Psoas Compartment Block for the Relief of Lumbar and Left Thigh Pain after Operation of Second Lumbar Compression Fracture -A case report- (척추 수술 후 발생된 요부 및 좌측 대퇴전부 통증 치료를 위한 대요근 근구 차단술의 효과 -증례 보고-)

  • Psoas compartment block has been used to provide anesthesia and analgesia of hip joint. This block is advocated for relief of pain of various origins in the thigh, leg and lumbar area. A-40-year-old women complained of pain in the left thigh and lumbar area after operation of the second lumbar vertebral compression fracture. To relieve pain, caudal block was performed. This block reduced in lumbar pain but left thigh pain persisted. So, we were performed psoas compartment block using mepivacaine and dexamethasone, which relieved the pain in the left thigh and lumbar area. We recommend psoas compartment block as useful and simple method for patients with thigh and lumbar area pain, especially when the epidural block is not feasible.

A Study of the Depth from the Skin to the Psoas Compartment under C-arm Guidance (C자형 영상증강장치를 사용한 방사선 투시하에 피부로부터 대요근 근구까지의 깊이에 대한 연구)

  • Background: Psoas compartment block with local anesthetics and corticosteroids is one of the treatments which provides long term analgesia of the lower back and anterior thigh unilaterally, and its technical easiness and safety allows blind application without C-arm guidance in the out-patient clinic. This study aimed to evaluate the mean of the depth from the skin to the psoas compartment, and its correlation to the following attributes: age, weight, height and PI (Ponderal Index). Methods: We investigated 28 patients who underwent psoas compartment block. All blocks were performed using Chayen's method (punctured at the point of 3 cm caudally and 5 cm laterally from the 4th lumbar vertebral spinous process) with a 22 G, 8 cm Tuohy needle under C-arm guidance. We recorded the depth from skin to the psoas compartment, height, weight and PI (weight (kg)/height (cm)$\times100$ (%)). Data were analyzed using the Pearson product-moment correlation coefficients. The correlations between the depth and other attributes identified by p-value of less than 0.05 were considered statistically significant. Results: The mean depths from skin to the psoas compartment were $6.02{\pm}0.28$ cm in men, $5.44{\pm}0.22$ cm in women. There is no significant correlation between the depth and other patient's attributes. Conclusions: The mean depths from skin to the psoas compartment may be one of the guide for psoas compartment block in outpatient clinics without C-arm guidance.

  • Psoas compartment block has been used to provide anesthesia for orthopedic surgical procedures and postoperative pain control. Nowadays this block is advocated for the management of pain in the thighs, legs and lumbar area for various reasons. To relieve pain in the thigh and leg, 21 times of block were performed and observed clinically in the 17 patients at our hospital. The results were excellent for the relief of pain in metastatic cancer patients. And this block is satisfactory in the thigh pain but not in the leg pain. So we recommend psoas compartment block as a useful and simple method for the relief of pain in metastatic cancer pain in the thigh.

  • The psoas compartment block has been advocated as the technique of choice for the surgical correction of the fractured neck of the femur. It is, moreover, widely used as a technique for postoperative analgesia. This block has not been discribed in the treatment of pain in cancer patients unitl 1988. A 54-year-old woman with advanced stomach cancer complained of pain in the right leg. Psoas compartment block using bupivacaine and methylprednisolone; was effective for pain control. Psoas compartment block is a useful procedure and can be of particular use in patients who have not responded to opiates or find the side effects intolerable.

  • A psoas compartment block has been used to provide anesthesia for orthopedic surgical procedures and analgesia for post-operative pain. Currently, this block is advocated for relieving pain in the lower extremity and pelvic area resulting from various origins. We report a case of a 69-year-old male patient who had gait abnormality with posterior pelvic and hip pain, which were both aggravated by hip extension. From the magnetic resonance image, the patient was found to have a laterally herniated intervertebral disc at the L2/3 level, which compressed the right psoas muscle. This was thought to be the origin of the pain, so a psoas compartment block was performed using 0.25% chirocaine with triamcinolone 5mg, and the pain in both the pelvis and hip were relieved.

  • Reactivation of the latent varicella zoster virus in the sensory ganglion causes herpes zoster (HZ). Its characteristic symptom is a painful rash in the involved dermatome. HZ-induced motor weakness is rare and is usually resolved within one year of the onset, but some patients permanently experience motor dysfunction. Epidural steroid administration, with antiviral therapy, can be effective in treating pain from HZ and preventing postherpetic neuralgia. But an epidural block is contraindicated in patients receiving thromboprophylaxis. A psoas compartment block (PCB) provides equivalent analgesic efficacy with significantly low incidence of complication, compared to an epidural block. A 68 year old male patient recieving thromboprophylaxis presented with motor weakness following painful rash in his left L4 dermatome. Ten days before presentation, herpetic rash occurred on his left leg. We performed PCB with a steroid and local anesthetic, which successfully and safely alleviated the pain and motor weakness from HZ.

  • Low back pain(LBP) is one of the most common ailment. There were two type of LBP in the clinic. One of them is low back pain with leg pain, the other one is low back pain without leg pain. Author explain the reasons, characters. mechanism, diagnosis, and reported 156 cases of LBP. There are various method to treatment of low back pain in recent. The first selection for treatment of LBP were intervertebral block added "+" type block(IVP "+" TB). according to author's experience, the rate of Excellent and good were account for 96.2%. Other method of LBPO therapy were also used in Pain clinic, such as psoas compartment block, caudal block, epidural steroid block, zygapophysial joint block, nerve root block, subarachnoid neural block, Lumbar sympathetic block, etc. Finally, author introduced Pain clinic in China. divided to three titles: (1) history, (2) CASP and scientific activity, (3) pain therapy in China.

  • Postpuncture headache is the most common complication of epidural block, others include abscission of the tip of catheter, epidural abscess and subarachnoid infection, etc. A 69-year-old female patient visited the Neuro-Pain Clinic of Seran General Hospital for treatment of lower back pain and both sciatica. She received continuous epidural block, psoas compartment block, lumbar facet joint block and lumbar facet thermocoagulation. During the epidural block procedure the dura was accidently punctured and auto-logous blood patch was performed. Three days later, she manifested fever, nausea, vomiting, mild neck stiffness and mental deterioration. Meningitis was suspected as the cause of these signs. The CSF study reported: protein 400 mg/dl, sugar 14 mg/dl, WBC $468/mm^3$. She was recovered from the meningitis after adequate antibiotic therapy.
