성인 알코올사용장애 환자에서 주의력결핍 과잉행동 증상 유무에 따른 임상적 및 신경심리학적 특성 (Clinical and Neuropsychological Characteristics of Adult Alcohol Use Disorder Patients with or without Attention-Deficit Hyperactivity Symptom)

  • Objectives : Alcohol use disorder (AUD) is often comorbid with adult attention-deficit hyperactivity disorder (ADHD) and other psychiatric illnesses. When associated with other mental problems, the prognosis of the AUD can be more serious. This study shows research on the clinical and neuropsychological characteristics according to whether or not ADHD symptoms and AUD were comorbid. Methods : A total of 64 adult inpatients who completed AUD scales about adult ADHD, alcohol dependence, depression, anxiety, and impulsiveness. They also completed neuropsychological tests about attention and executive function. According to the Adult ADHD Self-Report Scale score, patients were categorized into two groups (ADHD symptom positive/negative group). Results : Fourteen among the 64 subjects were part of the ADHD symptom positive group (21.9%). They had statistically significant shorter abstinence periods and a higher rate of history of 'rule violation during school' and 'physical damage in childhood' compared to the ADHD symptom negative group. Conclusion : The comorbidity rate with ADHD symptom in adult AUD is higher than the general population. The ADHD symptom positive group suffered from more severe and refractory AUD. Considering the more frequent history of rule violation during school in the ADHD symptom positive group, the association between ADHD and AUD may be mediated by conduct disorder. Therefore we suggest the necessity of careful evaluation and intervention in children and adolescents with ADHD and conduct disorder.

지역사회 통합 케어서비스 방안 마련을 위한 시립병원 입원환자의 동반질환 분포와 재원일수 및 진료비와의 관련성 (Relationship between the Distribution of Comorbidity and Length of Stay and Medical Cost for Planning Integrated Community Care Services among Inpatients at a Seoul Municipal Hospital)

  • Background: This study is to investigate the association between the distribution of multimorbidity and length of stay and medical expenses among inpatients in a municipal hospital to achieve an integrated care setting. Methods: We used the exploratory factor analysis and the generalized estimating equation model to analyze the data from patients living in the northeast region of Seoul, who were hospitalized from January 2017 to December 2017 in a municipal hospital. Results: As a result of the factor analysis, seven types of multiple chronic diseases were classified. Among the elderly patients admitted to municipal hospitals, the burden of medical expenses was mainly influenced by the length of stay (B=310,719, p-value <0.0001), not the type of disease (all not significant). Length of stay were mainly due to psychiatric illness (factor 1: B=4.323, p-value <0.0001) related to the brain and metabolic diseases (factor 2: B=2.364, p-value=0.003). Conclusion: This study showed that the medical expenses of the elderly patients were largely due to prolonged hospitalization, not multimorbidity. Therefore, it is necessary to develop an integrated care paradigm strategy cope with the multimorbidity of the elderly in the community and to alleviate the socio-economic burden.

주의력결핍 과잉행동장애에서 한국아동 인성평정척도의 임상적 적용 (Clinical Application of the Korean Personality Rating Scale for Children in Attention-Deficit Hyperactivity Disorder)

  • Objectives : The objective of this study was to compare the Korean Personality Rating Scale for Children (K-PRC) profile between children with attention-deficit hyperactivity disorder (ADHD) and typically developing children. We also aimed to investigate the association of K-PRC and ADHD symptoms. Methods : Ninety-nine youth (age $8.3{\pm}2.4$ years, 72 boys) with ADHD and 84 controls (age $9.2{\pm}2.5$ years, 43 boys) were recruited from the Department of Pediatric Psychiatry of the Asan Medical Center Children's Hospital. Diagnoses of ADHD and comorbid psychiatric disorders were confirmed with the Kiddie-Schedule for Affective Disorders and Schizophrenia-Present and Lifetime Version (K-SADS-PL). The parents of the subjects completed the ADHD rating scale, and K-PRC. Independent t-tests, analysis of covariance, partial correlation analyses, and Mc Nemar test were used for analysis. Results : Children and adolescents with ADHD showed higher K-PRC scores in verbal development, physical development, depression, delinquency, hyperactivity, family dysfunction and psychoticism. Delinquency and hyperactivity were significantly correlated with parent-rated ADHD rating scales and ADHD scores on K-SADS-PL. The hyperactive/impulsive and combined subtypes showed higher scores on hyperactivity and delinquency than the inattentive subtype, and the inattentive subtype showed higher scores on depression and social dysfunction of the K-PRC. Conclusion : Our results suggest that K-PRC could be used to comprehensively evaluate symptoms, combined psychopathologies, developmental delay and family dysfunction of children with ADHD.

청소년 자폐스펙트럼장애에서 사회기술훈련의 효과에 미치는 우울, 불안 증상의 영향 (The Effects of Anxiety and Depression on Treatment Outcome of Social Skills Training in Adolescents with Autism Spectrum Disorder)

  • Objective : The purpose of this study was to investigate effects of depression and anxiety on treatment response of social skills training in adolescents with autism spectrum disorders (ASD). Methods : A total of 107 teenagers that participated in the Program for the Education and Enrichment of Relational Skills (PEERS$^{(R)}$) between 2011 and 2015 were analyzed using linear regression. Outcome measures used were the Korean version of Social Responsiveness Scale (SRS), the Test of Adolescent Social Skills Knowledge-Revised (TASSK-R), and the Korean version of Autism Diagnostic Observation Schedule (K-ADOS). Depression and anxiety were assessed by the Korean version of Child Depression Inventory (CDI), the Korean version of State and Trait Anxiety Inventory for Children (STAIC), and the Korean version of Child Behavior Checklist (K-CBCL). Results : As a result, depressive symptoms measured by CDI (p<0.05) and anxiety symptoms measured by STAIC (p<0.05) had significant influence on score change of social interaction domain of ADOS. It remained significant even after the baseline score was adjusted as covariates (p<0.05). Conclusion : We observed that pre-treatment anxiety and depressive symptoms of teenagers had significant effects on the treatment outcome of PEERS$^{(R)}$, especially in terms of face-to-face social interaction.

Prevalence of Posttraumatic Stress Disorder Among North Korean Defectors During Preparation for Early Settlement

  • Objective : This study examined the prevalence of posttraumatic stress disorder (PTSD) among North Korean defectors during preparation for early settlement. In addition, the correlation between PTSD and other mental disorders with suicidal ideation was assessed. Methods : A total of 639 North Korean defectors aged 19-65 years were included from resettlement and training centers. PTSD was assessed using the PTSD Diagnostic Scale (PDS). The prevalence of other mental disorders and suicidal ideation were assessed using the Korean version of the Psychiatric Diagnostic Screening Questionnaire (K-PDSQ). Results : A total of 6.8% of North Korean defectors had a PDS score >23, which was suggestive of a diagnosis of PTSD. Prior repatriation to North Korea was significantly associated with PTSD. The defectors with PTSD had a higher prevalence of other mental disorders (major depressive disorder prevalence: adjusted odds ratio [AOR], 14.5; 95% confidence interval [CI], 6.7-31.2, panic disorder prevalence: AOR, 9.5; 95% CI, 4.6-20.0, generalized anxiety disorder prevalence: AOR, 5.6; 95% CI, 2.4-13.2). No significant association was found between suicidal ideation and PTSD. Conclusion : North Korean defectors had a relatively high prevalence of PTSD and suicidal ideation during preparation for early settlement. Cohort studies are needed to assess the lasting effects of PTSD on North Korean defectors' settlement in Korea for a longer period.

Prevalence and Impact of Comorbidities in Individuals with Chronic Obstructive Pulmonary Disease: A Systematic Review

  • This study aimed to describe the prevalence of comorbidities associated with chronic obstructive pulmonary disease (COPD) and their relation with relevant outcomes. A systematic review based on the PRISMA methodology was performed from January 2020 until July 2021. The MEDLINE, Lilacs, and Scielo databases were searched to identify studies related to COPD and its comorbidities. Observational studies on the prevalence of comorbidities in COPD patients and costs with health estimates, reduced quality of life, and mortality were included. Studies that were restricted to one or more COPD pain assessments and only specific comorbidities such as osteoporosis, bronchitis, and asthma were excluded. The initial search identified 1,409 studies and after applying the inclusion and exclusion criteria, 20 studies were finally selected for analysis (comprising data from 447,459 COPD subjects). The most frequent COPD comorbidities were: hypertension (range, 17%-64.7%), coronary artery disease (19.9%-47.8%), diabetes (10.2%-45%), osteoarthritis (18%-43.8%), psychiatric conditions (12.1%-33%), and asthma (14.7%-32.5%). Several comorbidities had an impact on the frequency and severity of COPD exacerbations, quality of life, and mortality risk, in particular malignancies, coronary artery disease, chronic heart failure, and cardiac arrhythmias. Comorbidities, especially cardiovascular diseases and diabetes, are frequent in COPD patients, and some of them are associated with higher mortality.

Preliminary Report of Validity for the Infant Comprehensive Evaluation for Neurodevelopmental Delay, a Newly Developed Inventory for Children Aged 12 to 71 Months

  • Objectives: Early detection of developmental issues in infants and necessary intervention are important. To identify the comorbid conditions, a comprehensive evaluation is required. The study's objectives were to 1) generate scale items by identifying and eliciting concepts relevant to young children (12-71 months) with developmental delays, 2) develop a comprehensive screening tool for developmental delay and comorbid conditions, and 3) assess the tool's validity and cut-off. Methods: Multidisciplinary experts devised the "Infant Comprehensive Evaluation for Neurodevelopmental Delay (ICEND)," an assessment method that comes in two versions depending on the age of the child: 12-36 months and 37-71 months, through monthly seminars and focused group interviews. The ICEND is composed of three parts: risk factors, resilience factors, and clinical scales. In parts 1 and 2, there were 41 caretakers responded to the questionnaires. Part 3 involved clinicians evaluating ten subscales using 98 and 114 questionnaires for younger and older versions, respectively. The Child Behavior Checklist, Strengths and Difficulties Questionnaire, Infant-Toddler Social Emotional Assessment, and Korean Developmental Screening Test for Infants and Children were employed to analyze concurrent validity with the ICEND. The analyses were performed on both typical and high-risk infants to identify concurrent validity, reliability, and cut-off scores. Results: A total of 296 people participated in the study, with 57 of them being high-risk (19.2%). The Cronbach's alpha was positive (0.533-0.928). In the majority of domains, the ICEND demonstrated a fair discriminatory ability, with a sensitivity of 0.5-0.7 and specificity 0.7-0.9. Conclusion: The ICEND is reliable and valid, indicating its potential as an auxiliary tool for assessing neurodevelopmental delay and comorbid conditions in children aged 12-36 months and 37-71 months.

불안 장애와 주요우울장애에서 나타나는 신체 증상과 증상군에 따른 자살 사고, 계획, 행동과의 관계 고찰 (Difference of Somatic Symptoms between Anxiety Disorder and Major Depressive Disorder and Their Domainal Association with Suicidal Idealization, Plan and Attempts)

  • 연구목적 본 연구에서는 불안장애와 주요우울장애 및 불안장애와 우울장애가 동반된 중복 진단군의 신체 증상의 빈도와 특성의 차이를 알아보고 여러 신체 증상군과 자살 사고, 자살 계획, 자살 시도와의 관련성에 대해 분석하고자 한다. 방 법 본 연구는 국가적 역학조사연구인 2011년 정신질환 실태 역학 조사 연구(Korean Epidemiologic Catchment Area Study-2011, KECA-2011)에 참여한 만 18세 이상에서 74세 이하의 참여자 6027명 중 지난 1 년 동안 주요우울장애 또는 불안장애의 진단 기준을 만족 하면서, 같은 시기에 최소 1가지 이상의 신체증상을 호소한 378명을 대상으로 하였다. 이후 조사된 신체 증상을 통증 증상군, 소화기 증상군, 가성신경학적 증상군의 세군으로 분류하여, 각 증상군에 따른 자살 사고, 자살 계획 및 자살 시도의 정도를 비교하였다. 면담 도구로는 한국어판 CIDI를 사용하였으며, 이 중 신체 증상에 대한 항목이 포함된 C장과 자살관련 항목이 포함된 S장의 일부 질문을 연구에 사용하였다. 결 과 진단별로 신체 증상의 차이를 비교하였을 때, 주요우울장애에서는 흉통(p=0.016, 95%CI)이, 불안장애에서는 두통(p=0.004, 95%CI)과 묽은 변이나 설사를 나타내는 증상(p=0.018, 95%CI)이, 주요우울장애와 불안 장애의 중복진단군에서는 균형을 잡기 힘든 것(p=0.006), 기절할 것 같은 느낌(p=0.020, 95%CI), 기억을 잃음(p=0.034, 95%CI)의 증상이 유의하게 많았다. 자살 계획의 경우 통증 증상군에서 자살 계획이 있었던 군의 증상의 평균 개수가 계획이 없는 군에 비해 유의하게 높았다(p=0.026, 95%CI). 자살 시도의 경우 소화기 증상군(p=0.004, 95%CI) 및 가성신경학적 증상군(p=0.013, 95%CI) 에서 자살 시도가 있었던 군의 증상의 개수가 시도가 없었던 군에 비해 유의하게 높았다. 자살 사고, 자살 계획, 자살 시도로 갈수록 각 증상군의 신체 증상의 평균의 개수는 증가하는 경향성을 보였다. 결 론 본 연구는 불안장애와 주요우울장애의 신체증상의 특성에 차이가 있음을 보여주었으며, 증상의 개수가 자살 사고에서 자살 계획, 자살 시도로 진행되는 단계로 갈수록 증가하며 일부 증상군에서는 자살 계획과 자살 시도의 유무의 유의한 차이를 보여주는 것으로 나타났다. 이 결과는 비 정신과적 임상 현장에서 다양하고 많은 숫자의 신체 증상을 호소하는 환자의 경우 정신과적인 적극적인 평가 및 자살 위험도에 대한 적극적인 평가가 필요함을 시사한다.

  • 주의력결핍 과잉행동장애아(ADHD)와 품행장애나 반항성장애와 같은 외면성 증후를 공존질환으로 갖는 주의력결핍 과잉행동장애아(ADHD + CD/ODD)간에 사회경제적지위, 부모 학력수준, 생활 스트레스 사건, 친인척의 정신병력을 내용으로 하는 가족 배경과 부모의 결혼 만족도, 양육자 스트레스, 양육태도 등의 심리사회적 변인에 차이가 있는지 검증하였다. 연구결과, ADHD+CD/ODD집단이 위의 가족배경에서 가장 열악한 수준을 보이고 있었고, 부모의 결혼적응도에서도 ADHD+CD/ODD 집단이 가장 낮았으며, 양육자 스트레스에서는 ADHD+CD/ODD>ADHD>통제집단 순으로 높았으며 부모가 지각한 양육태도에서 임상집단의 어머니들이 증오적, 거부적 의존성을 포함하는 양육태도를 가지고 있었으며, 아동이 지각한 부모의 양육태도에서는 임상집단의 부모가 아동에게 더 거부적이고 지배적이고, 모순적인 양육태도를 지닌 경향성을 알 수 있었다.

알코올 의존 환자에서 삼대립 세로토닌 수송체 유전자 다형성과 생활사건 스트레스가 우울증에 미치는 영향 (The Effects of Triallelic Serotonin Transporter Gene Polymorphism and Stressful Life Event on Depression in Patients with Alcohol Dependence)

  • Objectives : The purpose of this study is to investigate the relationship between the triallelic serotonin transporter gene and stressful life events to determine their effect on depression with alcohol dependence. Methods : Ninety-five hospitalized patients with alcohol dependence (73 male, 22 female) were enrolled in this study. Thirty-two (33.7%) of the total patients were diagnosed with major depressive disorder and dysthymic disorder by Structured Clinical Interview for Diagnostic and Statistical Manual of Mental Disorders-IV. The characteristics of stress were evaluated using the stressful life events scale, and depressive symptoms were assessed using the depression scale (Beck Depression Inventory, BDI). Alcoholism with depression (n = 32) and alcoholism without depression (n = 63) were genotyped for the triallelic serotonin transporter gene ($L_A$ : higher expressing allele, $L_G$/S : lower expressing allele). Results : There was no significant difference in the allele frequency between the depression group and the non-depression group (${\chi}^2$ = 0.345, p = 0.619). $L_G$/S alleles had more comorbid depression in the higher score of stressful life events scale [Mental-Haenszel (MH)-${\chi}^2$ = 4.477, p = 0.034]. But there was no significant difference in the comorbidity according to the scores from the stressful life event scale in the $L_A$ alleles (MH-${\chi}^2$ = 0.741, p = 0.399). In the results, alcohol-dependent individuals with $L_G$/S alleles had more comorbid depression than those with $L_A$ alleles when they had experienced severe stressful life events (MH-odds ratio = 2.699, p = 0.028). Conclusions : These results suggest that there is no direct relationship between triallelic serotonin transporter gene and depression in the alcohol dependent patients. But alcohol dependent individuals with the lower expressing alleles of the serotonin transporter gene were more susceptible to depression than those with the higher expressing alleles in response to stressful life events.
