목적 : 본 연구는 정신장애인의 직업재활 평가도구를 확인하고 분석함으로써 작업치료 영역에서 임상적으로 유용하게 활용할 수 있는 정신장애인 직업재활 평가도구의 개발을 위한 기초자료로 활용하는 것이다. 연구방법 : 문헌 검색은 Pubmed 데이터베이스를 사용하였으며, 분석 내용은 개발연도, 평가방식, 문항 수, 척도, 평가 문항을 분석하였다. 분석 방법은 각 평가 항목을 기능, 내적요인, 환경, 정신증상의 4가지 범주로 구분하여 각 평가도구의 평가 요소에 대해 파악하였다. 결과 : 검색어를 통한 Pubmed 검색 시 161개의 문헌이 검색되었고, 선정 방법에 따라 Griffiths Work Behavior Scale (GWBS), Occupational Functioning Scale (OFS), Social and Occupational Functioning Assessment Scale (SOFAS), Work Ability Index (WAI), Work Behaviour Inventory (WBI), Work Environment Impact Scale (WEIS), Work and Social Adjustment Scale (WSAS)의 7개 평가도구가 선별되었다. 모든 평가도구의 평가항목은 직무와 관련된 기능평가를 포함하였으며, 각 평가도구의 목적에 따라 내적요인, 환경, 정신증상에 대한 측정이 이루어지는 것으로 나타났다. 결론 : 직업 기술은 신체, 인지, 사회기술, 대처능력 등 다양한 기능이 복합적으로 작용하는 기술이다. 따라서 본 연구에서 분석한 기능, 내적요인, 환경, 정신증상의 4가지 요소가 포함될 필요가 있으며, 평가과정에서 발생하는 물리적 제약을 보완하기 위해 ICT 융합 평가도구의 개발함으로써 보다 효율적인 정신장애인 직업재활 평가가 이루어져야 할 것이다.
Introduction: Breast cancer is the most common cancer among Jordanians. Breast cancer patients suffer from several negative consequences after treatment and these include pain, fatigue, sexual problems, appearance and body image concerns, with psychological dysfunction. This could affect the patient quality of life and psychological well-being. To the best of our knowledge, there is no published quantitative data on the quality of life and psychological well-being of breast cancer patients in Jordan. The objective of this study was to obtain such data and assess predictors with calculated scores. Methods: In this cross-sectional study conducted among breast cancer patients in Jordan diagnosed in 2009 and 2010, assessment was performed using the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire (EORTC QLQ-C30), the Breast Module (QLQ-BR23) and the Hospital Anxiety and Depression Scale (HADS). Clinical, demographic and psychosocial indicators that could predict patient quality of life scores were collected. Results: The number of patients interviewed was 236 (mean age=$50.7{\pm}10.7$ years). The mean Global Health score for the QLQ-C30 was $63.7{\pm}20.2$ SD. Among functional scales, "social functioning" scored the highest ($mean=78.1{\pm}28.6$ SD), whereas "emotional functioning" scored the lowest ($mean=59.0{\pm}SD\;33.5$). For the QLQ-BR23, the worst scores within the functional scales were for "body image" ($mean=52.1{\pm}36.8$ SD) and "future perspective" ($mean=52.9{\pm}38.5$ SD). The worst symptom was "upset by hair loss" ($mean=69.8{\pm}43.0$). The mean HADS scores was $18.{\pm}9.0$ SD. Out of study participants, 53% scored abnormal on the anxiety scale and 45% on the depression scale. Severe depression and severe anxiety were detected among 8% and 14% of study participants, respectively. Statistically significant predictors for individual scores were similar to those reported in published studies, such as the presence of recurrence since baseline, family history of cancer, low educational status, current social problems, extent of the disease, presence of financial difficulties, and employment status. Conclusions and Recommendations: Breast cancer survivors in Jordan have overall good quality of life scores when compared with patients from Western countries. However, their psychological wellbeing is more impaired. There is an urgent need for psychosocial support programs and psychological screening and consultation for breast cancer patients at hospitals of the Ministry of Health in Jordan.
Purpose: Children treated for Hirschsprung disease (HD) are adversely affected by fecal incontinence and soiling. This can be detrimental to their physical, psychosocial quality of life (QoL) and impacts the normal functioning of their family. QoL studies in HD are predominantly from developed countries. We measured general quality of life, impact on family and functional bowel status using validated questionnaires in HD children in a developing country. Methods: Patients with HD, treated in a tertiary paediatric institution in India between 2010 and 2017, were identified. Patients and/or their proxy completed the Pediatric Quality of Life and Family Impact Module questionnaires. Functional outcomes were assessed using Rintala's score. Results: A 86 children and their parents participated in the study. Majority had rectosigmoid disease (67.4%) and underwent Soave's endoanal pull through (74.4%). A 21% of patients had low Rintala score indicating poor functional bowel outcomes. Only 11% of children had poor QoL scores. Family functioning outcomes were also severely affected in the same subgroup of patients. There was statistically significant correlation between Rintala score and QoL scores (p-value<0.001). Disease severity, type of surgery, and duration of follow-up did not have a statistically significant impact on the QoL. Conclusion: QoL in children with HD was comparable to the general population. Bowel dysfunction affects a notable number of children and was the most significant determinant of poor QoL.
Objectives : Antipsychotic-induced hyperprolactinemia causes physical symptoms, such as amenorrhea, galactorrhea, gynecomastia, sexual dysfunction, and bone density loss, as well as psychiatric symptoms, such as depression and cognitive impairments. This study aimed to clarify the associations among hyperprolactinemia caused by antipsychotics in patients with schizophrenia, psychiatric pathology, and psychosocial factors. Methods : Ninety-nine patients with schizophrenia in the psychiatry department of a university hospital were registered between 2015 and 2017. All participants were assessed using structured questionnaires to elucidate psychopathology, social function, quality of life, and hyperprolactinemia-related side effects. The standard levels for hyperprolactinemia were 24ng/mL for women and 20ng/mL for men. Results : The average prolactin levels were $73.45{\pm}49.37ng/mL$ in patients with hyperprolactinemia and $9.16{\pm}6.42ng/mL$ in those without hyperprolactinemia. The average prolactin level in women was significantly higher than that in men(p=0.04). Risperidone was most commonly administered in patients with hyperprolactinemia(58.1%, p<0.01), while aripiprazole was most commonly administered in those without hyperprolactinemia(44.7%, p<0.01). Patients with hyperprolactinemia had significantly higher Positive and Negative Syndrome Scale(p=0.03) and Patient Health Questionnaire-9(p=0.05) scores and had significantly lower Social and Occupational Functioning Assessment Scale(p=0.04) and Strauss-Carpenter Levels of Functioning Scale(p=0.03) scores than patients without hyperprolactinemia. There were no significant differences in side effects or quality of life between the two groups. Conclusion : These findings demonstrate that hyperprolactinemia confers negative effects on depression and social function, but does not directly affect the quality of life. These results suggest that patients with schizophrenia who take antipsychotics that increase prolactin or cause side effects of hyperprolactinemia need to be assessed and receive interventions for depression.
Purpose: The purpose of this study was to understand and describe the experiences of patients with Meniere disease. Methods: Data were collected from February 19, 2019, to February 5, 2020, through individual in-depth interviews with 13 Meniere patients. Verbatim transcripts were analyzed using Colaizzi's phenomenological analysis. Results: The experiences of patients with Meniere disease were clustered into the following four themes from 22 meaning units: 1) Complex unfamiliar symptoms that shatter both balance of the body and peace of the heart; 2) A disease that medical treatment and health professionals cannot cure; 3) Suffering hardships that cannot be understood by non-Meniere sufferers; and 4) Making daily efforts to become healthier. Symptoms of Meniere disease are life-shattering and depressing because they are neither visible nor easily curable. Over time, as they accepted the reality of living with the disease, the participants would shift their focus from complete symptomatic cure to leading a healthy and more balanced life. Conclusion: This study shows that Meniere disease has a pervasive impact on all aspects of the patients' lives. Patients are prone to experiencing restrictions in their social functioning and activities. They also experience psychosocial problems due to the unseen nature of their symptoms. This study elucidates the experiences of Meniere patients and the need for nursing intervention to help improve their quality of life and ability to self-manage. Lastly, this study shows the need for a coordinated interdisciplinary approach to raising public awareness of the disease.
This study is an attempt to evaluate the effect of the medication and symptoms self-care education on the psychiatric patients' quality of life. This exploratory study was designed to a quasi-experiment of pretest-postest with a non equivalent control group. Subjects of 40 patients who were discharged from a mental hospital in Pusan were selected to be experiment and control group, with each group consisting of 20. The research period was from March, 6 to May, 29, 1999 and the pre-post test was given before and after implementing MSSE to the both experiment and control group. The results finding were summarized as follows ; 1. subjects who attended self-care education scored significantly higher than control group in quality of life.(Z=-2.06, P=0.034) 2. Those who were in the experiment group reported more significant higher scores than control group in item of quality of life, living situation(A=-2.62 P=0.012), relations with spouse (Z=-2.31 P=0.038) and children (Z=3.37, P=0.008), fear (Z=-1 95, P=0.049) and anger(Z=-2.07 P=0.041), work functioning (Z=-2.34, P=0.021), environmental adjustment. Sexual behavior is crucial in life, yet comparatively little is known about the mechanisms in the sexual response in humans. A lot of theories and models have been developed to explain about the process of the sexual response in humans. The first model of sexual function was described by Masters and Johnson, defined the four-phase model (phases of excitation, plateau, orgasm and resolution). Helen Kaplan proposed a slightly different model of human sexual response by adding the conception of the desire phase. Some years later, a new model of circular sexual response pattern was described by Whipple and Brash-McGreer, who acknowledged the cyclic nature of women's sexual response. Basson presented an alternative model of women's normative sexual function, which featured a responsive form of desire in women's sexual response. Bancroft developed a new theoretical model, the Dual Control Model, which postulates sexual response and arousal is ultimately determined by the balance between the sexual activation or excitation system and the sexual inhibition system. The Sexual Tipping Point is a model created by Perelman, suggesting that a sexual response is determined by a balance between excitatory or inhibitory factors that may be psychological, organic, psychosocial, or cultural. A comprehensive understanding of sexual response and function is of paramount importance for the psychiatrist to study sex, offer counseling to the patient on sex, and practice sex therapy. In this literature, models of sexual response would be reviewed to understand the knowledge of the sexual functioning in humans.
Homelessness became a major social problem in Korea. And the homeless presents many kinds of problems in social functioning. In studies on the homeless, Physical and mental health, alcohol and subtance abuse, the vulnerability of the social support have been frequently described as major characteristics of the homeless. Of the homeless' psychosocial characteristics, this article focused on the lack of the social support, which plays a important role on the exit from homelessness. The lack of social support has been mentioned as cause for homelessness. But a few researchers stressed the perspective that the homelessness causes the lack of social support. This article analyzed empirically the lack of social support among the homeless in terms of "social disaffiliation". This article hypothesized: (a) as longer homeless duration, the extent of the perceived social support would be decreased, (b) as longer homeless duration, the magnitude of the social network would be decreased, (c) as longer homeless duration, the ratio of the homeless in social network would be increased. Survey data collected from 355 homeless persons (sheltered homeless) at 82 shelters in Seoul. The result of analysis supports 'social disaffiliation hypotheses'. The result of this study suggests that the social work intervention to strengthen social support and social network be critically important in homeless shelter.
본 연구의 목적은 생식기 기형의 대표적인 하위유형인 요도하열과 잠복고환 영유아와 양육자의 심리 사회적 특성을 탐색하는 것이다. 생후 6~38개월 요도하열(103명), 잠복고환(48명)을 대상으로 생식기 기형과 관련한 걱정과 사건 충격, 수치심을 탐색하였다. 이들을 정상통제군(131명) 양육자와 비교하여 영아의 기질과 양육자의 정신 건강, 양육자의 사회적 지지와 결혼 만족도, 양육 스트레스와 대처기제를 비교하였다. 결과, 요도하열군 모는 '배뇨와 신체기능'과 '출산 능력'을, 잠복고환 모는 '마취에 대한 부담'과 '출산 능력'을 가장 걱정하였다. 또 요도하열 모는 임상수준의 심리적 충격을 보고하였다. 정상군과의 비교에서, 요도하열 영아는 정상영아에 비해 부정적 정서가 높았고, 잠복고환 영아는 주의 전환성이 유의하게 낮았으며, 요도하열과 잠복고환 모는 정상군모보다 대인 예민성이 낮았다. 사회적 지지에서 세 집단 간 차이는 없었으나, 잠복고환 모는 정상군 모에 비해 원가족과의 갈등이 적다고 보고하였다. 요도하열과 잠복고환 모는 정상군 모보다 양육 스트레스 중 부모의 고통은 낮았고, 대처 전략을 많이 사용하였다. 양육 스트레스 총점이나 까다로운 자녀, 역기능적 상호작용에서는 집단 간 차이는 나타나지 않았다. 결과에 대해 임상적 함의와 연구적 함의를 논의하였다.
본 연구는 중학생들의 건강관련 삶의 질과 이에 영향을 미치는 요인으로 사회적 지지와 학급응집력의 영향을 알아보고자 실시하였다. 조사는 대구광역시에서 임의로 선정된 3개 학교의 516명을 대상으로 실시하였으며 설문지의 목적을 이해하고 동의한 학생들이 자기기입식으로 응답하도록 하였다. 연구결과 학급응집력에는 사회적 지지 중 친구지지와 교사지지가 유의하게 영향을 미쳤는데 친구지지와 교사지지가 높을수록 학급응집력은 유의하게 증가하였다. 중학생들의 건강관련 삶의 질 점수는 총점수 81.1점이었으며 사회적 영역이 87.1점으로 가장 높았으며 정서적 기능 점수가 73.8점으로 가장 낮았다. 학급응집력 및 사회적 지지와 건강관련 삶의 질의 관련성은 신체적 영역은 교사지지가 유의하게 영향을 미쳤으며 사회심리적 영역과 전체 영역은 학급응집력, 친구지지, 교사지지가 유의하게 영향을 미쳤다. 연구 결과 중학생에게는 본인이 속해 있는 학급에 대한 응집력을 높일 수 있도록 친구관계, 교사-학생관계를 원만하게 하여야 할 것이며, 이들 친구지지와 교사지지는 직접적으로도 삶의 질에 영향을 미치므로 사회적 지지를 받을 수 있도록 하여야 할 것이다.
