers in the study ranged from 20 to 59. Socio-demographic variables, cigarette consumption behaviors, quit attempts and intentions of quitting smoking, and use of clinical services provided by the local public health centers were analysed using SPSS program. Result: About 77.8% of the smokers consumed a pack of cigarettes or less per day. About 20% of the smokers have used quit aides such as nicotine patch, gum, quit cigarettes, and acupuncture. These variables were associated with prior quit attempts in the past 1 year. Older age, degree of addiction(less time spent without smoking after a smoker gets up in the morning), prior quit attempts, panning to quit in one month, doctor's advice to quit were the major predictors of using the smoking cessation services provided by the public health centers. Conclusion: Health educators have to entice physicians to give smoking cessation advice to smokers, and have to provide many different types of educational programs to help quitting smoking.

Objectives : The frequency of visiting clinics in Jejudo for treating atopic dermatitis (AD) has been reported to be higher than that for other counties of Korea. The aim of this work was to estimate the prevalence of AD for the students of the primary, middle and high schools in Jejudo, Korea. Methods : We planned to evaluate about 5,000 students, so classes were randomly selected by random systematic sampling methods. The parents of all the students in the selected classes were asked to respond to a structured questionnaire concerned with the prevalence of AD. Results : The prevalence of AD based on ever being diagnosed by a doctor, on currently being diagnosed by a doctor, on ever being treated for AD by a doctor, on currently being treated for AD by a doctor was 25.57% (95% CI=25.3-25.8), 14.67% (95% CI=14.5-14.9), 21.90% (95% CI=21.6-22.2) and 14.41% (95% CI=14.2-14.6), respectively. Conclusions : On comparing the prevalence of AD, the students in Jejudo had a higher number of recurrence events in spite of their lower number of cases. Further studies are needed to evaluate these aspects of AD.
This study aims to explore how the transformational leadership (consisting of three factors"charisma, individualized consideration, and intellectual stimulation) and the organizational culture (typified by developmental, rational, consensual, and hierarchical culture) are related to the organizational effectiveness (consisting of job satisfaction, organizational commitment, and intention to leave a job) of dental clinics. We found the following empirical results through a survey of 378 persons who work in dental clinics. Firstly, the respondents acknowledged charismatic leadership as the typical type of leadership ; as for organizational culture, a culture of consensus is recognized as the representative type. Secondly, transformational leadership has different effects according to the type of organizational culture: a positive influence in developmental and rational cultures, but a negative influence in consensual and hierarchical cultures. Thirdly, developmental culture has proven to be the most suitable for increasing job satisfaction and organizational commitment. The developmental, rational, and consensual cultures have all contributed to reducing intention to leave among clinic staff, whereas in the hierarchical culture, intention has been elevated. Fourthly, transformational leadership has a positive effect on the job satisfaction and organizational commitment; the intent to leave has been lessened under individualized consideration and charismatic leadership, but has been increased under leadership of intellectual stimulation. Fifthly, it is consideration and charisma that help to increase organizational effectiveness. These influences will become more effective through a developmental culture. Based on the above empirical results, we propose practical measures to improve the organizational effectiveness of clinics, in particular dental clinics. In order to build developmental culture, the doctor (manager) should produce an atmosphere in the clinic in which staff members are able to create and jointly own ideas and then promote awareness of staff participation. Additionally, in order to bring leadership of charisma and consideration into full play, the doctor should shape a relationship of mutual trust mainly by recognizing and praising the work of clinic staff. Finally, the doctor needs to acknowledge that organizational effectiveness can be significantly improved by increasing the transparency of the business.
